Resumen

FERNANDEZ-CASTILLO, A.; VILCHEZ-LARA, Mª J.  y  CABALLERO, J.. Post-surgical acute pain complementary assessment in an analgesic power contrast. Rev. Soc. Esp. Dolor [online]. 2006, vol.13, n.3, pp.151-158. ISSN 1134-8046.

Objectives: Our investigation pursues two basics objectives: a) in first place we try to analyze the possible congruity between two evaluation procedures of post-surgical pain: the Numerical Verbal Scale (NVS) and the self-administration of analgesia as a complementary criterion. b) In second place, this study tries to determine the previous objective in the framework or the compared effect of two analgesic medicine procedures, Ketorolac and Dexketoprofen trometamol, in patients submitted to orthopaedic surgery, concretely hip prosthesis. Material and Method: 92 patients ASA II was selected, and a double blind study was realized with the implantation of continuous per-fusions (PCA) analgesia during 72 hours, with the possibility of auto-administration. As evaluation methods of the postoperative acute pain was utilized a Numerical Verbal Scale and the self-administration of analgesia as complementary criterion. The subjects that satisfy the inclusion criterion were randomly assigned, after reported consent, to one of the two established experimental conditions. Results: There are not significant differences in the analgesic power of both NSAIDs during the 72 studied hours. The Dexketoprofen trometamol does not present important analgesic additional advantages. The analgesic behaviour during 72 h, the requests of extra analgesics and the need of rescue analgesia, was similar. A parallelism between the two post-surgical acute pain evaluation procedures used is observed. They present a similar validity and a significant correlation among them [r =. 49, (p<0.00)]. Conclusions: In relationship with the frequent criticisms on the evaluation of the pain with the Numerical Verbal Scale, other more objective assessment procedures, permit us to determine the level of pain, and therefore to increase the level of reliability and validity of the numerical and analogical scales. On the other hand, and according to other investigations that have shown the individual or separated effectiveness of the two considered NSAIDs, our results confirm the non-existence of significant differences between both analgesics, showing both a similar analgesic power in the treatment of the postoperative acute pain after orthopaedic surgery.
