Material y métodos: Desde enero de 2006 a julio de 2009, 51 pacientes fueron sometidos a colocación de catéter de diálisis peritoneal usando una nueva técnica. Todos los procedimientos se completaron laparoscópicamente usando dos puertos de 12 mm. Nuestra técnica se basa en la colocación de un catéter de tipo Oreopoulos-Zellerman sobre una guía de Guyon recta con punta atraumática, y garantiza la óptima colocación del catéter. En caso necesario, éste se puede poner de nuevo mediante la recolocación de la guía. El seguimiento medio ha sido de 25 meses. Resultados: Tiempo quirúrgico medio: 32 minutos (rango 15-55 minutos). Un paciente presentó una obstrucción del catéter en el postoperatorio inmediato, que requirió recolocación quirúrgica. No se han producido otras complicaciones técnicas durante la cirugía o el postoperatorio inmediato. Media de tiempo al alta: 1,02 &plusmn; 2,2 días. Tasa de obstrucción del catéter: 7,6%. Tasa de conversión a hemodiálisis secundaria a peritonitis: 13%. Episodios de peritonitis por paciente-año: 0,27. Supervivencia del catéter a los 6 meses, un año y 5 años: 94, 87 y 72%, respectivamente. Tasa de migración de catéter: 4%. No se han comentado casos de fístula de líquido peritoneal. Conclusiones: La técnica de dos puertos descrita es un procedimiento sencillo y rápido, con pocas complicaciones y alta hospitalaria inmediata. Debido a su fiabilidad, ofrece buenos resultados en la función del catéter.]]></p></abstract>

Due to its reliability, offers good catheter function outcome.</font></p>     <p><font face="Verdana" size="2"><b>Key words:</b> Peritoneal dialysis, Laparoscopy, Surgical technique.</font></p> <hr size="1">     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Introducci&oacute;n</b></font></p>     <p><font face="Verdana" size="2">La di&aacute;lisis peritoneal (DP) es una alternativa v&aacute;lida a la hemodi&aacute;lisis, que presenta algunas ventajas en comparaci&oacute;n con &eacute;sta. En lo que respecta a los pacientes, la DP permite mejor movilidad, mayor libertad en la dieta, mejor control hemodin&aacute;mico y menor complejidad t&eacute;cnica<sup>1</sup>. Desde un punto de vista econ&oacute;mico, la DP presenta un coste menor comparada con la hemodi&aacute;lisis. En suma, los pacientes en DP presentan mayor satisfacci&oacute;n que los pacientes en di&aacute;lisis<sup>2-8</sup>. Por otro lado, la DP presenta algunas desventajas, la mayor&iacute;a de ellas relacionadas con el cat&eacute;ter, como la infecci&oacute;n del cat&eacute;ter, la obstrucci&oacute;n o la migraci&oacute;n del cat&eacute;ter, la extrusi&oacute;n del <i>cuff</i>, las eventraciones y las fugas de l&iacute;quido<sup>5,9-12</sup>.</font></p>     <p><font face="Verdana" size="2">Las t&eacute;cnicas de colocaci&oacute;n del cat&eacute;ter han evolucionado desde la cirug&iacute;a abierta hacia los procedimientos m&iacute;nimamente invasivos durante las &uacute;ltimas dos d&eacute;cadas. Paralelamente, se ha empleado la colocaci&oacute;n percut&aacute;nea de cat&eacute;teres de di&aacute;lisis mediante la t&eacute;cnica de Seldinger<sup>13</sup>. En la actualidad, la colocaci&oacute;n del cat&eacute;ter se puede llevar a cabo mediante cirug&iacute;a abierta, inserci&oacute;n percut&aacute;nea o cirug&iacute;a laparosc&oacute;pica.</font></p>     <p><font face="Verdana" size="2">La cirug&iacute;a abierta es un procedimiento sencillo, que requiere una laparotom&iacute;a m&iacute;nima, por lo que ha sido el procedimiento de elecci&oacute;n m&aacute;s ampliamente usado<sup>2,7,14-16</sup>. Sin embargo, la cirug&iacute;a abierta permite una visi&oacute;n limitada, hecho especialmente importante en pacientes con antecedentes de cirug&iacute;a abdominal, en los cuales las adherencias intestinales pueden dificultar el procedimiento<sup>17-19</sup>. Por este motivo, la tasa de obstrucci&oacute;n de los cat&eacute;teres colocados por cirug&iacute;a abierta alcanza el 22%<sup>3,15,16,20</sup>.</font></p>     <p><font face="Verdana" size="2">Estos problemas t&eacute;cnicos con la cirug&iacute;a abierta llevaron, hace dos d&eacute;cadas, al desarrollo de nuevas estrategias para la colocaci&oacute;n de cat&eacute;teres de DP. La cirug&iacute;a laparosc&oacute;pica, llevada a cabo en su mayor&iacute;a con tres tr&oacute;cares, se desarroll&oacute; en este punto<sup>21,22</sup>. Al facilitar una visi&oacute;n &oacute;ptima de la cavidad peritoneal, y por tanto mejorar la obstrucci&oacute;n del cat&eacute;ter y las infecciones relacionadas, el abordaje laparosc&oacute;pico gan&oacute; amplia aceptaci&oacute;n<sup>23-25</sup>. La cirug&iacute;a laparosc&oacute;pica presenta una serie de claros beneficios, sobre todo en la reducci&oacute;n del dolor postoperatorio. El menor dolor postoperatorio permite un alta m&aacute;s precoz y una pronta normalizaci&oacute;n de la vida social<sup>4,26</sup>. Adem&aacute;s, la laparoscopia ofrece mejores resultados cosm&eacute;ticos.</font></p>     <p><font face="Verdana" size="2">En el presente estudio describimos una nueva t&eacute;cnica quir&uacute;rgica para la colocaci&oacute;n del cat&eacute;ter de DP, usando un abordaje laparosc&oacute;pico con dos puertos. Esta experiencia inicial examina su viabilidad, eficacia y seguridad.</font></p>     ]]></body>
<body><![CDATA[<p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Material y m&eacute;todos</b></font></p>     <p><font face="Verdana" size="2">Analizamos prospectivamente a 51 pacientes consecutivos, que fueron sometidos a colocaci&oacute;n de cat&eacute;ter de DP en nuestro centro, desde enero de 2006 hasta julio de 2009. Los datos demogr&aacute;ficos, cl&iacute;nicos, preoperatorios y postoperatorios se recogieron de forma prospectiva.</font></p>     <p><font face="Verdana" size="2"><b>Datos demogr&aacute;ficos</b></font></p>     <p><font face="Verdana" size="2">Los pacientes fueron 19 mujeres y 32 varones, con una edad media de 56 &plusmn; 18 a&ntilde;os. Todos los procedimientos se llevaron a cabo con anestesia general. El &iacute;ndice de masa corporal medio fue 24,5 &plusmn; 3,5 kg/m<sup>2</sup>. El riesgo anest&eacute;sico (ASA) medio fue III (40% ASA II, 48% ASA III, 12% ASA IV).</font></p>     <p><font face="Verdana" size="2"><b>T&eacute;cnica quir&uacute;rgica</b></font></p>     <p><font face="Verdana" size="2">Describimos una nueva t&eacute;cnica quir&uacute;rgica usando dos puertos de 12 mm para la colocaci&oacute;n del cat&eacute;ter de DP; para ello se usa una gu&iacute;a de Guyon de punta atraum&aacute;tica (<a href="#f1">figura 1</a>). Tras lubricar la gu&iacute;a de Guyon, el cat&eacute;ter se coloca sobre &eacute;sta y se obtiene un cat&eacute;ter tutorizado r&iacute;gido. <body><![CDATA[<p><font face="Verdana" size="2">Realizamos el neumoperitoneo mediante una laparotom&iacute;a m&iacute;nima periumbilical y se coloca un trocar de 12 mm. Bajo visi&oacute;n directa, se pone un trocar de 12 mm pararrectal izquierdo. Se sit&uacute;a la &oacute;ptica en &eacute;ste y se coloca el cat&eacute;ter tutorizado con la gu&iacute;a a trav&eacute;s del trocar periumbilical. Seguidamente, se instala la punta del cat&eacute;ter en el fondo de saco de Douglas y se retira la gu&iacute;a de Guyon. Comprobamos visualmente que el cat&eacute;ter queda adecuadamente colocado, tras lo que se retiran ambos tr&oacute;cares. Se crea un t&uacute;nel subcut&aacute;neo entre ambos tr&oacute;cares y se exterioriza el cat&eacute;ter a trav&eacute;s del orificio del trocar pararrectal izquierdo (<a href="#f2">figura 2</a>).</font></p>     <p align="center"><font face="Verdana" size="2"><a name="f2"><img src="/img/revistas/nefrologia/v30n3/nota2_figura2.jpg"></a>    <br><b>Figura 2.</b> Posición de los puertos. Puerto periumbilical de 12 mm para el catéter y    <br>puerto pararrectal izquierdo de 12 mm para la óptica.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Resultados</b></font></p>     <p><font face="Verdana" size="2">Todos los procedimientos se completaron laparosc&oacute;picamente con dos puertos de 12 mm. El tiempo quir&uacute;rgico medio fue de 32 minutos (rango, 15-55 minutos). Un paciente present&oacute; obstrucci&oacute;n de cat&eacute;ter en las primeras 24 horas tras la colocaci&oacute;n y requiri&oacute; revisi&oacute;n y recolocaci&oacute;n quir&uacute;rgica. No se produjeron otras complicaciones intraoperatorias o durante el postoperatorio inmediato. El seguimiento medio fue de 25 meses.</font></p>     <p><font face="Verdana" size="2">La estancia media fue de 1,02 &plusmn; 2,2 d&iacute;as. Aproximadamente dos tercios de los pacientes (65%) fueron dados de alta el mismo d&iacute;a de la intervenci&oacute;n, y hasta el 80% dentro de las primeras 24 horas postoperatorias. Los pacientes que permanecieron en el hospital pasadas 24 horas del procedimiento lo hicieron por problemas m&eacute;dicos no relacionados con el procedimiento.</font></p>     <p><font face="Verdana" size="2">No se han producido fugas de l&iacute;quido peritoneal ni infecciones de las heridas quir&uacute;rgicas durante el postoperatorio inmediato (&lt; 48 h). No se han producido extrusiones del <i>cuff</i> o eventraciones. La tasa de obstrucci&oacute;n del cat&eacute;ter fue del 7,8%, y la tasa de migraci&oacute;n del cat&eacute;ter, del 4% (2 pacientes). Uno de estos pacientes requiri&oacute; extracci&oacute;n del cat&eacute;ter debido a peritonitis grave. En el otro caso, el cat&eacute;ter no ha presentado obstrucci&oacute;n ni peritonitis, y actualmente funciona correctamente.</font></p>     <p><font face="Verdana" size="2">Un total de 3 pacientes fallecieron, a una media de 16 meses, tras la colocaci&oacute;n del cat&eacute;ter (2,35 muertes por cada 1.000 pacientes y mes de exposici&oacute;n). Las causas del fallecimiento fueron complicaciones cardiovasculares secundarias a la insuficiencia renal terminal. La mortalidad no estuvo relacionada con la DP o con el cat&eacute;ter.</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">La curva de supervivencia de los pacientes en programa de DP se muestra en el <a href="#f3">figura 3</a>, y la curva de supervivencia de los cat&eacute;teres, en el <a href="#f4">figura 4</a>. Un total de tres cat&eacute;teres (5,9%) tuvieron que ser retirados debido a peritonitis, todos ellos funcionando correctamente. En nuestra experiencia, hemos tenido 0,27 episodios de peritonitis por paciente y a&ntilde;o.</font></p>     <p>&nbsp;</p>     <p align="center"><font face="Verdana" size="2"><a name="f3"><img src="/img/revistas/nefrologia/v30n3/nota2_figura3.jpg"></a>    <br><b>Figura 3.</b> Curva KM que muestra la supervivencia de pacientes en diálisis peritoneal.</font></p>     <p align="center"><font face="Verdana" size="2"><a name="f4"><img src="/img/revistas/nefrologia/v30n3/nota2_figura4.jpg"></a>    <br><b>Figura 4.</b> Catéteres retirados o sustituidos debido a problemas técnicos    <br>(obstrucción, peritonitis o migración).</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2">Se retiraron dos cat&eacute;teres debido a complicaciones t&eacute;cnicas en el postoperatorio tard&iacute;o. En el primer caso, el paciente present&oacute; un cuadro de dolor abdominal que requiri&oacute; laparotom&iacute;a exploradora, y se advirti&oacute; dec&uacute;bito del cat&eacute;ter en un asa de intestino. Tras comprobar que el asa era viable, se retir&oacute; el cat&eacute;ter y se coloc&oacute; uno nuevo. El paciente sigue actualmente en programa de DP. El segundo paciente, 9 meses despu&eacute;s de la cirug&iacute;a, present&oacute; permeabilizaci&oacute;n del conducto peritoneo vaginal y apareci&oacute; un hidrocele. <body><![CDATA[<br>el inicio, tratamiento actual y estado del catéter en el momento de la exclusión    <br><img src="/img/revistas/nefrologia/v30n3/nota2_tabla1.jpg">    <br>TR: trasplante renal; AVC: accidente vascular cerebral.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Discusi&oacute;n</b></font></p>     <p><font face="Verdana" size="2">La DP es una opci&oacute;n segura y efectiva para el paciente con insuficiencia renal terminal. Adem&aacute;s, hay evidencia de una mejor preservaci&oacute;n de la funci&oacute;n renal residual al compararla con la hemodi&aacute;lisis<sup>27,28</sup>. A pesar de que la cirug&iacute;a abierta ha sido el m&eacute;todo de elecci&oacute;n, el abordaje laparosc&oacute;pico ha tenido amplia aceptaci&oacute;n<sup>29-31</sup>.</font></p>     <p><font face="Verdana" size="2">Nuestra t&eacute;cnica, descrita anteriormente, es un procedimiento sencillo a trav&eacute;s de dos tr&oacute;cares de 12 mm. Adem&aacute;s, es un procedimiento r&aacute;pido, con un tiempo quir&uacute;rgico corto. Respecto a las ventajas intraoperatorias, la laparoscopia permite una visi&oacute;n y una evaluaci&oacute;n de la cavidad peritoneal &oacute;ptimas, y permiten una precisa colocaci&oacute;n del cat&eacute;ter. Adem&aacute;s, la laparoscopia deja liberar adherencias peritoneales si es necesario.</font></p>     <p><font face="Verdana" size="2">El uso de una gu&iacute;a de Guyon ha sido de gran ayuda para la precisa colocaci&oacute;n del cat&eacute;ter, ya que su punta atraum&aacute;tica y su rigidez hacen posible tanto guiar el cat&eacute;ter hasta el fondo del saco como reposicionarlo si es necesario. La incidencia de obstrucci&oacute;n del cat&eacute;ter var&iacute;a en la literatura entre el 10 y el 22% en los procedimientos por cirug&iacute;a abierta con colocaci&oacute;n ciega. Por otro lado, la cirug&iacute;a laparosc&oacute;pica presenta tasas de obstrucci&oacute;n mucho menores, de entre el 4 y el 13%. En nuestra experiencia, la tasa de obstrucci&oacute;n fue del 3,9%. A pesar de esta baja tasa, debe tenerse en cuenta nuestro limitado seguimiento.</font></p>     <p><font face="Verdana" size="2">La tasa de fuga de l&iacute;quido peritoneal var&iacute;a entre el 2,6 y el 22%. En nuestra experiencia, no hemos tenido ning&uacute;n caso de fuga. Esta complicaci&oacute;n no s&oacute;lo se asocia a cirug&iacute;a abierta, sino tambi&eacute;n al abordaje laparosc&oacute;pico. La colocaci&oacute;n paramedial y la creaci&oacute;n de un t&uacute;nel subcut&aacute;neo largo son estrategias para tratar de disminuir esta complicaci&oacute;n<sup>32,33</sup> y podr&iacute;an explicar la ausencia de f&iacute;stula en nuestra serie.</font></p>     <p><font face="Verdana" size="2">Al analizar nuestra t&eacute;cnica en comparaci&oacute;n con otras t&eacute;cnicas laparosc&oacute;picas de tres puertos, nuestra experiencia es comparable en cuanto a tiempo quir&uacute;rgico, tiempo de ingreso y tasa de obstrucci&oacute;n del cat&eacute;ter<sup>30,34,35</sup>. Las tasas de f&iacute;stula de l&iacute;quido peritoneal con las t&eacute;cnicas de tres puertos oscilan entre el 0 y el 4,7%. Aceptando nuestro seguimiento limitado, nuestros resultados ser&iacute;an como m&iacute;nimo iguales<sup>25,30</sup>. No hemos tenido casos de infecci&oacute;n de las incisiones quir&uacute;rgicas. Podr&iacute;a aducirse que un tiempo quir&uacute;rgico corto es importante para limitar las infecciones de las heridas, pero otros centros con tiempos quir&uacute;rgicos similares reportan infecciones de los puertos de hasta el 21%<sup>35</sup>.</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">No hemos tenido peritonitis en el postoperatorio precoz (dos primeras semanas) tras el implante del cat&eacute;ter, pero s&iacute; un episodio de peritonitis por paciente cada 32,4 meses (0,27 episodios por paciente y a&ntilde;o), que es m&aacute;s bajo que lo recomendado en la literatura<sup>36</sup>.
