The world of work is changing and this is impacting on the nature of occupational diseases (ODS). Globally, ODS are increasingly challenging. This article focus on these challenges in particular, issues regarding the notification, registration and monitoring of ODS, occupational disease is a legal and not a medical term, new and emerging types of diseases, ODS with long latency periods, even more complex than the definition are posible causes of occupational diseases. In many cases, work related factors increment the risk of a disease together with other, not work related factors. It is this complex multicausality of ODS which makes them not only difficult to recognize and record, but also highly difficult to prevent and insure. At the end, some practically relevant methods and practices should be derived in order to propose strategies to answer these challengers: ensure sound reporting, motivate to report, strengthen diagnostic quality, follow up on exposed workers, review/update lists of ODS and foster prevention of occupational diseases.
