Approaching the health-disease process is an initial step to strengthen collective's own care. Hence, perceptions on health-disease and care paths of pauper afrocolombian families at urban zone of Tumaco (Pacific Region, Colombia) are described. Through an ethnographic qualitative perspective, pauper afrocolombian extended families at Tumaco were studied, selected by convenience sampling. Ethnographic interviews and participant observations were used as research tools. Disease emerged as deterioration of body; to be healthy meant its vitality. Care paths showed up priority health imbalances, different routes and health workers for their care. Getting close to meanings of health-disease process and to care paths rendered key concepts to guide health interventions fine-tuned to culture and to address actions in research and nursing teaching.
