Abstract

Reactivation of VHB infection is a well documented complication of cytotoxic treatment. It appears in 20-50% of the patients with positive HBsAg under treatment, and has a mortality ranging from 5 to 12%. There is no standard treatment for the infection, and the current practice is observation. Nevertheless, treatment with lamivudine and adefovir dipivoxil have been tried with promising results. Serum determination of HBsAg must be done to all patients with cancer before initiating chemotherapy, admninistering lamivudine if analytical data indicate the existence of viral replication. A case of subfulminant acute hepatitis of a patient with prostate adenocarcinoma treated with adefovir dipivoxil is presented in this article. Treatment and prevention of viral reactivation are reviewed.
