El objetivo del presente estudio es validar un cuestionario de frecuencia de consumo de alimentos aplicado a la estimación de la relación entre los hábitos alimentarios y la densidad mineral ósea de una población adulta sana. Métodos: Los resultados obtenidos mediante el cuestionario de frecuencia de consumo de alimentos se compararon con los de recordatorios de 24 horas. Las medidas de la densidad mineral ósea se realizaron mediante densitometría de calcáneo. Resultados: Se demuestra la validez del cuestionario al obtener coeficientes de correlación de Spearman entre 0,014 y 0,467. Asimismo el test de Bland-Altman muestra que no existe variación entre los dos métodos para las variables analizadas. El análisis de correlación muestra que la densidad mineral ósea está asociada significativamente al consumo de vitamina D, vitamina A, vitamina B12, folato, tiamina y hierro. El consumo de lípidos totales no fue asociado con la densidad mineral ósea, sin embargo la ingesta de ácidos grasos monoinsaturados, EPA y DHA y colesterol muestra una correlación estadísticamente significativa. Conclusión: El cuestionario estima el consumo de energía y nutrientes con adecuada validez. Su aplicación nos ha permitido deducir la importancia de una dieta rica en vitaminas del grupo B, vitamina D, calcio, hierro y ácidos grasos monoinsaturados y n-3 en la salud ósea.]]></p></abstract>

The objective of the present study was to validate a food frequency questionnaire applied to estimate the relationship between food habits and bone mineral density (BMD) in a healthy adult population. Methods: The results of the food frequency questionnaire were compared with 24-hr recall findings. Calcaneus BMD was measured by densitometry. Results: The validity of the questionnaire was demonstrated, with Spearman correlation coefficients of 0.014 to 0.467. The Bland-Altman test also found no differences in study variables between the two methods. Correlation analysis showed that the BMD was significantly associated with the intake of vitamin D, vitamin A, vitamin B12, folate, thiamine and iron. Total fat consumption was not associated with BMD but the intake of monounsaturated fatty acids, EPA, DHA and cholesterol showed a significant correlation. Conclusion: The questionnaire evaluates the consumption of energy and nutrients with adequate validity. Foods habits.</font></p> <hr size="1">     <p>&nbsp;</p>     <p><font size="2" face="Verdana"><b>Introducci&oacute;n</b></font></p>     <p><font size="2" face="Verdana">La osteoporosis ha sido considerada por la Organizaci&oacute;n Mundial de la salud (OMS) como el segundo problema sanitario asistencial en el mundo despu&eacute;s de las enfermedades cardiovasculares. El estudio de esta enfermedad, caracterizada por la p&eacute;rdida de masa y densidad de los huesos, ha despertando un particular inter&eacute;s por la elevada magnitud del problema a nivel poblacional<sup>1-2</sup>. Su importancia para la persona que la padece recae en el gran riesgo de fracturas y las graves consecuencias que acarrean<sup>3</sup>.</font></p>     <p><font size="2" face="Verdana">Existen m&uacute;ltiples factores relacionados con el desarrollo de osteoporosis. El modelado y remodelado &oacute;seo presentan unas caracter&iacute;sticas individuales en funci&oacute;n de la raza y la herencia. Est&aacute;n condicionados por una serie de factores nutricionales, mec&aacute;nicos y hormonales<sup>4-7</sup>. La incidencia sobre estos factores condicionantes puede predisponer a la p&eacute;rdida o a la mejora y/o al mantenimiento, en funci&oacute;n de la etapa de la vida, de la calidad y cantidad de hueso.</font></p>     <p><font size="2" face="Verdana">Los factores nutricionales tienen m&uacute;ltiples efectos, actuando sobre el pico de masa &oacute;sea, la p&eacute;rdida &oacute;sea relacionada con la edad y la fortaleza muscular<sup>8</sup>. Por lo tanto, no podemos olvidar la importancia de mantener h&aacute;bitos de alimentaci&oacute;n saludables para la consecuci&oacute;n del objetivo de salud &oacute;sea<sup>9</sup>. Aunque los principales nutrientes de inter&eacute;s son el calcio y la vitamina D, dada su importancia en la optimizaci&oacute;n del pico de masa &oacute;sea<sup>10</sup>; sin embargo, en un informe realizado por la Uni&oacute;n Europea se reconoc&iacute;a la importancia de otros nutrientes y se instaba a incrementar la investigaci&oacute;n al respecto, desarroll&aacute;ndose diversos proyectos de investigaci&oacute;n europeos a este respecto<sup>10-11</sup>.</font></p>     ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana">Uno de los m&eacute;todos m&aacute;s com&uacute;nmente utilizados para investigar la asociaci&oacute;n entre dieta y salud &oacute;sea es el Cuestionario de Frecuencia de Consumo de Alimentos (FCA) debido a que permite obtener informaci&oacute;n del modelo de consumo habitual a largo plazo en poblaciones grandes. Adem&aacute;s de tratarse de un m&eacute;todo relativamente barato, r&aacute;pido y f&aacute;cil de aplicar<sup>12-13</sup>. En Espa&ntilde;a se han realizado pocos estudios de este tipo<sup>14</sup> y ser&iacute;a de gran utilidad disponer de cuestionarios validados adaptados a regiones geogr&aacute;ficas espec&iacute;ficas<sup>13</sup>.</font></p>     <p><font size="2" face="Verdana">Por todas las razones anteriormente expuestas y por el enorme inter&eacute;s que este tema presenta en la actualidad, nos planteamos el presente estudio con el objetivo de validar un cuestionario de frecuencia de consumo de alimentos que estime la relaci&oacute;n entre los h&aacute;bitos alimentarios y la densidad mineral &oacute;sea de una poblaci&oacute;n adulta sana. Para ello se han comparado sus resultados con los obtenidos por recordatorios de 24 horas considerado como uno de los m&eacute;todos de referencia. Asimismo aplicando este cuestionario se ha estudiado la relaci&oacute;n entre h&aacute;bitos alimentarios y densidad mineral &oacute;sea en una muestra de 91 mujeres sanas de la provincia de Granada.</font></p>     <p>&nbsp;</p>     <p><font size="2" face="Verdana"><b>Material y m&eacute;todos</b></font></p>     <p><font size="2" face="Verdana"><i>Sujetos</i></font></p>     <p><font size="2" face="Verdana">La muestra est&aacute; constituida por un grupo de 91 mujeres de edades comprendidas entre 18 y 65 a&ntilde;os. Todos los sujetos seleccionados son participantes en las actividades de las Escuelas de Nataci&oacute;n de la Facultad de Ciencias de la Actividad F&iacute;sica y el Deporte de la Universidad de Granada. Las mujeres participantes no presentaron ninguna patolog&iacute;a cardiaca, metab&oacute;lica o desorden endocrino. A su vez se excluyeron del estudio aquellos sujetos que estuvieran recibiendo medicaci&oacute;n que pudiera afectar al metabolismo del calcio y a la homeostasis del hueso.</font></p>     <p><font size="2" face="Verdana"><i>Cuestionario de Frecuencia de Consumo de Alimentos (FCA)</i></font></p>     <p><font size="2" face="Verdana">El cuestionario de frecuencia de consumo de alimentos utilizado fue semicuantitativo e incluy&oacute;, una lista cerrada de 24 items, en los que se pregunt&oacute;, por entrevistadores entrenados con qu&eacute; frecuencia (diaria, semanal, mensual o anual) y en que cantidad se consum&iacute;an los alimentos incluidos. El listado de alimentos fue creado partiendo de datos de CFA realizados a poblaci&oacute;n femenina del mismo &aacute;rea geogr&aacute;fica en varios proyectos realizados por integrantes del grupo de investigaci&oacute;n<sup>15-16</sup>.</font></p>     <p><font size="2" face="Verdana"><i>Recordatorio de 24 horas (R24H)</i></font></p>     <p><font size="2" face="Verdana">Se llevaron a t&eacute;rmino 3 recordatorios de 24 horas. Para evitar influencias estacionales, se realiz&oacute; uno de los recordatorios en el per&iacute;odo de octubre a diciembre, un segundo durante los meses de febrero y marzo, y el &uacute;ltimo en el periodo de mayo a julio. El R24H fue completado por los sujetos con la ayuda de un entrevistador, previamente entrenado.</font></p>     ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana">Para la estimaci&oacute;n de la ingesta de energ&iacute;a y nutrientes, de ambos cuestionarios, se utiliz&oacute; el programa inform&aacute;tico DIETSOURCE versi&oacute;n 1.2<sup>15-16</sup>.</font></p>     <p><font size="2" face="Verdana"><i>Medidas de la Densidad Mineral &Oacute;sea (DMO)</i></font></p>     <p><font size="2" face="Verdana">Las medidas de la densidad mineral &oacute;sea se realizaron por un especialista en Reumatolog&iacute;a mediante densitometr&iacute;a de calc&aacute;neo, realizada con un densit&oacute;metro port&aacute;til Hologic de absorciometria dual de rayos x (DEXA) que mide la densidad mineral &oacute;sea del &aacute;rea microrradiografiada del tal&oacute;n expresada en grs/cm<sup>2</sup>. La comparativa fue expresada en desviaciones st&aacute;ndard con respecto a la media poblacional en relaci&oacute;n a la edad y al pico de masa &oacute;sea (Z-score y T-score) respectivamente.</font></p>     <p>&nbsp;</p>     <p><font size="2" face="Verdana"><b>An&aacute;lisis estad&iacute;stico</b></font></p>     <p><font size="2" face="Verdana">Los datos fueron analizados mediante el programa SPSS, versi&oacute;n 15.0. Para evaluar el nivel de correlaci&oacute;n entre ambas encuestas diet&eacute;ticas se calcularon los coeficientes de correlaci&oacute;n de Spearman. Asimismo se realiz&oacute; el an&aacute;lisis de Bland-Altman<sup>17</sup> para valorar el nivel de acuerdo entre ambos cuestionarios, al ser el m&aacute;s utilizado en los estudios recientes para validar los cuestionarios de FCA<sup>18,19</sup>. Los l&iacute;mites de concordancia de Bland-Altman se calculan sum&aacute;ndole y rest&aacute;ndole 2 veces la desviaci&oacute;n est&aacute;ndar a la media de las diferencias entre ambos m&eacute;todos. El nivel de significaci&oacute;n estad&iacute;stica que se emple&oacute; en todos los casos fue de p &lt; 0.05.</font></p>     <p>&nbsp;</p>     <p><font size="2" face="Verdana"><b>Resultados</b></font></p>     <p><font size="2" face="Verdana">En la <a href="#t1">tabla I</a> se presentan caracter&iacute;sticas y datos correspondientes a las medidas de densidad mineral &oacute;sea de la poblaci&oacute;n de estudio. La edad media de las mujeres participantes es de 41,95 a&ntilde;os, presentando un &iacute;ndice de masa corporal de 26,73 (&plusmn; 4,53). La media de DMO de la poblaci&oacute;n de estudio fue de 0,51 &plusmn; 0,09. La  <a href="#t2">tabla II</a> muestra el porcentaje de ingesta de nutrientes respecto a las recomendaciones. La ingesta cal&oacute;rica de la poblaci&oacute;n es inferior a la recomendada, presentando un valor medio del 85%. Los sujetos estudiados tienen un consumo de prote&iacute;nas y l&iacute;pidos que superan como media el 35,63% y el 34,63% de los objetivos diet&eacute;ticos. Por el contrario solo se alcanza el 70% del porcentaje de carbohidratos recomendado (50-60%). La media del porcentaje de consumo de Ca y vitamina D por la poblaci&oacute;n se aproxima a las recomendaciones en ambos casos (99,17% y 97,54%, respectivamente). Por el contrario la ingesta recomendada de f&oacute;sforo (700 mg) es superada en un 91%.</font></p>     <p align="center"><font size="2" face="Verdana"><a name="t1"><img src="/img/revistas/nh/v24n5/original1_t1.gif" align="top"></a></font></p>     ]]></body>
<body><![CDATA[<p align="center"><font size="2" face="Verdana"><a name="t2"><img src="/img/revistas/nh/v24n5/original1_t2.gif" align="top"></a></font></p>     <p><font size="2" face="Verdana">En la <a href="#t3">tabla III</a> se presentan los resultados del calculo de la mediana y la amplitud interquartil de la ingesta de nutrientes relacionados con la DMO utilizando los cuestionarios de FCA y R24H. En general, la ingesta es superior cuando se calcula utilizando el R24H, aunque los coeficientes de correlaci&oacute;n obtenidos demuestran que ambos m&eacute;todos son comparables (p &le; 0,05, <a href="#t3">tabla III</a>). Se han observado mejor ajuste en los valores de correlaci&oacute;n para la energ&iacute;a, y los macronutrientes, siendo los coeficientes m&aacute;s bajos los correspondientes al selenio, la riboflavina y la cianocobalamina, cuyas correlaciones no fueron significativas. Asimismo, el test de Blan-Atlman muestra que no existen variaciones significativas entre ambos cuestionarios (<a href="#t3">tabla III</a> y <a href="#f1">fig. 1</a>).</font></p>     <p align="center"><font size="2" face="Verdana"><a name="t3"><img src="/img/revistas/nh/v24n5/original1_t3.gif" align="top"></a></font></p>     <p align="center">&nbsp;</p>     <p align="center"><font size="2" face="Verdana"><a name="f1"><img src="/img/revistas/nh/v24n5/original1_f1.gif" align="top"></a></font></p>     <p><font size="2" face="Verdana">Se han seleccionado para los gr&aacute;ficos del test de Bland-Altman aquellos nutrientes que han demostrado en numerosos estudios un efecto en la masa &oacute;sea. Como se observa no existe variaci&oacute;n entre los dos m&eacute;todos para dichos nutrientes, es decir el FCA no estima valores de nutrientes superiores o inferiores que el R24H.</font></p>     <p><font size="2" face="Verdana">En la <a href="#t4">tabla IV</a> se muestra el resultado del estudio de la relaci&oacute;n existente entre la ingesta de nutrientes y la densidad mineral &oacute;sea. El an&aacute;lisis de correlaci&oacute;n muestra que la DMO est&aacute; asociada significativamente al consumo de vitamina D, vitamina A, vitamina B<sub>12</sub>, folato y tiamina. Asimismo las ingestas de calcio y hierro han sido significativamente correlacionadas con la DMO. <body><![CDATA[<p><font size="2" face="Verdana">En el presente estudio se ha validado una encuesta de FCA para el an&aacute;lisis de la relaci&oacute;n entre los h&aacute;bitos nutricionales de una poblaci&oacute;n adulta y su salud &oacute;sea compar&aacute;ndolo con una encuesta de R24H. Este cuestionario se ha aplicado a 91 mujeres adultas de la provincia de Granada que comprend&iacute;a un amplio rango de edad (18 a 65 a&ntilde;os).</font></p>     <p><font size="2" face="Verdana">El n&uacute;mero de muestra es suficiente para la validaci&oacute;n de un cuestionario, teniendo en cuenta que numerosos estudios han utilizado un n&uacute;mero similar de individuos para la validaci&oacute;n de cuestionarios obteni&eacute;ndose resultados aceptables<sup>12,13,20-22</sup>. Asimismo la utilizaci&oacute;n del m&eacute;todo de R24H para validar el FCA ha sido utilizado en numerosos trabajos<sup>23,24</sup>. La administraci&oacute;n del R24H en diferentes momentos del a&ntilde;o elimin&oacute; los posibles errores correspondientes a las variabilidades estacionales.</font></p>     <p><font size="2" face="Verdana">La validez del FCA para determinar la exactitud de la ingesta alimentaria ha sido demostrada obteni&eacute;ndose coeficientes de correlaci&oacute;n de Spearman entre 0,014 y 0,467, siendo significativas para todos los nutrientes con la excepci&oacute;n del selenio, riboflavina y cianocobalamina. Al igual que en otros estudios<sup>13</sup> los mayores coeficientes de correlaci&oacute;n fueron para la energ&iacute;a y los macronutrientes. Asimismo el test de Bland-Altman muestra que no existe variaci&oacute;n entre los dos m&eacute;todos para los nutrientes investigados.</font></p>     <p><font size="2" face="Verdana">Los resultados del an&aacute;lisis de la relaci&oacute;n existente entre la ingesta de nutrientes y la densidad mineral &oacute;sea nos ha llevado a encontrar asociaciones estad&iacute;sticamente significativas entre ambos en la poblaci&oacute;n estudiada. Entre los nutrientes m&aacute;s conocidos por su relaci&oacute;n con la masa &oacute;sea se encuentran el calcio y la vitamina D. La ingesta de calcio es considerada como un factor decisivo en la formaci&oacute;n &oacute;sea<sup>25</sup> debido a que una ingesta deficiente puede afectar adversamente a la p&eacute;rdida de hueso relacionada con la edad<sup>26-27</sup>. Asimismo la vitamina D es esencial para la mineralizaci&oacute;n &oacute;sea, ya que mantiene las adecuadas cantidades de Ca y P en el suero y en los espacios extracelulares<sup>14</sup>. En nuestro estudio la ingesta de ambos nutrientes se asocia de manera significativa a la densidad mineral &oacute;sea.</font></p>     <p><font size="2" face="Verdana">En varios estudios recientes se ha encontrado una asociaci&oacute;n positiva entre la DMO y los niveles plasm&aacute;ticos  de vitamina B<sub>12</sub> y folatos<sup>28-32</sup>. Asimismo, se ha demostrado que niveles bajos de &aacute;cido f&oacute;lico en plasma incrementan el riesgo de fractura<sup>32,33</sup>. Nuestro trabajo confirma estos resultados al encontrar una asociaci&oacute;n estad&iacute;sticamente significativa entre la DMO y la ingesta de folatos y vitamina B<sub>12</sub>. El efecto de la vitamina B<sub>12</sub> y el &aacute;cido f&oacute;lico en el hueso puede ser directo<sup>34</sup> o indirecto, habi&eacute;ndose demostrado que la deficiencia de &aacute;cido f&oacute;lico y vitamina B<sub>12</sub> incrementa los niveles plasm&aacute;ticos de homociste&iacute;na, lo que origina una aparici&oacute;n temprana de la osteoporosis y un incremento en el riesgo de fracturas relacionadas con la edad<sup>32,34,35</sup>.</font></p>     <p><font size="2" face="Verdana">El hierro es fundamental para todas las c&eacute;lulas incluidos los osteoblastos, relacionados con la formaci&oacute;n &oacute;sea<sup>36</sup>. La ingesta diet&eacute;tica de hierro se ha asociado a un incremento en la densidad mineral &oacute;sea en mujeres postmenop&aacute;usicas<sup>36-38</sup>. Asimismo en estudios realizados en ratas se ha observado una disminuci&oacute;n en la DMO en animales con deficiencia en hierro<sup>39-41</sup>. Nuestro estudio se&ntilde;ala que la ingesta de hierro puede ser un factor importante en la mineralizaci&oacute;n &oacute;sea al encontrar significaci&oacute;n estad&iacute;stica en la correlaci&oacute;n entre dicha ingesta y la DMO en mujeres sanas.</font></p>     <p><font size="2" face="Verdana">Algunas publicaciones sugieren que la cantidad y el tipo de grasa de la dieta pueden tener importantes efectos en la salud &oacute;sea<sup>42</sup>. Estudios <i>in vivo</i> han demostrado que la ingesta de &aacute;cidos grasos polinsaturados n-3, pueden influenciar la formaci&oacute;n y reabsorci&oacute;n del hueso<sup>43-45</sup>. Asimismo en estudios realizados en humanos se ha comprobado que la concentraci&oacute;n de &aacute;cidos grasos n-3, especialmente DHA, est&aacute; positivamente asociada con la DMO<sup>42,46</sup>. Nuestros datos apoyan esta hip&oacute;tesis al encontrar una asociaci&oacute;n significativa entre la ingesta de EPA y DHA y la DMO.</font></p>     <p><font size="2" face="Verdana">Con respecto a los monoinsaturados, un estudio realizado en Grecia<sup>47</sup> ha mostrado la existencia de una asociaci&oacute;n positiva, tanto en hombres como en mujeres adultas, entre la ingesta de grasa monoinsaturada y la densidad mineral del hueso. Los autores subrayan que las fracturas de cadera son substancialmente menores en Grecia que en USA o los pa&iacute;ses del norte de Europa. Asimismo, en nuestro trabajo el consumo de monoinsaturados se asocia significativamente con la masa &oacute;sea.</font></p>     <p><font size="2" face="Verdana">En conclusi&oacute;n, este trabajo valida un cuestionario de FCA que pretende valorar la relaci&oacute;n entre h&aacute;bitos alimentarios y masa &oacute;sea. La aplicaci&oacute;n de dicho cuestionario a una poblaci&oacute;n de mujeres sanas nos ha permitido deducir la importancia de una dieta rica en vitaminasdel grupo B, vitamina D, calcio, hierro y &aacute;cidos grasos monoinsaturados y n-3 en la salud &oacute;sea.</font></p>     <p>&nbsp;</p>     ]]></body>
