Pain is the second reason for assistance in Primary Care, where up to 50% of consultations are related to chronic pain. Chronic pain is a relevant health problem due to its high incidence, its complexity and its consequences on the patient's quality of life. Primary care professionals are often the first to treat patients with pain, both chronic and acute, and are thus in a unique position to treat, address pain and improve the patient's quality of life.
Mr. V.R. is a 55-year-old male patient with osteochondropathy, meniscopathy and phlebitis and thrombophlebitis, who, after several interventions of the meniscus of the right lower limb, presents two ulcers in the internal malleolar area of the right lower limb and chronic pain in the lower limbs. Poor pain control and the slow healing process of ulcers cause a decrease in the patient's quality of life.
Thanks to the novelties in the use of tools for nursing assessment, a care plan was carried out applying a standardized language through the NANDA-I, NOC and NIC taxonomy, with evidence-based interventions.
