Las nuevas técnicas diagnósticas y terapéuticas han permitido mayor supervivencia y calidad de vida de unos pacientes que desean realizar deporte. Hasta donde pueden llegar no es siempre fácil de determinar. Las guías nos ayudan a la hora de decidir, aunque finalmente es el médico el que debe determinar en cada caso la situación del paciente y el tipo de deporte que podrá realizar dependiendo de la severidad y el tipo de cardiopatía.]]></p></abstract>

Exercise.</font></p>  <hr size="1">      <p>&nbsp;</p>     <p><font face="Verdana"><b>Introducción</b></font></p>     <p><font face="Verdana" size="2">Las cardiopat&iacute;as cong&eacute;nitas constituyen, cada vez m&aacute;s, una causa frecuente de consulta cardiol&oacute;gica, permitiendo los avances diagn&oacute;sticos y terap&eacute;uticos que estos ni&ntilde;os lleguen a la adolescencia y a la edad adulta solicitando realizar deporte.</font></p>    <p> <font face="Verdana" size="2">El m&eacute;dico debe intentar detectar las anomal&iacute;as cardiovasculares que puedan predisponer a muerte s&uacute;bita durante la pr&aacute;ctica deportiva, ya que la mayor parte de los eventos cardiol&oacute;gicos durante el ejercicio son debidos a enfermedades cardiacas subyacentes (1,2). De ah&iacute; la importancia de realizar una historia cl&iacute;nica y exploraci&oacute;n f&iacute;sica adecuada incluyendo la existencia de antecedentes familiares de muerte s&uacute;bita o cardiopat&iacute;a, s&iacute;ntomas relacionados con el esfuerzo, s&iacute;ncope, palpitaciones o dolor tor&aacute;cico junto con la determinaci&oacute;n de la tensi&oacute;n arterial, palpaci&oacute;n de pulsos radiales y femorales, auscultaci&oacute;n cardiaca o descartar fenotipos marfanoides (3).</font></p>    ]]></body>
<body><![CDATA[<p> <font face="Verdana" size="2">Sin embargo debemos ser conscientes de que la realizaci&oacute;n de ejercicio f&iacute;sico o deporte de competici&oacute;n es una parte importante en el desarrollo f&iacute;sico, ps&iacute;quico y metab&oacute;lico de ni&ntilde;os y adolescentes, estando aconsejado la realizaci&oacute;n de ejercicio regular a los niveles indicados a todos los pacientes con cardiopat&iacute;as cong&eacute;nitas (4).</font></p>    <p> <font face="Verdana" size="2">En t&eacute;rminos generales aquellos pacientes con cardiopat&iacute;as cong&eacute;nitas simples con reparaci&oacute;n adecuada o sin necesidad de intervenci&oacute;n tendr&aacute;n una capacidad pr&aacute;cticamente normal de realizar cualquier tipo de deporte, mientras que aquellos con lesiones m&aacute;s complejas tendr&aacute;n mayor restricci&oacute;n en la pr&aacute;ctica deportiva, siendo la ergometr&iacute;a una herramienta ideal para la identificaci&oacute;n de arritmias o inestabilidad hemodin&aacute;mica con el ejercicio (5).</font></p>     <p>&nbsp;</p>     <p><font face="Verdana"><B>Tipos de cardiopatías congénitas y de actividad deportiva</B></font></p>     <p><font face="Verdana" size="2">Dada la diversidad de cardiopat&iacute;as cong&eacute;nitas las clasificaremos en:</font></p>    <p> <font face="Verdana" size="2">-Cardiopat&iacute;as con shunt izquierda-derecha: En este grupo se incluyen la comunicaci&oacute;n interauricular (CIA), la comunicaci&oacute;n interventricular (CIV) y el ductus.</font></p>    <p> <font face="Verdana" size="2">-Cardiopat&iacute;as con lesiones obstructivas o insuficientes: estenosis valvular, subvalvular o supravalvular y las insuficiencias valvulares.</font></p>    <p> <font face="Verdana" size="2">-Cardiopat&iacute;as cian&oacute;ticas: en este grupo est&aacute;n incluidas un gran n&uacute;mero de cardiopat&iacute;as que presenta cianosis como la tetralog&iacute;a de Fallot, la transposici&oacute;n de grandes arterias (DTGA), la transposici&oacute;n corregida de grandes arterias (LTGA) con alteraciones asociadas, la atresia valvular mitral o trisc&uacute;spide, el truncus, el ventr&iacute;culo &uacute;nico o la hipoplasia de cavidades izquierdas.</font></p>    <p> <font face="Verdana" size="2">-Otros tipos de cardiopat&iacute;as: como son las anomal&iacute;as coronarias, el s&iacute;ndrome de Marfan, la coartaci&oacute;n de aorta o las f&iacute;stulas.</font></p>    <p> <font face="Verdana" size="2">Dentro de cada grupo de cardiopat&iacute;as podremos encontrar diferentes grados que van desde las que tienen un comportamiento benigno a las que son severas.</font></p>    ]]></body>
<body><![CDATA[<p> <font face="Verdana" size="2">En lo referente al deporte existen diferentes clasificaciones seg&uacute;n su intensidad, que sean din&aacute;micos, est&aacute;ticos, recreativos o competitivos (en estos &uacute;ltimos el fin es obtener un premio mediante la competici&oacute;n con otros, lo que requiere un entrenamiento regular y de alta intensidad) (6,7) (Tabla  <a href="#t1">I</a> y <a target="_blank" href="/img/revistas/ami/v25n4/revision2_t2.jpg">II</a>).</font></p>     <p align="center"><a name="t1"><img src="/img/revistas/ami/v25n4/revision2_t1.jpg" width="386" height="611"></a></p>      <p align="center">&nbsp;</p>     <p align="left"><font face="Verdana"><B>Clasificación de las cardiopatías congénitas según su grado de severidad</B></font></p>     <p><font face="Verdana" size="2"><i>CARDIOPAT&Iacute;AS CONG&Eacute;NITAS BENIGNAS</i></font></p>     <p><font face="Verdana" size="2">Son aquellas comunicaciones con shunt izquierda-derecha con QP/QS &lt; 1,5, obstrucciones con gradiente &lt; 20 mmHg o insuficiencias valvulares de grado ligero.</font></p>    <p> <font face="Verdana" size="2">Estos grupos de cardiopat&iacute;as son clasificadas como benignas no teniendo limitaci&oacute;n alguna para la pr&aacute;ctica de deportes recreativos o de competici&oacute;n (9).</font></p>      <p><font face="Verdana" size="2"><i>    <br> CARDIOPAT&Iacute;AS CONG&Eacute;NITAS LEVES</i></font></p>     <p><font face="Verdana" size="2">En este grupo incluimos las comunicaciones con shunt izquierda-derecha con QP/QS entre 1,5-2 y presi&oacute;n en arteria pulmonar sist&oacute;lica &lt; 40 mm Hg, la estenosis pulmonar y a&oacute;rtica con gradiente pico ecocardiogr&aacute;fico &lt; 40 mmHg y/o pico hemodin&aacute;mico &lt; 30 mmHg, las insuficiencias valvulares mitral o a&oacute;rticas ligeras-moderadas sin disfunci&oacute;n ni dilataci&oacute;n ventricular y la coartaci&oacute;n de aorta nativa u operada con gradiente basal inferior a 20 mmHg (9,10).</font></p>    ]]></body>
<body><![CDATA[<p> <font face="Verdana" size="2">En este grupo de pacientes catalogados como de riesgo ligero se permiten las actividades recreativas, los deportes de competici&oacute;n poco o moderadamente intensos y tan solo los de alta intensidad si se consigue realizar una prueba de esfuerzo a m&aacute;xima intensidad sin evidenciarse alteraciones isqu&eacute;micas electrocardiogr&aacute;ficas ni alteraciones del ritmo (9,11).</font></p>      <p><font face="Verdana" size="2"><i>    <br> CARDIOPAT&Iacute;AS CONG&Eacute;NITAS MODERADAS</i></font></p>     <p><font face="Verdana" size="2">En este grupo est&aacute;n las comunicaciones con shunt izquierda-derecha con QP/QS &gt; 2 y presi&oacute;n en la arteria pulmonar sist&oacute;lica &gt; 40 mm Hg, la estenosis a&oacute;rtica moderada (gradiente pico ecocardiogr&aacute;fico entre 40-70 mmHg y/o gradiente pico hemodin&aacute;mico entre 30-50 mmHg) sin datos de hipertrofia el&eacute;ctrica ni ecocardiogr&aacute;fica y asintom&aacute;ticos, la coartaci&oacute;n de aorta nativa u operada con gradiente basal entre 20-50 mmHg, las cardiopat&iacute;as cong&eacute;nitas cian&oacute;ticas corregidas totalmente, las pr&oacute;tesis valvulares y las insuficiencias a&oacute;rtica y mitral moderada-severa sin disfunci&oacute;n ni dilataci&oacute;n ventricular (9-11).</font></p>    <p> <font face="Verdana" size="2">En estos pacientes, catalogados como de riesgo moderado, est&aacute;n permitidas las actividades recreativas poco o moderadamente intensas si en la prueba de esfuerzo no se evidencia alteraciones isqu&eacute;micas ni arr&iacute;tmicas y el paciente presenta una buena tolerancia al esfuerzo. Sin embargo tan solo se recomienda deportes de competici&oacute;n de baja carga est&aacute;tica y din&aacute;mica, prohibi&eacute;ndose los de alta intensidad (8-11).</font></p>    <p> <font face="Verdana" size="2">En el caso de las comunicaciones, cuando &eacute;stas son moderadas o grandes y no existe hipertensi&oacute;n pulmonar fija, se debe plantear el cierre percut&aacute;neo o quir&uacute;rgico. Tras la reparaci&oacute;n se debe esperar entre 3-6 meses antes de comenzar la realizaci&oacute;n de deportes, pudiendo participar en todo tipo de deportes aquellos que se encuentran asintom&aacute;ticos, sin datos de hipertensi&oacute;n pulmonar, arritmias supra o ventriculares o disfunci&oacute;n/dilataci&oacute;n ventricular. De igual forma ocurre con las estenosis pulmonar y a&oacute;rtica en las cuales se recomienda valvuloplastia o valvulotom&iacute;a antes de realizar deportes de competici&oacute;n, desaconsej&aacute;ndose en aquellos en los que exista disfunci&oacute;n/dilataci&oacute;n ventricular (9,10).</font></p>    <p> <font face="Verdana" size="2">En la coartaci&oacute;n de aorta, tras su reparaci&oacute;n, se desaconseja la realizaci&oacute;n de deportes con alto componente est&aacute;tico y aquellos en los que puedan existir golpes durante el primer a&ntilde;o. Para aquellos deportistas con dilataci&oacute;n significativa de la aorta, adelgazamiento de la pared o formaci&oacute;n de aneurismas, el deporte competitivo se debe restringir a aquellos de baja intensidad (9).</font></p>    <p> <font face="Verdana" size="2">Dentro de las cardiopat&iacute;as ci&aacute;noticas corregidas quirurgicamente hay que tener en cuenta que las cirug&iacute;as previas conllevan que pueda existir cicatrices, parches o pr&oacute;tesis que puedan dar lugar a un mayor n&uacute;mero de complicaciones mec&aacute;nicas o el&eacute;ctricas a largo plazo. Ejemplo de ello son las taquicardias ventriculares a nivel de la cicatriz de ampliaci&oacute;n del tracto de salida del ventr&iacute;culo derecho en pacientes operados de tetralog&iacute;a de Fallot (12), la aparici&oacute;n de bradi-taquiarritmias supraventriculares o insuficiencia cardiaca en paciente operados de transposici&oacute;n de grandes arterias con la t&eacute;cnica de Mustard/Senning (13,14) o isquemia coronaria con la t&eacute;cnica de Jatene (15,16).</font></p>      <p><font face="Verdana" size="2"><i>    <br> CARDIOPAT&Iacute;AS CONG&Eacute;NITAS SEVERAS</i></font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">Dentro de las cardiopat&iacute;as cong&eacute;nitas severas se encuentran las cian&oacute;ticas que no se han corregido o se ha hecho parcialmente, los shunt de gran tama&ntilde;o que han desarrollado s&iacute;ndrome de Eisenmenger (17), la estenosis valvular severa a&oacute;rtica y pulmonar debido a la dificultad que existe en mantener un gasto cardiaco adecuado en situaciones de aumento de la demanda ante un esfuerzo f&iacute;sico y la coartaci&oacute;n de aorta con gradiente &gt; 50 mmHg. Este grado de coartaci&oacute;n hace que un aumento de presi&oacute;n con el ejercicio pueda favorecer la disecci&oacute;n a&oacute;rtica, la ruptura a nivel de la zona coartada o eventos cerebrales (18). Algo similar ocurre en el s&iacute;ndrome de Marfan en el que existe una enfermedad de la media de la pared a&oacute;rtica lo cual favorece la dilataci&oacute;n a&oacute;rtica y la posibilidad de disecci&oacute;n/ruptura con el incremento de la tensi&oacute;n arterial y el gasto cardiaco (19-23 ).</font></p>    <p> <font face="Verdana" size="2">Las anomal&iacute;as coronarias y la disfunci&oacute;n ventricular por miocarditis o fibroelastosis endoc&aacute;rdica tambi&eacute;n constituyen un grupo de cardiopat&iacute;as en las que el ejercicio puede favorecer la aparici&oacute;n de muerte s&uacute;bita por fen&oacute;menos isqu&eacute;micos o arr&iacute;tmicos. (24,25).</font></p>    <p> <font face="Verdana" size="2">En estos pacientes con cardiopat&iacute;as severas tan solo se permitir&aacute;n las actividades recreativas de baja intensidad o las moderadamente intensas si la prueba de esfuerzo (con car&aacute;cter anual) no muestra peligro. Los deportes recreativos intensos o cualquier tipo de deporte de competici&oacute;n est&aacute;n prohibidos (9,11).</font></p>     <p> &nbsp;</p>     <p> <font face="Verdana"><B>Arritmias</B></font></p>     <p><font face="Verdana" size="2">Las arritmias constituyen un hallazgo frecuente en la evoluci&oacute;n a largo plazo de los pacientes con cardiopat&iacute;as cong&eacute;nitas. Dependiendo de la existencia o no de cardiopat&iacute;a estructural existir&aacute; mayor o menor limitaci&oacute;n a la pr&aacute;ctica deportiva.</font></p>     <p><font face="Verdana" size="2">Entre la arritmias m&aacute;s frecuentes que existen en la pr&aacute;ctica diaria encontramos las extras&iacute;stoles supraventriculares que cuando son asintom&aacute;ticas y no se asocian a cardiopat&iacute;a de base no producen ning&uacute;n tipo de limitaci&oacute;n deportiva. En cuanto a las taquicardias supraventriculares se recomienda la ablaci&oacute;n en la taquicardia intranodal y se considera mandataria la ablaci&oacute;n en los pacientes con pre-excitaci&oacute;n ventricular asociado a taquicardia por reentrada, fibrilaci&oacute;n auricular o flutter dada la mayor posibilidad de presentaci&oacute;n de muerte s&uacute;bita. Despu&eacute;s de 3 meses tras la ablaci&oacute;n, si no existen s&iacute;ntomas, recurrencias o cardiopat&iacute;a de base se podr&aacute; realizar cualquier tipo de ejercicio. En lo que respecta a la fibrilaci&oacute;n auricular, cuando esta es parox&iacute;stica, no presenta cardiopat&iacute;a de base y existe un ritmo sinusal estable en los &uacute;ltimos tres meses no contraindica la pr&aacute;ctica deportiva.</font></p>    <p> <font face="Verdana" size="2">En cuanto a las arritmias ventriculares, las extras&iacute;stoles ventriculares, la taquicardia ventricular monom&oacute;rfica no sostenida (TVNS), las taquicardias ventriculares fasciculares o las del tracto de salida del ventr&iacute;culo derecho, tampoco requieren limitaci&oacute;n de la pr&aacute;ctica deportiva siempre y cuando no exista asociado cardiopat&iacute;a estructural o arritmog&eacute;nica, antecedentes familiares de muerte s&uacute;bita, s&iacute;ntomas con el ejercicio, extrasistolia ventricular polim&oacute;rfica o frecuente con intervalo RR corto o TVNS con episodios m&uacute;ltiples. Sin embargo en aquellos pacientes con s&iacute;ndrome de Brugada o QT largo se contraindica cualquier tipo de deporte de competici&oacute;n (25,26).</font></p>    <p> <font face="Verdana" size="2">En los pacientes con implante de marcapasos que tengan una adecuada respuesta cronotr&oacute;pica con el ejercicio con buena funci&oacute;n ventricular y sin arritmias significativas de base se permitir&aacute;n los deportes de bajo-moderado componente din&aacute;mico con bajo componente est&aacute;tico, exceptuando los de contacto.</font></p>    <p> <font face="Verdana" size="2">Con respecto a las bradicardias los pacientes con frecuencia cardiaca inferior a 40 latidos/minuto o pausas sinusales mayores de 3 segundos deben interrumpir temporalmente el deporte si se acompa&ntilde;a de sintomatolog&iacute;a. Los pacientes con bloqueo auriculoventricular de primer grado y segundo grado tipo 1 podr&aacute;n realizar todo tipo de deportes, a diferencia de los que tienen un mayor grado de bloqueo, aquellos con bloqueo auriculoventricular de segundo grado tipo 2 o bloqueo completo, en los que se permitir&aacute; deportes con bajo-moderado componente est&aacute;tico y din&aacute;mico si est&aacute;n asintom&aacute;ticos, no presentan cardiopat&iacute;a estructural y la frecuencia basal est&aacute; por encima de los 40 latidos/minuto (25).</font></p>     ]]></body>
<body><![CDATA[<p> &nbsp;</p>     <p> <font face="Verdana"><B>Muerte súbita</B></font></p>     <p><font face="Verdana" size="2">El riesgo de muerte s&uacute;bita es otra de las causas que entra&ntilde;an siempre una preocupaci&oacute;n para el m&eacute;dico. La miocardiopat&iacute;a hipertr&oacute;fica constituye dentro de las cardiopat&iacute;as cong&eacute;nitas una de las causas m&aacute;s frecuentes. Es por ello que estos pacientes se clasifican como de alto riesgo conjuntamente con otras patolog&iacute;as que pueden producir muerte s&uacute;bita como son las anomal&iacute;as o la arteroesclerosis coronaria, el s&iacute;ndrome de Marfan, la displasia del ventr&iacute;culo derecho, alteraciones graves del ritmo, las cardiopat&iacute;as cong&eacute;nitas cian&oacute;genas o la hipertensi&oacute;n pulmonar (27-29).</font></p>     <p>&nbsp;</p>     <p><font face="Verdana"><B>Anticoagulación</B></font></p>     <p><font face="Verdana" size="2">Los pacientes que se encuentran anticoagulados por diferentes motivos (arritmias supraventriculares, disfunci&oacute;n ventricular, pr&oacute;tesis met&aacute;licas, hipertensi&oacute;n pulmonar idiop&aacute;tica, etc.) tambi&eacute;n deben evitar los traumatismos o golpes durante la realizaci&oacute;n de deportes dado el mayor riesgo hemorr&aacute;gico que tienen. Por eso se deben evitar los deportes de contacto (p. ej.: boxeo, k&aacute;rate). Igualmente a los pacientes con cuadros sincopales se les debe indicar que no practiquen deportes en los que un cuadro sincopal pueda poner en riesgo sus vidas (p. ej.: escalada, buceo, automovilismo).</font></p>     <p>&nbsp;</p>     <p><font face="Verdana"><b>Conclusiones</b></font></p>     <p><font face="Verdana" size="2">Las cardiopat&iacute;as cong&eacute;nitas son cada vez m&aacute;s una causa frecuente de consulta m&eacute;dica debido a la mayor y mejor expectativa de vida de estos pacientes. Reconocer a aquellos pacientes con mayor riesgo de presentar muerte s&uacute;bita y adecuar el deporte que pueden realizar es de vital importancia para reducir la mortalidad durante la pr&aacute;ctica deportiva y conseguir un buen desarrollo ps&iacute;quico y f&iacute;sico.</font></p>     <p>&nbsp;</p>     ]]></body>
