Metodología: Realizamos una revisión no sistemática con el objetivo de evaluar la relación entre el estrés oxidativo y la diabetes, y los posibles efectos de los antioxidantes en la prevención y tratamiento de la diabetes y sus complicaciones. Resultados: Los estudios de intervención con diferentes combinaciones de antioxidantes no han demostrado un efecto beneficioso sobre la morbimortalidad cardiovascular y global en diferentes poblaciones, incluidos los pacientes con diabetes mellitus. Tampoco en estos estudios se ha demostrado un efecto beneficioso de estas sustancias en la prevención de la diabetes. La evidencia científica actual apoya que estas sustancias pueden disminuir la peroxidación lipídica, la oxidación de las partículas de LDL-colesterol y mejorar la función endotelial y la vasodilatación dependiente del endotelio, sin mejorar de forma significativa el control metabólico de estos pacientes. <abstract abstract-type="short" xml:lang="en"><p><![CDATA[Introduction: An increase in the oxidative stress and a decrease in the antioxidant levels have been described in diabetic patients, that have been related with the etiopathogenesis of diabetes and its chronic complications. Methods: We performed a non-systematic review to evaluate the relationship between oxidative stress and diabetes, and the possible effects of antioxidants in the prevention and treatment of diabetes and its complications. Results: The intervention studies including different antioxidants have not demonstrated any beneficial effect on cardiovascular and global morbimortality in different populations, including diabetic patients. Neither of these studies has demonstrated a beneficial effect of antioxidant supplementation on the prevention of diabetes. According to these studies, these substances can decrease lipid peroxidation, LDL-cholesterol particles oxidation and improve endothelial function and endothelial-dependent vasodilatation, without significant improvement in the metabolic control of these patients. Conclusions: The current evidence does not support the use of high doses of antioxidants on the prevention and treatment of diabetes and its complications.]]></p></abstract>

<body><![CDATA[<p><font face="Verdana" size="2"><b>Introducción</b></font></p>     <p><font face="Verdana" size="2">Los seres vivos aerobios necesitan oxígeno para generar energía. El oxígeno puede reaccionar con otros elementos químicos, produciendo radicales libres. Estas sustancias son especies químicas que contienen uno o más electrones desapareados en su órbita externa. Son inestables y muy reactivas, ya que una vez formadas pueden captar un electrón de otras moléculas cercanas. El organismo dispone de un sistema de defensa antioxidante capaz de eliminar estos radicales libres, una vez formados. Estos incluyen enzimas como las superóxido dismutasas, las glutatión peroxidasas, la glutatión reductasa, la catalasa, las fosfolipasas y la poliADPribosa sintetasa, entre otras. Además, dispone de diferentes sustancias no enzimáticas como glutatión, taurina, coenzima Q, vitamina C, E y carotenoides.</font></p>     <p><font face="Verdana" size="2">En condiciones fisiológicas, existe un equilibrio entre la generación y degradación de radicales libres. Cuando este equilibrio se rompe, bien por producción de radicales en exceso, bien por disminución de los sistemas de defensa, se origina lo que se conoce como daño oxidativo. Este daño se debe a la capacidad que tienen los radicales libres de actuar sobre las proteínas, carbohidratos, lípidos y ácidos nucleicos de la célula. Cuando los radicales libres interactúan con estos componentes celulares, se originan alteraciones estructurales y funcionales. Como consecuencia de ello, se produce un deterioro de la homeostasis de la célula y la aparición de diferentes enfermedades crónicas, e incluso la muerte celular.</font></p>     <p><font face="Verdana" size="2">El estrés oxidativo se ha implicado en la patogénesis de la diabetes mellitus. El aumento de los radicales libres empeora la acción de la insulina a nivel periférico, contribuye a la disfunción de la célula beta pancreática<sup>1,2</sup> y está implicado en el desarrollo de las complicaciones crónicas<sup>3-6</sup>. En pacientes diabéticos existe un desequilibrio entre los mecanismos antioxidantes y oxidantes. Se ha demostrado una disminución de los niveles plasmáticos de enzimas antioxidantes, de glutatión y de vitaminas antioxidantes. Por otro lado, existe evidencia de un aumento de la peroxidación lipídica mediada por radicales libres en estos enfermos.</font></p>     <p><font face="Verdana" size="2">Los antioxidantes de la dieta juegan un papel importante en la defensa frente al envejecimiento y frente a las enfermedades crónicas como la diabetes mellitus, el cáncer y la enfermedad cardiovascular. Estas sustancias inactivan los radicales libres implicados en el estrés oxidativo e impiden su propagación. La suplementación con antioxidantes naturales podría tener un efecto beneficioso por mejorar la morbimortalidad de los pacientes diabéticos<sup>7,8</sup>, de tal forma que podrían prevenir y retrasar el desarrollo de las complicaciones crónicas de la diabetes<sup>9</sup>. Sin embargo, los resultados obtenidos con estas sustancias en los diferentes estudios de intervención han sido controvertidos. El objetivo de esta revisión ha sido analizar la evidencia científica disponible en relación al papel de diversos antioxidantes (vitamina E, vitamina C, betacaroteno, selenio, entre otros) en la diabetes mellitus.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Metodología</b></font></p>     <p><font face="Verdana" size="2">Se planteó una revisión no sistemática con el objetivo de evaluar la relación entre el estrés oxidativo y la diabetes, y los posibles efectos de los antioxidantes en la prevención y tratamiento de la diabetes y sus complicaciones.</font></p>     <p><font face="Verdana" size="2">La búsqueda bibliográfica se realizó en las bases Medline (PubMed) y Central (Cochrane Library). La estrategia de búsqueda de artículos sobre la relación entre estrés oxidativo y diabetes fue la siguiente: "Oxidative stress" AND "Diabetes Mellitus", y se obtuvieron 2.555 referencias. La estrategia de búsqueda sobre antioxidantes y estrés oxidativo en diabetes fue "Antioxidants" OR "Flavonoids" OR "Carotenoids" OR "Ubiquinone" OR "Selenium" OR "Zinc" OR "Chromium" OR "Glycine" OR "Glutamine" OR "Vitamin E" OR "Ascorbic Acid" AND "Diabetes Mellitus." La búsqueda inicial obtuvo 7.393 artículos en Medline y 24 en la Cochrane Library.</font></p>     <p><font face="Verdana" size="2">Se realizaron dos procesos de filtrado, el primero automático, seguido de otro segundo manual mediante la lectura de la bibliografía obtenida. Se seleccionaron los artículos según los siguientes criterios: tipo de estudio (estudios clínicos, ensayos aleatorizados y controlados, estudios de casos y controles), otros tipos de publicación (metanálisis, guías de práctica clínica, revisión), sujetos (humanos, mayores de edad), idiomas (inglés, español) y tipo de revista (core clinical journals). El número de artículos tras el filtro automático fue de 48 y 41 respectivamente. Se completó el proceso con una búsqueda en racimo a partir de la bibliografía de las publicaciones halladas. El número final de artículos incluidos fue de 74.</font></p>     ]]></body>
<body><![CDATA[<p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Estudios realizados con antioxidantes en pacientes diabéticos</b></font></p>     <p><font face="Verdana" size="2">La mayoría de los estudios con antioxidantes se han realizado en población general y en pacientes con riesgo cardiovascular (diabéticos, hipertensos, fumadores..). Son menos los estudios que incluyen únicamente pacientes diabéticos, y dentro de ellos la mayoría están realizados en pacientes con diabetes tipo 2.</font></p>     <p><font face="Verdana" size="2">Hay dos tipos de estudios: epidemiológicos y de intervención. Los primeros se realizan en población general y relacionan los niveles de ingesta de ciertos antioxidantes con la morbimortalidad cardiovascular y por otras causas. Los estudios de intervención estudian el efecto de la suplementación de antioxidantes sobre la morbimortalidad cardiovascular o por otras causas, o el riesgo de desarrollar diabetes.</font></p>     <p><font face="Verdana" size="2">Otro grupo de estudios analizan el efecto de la administración de antioxidantes de forma puntual o por un período corto de tiempo, sobre diferentes componentes de la función endotelial, perfil lipídico, estrés oxidativo, etc.</u></font></p>     <p><font face="Verdana" size="2">A continuación, presentamos una revisión de los estudios encontrados sobre el efecto de los antioxidantes en pacientes con diabetes mellitus. Los hemos dividido teniendo en cuenta el tipo de antioxidante utilizado, si incluía o no vitamina E, y si ésta se administraba de forma aislada o combinada. Asimismo los hemos estructurado según el objetivo del estudio: morbimortalidad cardiovascular, prevención de diabetes, función endotelial y control metabólico. No hemos incluído como objetivos el cáncer, cataratas o degeneración macular.</font></p>     <p><font face="Verdana" size="2"><i>Estudios con vitamina E aislada</i></font></p>     <p><font face="Verdana" size="2">La mayoría de los estudios realizados con antioxidantes tanto en población general como en diabéticos han utilizado la vitamina E aislada o en combinación con otros antioxidantes. Las dosis de vitamina E empleadas son de 300 a 1800 UI al día, generalmente en forma de alfa-tocoferol.</font></p>     <p><font face="Verdana" size="2">Morbimortalidad cardiovascular</font></p>     <p><font face="Verdana" size="2">Los estudios epidemiológicos en población general muestran resultados positivos que correlacionan la ingesta de vitamina E con una menor morbimortalidad cardiovascular (<a target="_blank" href="/img/revistas/nh/v26n1/revision_4_t1.gif">tabla I</a>)<sup>10-14</sup>. No hemos encontrado estudios que correlacionen la ingesta de vitamina E con la morbimortalidad cardiovascular en pacientes diabéticos.</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">Por el contrario, los estudios de intervención no han demostrado un beneficio claro de la suplementación con vitamina E sobre la morbimortalidad cardiovascular ni en la población general ni en pacientes con riesgo cardiovascular o diabetes. En la <a target="_blank" href="/img/revistas/nh/v26n1/revision_4_t2.gif">tabla II</a> se resumen los resultados de varios estudios<sup>15-19</sup>. Uno de ellos se realizó en mujeres sanas<sup>15</sup>, 2 en pacientes con enfermedad cardiovascular<sup>16,17</sup> y otros 2 en pacientes con enfermedad cardiovascular o diabetes<sup>18,19</sup>. Las dosis de vitamina E variaron entre 300 y 800 UI/día y el período de seguimiento entre 1,5 y 10 años.</font></p>     <p><font face="Verdana" size="2">Prevención de diabetes</font></p>     <p><font face="Verdana" size="2">La evidencia actual no apoya un efecto beneficioso de la suplementación de vitamina E en la prevención de la diabetes. En el Women´s Health Study<sup>20</sup>, que reclutó 38.716 mujeres sanas tratadas durante 10 años con ácido acetilsalicílico (AAS) 100 mg/48 h, alfa-tocoferol 600 UI/ 48 h, ambos o placebo, no se observó un efecto significativo de la vitamina E en el desarrollo de diabetes tipo 2 (RR 0,95, IC 95% 0,87-1,05). Sólo se observó una reducción significativa marginal en el riesgo de diabetes tipo 2 en mujeres asignadas al grupo de vitamina E sin historia familiar de diabetes (RR 0,88, IC 95% 0,78-1,00).</font></p>     <p><font face="Verdana" size="2">Función endotelial y actividad plaquetaria</font></p>     <p><font face="Verdana" size="2">En modelos animales, el alfa-tocoferol preserva la vasodilatación dependiente del óxido nítrico. En estudios in vitro, produce disminución en la expresión de moléculas de adhesión, en la adhesión de los monocitos al endotelio y mejora la vasodilatación mediada por el endotelio tras incubación con antioxidantes.</font></p>     <p><font face="Verdana" size="2">Además de los estudios in vitro, varios estudios clínicos apoyan este efecto beneficioso. Dos de ellos muestran que dosis altas de vitamina E disminuyen los marcadores solubles de activación endotelial como la P-selectina y reducen la adhesión de los monocitos en pacientes con hipercolesterolemia<sup>21,22</sup> y en diabéticos<sup>23</sup>.</font></p>     <p><font face="Verdana" size="2">Otros autores han estudiado el efecto de la administración de antioxidantes durante un tiempo variable (1-12 meses) sobre la vasodilatación dependiente del endotelio (<a target="_blank" href="/img/revistas/nh/v26n1/revision_4_t3.gif">tabla III</a>)<sup>24-30</sup>.</font></p>     <p><font face="Verdana" size="2">En conjunto, la mayoría de estos estudios muestran un papel beneficioso de la vitamina E en la vasodilatación dependiente del endotelio en sujetos con riesgo cardiovascular como dislipemia, diabetes o enfermedad cardiovascular establecida. En alguno de ellos, la mejoría de la función endotelial se relacionó directamente con la reducción del estrés oxidativo, apoyando que el beneficio de la vitamina E sobre la función endotelial depende en parte de sus efectos antioxidantes.</font></p>     <p><font face="Verdana" size="2">Al menos tres estudios han investigado la influencia de la vitamina E sobre la función plaquetaria en diabéticos con diferentes resultados (<a target="_blank" href="/img/revistas/nh/v26n1/revision_4_t4.gif">tabla IV</a>)<sup>31-33</sup>. En dos de ellos<sup>32,33</sup>, la suplementación con vitamina E produjo una disminución de la agregación plaquetaria.</font></p>     <p><font face="Verdana" size="2">Efectos sobre el control metabólico en pacientes diabéticos</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">Diferentes estudios han evaluado los efectos de la suplementación con vitamina E sobre el control glucémico, lipídico y el stress oxidativo en pacientes diabéticos<sup>34-40</sup> (<a target="_blank" href="/img/revistas/nh/v26n1/revision_4_t5.gif">tabla V</a>). En general estos estudios muestran que la suplementación con vitamina E disminuye la peroxidación lipídica y la oxidación de las lipoproteínas, sin mejoría significativa en el control glucémico, resistencia insulínica, perfil lipídico ni en la presión arterial en la mayoría de ellos.</font></p>     <p><font face="Verdana" size="2"><i>Estudios con varios antioxidantes que incluyan vitamina E</i></font></p>     <p><font face="Verdana" size="2">Morbimortalidad cardiovascular</font></p>     <p><font face="Verdana" size="2">El estudio epidemiológico de Losonczy et al. en ancianos, encontró que la ingesta o suplementación con vitaminas E y C produjo una mayor disminución del riesgo de mortalidad coronaria y global que de forma aislada, sugiriendo un efecto sinérgico de ambas vitaminas<sup>14</sup>.</font></p>     <p><font face="Verdana" size="2">Los estudios de intervención con diferentes pautas de antioxidantes incluyendo la vitamina E no han demostrado beneficios en la morbimortalidad cardiovascular en sujetos sanos ni en pacientes con riesgo cardiovascular o diabetes (<a target="_blank" href="/img/revistas/nh/v26n1/revision_4_t6.gif">tabla VI</a>)<sup>41-45</sup>.</font></p>     <p><font face="Verdana" size="2">Prevención de diabetes</font></p>     <p><font face="Verdana" size="2">Los datos de los estudios disponibles no son concluyentes sobre los efectos de diferentes combinaciones de antioxidantes en la prevención de la diabetes (<a target="_blank" href="/img/revistas/nh/v26n1/revision_4_t7.gif">tabla VII</a>)<sup>46,47</sup>. En el estudio SUVIMAX se observó una correlación directa entre los niveles plasmáticos basales de glucemia y selenio, e inversa para los de vitamina C y beta-caroteno<sup>47</sup>.</font></p>     <p><font face="Verdana" size="2">Función endotelial</font></p>     <p><font face="Verdana" size="2">Los estudios sobre función endotelial en pacientes diabéticos y con riesgo cardiovascular muestran en su mayoría una disminución en la producción de moléculas de adhesión en los pacientes suplementados. Además algunos autores han estudiado el efecto de la suplementación con antioxidantes sobre la función endotelial tras una comida rica en grasa. Estos estudios se resumen en la <a target="_blank" href="/img/revistas/nh/v26n1/revision_4_t8.gif">tabla VIII</a><sup>48-51</sup>.</font></p>     <p><font face="Verdana" size="2">Otros estudios</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">Los resultados de la suplementación con antioxidantes en sujetos diabéticos sobre el estrés oxidativo, control glucémico y perfil lipídico se muestran en la <a target="_blank" href="/img/revistas/nh/v26n1/revision_4_t9.gif">tabla IX</a><sup>51-54</sup>. En general, se observa una disminución de la peroxidación lipídica y de las LDL. En uno de ellos realizado con 2 fórmulas de nutrición enteral, una específica para diabetes que estaba enriquecida en antioxidantes y la otra una fórmula estándar, se observó una disminución significativa de la dosis total de insulina, glucemia basal y postprandrial, sin variaciones en los niveles de HbA1c ni en los lípidos en los sujetos tratados con la dieta específica, si bien probablemente estos cambios se deban al efecto de la diferente composición de macronutrientes de ambas dietas<sup>54</sup>.</font></p>     <p><font face="Verdana" size="2"><i>Estudios con antioxidantes sin vitamina E</i></font></p>     <p><font face="Verdana" size="2">Estos estudios incluyen diferentes antioxidantes en dosis diarias variables: vitamina C 100-3000 mg, betacaroteno 6-60 mg, Zn 10-30 mg, selenio 100-800 mcg, cromo 400-1.000 mcg, entre otros.</font></p>     <p><font face="Verdana" size="2">Morbimortalidad cardiovascular</font></p>     <p><font face="Verdana" size="2">No se han encontrado estudios sobre el efecto de la suplementación con antioxidantes (sin vitamina E) sobre la morbimortalidad cardiovascular en pacientes diabéticos entre las referencias evaluadas.</font></p>     <p><font face="Verdana" size="2">Prevención de diabetes</font></p>     <p><font face="Verdana" size="2">Los estudios disponibles no apoyan un efecto beneficioso de la suplementación con estos antioxidantes en la prevención de la diabetes<sup>55</sup>. De hecho, la suplementación con algunos antioxidantes como el selenio parece asociarse a un aumento en el riesgo de esta enfermedad<sup>56</sup> (<a target="_blank" href="/img/revistas/nh/v26n1/revision_4_t10.gif">tabla X</a>).</font></p>     <p><font face="Verdana" size="2">Función endotelial y actividad plaquetaria</font></p>     <p><font face="Verdana" size="2">En una revisión publicada por Brown y Hu en 2001<sup>57</sup>, se repasan los resultados de varios estudios clínicos que examinan los efectos de distintos nutrientes sobre la función endotelial. En uno de ellos se ve cómo 2.000 mg/día de vitamina C durante 10 días en 10 fumadores disminuye la adhesión de los monocitos a los valores de los no fumadores<sup>58</sup>.</font></p>     <p><font face="Verdana" size="2">Los resultados de los estudios con suplementación con antioxidantes sobre el estrés oxidativo y la función endotelial en los diabéticos se resumen en la <a target="_blank" href="/img/revistas/nh/v26n1/revision_4_t11.gif">tabla XI</a><sup>59-64</sup>. En ellos se pone de manifiesto el efecto positivo de estos antioxidantes sobre la disminución de los marcadores de peroxidación lipídica.</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">Respecto a la vasodilatación dependiente del endotelio, la mencionada revisión<sup>57</sup> evalúa cuatro ensayos clínicos controlados con placebo<sup>65-68</sup>, los tres últimos aleatorizados, realizados en un total de 93 sujetos sanos, fumadores, con insuficiencia cardiaca o cardiopatía isquémica a los que se les administraron entre 500 y 2.000 mg de vitamina C diarios durante un periodo de entre 1 y 8 semanas y, en todos los casos excepto en el estudio realizado en fumadores<sup>68</sup>, se vio una mejoría de la dilatación mediada por el flujo de las arterias braquial o radial.</font></p>     <p><font face="Verdana" size="2">En un estudio experimental<sup>69</sup>, se administraron 2.000 mg/día de vitamina C a sujetos fumadores o no y en programa de by-pass coronario, y se logró un aumento de la superficie arterial tanto en fumadores como en no fumadores, pero fue mayor en el primer grupo. En una segunda fase del mismo estudio, se comparó la administración de vitamina C con la de diltiazem 120 mg/día en los pacientes pendientes de bypass coronario y se lograron mejores resultados en cuanto incremento de la superficie arterial con la vitamina C.</font></p>     <p><font face="Verdana" size="2">En la <a target="_blank" href="/img/revistas/nh/v26n1/revision_4_t12.gif">tabla XII</a> se resumen los estudios realizados con vitamina C sobre la vasodilatación dependiente de endotelio en diabéticos, que muestran un efecto positivo de la misma<sup>59,70,71</sup>.</font></p>     <p><font face="Verdana" size="2">Efectos sobre el control metabólico en pacientes diabéticos</font></p>     <p><font face="Verdana" size="2">Hay dos ensayos clínicos aleatorizados<sup>72,73</sup> que evaluaron los efectos metabólicos de suplementar cromo en distintas dosis o cromo combinado con biotina a un total de 216 pacientes diabéticos tipo 2 y obtuvieron una disminución de la hemoglobina glicosilada de hasta 2%, la glucemia postprandial, la fructosamina, la insulinemia, el colesterol total, el cociente HDL/LDL, los triglicéridos y el índice de aterogenicidad.</font></p>     <p><font face="Verdana" size="2">En el ensayo clínico comentado anteriormente que estudió los resultados de suplementar zinc, cromo o ambos combinados frente a placebo<sup>62</sup>, no se obtuvieron cambios en la hemoglobina glicosilada ni otros parámetros de homeostasis de glucosa, al igual que en el ensayo en el que se suplementaba con 30 mg/día de zinc<sup>60</sup>.</font></p>     <p><font face="Verdana" size="2">Se han encontrado ciertos beneficios metabólicos de suplementar con 3.000 mg/día de vitamina C a pacientes con diabetes mellitus tipo 2 en un ensayo clínico referido anteriormente<sup>71</sup>, al obtenerse una tendencia a disminuir la hemoglobina glicosilada y el colesterol total en el grupo de vitamina C, aunque no hubo cambios en los niveles de interleukinas, proteína C reactiva ni en la oxidación de las partículas de LDL colesterol.</font></p>     <p><font face="Verdana" size="2">Por último, un estudio caso-control<sup>74</sup> evaluó el efecto sobre la oxidación del colesterol LDL de administrar 60 mg/día de beta-caroteno a 20 pacientes con diabetes mellitus tipo 2 y a 20 sujetos sanos durante 3 semanas. Los pacientes diabéticos tenían diferente composiciónlipídica del colesterol LDL, que se oxidaba más que en los controles sanos, y esta oxidación disminuía tras 3 semanas de suplementar con beta-caroteno.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Conclusiones</b></font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">Aunque en la diabetes mellitus existe un aumento del estrés oxidativo y una disminución de los sistemas de defensa antioxidante, los estudios de intervención con diferentes combinaciones de antioxidantes no han demostrado un efecto beneficioso sobre la morbimortalidad cardiovascular y global en diferentes poblaciones, incluídos los pacientes con diabetes mellitus. Tampoco en estos estudios se ha demostrado un efecto beneficioso de estas sustancias en la prevención de la diabetes.
