Una nutrición inadecuada puede inducir a la disfunción en eslabones de la cadena metabólica. Todos los nutrientes esenciales son imprescindibles y el déficit o exceso de cualquiera de ellos conlleva a efectos adversos en la salud. El cinc es un micronutriente extensamente demandado en el organismo, como lo demuestra la amplia diversidad de funciones biológicas que presenta. La ingesta de cinc presenta un gran margen en la población mundial actual, pudiendo ser de 7 mg/d en Reino Unido y llegando a 15 mg/d en EEUU, aunque por supuesto, las RDAs se fijan en función de la edad, sexo, situación fisiológica (embarazo, lactancia, etc.), o enfermedad. Se conoce que el cinc es fundamental para la estructura y funcionamiento del DNA y así como de enzimas, coenzimas, hormonas, etc. La vida del cinc es corta, dado que la mayor cantidad se absorbe y rápidamente es transferida a depósitos, donde se almacena, por lo tanto la cantidad se cinc disponible en sangre puede no ser la cantidad "real". En el presente trabajo hemos realizado una breve revisión del paso del cinc por nuestro organismo, tratando desde su ingesta hasta su recorrido por la sangre tanto en personas sanas como enfermas.]]></p></abstract>

One of the most important micronutrients is zinc, as evidenced by its wide range of carriers in the body. Zinc intake has a large margin in the current world population, may be 7 mg/d in the UK, reaching 15 mg/d in the U.S., although of course, the RDA's are set according to age, sex , physiological status (pregnancy, lactation, etc..), or disease. It is known that zinc is essential for the structure and function as well as DNA and enzymes, coenzymes, hormones and so on. Life is short, zinc, since the most rapidly absorbed and is transferred to tanks where it is stored, so the amount available zinc in the blood cannot be the amount "real". In this work we have done a mini-review of the passage of zinc by the body trying since their intake to their tour of the blood in both healthy and sick people.]]></p></abstract>

<body><![CDATA[<p><font face="Verdana" size="2"><b>Introducción</b></font></p>     <p><font face="Verdana" size="2">La Nutrición es la base de los procesos fisiológicos humanos. Una nutrición inadecuada puede inducir a la disfunción en eslabones de la cadena metabólica. Los macronutrientes y micronutrientes deben estar en cantidades adecuadas para el equilibrio metabólico. Los micronutrientes son necesarios en pequeñas cantidades y en general son utilizados en estructuras enzimáticas, coenzimáticas, hormonales, etc.</font></p>     <p><font face="Verdana" size="2">Uno de los micronutrientes más conocidos situado en segundo lugar tras el hierro, es el cinc. La doctora Ananda S. Prasad en la década de los sesenta, comprobó por primera vez la importancia del cinc en la nutrición humana, encontrando, primero en los pacientes iraníes y luego en pacientes egipcios, un cuadro caracterizado por anemia, déficit de hierro, hipogonadismo, enanismo, hepatoesplenomegalia, y geofagia. Todos estos pacientes sufrían de malnutrición<sup>1,2</sup>.</font></p>     <p><font face="Verdana" size="2">Las primeras referencias en las que el cinc se reconoció por primera vez como esencial en un sistema biológico humano, datan de 1956, cuando se estudió el metabolismo del cinc en pacientes que padecían cirrosis<sup>1,3</sup>. Aún al inicio de los sesenta se creía que la deficiencia de cinc nunca podía ocurrir en humanos ya que se contaba con muy pocos datos de contenido de Zn en alimentos. Sin embargo, es en 1961 cuando Prasad y colaboradores describen lo que hoy se conoce como Síndrome de deficiencia de cinc<sup>1,2</sup>.</font></p>     <p><font face="Verdana" size="2">Las recomendaciones de nutrientes (RDA = Recommended Dietary Allowances o IDR = Ingesta Diaria Recomendada) es el nivel de ingesta diaria de un nutriente que resulta suficiente para cubrir las necesidades de casi todos (97,5%) los individuos sanos, según edades, sexo y situaciones de embarazo y lactancia. Los valores de RDA se fijan en función de la edad, sexo, situación fisiológica (embarazo, lactancia, etc.)<sup>4-10</sup>.</font></p>     <p><font face="Verdana" size="2">La ingesta de cinc es muy amplia en la población mundial en general, siendo entre 7,3 mg/día para hombres y 5,5 mg/día para mujeres, en Reino Unido, y hasta 15 mg/día para hombres y 12 mg/día para mujeres, en EE.UU. En la <a target="_blank" href="/img/revistas/nh/v27n3/03_revision_02_t1.gif">tabla I</a>, se describen las RDA's para el cinc en distintos tipos de poblaciones y recomendaciones de cinc para nutrición parenteral en función de la situación de enfermedad.</font></p>     <p><font face="Verdana" size="2">La recomendación de cinc en pacientes es diferente dependiendo de la enfermedad, en especial los que reciben nutrición artificial, que varía fundamentalmente en función del tipo de nutrición. En caso de nutrición enteral, las recomendaciones son casi las mismas que en personas sanas. Se sigue con las mismas recomendaciones que en una digestión normal, ya que las vías de nutrición son parcialmente naturales y la dieta llega, o al estómago o al intestino delgada. En caso de nutrición parenteral las recomendaciones están en un nivel más bajo porque los nutrientes llegan directamente al flujo sanguíneo. No obstante, es de destacar que los estudios sobre el cinc en distintos tipos de enfermedades se sitúan hoy aún en su inicio. Por lo tanto, es importante indicar la necesidad de realizar estudios nutricionales generales y por cada nutriente por separado.</font></p>     <p><font face="Verdana" size="2">El nivel más alto tolerable de ingesta de cinc es de 25 mg/día<sup>15</sup>. Existen factores que pueden aumentar o disminuir la biodisponibilidad del cinc ingerido, como el ácido fítico, la fibra, el calcio, ligando orgánicos, etc. Asimismo, el consumo paralelo de suplementos o alimentos enriquecidos con calcio, cobre o hierro pueden dificultar la absorción del elemento<sup>7,16,17</sup>.</font></p>     <p><font face="Verdana" size="2">En el mundo, la ingesta de cinc es diferente de unos países a otros, como viene reflejado en la <a target="_blank" href="/img/revistas/nh/v27n3/03_revision_02_f1.gif">figura 1</a>.</font></p>     <p><font face="Verdana" size="2">Los grupos poblacionales que tienen riesgo alto de deficiente de ingesta de cinc son<sup>3,20-25</sup>:</font></p>     ]]></body>
<body><![CDATA[<blockquote> 	    <p><font face="Verdana" size="2">- Personas vegetarianas.</font></p> 	    <p><font face="Verdana" size="2">- Población infantil (especialmente desnutridos).</font></p> 	    <p><font face="Verdana" size="2">- Recién nacidos prematuros.</font></p> 	    <p><font face="Verdana" size="2">- Pacientes con infecciones severas.</font></p> 	    <p><font face="Verdana" size="2">- Pacientes con quemaduras severas.</font></p> 	    <p><font face="Verdana" size="2">- Pacientes politraumatizados.</font></p> 	    <p><font face="Verdana" size="2">- Pacientes que reciben alimentación parenteral total.</font></p> 	    <p><font face="Verdana" size="2">- Pacientes con insuficiencia renal en tratamiento de hemodiálisis.</font></p> 	    <p><font face="Verdana" size="2">- Personas mal nutridas incluyendo pacientes con anorexia nerviosa.</font></p> 	    ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">- Pacientes con enfermedad alcohólica del hígado, cirrosis hepática o enfermedad de Wilson.</font></p> 	    <p><font face="Verdana" size="2">- Pacientes con síndromes de malabsorción intestinal.</font></p> 	    <p><font face="Verdana" size="2">- Pacientes con diarrea severa o crónica de cualquier etiología.</font></p> 	    <p><font face="Verdana" size="2">- Pacientes con enfermedades inflamatorias intestinales tales como la enfermedad de Crohn o colitis ulcerosa.</font></p> 	    <p><font face="Verdana" size="2">- Pacientes con fístulas intestinales donde hay pérdida constante de fluidos.</font></p> 	    <p><font face="Verdana" size="2">- Pacientes ancianos (65 o más años).</font></p> 	    <p><font face="Verdana" size="2">- Pacientes con enfermedad del páncreas (pancreatitis crónica).</font></p> 	    <p><font face="Verdana" size="2">- Mujeres embarazadas y lactando.</font></p> 	    <p><font face="Verdana" size="2">- Pacientes con diabetes mellitus.</font></p> 	    <p><font face="Verdana" size="2">- Vegetarianos estrictos o personas que ingieren dietas cuya fuente proteica es sólo cereales.</font></p> 	    ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">- Pacientes con Acrodermatitis Enteropática.</font></p> 	    <p><font face="Verdana" size="2">- Pacientes con Anemia Drepanocítica (cursa con bajos niveles de cinc).</font></p> </blockquote>     <p><font face="Verdana" size="2">El cinc está extensamente distribuido en alimentos y bebidas, pero tal como ocurre con otros elementos, los contenidos son muy variables y en general bajos. Los productos de origen marino, principalmente los mariscos (ostras y crustáceos), son los alimentos más ricos en cinc, seguidos de las carnes rojas, derivados lácteos y huevos, así como los cereales integrales. Los vegetales, con excepción de las leguminosas, no son alimentos que presenten contenido de cinc alto. Por todo ello, las verduras, hortalizas, frutas, grasas, pescados y dulces son fuentes pobres del mineral<sup>6,10,26</sup>.</font></p>     <p><font face="Verdana" size="2">La homeostasis del cinc se logra principalmente por adaptaciones a nivel del tracto gastrointestinal. Además, éste puede contribuir a la redistribución tisular y celular, y solo en casos extremos, se hacen ajustes renales. Las adaptaciones intestinales a cambios en la ingesta del mineral consisten en aumentar la absorción y disminuir la excreción fecal de cinc endógeno ante consumos deficientes. Por el contrario, cuando el consumo es excesivo, disminuye la absorción y aumenta la excreción<sup>27,28</sup>. El recambio de cinc en el organismo es lento, con una vida media biológica de 250 días. Sin embargo, las reservas corporales de cinc son relativamente pequeñas, y tiene una rápida tasa de recambio. Por consiguiente, es necesario el aporte continuo de cinc para que pueda eliminarse a través de las secreciones pancreáticas e intestinales y excretado diariamente en la orina (sólo un 5%)<sup>29,30</sup>.</font></p>     <p><font face="Verdana" size="2">Entre el 3 y el 38% del cinc de la dieta se absorbe en el tubo digestivo proximal. Esta absorción del cinc parece estar regulada por la síntesis de una proteína intestinal denominada metalotioneína (MT, proteína de bajo peso molecular rica en cisteína), que tiene la capacidad de ligar diferentes metales divalentes como el Zn<sup>2+</sup>, Cu<sup>2+</sup> y Cd<sup>2+</sup>. Esta proteína actúa como ligando que amortigua la absorción del cinc<sup>1,31</sup>.</font></p>     <p><font face="Verdana" size="2">Los músculos y huesos contienen un 90% de la cantidad total de cinc<sup>10</sup>. También se ha demostrado que la mayor concentración del cinc en los órganos se encuentra en hígado, tracto gastrointestinal, riñón, piel, pulmón, cerebro, ojo, corazón y páncreas. Se han detectado también cantidades importantes en próstata, retina y esperma. Por otro lado, Forssen (1972) demostró que la cantidad de cinc es muy variable de un individuo a otro<sup>17,32,33</sup>.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Biomarcadores del estatus de cinc en humanos</b></font></p>     <p><font face="Verdana" size="2">Los biomarcadores se pueden definir como cualquier parámetro bioquímico, fisiológico u otra alteración del organismo que se puede reconocer y establecer con un potencial efecto sobre salud o enfermedad (NAS/NRC, 1989), incluyendo cualquier signo bioquímico o celular de los tejidos con mal funcionamiento, como podría ser un incremento o disminución en la actividad enzimática provocado por cambios patológicos.</font></p>     <p><font face="Verdana" size="2">Las sociedades internacionales así como distintos autores, han investigado la determinación óptima de cinc en medios biológicos, siendo uno de los más estudiado, fáciles, fiable y comparable, la sangre. Como marcador, las muestras más utilizadas para determinar la concentración de cinc son el suero/plasma y/o eritrocito. Por lo tanto la concertación mínima de cinc en suero/plasma se considera ser alrededor de 10,9 &mu;moles/L y con valores más elevadas en los sujetos que pueden ser expuestos a la contaminación con cinc. Los niveles de cinc que producen efectos adversos en la salud son mucho mayores (10-15 veces) que las RDAs para el cinc<sup>34,78</sup>.</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">Los medios biológicos más usuales para determinar el cinc son en sangre (suero, plasma, células), orina, pelo y uñas. En orina la concentración aproximada es de 0,5 mg Zn/g de creatinina<sup>17</sup>. Las determinaciones de cinc en pelo y uñas informan del historial de ingesta de cinc La concentración de cinc en uñas es estimada entre 129-179 &mu;g/g, y en pelo 102-258 &mu;g/g<sup>17</sup>. Un inconveniente es que hay poca correlación entre el cinc en pelo y el cinc plasmático, ya que el cinc del pelo no se recambia. Los valores de cinc en saliva (parótida) son entre 23-70 ng/g y en sudor 0,55-1,75 mg/L<sup>30</sup>, aunque no son medios frecuentemente utilizados para la determinación de cinc y, en general, los minerales.</font></p>     <p><font face="Verdana" size="2">Se ha realizado el análisis de varias enzimas empleadas como biomarcadores dependientes de cinc, pero ninguna por sí sola puede reflejar claramente el estatus real de cinc. Por ejemplo, la fosfatasa alcalina ha sido la más estudiada, y se ha demostrado que en una deficiencia de cinc severa la actividad de la enzima disminuye<sup>35</sup>, aunque es un marcador muy inespecífico por sí sólo.</font></p>     <p><font face="Verdana" size="2">Otros autores relacionan la cantidad de cinc en el organismo con de la concentraciones de proteínas asociadas al cinc, como la albúmina y a la &alpha;-2-macroglobulina<sup>34,36</sup>. En sangre el cinc está presente tanto en plasma como en eritrocitos, leucocitos y plaquetas, aunque la mayor cantidad se encuentra en eritrocitos (el 87% se une a la anhidrasa carbónica). Diversos autores han encontrado valores de cinc de referencia en eritrocito de 20-50 &mu;g Zn/g Hb (0,31-0,76 &mu;mol/g) y como cinc en eritrocito/10<sup>9</sup> células 0,50-1,50 &mu;g (0,0076-0,0229 &mu;mol/10<sup>9</sup> células)<sup>37</sup>.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Los inconvenientes</b></font></p>     <p><font face="Verdana" size="2">Los valores de plasma o suero son muy discutidos por los inconvenientes que presentan:</font></p>     <blockquote> 	    <p><font face="Verdana" size="2">a) No refleja los depósitos en el organismo con un rango normal.</font></p> 	    <p><font face="Verdana" size="2">b) Fluctúa más de un 20% en un día.</font></p> 	    <p><font face="Verdana" size="2">c) Los valores pueden verse afectados en situaciones de inflamación e infección aguda, unido a otros factores como hipoalbuminemía, contraceptivos orales, embarazo, cirrosis, etc., o por interacción con otros minerales<sup>38,39</sup>.</font></p> </blockquote>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">Existen varios factores que pueden influir sobre la cantidad de cinc presente en los medios biológicos, como se refleja en la <a target="_blank" href="/img/revistas/nh/v27n3/03_revision_02_t2.gif">tabla II</a>.</font></p>     <p><font face="Verdana" size="2">Un estudio reciente efectuado en adultos ha mostrado que una hipercinquemia acentuada (77-200 &mu;mol/l), asociada a hipercalprotectinemia, parece ser una nueva alteración genético-metabólica, que se traduce en infecciones recurrentes, hepatoesplenomegalia, anemia y evidencia de inflamación sistémica. También está descrita la hipercinquemia familiar sin asociación a alteraciones clínicas<sup>42</sup>.</font></p>     <p><font face="Verdana" size="2">Se sabe que los niveles de cinc en sangre de paciente crítico bajan en caso de inflamación<sup>12,58</sup>. Se han realizado estudios sobre otros marcadores de cinc pero hasta en día de hoy no se encontró ninguno completamente fiable. Por otro lado, un estudio realizado por Lowe et al. (2004) confirma que el cinc plasmático puede resultar válido como indicador del estatus del mineral<sup>43</sup>.</font></p>     <p><font face="Verdana" size="2">En la <a target="_blank" href="/img/revistas/nh/v27n3/03_revision_02_t3.gif">tabla III</a> se muestran los puntos de corte propuestos para determinar la deficiencia de cinc según la Sociedad Internacional IZiNCG 2007<sup>45</sup>.</font></p>     <p><font face="Verdana" size="2">La IZiNCG afirma que si existe un porcentaje mayor al 20% de la población que presenta una concentración insuficiente de cinc en suero, se considera que dicha población está en riesgo de deficiencia de este mineral<sup>44,45</sup>.</font></p>     <p><font face="Verdana" size="2">Dada la importancia de conocer la cantidad real de cinc existente en el cuerpo humano, se ha realizado una revisión sobre los estudios de valoración del estatus de cinc realizados en la última década, tanto en personas sanas (<a target="_blank" href="/img/revistas/nh/v27n3/03_revision_02_tabla4.html">tabla IV</a>) como con diferentes enfermedades (<a target="_blank" href="/img/revistas/nh/v27n3/03_revision_02_tabla5.html">tabla V</a>).</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Conclusiones</b></font></p>     <p><font face="Verdana" size="2">Como podemos observar, existe controversia al intentar establecer unos valores bioquímicos de referencia de cinc en sangre, plasma o eritrocitos en los diferentes grupos de población sana o enferma, que sirvan para determinar el riesgo de deficiencia en éste mineral. Es por ello por lo que es recomendable evaluarlo de forma separada en cada estudio concreto para poder comparar con la propia muestra.</font></p>     <p>&nbsp;</p>     ]]></body>
