Existen datos contradictorios sobre si la prevalencia de malnutrición es mayor en los servicios médicos o quirúrgicos. Estudiamos la prevalencia de este trastorno en pacientes de ambos tipos de servicio. Métodos: Se evaluó el estado nutricional de 189 pacientes de servicios médicos y quirúrgicos mediante la Valoración Global Subjetiva, y se recogieron datos sobre su sexo, edad, estancia hospitalaria, mortalidad, patología y servicio de ingreso. Las variables cualitativas se compararon mediante chi-cuadrado, y las variables cuantitativas independientes mediante el test t de Student. Consideramos significativa una p < 0,05. Resultados: La prevalencia de malnutrición fue 40,2%. Los pacientes malnutridos habían perdido un 7,3% de peso, el 67,1% habían reducido su ingesta oral, y el 53,9% referían anorexia. La malnutrición se asoció significativamente al sexo masculino, a una estancia hospitalaria más larga, y a padecer enfermedades crónicas o cáncer. La prevalencia y gravedad de malnutrición fueron similares en servicios médicos y quirúrgicos, pero presentaron distintas patologías de base (enfermedades crónicas en los servicios médicos, intervenciones y cáncer en los quirúrgicos). Conclusiones: La malnutrición es igual de frecuente entre pacientes médicos y quirúrgico, probablemente debido a diferentes pero igualmente frecuentes factores de riesgo, y se asocia a una estancia hospitalaria más larga.]]></p></abstract>

Length of stay.</font></p> <hr size="1">     <p>&nbsp;</p>     <p><font face="Verdana"><b>Introducci&oacute;n</b></font></p>     <p><font face="Verdana" size="2">La malnutrici&oacute;n es un trastorno metab&oacute;lico causado por el desequilibrio entre la ingesta de nutrientes y las necesidades corporales. Es altamente prevalente en pa&iacute;ses subdesarrollados, causada por la carencia de alimentos en cantidad y calidad suficiente (malnutrici&oacute;n primaria). En las sociedades industrializadas la malnutrici&oacute;n normalmente est&aacute; relacionada con alguna enfermedad (malnutrici&oacute;n secundaria) y se encuentra con frecuencia en pacientes hospitalizados o institucionalizados. Hace treinta a&ntilde;os Bistrian y cols., publicaron los primeros trabajos sobre prevalencia de malnutrici&oacute;n hospitalaria, hall&aacute;ndola en el 45% de los pacientes m&eacute;dicos y el 54% de los quir&uacute;rgicos<sup>2, 3</sup>. M&aacute;s all&aacute; de su frecuencia, la importancia de la malnutrici&oacute;n reside en su asociaci&oacute;n a una peor calidad de vida<sup>4</sup>, al aumento de la morbi-mortalidad<sup>5, 6</sup>, a estancias hospitalarias m&aacute;s prolongadas<sup>7, 8</sup>, y al incremento de los costes sanitarios<sup>5, 9, 10</sup>. El despistaje y tratamiento precoz de la malnutrici&oacute;n es un modo coste-efectivo de mejorar estos par&aacute;metros<sup>11, 12</sup>.</font></p>     <p><font face="Verdana" size="2">Tanto los pacientes m&eacute;dicos como los quir&uacute;rgicos est&aacute;n expuestos a factores de riesgo de desarrollar malnutrici&oacute;n: estr&eacute;s metab&oacute;lico asociado a las enfermedades subyacentes, s&iacute;ntomas gastrointestinales, dificultades para cubrir las necesidades nutricionales, efectos de f&aacute;rmacos, etc. Los pacientes quir&uacute;rgicos presentan ciertas desventajas desde el punto de vista nutricional, como los periodos de ayuno pre y postoperatorios, la maldigesti&oacute;n y malabsorci&oacute;n tras cirug&iacute;as del aparato gastrointestinal, etc., por lo que es de esperar una prevalencia de malnutrici&oacute;n mayor en este tipo de enfermos como reflejan los datos de Bistrian. Sin embargo, otros estudios han hallado resultados contradictorios a este respecto<sup>13-29</sup>.</font></p>     <p><font face="Verdana" size="2">El objetivo de este estudio es determinar la prevalencia de malnutrici&oacute;n en un hospital universitario de tercer nivel, y comparar las tasas de malnutrici&oacute;n y las caracter&iacute;sticas de los pacientes malnutridos en servicios m&eacute;dicos y quir&uacute;rgicos.</font></p>     ]]></body>
<body><![CDATA[<p>&nbsp;</p>     <p><font face="Verdana"><b>Material y m&eacute;todos</b></font>     <p><font face="Verdana" size="2">Se dise&ntilde;&oacute; un estudio transversal de prevalencia en que se estudi&oacute; una muestra aleatoria de 189 pacientes (tama&ntilde;o muestral calculado para una seguridad del 90%, una precisi&oacute;n del 7% y una prevalencia estimada en torno al 40%) obtenida del censo del hospital. Los pacientes estaban ingresados en servicios m&eacute;dicos (Medicina Interna, Hematolog&iacute;a, Oncolog&iacute;a, Cardiolog&iacute;a, Neumolog&iacute;a, Reumatolog&iacute;a, Endocrinolog&iacute;a, Enfermedades Infecciosas, Neurolog&iacute;a y Enfermedades Infecciosas) y quir&uacute;rgicos (Cirug&iacute;a General, Ginecolog&iacute;a, Urolog&iacute;a, Neurocirug&iacute;a, Cirug&iacute;a Cardiovascular, Otorrinolaringolog&iacute;a, Cirug&iacute;a Tor&aacute;cica y Traumatolog&iacute;a). Los criterios de exclusi&oacute;n fueron ser menor de edad, embarazo, ingresar en la Unidad de Cuidados Intensivos, padecer un trastorno psiqui&aacute;trico mayor, o estar en situaci&oacute;n vital terminal.</font></p>     <p><font face="Verdana" size="2">El despistaje de malnutrici&oacute;n se realiz&oacute; mediante Valoraci&oacute;n Global Subjetiva (VGS), un cuestionario descrito por primera vez por Detsky y cols., en 1987<sup>30</sup>que recoge signos f&iacute;sicos (p&eacute;rdida de peso, disminuci&oacute;n de la masa muscular y grasa, edemas), reducciones de la ingesta oral, el deterioro de la capacidad funcional y el estr&eacute;s metab&oacute;lico producido por la enfermedad. Los pacientes se clasifican en tres categor&iacute;as: VGS-A para los bien nutridos, VGS-B para la malnutrici&oacute;n moderada o riesgo de malnutrici&oacute;n, y VGS-C para la malnutrici&oacute;n grave. La valoraci&oacute;n nutricional se realiz&oacute; en el momento en que se obtuvo la muestra aleatoria del censo, independientemente del tiempo que llevase ingresado el paciente.</font></p>     <p><font face="Verdana" size="2">Recogimos datos sobre la edad, sexo, enfermedad, mortalidad y estancia hospitalaria de los pacientes estudiados. Las distintas patolog&iacute;a se agruparon en 6 categor&iacute;as: enfermedades agudas (p. ej. infecciones, traumatismos y fracturas, pancreatitis aguda, angor, etc.), enfermedades cr&oacute;nicas (p. ej. EPOC, cirrosis hep&aacute;tica, insuficiencia card&iacute;aca, insuficiencia renal cr&oacute;nica, VIH, etc&eacute;tera), c&aacute;ncer, intervenci&oacute;n quir&uacute;rgica, trasplantes (de &oacute;rganos s&oacute;lidos y de m&eacute;dula &oacute;sea) y enfermedades neurol&oacute;gicas (p. ej. Ictus, demencia, etc.).</font></p>     <p><font face="Verdana" size="2">Las variables cuantitativas se expresan como media (y desviaci&oacute;n est&aacute;ndar) y se compararon mediante el test t de Student para variables independientes. Las variables cualitativas se expresan en porcentajes, y se compararon mediante el test chi-cuadrado. Consideramos estad&iacute;sticamente significativa una p &lt; 0,05.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana"><b>Resultados</b></font>     <p><font face="Verdana" size="2">Las caracter&iacute;sticas generales de los pacientes estudiados se resumen en la <a href="#t1"> tabla I</a>. La mayor&iacute;a de presentaban un estado nutricional normal (VGS-A 59,8%); el 24,9% estaban moderadamente malnutridos (VGS-B), y el 15,3% presentaban una malnutrici&oacute;n grave (VGSC). La prevalencia de malnutrici&oacute;n en la poblaci&oacute;n estudiada fue del 40,2%. Los pacientes malnutridos presentaban una p&eacute;rdida de peso del 7,3 (6,4%), y el 67,1% refer&iacute;an haber disminuido su ingesta en las &uacute;ltimas 4 semanas (expresado como mediana; rango 1-52 semanas). Los s&iacute;ntomas m&aacute;s frecuentemente descritos fueron anorexia (53,9%), n&aacute;useas (19,7%), diarrea (11,8%) y v&oacute;mitos (11,8%).</font></p>     <p align="center"><font face="Verdana" size="2"><a name="t1"><img src="/img/revistas/nh/v23n3/original9_t1.gif"></a></font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">La mayor&iacute;a de los pacientes malnutridos fueron varones (70,7% vs 29,3%; p &lt; 0,01), sin hallar diferencias significativas en cuanto a la edad respecto a los bien nutridos &#091;63 (17,7) vs 62,7 (19,3) a&ntilde;os; p = 0,63&#093;. La estancia hospitalaria fue m&aacute;s larga en los pacientes malnutridos &#091;12 (7,2) vs 8 ( 5,3) d&iacute;as; p &lt; 0,01), y la tasa de mortalidad fue similar entre malnutridos y bien nutridos (7,9% vs 3,6%; p = 0,26).</font></p>     <p><font face="Verdana" size="2">Las enfermedades agudas fueron m&aacute;s frecuentes entre los pacientes con un buen estado nutricional (48,1% vs 16%; p &lt; 0,01), y el c&aacute;ncer entre los malnutridos (24% vs 12%; p &lt; 0,05); no encontramos diferencias en la frecuencia de enfermedades cr&oacute;nicas (30,1% vs 37,3%; p = 0,054), enfermedades neurol&oacute;gicas (3,6% vs 5,3%; p = 0,33), intervenciones quir&uacute;rgicas (6% vs 12%; p = 0,12) o trasplantes (6% vs 4%; p =0,76) entre los bien y malnutridos.</font></p>     <p><font face="Verdana" size="2">Las caracter&iacute;sticas generales de los pacientes m&eacute;dicos y quir&uacute;rgicos se comparan en la <a href="#t2"> tabla II</a>. El estado nutricional no difiri&oacute; entre ambos grupos: VGS-A 59,4% vs 59,6% (p = 0,97); VGS-B 26% vs 23,4% (p = 0,83); VGS-C 14,6% vs 16% (p = 0,92), respectivamente. Entre los pacientes malnutridos de ambos grupos no encontramos diferencias significativas respecto a la edad (62,2 &plusmn; 22 vs 63,9 &plusmn; 13,4 a&ntilde;os; p = 0,70) ni el sexo (varones 54,7% vs 63,05%; p = 0,31). La tasa de mortalidad (7,7% vs 5,4%; p = 0,95) y la estancia hospitalaria &#091;10,7 (8,4) vs 10,9 (9,5) d&iacute;as; p = 0,91) fueron similares. La comparaci&oacute;n de s&iacute;ntomas, signos y enfermedades se resume en la <a href="#t3"> tabla III</a>.</font></p>     <p align="center"><font face="Verdana" size="2"><a name="t2"><img src="/img/revistas/nh/v23n3/original9_t2.gif"></a></font></p>     <p align="center"><font face="Verdana" size="2"><a name="t3"><img src="/img/revistas/nh/v23n3/original9_t3.gif"></a></font></p>     <p>&nbsp;    <p><font face="Verdana"><b>Discusi&oacute;n</b></font>     <p><font face="Verdana" size="2">Presentamos un estudio sobre la prevalencia de malnutrici&oacute;n en un hospital universitario de m&aacute;s de 1.300 camas. Hallamos una elevada prevalencia (40,2%), similar a la reflejada en otros estudios (30-50%)<sup>2, 3, 13-29</sup>. Parte de la variaci&oacute;n en esta tasa que se observa entre estudios puede explicarse por causas metodol&oacute;gicas y a diferencias en las caracter&iacute;sticas de diferentes hospitales. Se han desarrollado decenas de herramientas para el cribaje de malnutrici&oacute;n con diferentes sensibilidades y especificidades y que tampoco son igualmente aplicables a todos los pacientes<sup>31-34</sup>, lo que explica que la prevalencia var&iacute;e en funci&oacute;n del m&eacute;todo empleado. La VGS fue una de las primeras herramientas desarrolladas, y actualmente se considera el <i>gold standard</i> con el que se comparan y estandarizan los otros m&eacute;todos de cribaje; es r&aacute;pida, f&aacute;cil, reproducible y barata, y tiene la capacidad de predecir el desarrollo de complicaciones y la mortalidad<sup>35</sup>, por lo que consideramos que es la herramienta m&aacute;s adecuada para el cribaje de malnutrici&oacute;n en los centros hospitalarios, si bien por su car&aacute;cter subjetivo requiere de cierta experiencia por parte del cl&iacute;nico. Por otro lado, los hospitales de tercer nivel suelen acoger a pacientes m&aacute;s complejos, con enfermedades m&aacute;s graves, y con mayor frecuencia se emplean terapias y cirug&iacute;as m&aacute;s agresivas; todos estos factores predisponen al desarrollo de malnutrici&oacute;n y pueden explicar que este tipo de centros su prevalencia sea mayor.</font></p>     <p><font face="Verdana" size="2">Las caracter&iacute;sticas principales de los pacientes malnutridos de nuestro centro son el sexo masculino, padecer un c&aacute;ncer y presentar anorexia. Estos tres factores est&aacute;n relacionados, en cuanto que las enfermedades oncol&oacute;gicas son m&aacute;s frecuentes en varones, y la anorexia es un s&iacute;ntoma frecuente de la caquexia tumoral. La estancia hospitalaria fue mayor en los pacientes malnutridos, como era de esperar por la asociaci&oacute;n de malnutrici&oacute;n y complicaciones. La estancia hospitalaria fue similar en los malnutridos m&eacute;dicos y quir&uacute;rgicos, probablemente debido a que ambos grupos presentaron similares tasas y gravedad de malnutrici&oacute;n. Hallamos una tendencia a presentar mayor mortalidad entre los pacientes malnutridos respecto a los bien nutridos, aunque sin significaci&oacute;n estad&iacute;stica, probablemente por carecer nuestro estudio de la potencia estad&iacute;stica suficiente. El tama&ntilde;o muestral se calcul&oacute; teniendo en cuenta la prevalencia de malnutrici&oacute;n esperada, pero no una estimaci&oacute;n de la mortalidad en estos pacientes.</font></p>     <p><font face="Verdana" size="2">La prevalencia de malnutrici&oacute;n fue similar en los servicios m&eacute;dicos y quir&uacute;rgicos, sin diferencias en cuanto a la gravedad de la misma. Esper&aacute;bamos encontrar una mayor prevalencia entre los pacientes quir&uacute;rgicos. Los pacientes m&eacute;dicos y quir&uacute;rgicos de nuestro centro difieren en la edad (mayores en servicios m&eacute;dicos) y en las enfermedades de base (agudas, cr&oacute;nicas y neurol&oacute;gicas m&aacute;s frecuentes en servicios m&eacute;dicos; c&aacute;ncer e intervenciones en cirug&iacute;a), pero los pacientes malnutridos s&oacute;lo se diferencian en su patolog&iacute;a. Las enfermedades cr&oacute;nicas predominan en los pacientes m&eacute;dicos malnutridos (53,8%), y el c&aacute;ncer y las intervenciones quir&uacute;rgicas entre los quir&uacute;rgicos tambi&eacute;n malnutridos (64,8%). Todas estas patolog&iacute;as son causas bien conocidas de malnutrici&oacute;n de modo que, aunque en los pacientes m&eacute;dicos y quir&uacute;rgicos son diferentes los factores de riesgo de malnutrici&oacute;n a los que est&aacute;n expuestos, &eacute;stos son igualmente frecuentes, y esto puede explicar las prevalencias similares que hemos hallado.</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">Desde que Bistrian public&oacute; sus estudios se han desarrollado m&eacute;todos m&aacute;s eficaces para el diagn&oacute;stico y el tratamiento de la malnutrici&oacute;n, y sin embargo su prevalencia no ha disminuido. El personal sanitario normalmente desconoce la importancia de una detecci&oacute;n precoz de este problema y no realiza el despistaje de malnutrici&oacute;n, lo que impide el desarrollo de planes de soporte nutricional<sup>36, 37</sup>. El aumento de la prevalencia del c&aacute;ncer y de las enfermedades cr&oacute;nicas, el envejecimiento de la poblaci&oacute;n, y el desarrollo de nuevas terapias que prolongan la vida en enfermedades graves exponen a m&aacute;s pacientes al riesgo de padecer malnutrici&oacute;n.</font></p>     <p><font face="Verdana" size="2">El presente estudio demuestra que la prevalencia de malnutrici&oacute;n puede ser muy elevada en los hospitales de tercer nivel, que es igualmente frecuente en los servicios m&eacute;dicos y quir&uacute;rgicos, y que se asocia a una estancia hospitalaria m&aacute;s larga.
