Silent sinus syndrome consists of the implosion of the maxillary sinus due to improper pneumatization. It is a rare condition that can cause facial asymmetry and diplopia. Its diagnosis is both clinical and radiological. Patients present with hypoglobus and/or enophthalmos, and on CT scans or MRI, a reduction in the volume of the affected sinus is observed, along with the displacement of its walls, which appears postnatally, differentiating it from sinus atresias. The treatment is based on endoscopic surgery to restore proper sinus ventilation, with orbital reconstruction as an option to be performed either simultaneously or in a second intervention. In this article, we describe the evolution, diagnosis, and treatment of a patient with silent sinus syndrome, and we address the most controversial and still unknown aspects of the condition.
