The evidence currently available suggest that the safety and quality of care can be improved. In this article presents some of the strategies and interventions developed to optimize the processes of care in critically ill patients, and improve the safety culture in the ICU.]]></p></abstract>

<body><![CDATA[<br>Thomas Edison (1847-1931)</font></p> </blockquote>     <p><font face="Verdana" size="2">La monitorización, medición y mejoramiento de la seguridad y la calidad de la atención en la unidad de cuidados intensivos (UCI) representa un gran desafío para la comunidad de los cuidados críticos. Seguridad y calidad son 2 atributos del sistema de salud que se encuentran estrechamente entrelazados. El Instituto de Medicina estadounidense define <i>calidad de la atención de la salud</i> como el "grado en que los servicios sanitarios incrementan la probabilidad de resultados de salud deseables para individuos o poblaciones, y que son coherentes con los conocimientos actuales de la profesión"<sup>1</sup>. En otras palabras, la calidad es una característica del sistema en que se entrega la atención de salud y, por ende, para mejorarla se necesita una adecuada reorganización del trabajo en equipo. Adicionalmente, en el documento se establece que la atención de salud debe cumplir 6 objetivos: ser segura, efectiva, centrada en el paciente, oportuna, eficiente y equitativa<sup>1</sup>.</font></p>     <p><font face="Verdana" size="2">Un modelo conceptual habitualmente utilizado para la medición de calidad de la atención se basa en la trilogía de Donabedian: <i>estructura</i> (cómo se encuentra organizada la atención), <i>procesos</i> (qué hacen los proveedores de salud) y <i>desenlace</i> (resultados alcanzados con la atención de salud)<sup>2</sup>. Pronovost et al han adicionado un cuarto componente a esta trilogía: <i>contexto</i> en el que la atención de salud es entregada, y lo han denominado <i>cultura de la seguridad</i> debido a que existe evidencia que muestra que la cultura local se asocia a resultados clínicos relevantes<sup>3-5</sup>.</font></p>     <p><font face="Verdana" size="2">Se han desarrollado y propuesto múltiples indicadores de calidad para su evaluación en los pacientes críticos<sup>6,7,8,9</sup>. Para desarrollar una medida de calidad se necesita: 1) priorizar el área clínica a evaluarse; 2) seleccionar el tipo de medida; 3) definir y diseñar especificaciones; 4) desarrollar elementos para recolectar la información; 5) probar los elementos de recolección de los datos y constatar la validez, la reproducibilidad y la factibilidad de las mediciones; 6) desarrollar puntuaciones y especificaciones analíticas, y 7) obtener los datos basales<sup>10</sup>. Las mediciones de calidad tienen como objetivo evaluar si el proceso completo de atención de salud alcanza los objetivos deseados mientras evita los procesos que predisponen al daño del paciente.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Cultura de la seguridad</b></font></p>     <p><font face="Verdana" size="2">La <i>cultura de la seguridad</i> hace referencia a las actitudes colectivas y a las creencias locales de los trabajadores de la salud que impactan en el desenlace de los pacientes. Una organización comprometida con la cultura de la seguridad es aquélla en la que los estándares de seguridad son reforzados a nivel personal y a nivel grupal. Para mejorar la cultura de la seguridad, las organizaciones deben identificar los puntos débiles o deficientes del sistema y corregirlos. Los sistemas tienen que ver tanto con la forma en que se encuentra organizado el trabajo como con la cultura de la organización. Una cultura de la seguridad constructiva y dinámica debe reunir varias características: 1) compromiso de los líderes para discutir y aprender de los errores (acento en la educación); 2) documentar y mejorar la seguridad de los pacientes; 3) fomentar y practicar el trabajo en equipo; 4) detectar los peligros potenciales; 5) usar sistemas de reporte y análisis de efectos adversos, y 6) reconocer y elogiar el esfuerzo desplegado por el grupo para lograr el fortalecimiento de un clima de trabajo seguro<sup>11</sup>. En la última década se ha podido apreciar un creciente interés por la monitorización y el mejoramiento de la seguridad de los pacientes críticos<sup>2</sup>.</font></p>     <p><font face="Verdana" size="2">El grupo para la investigación en seguridad y calidad de la Universidad Johns Hopkins ha desarrollado una tarjeta para monitorizar y medir la seguridad de los pacientes (<i>patient safety scorecard</i>) en la UCI. Este instrumento estratifica las mediciones en 2 categorías. Una categoría usa mediciones basadas en tasas válidas: ¿cuán a menudo nosotros dañamos a los pacientes? (medida de <i>desenlace</i>) y ¿cuán a menudo nosotros proveemos las intervenciones que los pacientes deberían recibir? (medida de <i>proceso</i>). La segunda categoría incluye mediciones que no pueden expresarse como tasas válidas: ¿cómo sabemos que hemos aprendido de los defectos? (medida de <i>estructura</i>) y ¿cuán bien hemos creado una cultura de seguridad? (medida de <i>contexto</i>)<sup>5</sup>.</font></p>     <p><font face="Verdana" size="2">Los parámetros objetivos para evaluar cada una de estas categorías se han desarrollado e implementado en poblaciones de pacientes críticos. Los investigadores de la Universidad Johns Hopkins han propuesto la tasa de infecciones del torrente sanguíneo (ITS) asociada a catéter venoso central (CVC) para medir <i>desenlace</i>; la tasa de cumplimiento de intervenciones basadas en medicina de evidencia para el cuidado de pacientes en ventilación mecánica (VM) para medir <i>proceso</i>; el porcentaje de meses en el año en que la unidad aprendió de un error para monitorizar <i>estructura</i>, y el porcentaje de miembros del equipo de la UCI que reportan un trabajo en equipo y un clima de seguridad positivos para monitorizar <i>contexto</i><sup>12</sup>.</font></p>     <p><font face="Verdana" size="2">Recientemente, Pronovost et al<sup>13</sup> reportaron el resultado de un notable esfuerzo por mejorar la seguridad en la atención de los pacientes críticos. El objetivo general de este estudio fue describir el diseño y las lecciones aprendidas de la implementación de un programa colaborativo a gran escala sobre la seguridad de los pacientes medida a través de un instrumento validado (<i>Safety Attitudes Questionnaire</i>)<sup>14</sup> y el impacto de una intervención (<i>Comprehensive Unit-based Safety Program</i>) sobre el clima de trabajo en equipo en las UCI del estado de Michigan<sup>15</sup>. Estos instrumentos buscaron básicamente lo siguiente: 1) evaluar el estado actual de la situación; 2) educar al equipo de la UCI; 3) promover la identificación de los defectos o los errores; 4) incorporar a los directores administrativos del hospital para facilitar el cambio y la provisión de recursos; 5) motivar al personal para aprender de un error por mes e implementar elementos de trabajo en equipo y, 6) una vez culminado el proceso, volver a evaluar la cultura de seguridad en la UCI.</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">En el proceso participaron 99 UCI de 72 hospitales, y se obtuvo la respuesta de 5.440 (72%) de las 7.510 encuestas distribuidas para evaluar el clima de trabajo en equipo. Los autores encontraron un mejoramiento significativo en el clima laboral del equipo de la UCI después de la intervención. Este trabajo es muy relevante por cuanto demuestra que con la estrategia apropiada es posible mejorar el clima de trabajo en equipo, lo cual es fundamental para una atención de salud segura y con adecuados estándares de calidad para los pacientes<sup>16,17</sup>.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Errores clínicos y efectos adversos</b></font></p>     <p><font face="Verdana" size="2">En EE. UU. cada año mueren entre 44.000 y 98.000 pacientes como consecuencia de errores clínicos, lo que lo coloca como la séptima causa de muerte en ese país<sup>18-20</sup>. La UCI, sin lugar a dudas, no es el servicio más seguro de un hospital<sup>21,22</sup>. Los errores clínicos y los efectos adversos ocurren más frecuentemente en las UCI que en otras áreas de un hospital porque es ahí donde se encuentran los enfermos más complejos, afectados por múltiples enfermedades que amenazan su vida, por ende, reciben un elevado número de medicaciones y requieren de diversos procedimientos e intervenciones para su cuidado<sup>23</sup>. Debido a todo ello, el riesgo de exponerse a un error clínico o a experimentar un efecto adverso en este grupo de pacientes es ostensiblemente mayor que el riesgo para un paciente hospitalizado en una sala general, lo cual tiene implicaciones en morbimortalidad y costos<sup>24-27</sup>. La principal causa predisponente para errores clínicos es una comunicación deficiente entre los trabajadores de la salud<sup>28</sup>. El desarrollo y la aplicación de instrumentos preventivos (<i>checklist</i>)<sup>29</sup> así como la confección de reportes de incidentes o episodios centinela que identifiquen en qué punto el sistema se vulneró<sup>30-34</sup> son necesarios para que aprendamos de los errores y podamos mejorar la seguridad de la atención en la UCI. Los pacientes críticos pueden estar expuestos, con relativa frecuencia, a efectos adversos prevenibles asociados a medicamentos, a VM y a dispositivos intravasculares.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Efectos adversos asociados a medicamentos</b></font></p>     <p><font face="Verdana" size="2">Los enfermos críticos están expuestos a un elevado riesgo de experimentar efectos adversos por medicamentos<sup>35</sup>. Los errores en el tratamiento de la medicación pueden ser por prescripción, transcripción, dispensación, preparación o administración<sup>36</sup>. Los errores en la administración de medicamentos pueden ocurrir en un tercio de pacientes hospitalizados en una UCI<sup>37</sup>. Varias estrategias se han indicado en la literatura médica para prevenir este tipo de complicaciones iatrogénicas<sup>36,38</sup>, entre ellas la implementación de un "procedimiento estandarizado de trabajo" (<i>standard operating procedure</i>) para el tratamiento de medicamentos<sup>39</sup>.</font></p>     <p><font face="Verdana" size="2">El empleo de bombas de infusión ha mejorado la precisión en la administración de fármacos en pacientes críticos; sin embargo, estos dispositivos están involucrados en el 35 al 60% de los 700.000 incidentes relacionados con medicamentos que ocurren anualmente en EE. UU<sup>40</sup>. Los errores en la programación de las bombas de infusión son los que tienen la mayor probabilidad de causar daño al paciente<sup>41</sup>. Las bombas de infusión convencionales no alertan ni previenen en tiempo real sobre estos potenciales errores. Recientemente, se ha desarrollado una nueva generación de bombas de infusión con tecnología inalámbrica denominadas "bombas de infusión inteligentes"<sup>42</sup>. Estas bombas de infusión inteligentes incluyen un <i>software</i> de seguridad que permite la creación de una biblioteca de fármacos, lo que hace posible la protocolización de los diferentes tratamientos de infusión, mediante la programación de niveles de alarma con límites prestablecidos en relación con dosis de los medicamentos, unidad de dosificación, velocidad de dosificación o concentración de los medicamentos<sup>43</sup>. Trabajos recientes han mostrado un adecuado desempeño del <i>software</i> de seguridad de las bombas de infusión inteligentes y una reducción en el número de errores asociados a la administración de medicamentos<sup>44,45</sup>.</font></p>     <p><font face="Verdana" size="2">En 2006, el Instituto de Medicina estadounidense reportó que "cuando todos los tipos de errores son tomados en consideración, un paciente hospitalizado puede estar expuesto en promedio a uno o más errores de medicación cada día". En este mismo documento se consigna que "gran parte del daño se puede prevenir" e indica que los hospitales "deberían hacer mayor uso de la tecnología de la información en prescribir y dispensar medicamentos, incluida la adopción de bombas de infusión inteligentes"<sup>46</sup>. De hecho, algunos autores han llegado a plantear que las bombas de infusión inteligentes deberían considerarse actualmente como el estándar mínimo de seguridad en la UCI<sup>47</sup>.</font></p>     <p>&nbsp;</p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2"><b>Efectos adversos asociados a la ventilación mecánica</b></font></p>     <p><font face="Verdana" size="2">La neumonía asociada a VM (NAVM) es una complicación que ocurre del 20 al 25% de pacientes que reciben VM por más de 48h<sup>48</sup>. La NAVM es la infección nosocomial más frecuente en la UCI y la principal causa de muerte por infecciones en enfermos críticos<sup>49</sup>. Además, es la causante de más de la mitad de los antibióticos prescritos en la UCI y genera un importante incremento en morbilidad y costos de la atención<sup>50-52</sup>. Varias medidas se han desarrollado con la finalidad de reducir la incidencia de esta complicación, sin embargo, aún no se implementan habitualmente<sup>53-55</sup>. La optimización de la administración de la sedación mediante un tratamiento protocolizado ha demostrado reducir significativamente los días de VM, la incidencia de NAVM y la estadía en la UCI<sup>56-60</sup>. El desarrollo de agentes sedantes de nueva generación puede promover una mejor calidad de la sedación y resulta prometedor<sup>61-63</sup>.</font></p>     <p><font face="Verdana" size="2">Mientras más tiempo se encuentren los pacientes en VM, mayor el riesgo de que desarrollen una NAVM, la que prolongará los días en el ventilador, cerra´ndose un círculo vicioso que puede terminar cobrando la vida del paciente. De ahí la necesidad de evitar que los pacientes permanezcan bajo sedación profunda y en VM más allá del tiempo estrictamente necesario. La liberación de la VM continúa siendo desafiante, sin que hasta el momento existan predictores completamente confiables<sup>64,65</sup>; sin embargo, un enfoque protocolizado y multidisciplinario mediante ensayos diarios de respiración espontánea ha demostrado reducir los días de VM<sup>66-69</sup>. Recientemente, Girard et al<sup>70</sup> encontraron una reducción de los días de VM, estadía en la UCI y en el hospital y menor mortalidad al año de seguimiento aplicando un protocolo que combinó interrupción diaria de la sedación y ensayos de respiración espontánea.</font></p>     <p><font face="Verdana" size="2">Una de las iniciativas más interesantes ha sido el desarrollo del concepto de <i>bundles</i> o paquete de medidas por el Instituto para el Mejoramiento de la Asistencia Sanitaria de EE. UU. La estrategia del <i>bundle</i> consiste en incorporar un pequeño grupo de intervenciones basadas en medicina de evidencia<sup>71</sup> para mejorar diferentes procesos del cuidado de los pacientes críticos. El <i>bundle</i> específico para mejorar el cuidado de pacientes en VM y reducir la incidencia de NAVM consta de 5 intervenciones: 1) elevar la cabecera de la cama al menos a 30<sup>o</sup>; 2) sedación protocolizada; 3) evaluación diaria de las posibilidades de extubación; 4) profilaxis de trombosis venosa profunda, y 5) profilaxis de úlceras de estrés. Varios estudios han demostrado que la implementación y la adherencia a este <i>bundle</i> reducen la incidencia de NAVM<sup>72-74</sup>.</font></p>     <p><font face="Verdana" size="2">Recientemente, un estudio nacional multicéntrico<sup>75</sup> encontró que la insuficiencia respiratoria aguda fue la causa que llevó a la VM en el 70% de los pacientes, y cerca de la mitad de los casos tuvieron como origen una sepsis grave. La sepsis grave y el <i>shock</i> séptico son sin lugar a dudas entidades catastróficas y constituyen un verdadero problema de salud pública, tanto por las vidas que cobran anualmente como por los recursos que se destinan para su atención. Dougnac et al, en un estudio multicéntrico chileno, encontraron una prevalencia de sepsis grave en pacientes críticos de un 40%, con una mortalidad asociada de un 27%<sup>76</sup>. En este escenario, toma trascendental relevancia el tratamiento oportuno e integral de esta enfermedad mediante un diagnóstico precoz y la implementación de una reanimación intensiva guiada por metas claras<sup>77,78</sup>. El enfrentamiento protocolizado de esta enfermedad puede generar un mejoramiento significativo en las posibilidades de supervivencia de estos pacientes<sup>79,80</sup>.</font></p>     <p><font face="Verdana" size="2">La Campaña Sobreviviendo a la Sepsis o Surviving Sepsis Campaing ha desarrollado un <i>bundle</i> para la reanimación inicial de pacientes con sepsis grave<sup>81,82</sup>. En el paquete están incluidas todas las intervenciones y los tratamientos que debe recibir obligatoriamente un paciente durante las primeras 6h de diagnosticada la sepsis grave. Este <i>bundle</i> consta de 5 intervenciones:</font></p>     <p><font face="Verdana" size="2">1. Medir lactato arterial (LA).</font></p>     <p><font face="Verdana" size="2">2. <body><![CDATA[<p><font face="Verdana" size="2">La inserción, el uso o la remoción de un CVC pueden ocasionar complicaciones mecánicas, infecciosas o trombóticas hasta en un 26% de pacientes críticos<sup>26</sup>. Se han desarrollado guías clínicas con diferentes intervenciones para evitar o reducir complicaciones asociadas a dispositivos intravasculares, tanto durante la inserción como durante la mantención<sup>85-88</sup>; sin embargo, su implementación en la práctica diaria continúa siendo pobre<sup>89</sup>. Esfuerzos adicionales se han realizado en el diseño y la selección de la materia prima para la confección de los catéteres<sup>90,91</sup>, así como en el desarrollo de dispositivos de sistema cerrado y válvulas antirreflujo que promuevan un tratamiento más seguro y evite oclusiones trombóticas y riesgos de infección<sup>92-97</sup>.</font></p>     <p><font face="Verdana" size="2">El grupo para la investigación en seguridad y calidad de la Universidad Johns Hopkins desarrolló un <i>bundle</i> específico para el control de las ITS asociadas a CVC. Cada una de las intervenciones por separado ha mostrado reducir la incidencia de ITS asociadas a CVC, aplicadas en conjunto pueden tener no sólo un efecto aditivo, sino un efecto sinérgico. Este <i>bundle</i> consta de 5 intervenciones: 1) lavado de las manos; 2) empleo completo de barreras de protección; 3) limpieza de la piel con clorhexidina; 4) evitar la posición femoral del catéter, y 5) retirar el CVC tan pronto sea necesario. Su implementación ha demostrado reducir considerablemente las tasas de ITS asociadas a CVC y tener un efecto sostenido en el tiempo<sup>98</sup>.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Consideraciones finales</b></font></p>     <p><font face="Verdana" size="2">La ciencia de la seguridad y la calidad en Medicina Intensiva se encuentra en evolución; sin embargo, en esta primera etapa de su desarrollo ya ha generado algunos resultados alentadores. Monitorizar y medir la cultura de la seguridad con instrumentos validados e implementar medidas que mejoren el clima de trabajo en equipo en las UCI son elementos fundamentales para mejorar la calidad de la atención y la seguridad de los pacientes críticos. La evidencia disponible nos indica que es posible alcanzar este objetivo. La estrategia de los <i>bundles de cuidado</i> puede ayudar a mejorar nuestros procesos de atención y asegurar que los pacientes reciban las intervenciones necesarias para mejorar su desenlace<sup>99</sup>. Algo importante de enfatizar es que muchas de la intervenciones que han demostrado generar un impacto significativo en la reducción de errores clínicos y en disminuir la tasa de infecciones intrahospitalarias no requieren de grandes inversiones de dinero ni de tecnología sofisticada, sino de un cambio de mentalidad y una reorganización del trabajo en equipo que permita el desarrollo y el fortalecimiento de una cultura de la seguridad en las UCI.
