Introduction:

Chronic non-communicable diseases are the leading cause of death in the world population, limited disability, people's quality of life, and dependency.

Methodology:

Descriptive cross-sectional design with an approach to health assets in a community case management model for complex chronic patients in six locations in the city of Bogotá-Colombia due to their population and demographic characteristics.

Conclusions:

The mapping of assets that promotes self-management of health in the community allows to promote the exercise of their rights under their conditions and needs.
