    <p><font face="Verdana" size="2">En el contexto actual de una medicina cada vez m&aacute;s personalizada, sin duda la aportaci&oacute;n de la gen&eacute;tica juega un papel cada vez m&aacute;s relevante (1). Probablemente, una de las aproximaciones m&aacute;s sencillas a esta &aacute;rea de conocimiento es el an&aacute;lisis de diferentes polimorfismos de un &uacute;nico nucle&oacute;tido (SNP) y su relaci&oacute;n con diferentes par&aacute;metros antropom&eacute;tricos (2), factores de riesgo cardiovascular (3), diabetes mellitus (4), respuesta a diferentes modelos de dieta (5), cirug&iacute;a bari&aacute;trica (6) y f&aacute;rmacos (7). Como vemos, estas m&iacute;nimas modificaciones de un nucle&oacute;tido en el genoma humano pueden relacionarse con diferentes situaciones, as&iacute; como la respuesta a diversos tratamientos, en el contexto de la nutrici&oacute;n cl&iacute;nica.</font></p>
    <p><font face="Verdana" size="2">En este n&uacute;mero de <i>Nutrici&oacute;n Hospitalaria</i> se publican dos trabajos que abordan el uso de estos marcadores gen&eacute;ticos en dos escenarios diferentes de la composici&oacute;n corporal. En primer lugar, el trabajo de Ponce Gonz&aacute;lez y cols. (8) muestra c&oacute;mo, a pesar del bien conocido efecto de los andr&oacute;genos en la oxidaci&oacute;n de grasas y composici&oacute;n corporal, este efecto depende de las caracter&iacute;sticas intr&iacute;nsecas y posibles variaciones gen&eacute;ticas del receptor de andr&oacute;genos (RA). De este modo, los autores dise&ntilde;an un estudio en 319 hombres sanos, seleccionando los sujetos con repeticiones del CAG extremas cortas (CAG<sub>S</sub> &le; 19; n = 7) y largas (CAG<sub>L</sub> &ge; 24; n = 10), y los sujetos con repeticiones del GGN extremas cortas (GGN<sub>S</sub> &le; 22; n = 9) y largas (GGN<sub>L</sub> &ge; 25; n = 10) en el RA. El grupo CAG<sub>L</sub> tuvo mayor capacidad de oxidaci&oacute;n de grasa (COG) y masa libre de grasa que el grupo CAG<sub>S</sub>. Los hombres con GGN<sub>S</sub> acumularon mayor cantidad de masa grasa total que los hombres con GGN<sub>L</sub>, particularmente en la regi&oacute;n del tronco. Esto se acompa&ntilde;&oacute; con un mayor COG en el grupo GGN<sub>L</sub>, que acumul&oacute; menos masa grasa. Por tanto, las repeticiones del polimorfismo del CAG y GGN en el receptor de andr&oacute;genos pueden influenciar la capacidad muscular de oxidaci&oacute;n de grasas y tener un claro papel en la acumulaci&oacute;n de tejido adiposo.</font></p>
    <p><font face="Verdana" size="2">En el segundo trabajo de Romero y cols. (9) se eval&uacute;a el problema de la obesidad sarcop&eacute;nica. Se estudiaron 300 mujeres mayores de 60 a&ntilde;os que realizaban ejercicio f&iacute;sico al menos dos veces a la semana en los dos &uacute;ltimos a&ntilde;os. Se encontraron diferencias significativas en la obesidad sarcop&eacute;nica en funci&oacute;n del genotipo I/D del enzima convertidor de la angiotensina (ACE). Las mujeres del grupo ACE DD presentaron menor riesgo de padecer obesidad sarcop&eacute;nica que aquellas con genotipo ACE II y ACE ID. Por tanto, las mujeres mayores de 60 a&ntilde;os con genotipo ACE DD que practican ejercicio regularmente presentan menor riesgo de padecer obesidad sarcop&eacute;nica, debi&eacute;ndose por ello implementar otras estrategias para evitar esta situaci&oacute;n en las mujeres con los otros genotipos.</font></p>
    <p><font face="Verdana" size="2">Como podemos comprobar con estos dos magn&iacute;ficos trabajos, sin duda, la evaluaci&oacute;n gen&eacute;tica en forma de an&aacute;lisis de SNP nos permitir&aacute; una mejor aproximaci&oacute;n diagn&oacute;stica y terap&eacute;utica para nuestros pacientes en el &aacute;mbito de la nutrici&oacute;n cl&iacute;nica. Todo esto pasa por una mayor cercan&iacute;a del cl&iacute;nico a los laboratorios y de los especialistas que trabajan en esos laboratorios, en especial de los genetistas, al cl&iacute;nico. Previsiblemente, todo ello redundar&aacute; en una medicina m&aacute;s personalizada.</font></p>
