This manuscript presents a narrative review aimed at characterizing encephalopathy caused by beta-lactams, including its clinical presentation, diagnostic work-up, and therapeutic management. A systematic search was conducted using the PubMed, UpToDate, and StatPearls databases. The review included a range of studies, such as systematic reviews, observational studies, case series, and case reports, all of which examined the relationship between encephalopathy, beta-lactams, and neurotoxicity. Encephalopathy is a disorder of generalized attention, cognition, or consciousness, and its acute form can manifest with a wide range of symptoms, from mild confusion to delirium or coma. One possible cause of encephalopathy is the use of antibiotics, particularly beta-lactams. Neurotoxicity induced by beta-lactams presents with a rapid onset of symptoms within days, including myoclonus or seizures, abnormal electroencephalogram findings, and a normal magnetic resonance imaging scan. Treatment involves discontinuing the offending drug, which leads to a resolution of symptoms within a few days. This review underscores the importance of conducting more comprehensive studies with clearly defined inclusion criteria in order to improve the understanding and diagnosis of this condition.
