Introduction: Infection is a postoperative complication after third molar removal. Most infections are mixed in origin; they can appear early after the surgery or as a secondary infection later on, producing a higher morbidity and delaying the patient recovery. Objective: To evaluate the efficacy in preventing postoperative infections of Sultamicillin (Sulbactam- Ampicillin); a penicillin-derived antibiotic, resistant to beta-lactamases. Method: 476 impacted third molars were removed in 238 patients, all of them received Sultamicillin pre- and postoperatively. Factors evaluated were statistically analysed during a follow-up period of three months. Results: In our own study, we found 0.63% of alveolitis, in contrast with a 0.5% to 30% incidence level, reported from other authors. Postoperative secondary infections rate was 0,42%, the lowest level of incidence we have found in the literature, which reports rates from 1,5% to 5,8%. Conclusion: The excellent results we have obtained demonstrate the efficacy of Sultamicillin in preventing postoperative infections after third molar removal.
