Es frecuente que se atribuya el origen al estómago o a problemas nasales o pulmonares. No obstante, sabemos que hasta en el 90% de los casos, el origen se encuentra en la cavidad oral, y de forma preponderante debido al acúmulo de placa en el dorso lingual "cubierta lingual". La enfermedad periodontal también está directamente relacionada con el mal aliento, de manera que el acumulo de placa, bolsas y/o sangrado son aspectos directamente relacionados con el problema. Desde el punto de vista patogénico, las proteínas de la dieta, las de las bacterias y las de los propios tejidos de la boca, son degradadas por la acción de las proteasas bacterianas hasta aminoácidos. Los aminoácidos ricos en azufre son los máximos responsables del mal aliento. De hecho los tres productos más directamente relacionados son el sulfuro de hidrógeno, el metil mercaptano y el dimetil sulfuro. En la presente revisión repasamos todos estos aspectos y las causas más directamente relacionadas con el problema, revisando igualmente el concepto de halitofobia y pseudohalitosis.]]></p></abstract>

Periodontal disease is also directly related to bad breath, so that the accumulation of plaque, periodontal pockets and/or bleeding are aspects directly related to the problem. <body><![CDATA[<p><font face="Verdana" size="2">La halitosis es una afectación bucal frecuente que compromete las relaciones sociales del paciente y en alguna ocasión puede encubrir un problema médico de relevancia (1). Si realizamos una búsqueda bibliográfica para evaluar la relevancia de esta entidad, nos encontramos que el problema preocupa a la población en general y también a la comunicad científica (<a href="#t1">Tabla 1</a>). Una manera de afrontar el problema es plantearnos tres preguntas: &#191;Por qué tenemos halitosis, qué es? &#191;Cuáles son las causas de la halitosis?</font></p>     <p>&nbsp;</p>     <p align=center><a name="t1"><img src="/img/revistas/odonto/v30n3/original6_tab_1.jpg"></a></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>&#191;Qué es y por qué tenemos halitosis?</b></font></p>     <p><font face="Verdana" size="2">El olor que emana de la cavidad bucal, igual que cualquier otro olor que detectamos, lo podemos detectar gracias al bulbo olfativo situado en la parte superior de las fosas nasales y cuyas células (capaces de detectar de 2.000 a 4.000 olores diferentes) enviarán la información. Esta información se procesará en el córtex cerebral de manera directa (superficie inferior del hemisferio cerebral que son las áreas olfatorias primarias: corteza entorrinal, corteza piriforme y periamigdalina, tubérculo olfatorio, amígdala, núcleo olfatorio anterior) y a través del tálamo (2). Hoy sabemos, mediante la cromatografía de gases, que hay unos 200 compuestos en el aliento y si unimos el aliento de varias personas puede subir hasta los 3.000 (3).</font></p>     <p><font face="Verdana" size="2">Desde hace unos años disponemos de una asociación que se preocupa del estudio y análisis del aliento porque, entre otras cosas, se cree que puede ayudar de manera importante a la medicina. El análisis del aliento se utiliza de manera rutinaria en múltiples situaciones, una de las más conocidas es su uso para evaluar la concentración de alcohol en los conductores. Pero también se utiliza para el diagnóstico y control en numerosas enfermedades, destacando el estudio del <i>Helicobacter pylori, </i>el estudio del asma o el cáncer de pulmón (4,5).</font></p>     <p><font face="Verdana" size="2">Este aliento depende de múltiples factores pero a nosotros nos interesa el mal aliento y éste se debe a varios grupos de compuestos:</font></p>     <blockquote>     <p><font face="Verdana" size="2">1. Compuestos orgánicos volátiles (COVs) de origen sistémico, como la urea, la acetona, el dimetil sulfuro, etc.</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">2. COVs de origen extrasistémico, como los fenoles, los índoles, la putrescina, la cadaverina, etc.</font></p>     <p><font face="Verdana" size="2">3. Compuestos orgánicos sulfurados (CSVs). En mucha menor medida también participan: Compuestos inorgánicos volátiles, como el óxido nítrico, oxígeno, sulfuro de carbono, etc. y los compuestos no volátiles, como: los isoprostanos, las citocinas, los leucotrienos, etc. (<a href="#t2">Tabla 2</a>) (6,7).</font></p> </blockquote>     <p>&nbsp;</p>     <p align=center><a name="t2"><img src="/img/revistas/odonto/v30n3/original6_tab_2.jpg"></a></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2">De todos ellos, sin duda, los más implicados son los compuestos sulfurados volátiles y de estos hay tres fundamentales (<a href="#t3">Tabla 3</a>). Estos tres productos representan el 90% del problema. Se producen a partir de la metabolización de los aminoácidos cisteína, metionina y cistina, provenientes de la putrefacción de la materia orgánica o de la degradación de las proteínas. Los detritus que se acumulan en la superficie lingual son uno de los mayores reservorios de estos productos y sabemos, por ejemplo, que el metil mercaptano se suele asociar a la enfermedad periodontal (8,9). Además, sabemos que la producción y liberación de estos productos depende de múltiples factores, destacando la población bacteriana (en especial G-anaerobios) y las condiciones fisicoquímicas: pH de la saliva, O<sub>2</sub>, sustratos disueltos en la saliva, fluido crevicular, etc. A la dieta se le atribuye un valor menos importante (10).</font></p>     <p>&nbsp;</p>     <p align=center><a name="t3"><img src="/img/revistas/odonto/v30n3/original6_tab_3.jpg"></a></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2">Podemos repasar brevemente el mecanismo de producción de esos compuesto sulfurados (11). Las proteínas de la dieta, las de las bacterias y las de los propios tejidos de la boca por la acción de las proteasas se degradan a péptidos. Estos péptidos se descomponen en aminoácidos con y sin azufre. Los que contienen azufre (cisteína, metionina y cistina), debido a la acción bacteriana, en especial las gram negativas, producen los compuestos sulfurados volátiles. Algunos de estos aminoácidos también se pueden encontrar disueltos en la boca. Todos estos compuestos son los responsables de la halitosis.</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">La producción de estos compuestos también dependerá del ecosistema salival, a modo de resumen, podemos decir que el aumento de proteínas y urea, es perjudicial. Igual sucede si aumenta la glucosa o carbohidratos en saliva (habitualmente es pobre) (12,13). Cuando disminuye el flujo salival el problema se incrementa y también conocemos mecanismos más complejos, como por ejemplo que la disminución de la actividad de &#946;-galactosidasa también es nocivo (14).</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Causas de la halitosis</b></font></p>     <p><font face="Verdana" size="2">Es un padecimiento que presenta el 50% de la población y es frecuente, incluso entre los profesionales sanitarios, que se opine que una causa importante del problema es el estómago (15). Si analizamos las diferentes causas, muy bien descritas por Aylikci &amp; Colak (16) (<a href="#t4">Tabla 4</a>). Podemos observar que en el 90% de los casos el problema se encuentra en la boca (17). A continuación iremos desgranando cada grupo de causas, pero la verdad es que de la mayoría de ellas no hay mucha evidencia científica, en muchas ocasiones son opiniones de expertos o casos clínicos (18). En la revisión de Aylikci &amp; Colak (16) nos refiere que el 90% se origina en la cavidad oral, el 8% es extraoral y un 2% ocasionado por otros problemas.</font></p>     <p>&nbsp;</p>     <p align=center><a name="t4"><img src="/img/revistas/odonto/v30n3/original6_tab_4.jpg"></a></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>1. Cavidad oral</b></font></p>     <p><font face="Verdana" size="2">Es el origen del que tenemos más evidencia científica, son variados los trabajos que nos refieren la relación de la halitosis con el incremento de bacterias, el consumo de péptidos y aminoácidos en la dieta, la disminución del cepillado (ocasiona incremento de los microorganismos) y el uso de antimicrobianos tópicos, disminuyen la halitosis. De estos últimos productos, algunos preparados tienen especial importancia porque no solo disminuyen el número de bacterias de manera global, sino que de manera específica actúan sobre aquellas que más relacionadas están con la halitosis (19-21).</font></p>     <p><font face="Verdana" size="2">En la boca, el problema son las bacterias que actúan sobre el sustrato de materia orgánica, la sangre es un factor agravante que además aumenta la supervivencia de las <i>Porphyromona gingivalis. Recordemos</i> que la simple presencia de sangre provoca incremente de halitosis por aumento de las bacterias proteolíticas en general (22,23).</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">Sin olvidar que la higiene es fundamental para el control de la halitosis, diferentes enfermedades o procedimientos quirúrgicos en la boca pueden ser también responsables (24) (<a href="#f1">Figura 1</a>). Por otro lado los abscesos de repetición, enfermedades sistémicas con repercusión oral, en especial si ocasionan sangrado, también pueden ser responsables (25,26) (<a href="#f2">Figura 2</a>). También en la literatura, generalmente en forma de casos clínicos encontramos, otras situaciones menos frecuentes, como el uso de piercings linguales (27,28) (<a href="#f3">Figura 3</a>).</font></p>     <p>&nbsp;</p>     <p align=center><a name="f1"><img src="/img/revistas/odonto/v30n3/original6_fig_1.jpg"></a></p>     <p>&nbsp;</p>     <p align=center><a name="f2"><img src="/img/revistas/odonto/v30n3/original6_fig_2.jpg"></a></p>     <p>&nbsp;</p>     <p align=center><a name="f3"><img src="/img/revistas/odonto/v30n3/original6_fig_3.jpg"></a></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>2. Otras causas</b></font></p>     <p><font face="Verdana" size="2">Bien, y el 10% que nos citaban Aylikci &amp; Colak (16), a que se debe. En primer lugar un 8% le podemos atribuir un origen orgánico atribuible a otros aparatos o sistemas. De ellos las más frecuentes son las alteraciones relacionadas con el tracto aéreo superior (nariz y senos paranasales) y de forma mucho menos importante, relacionadas con el tracto aéreo inferior (tráquea y parénquima pulmonar) (29). Suelen ser patologías infecciosas crónicas y las más frecuentes son la rinosinusitis y la amigdalitis crónica. La ocena suele cursar específicamente con mal olor (30). Dentro de este campo, aunque no es muy frecuente que se asocie a mal aliento, tienen especial relevancia para nosotros, los respiradores orales (31). Si nos centramos en la patología pulmonar, solo parece haber una evidencia clara en el caso del cáncer de pulmón (29). Las bronquiectasias clásicamente se han relacionado con la halitosis, pero es una entidad muy bien controlada en la actualidad (32-34).</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">A continuación, en orden de frecuencia, se encontraría la patología digestiva. Dentro de ésta una en la que parece que existe evidencia es la relacionada con el <i>Helicobacter pylori</i> (35), si bien todos los autores no le otorgan el mismo valor (36,37). Hoy sabemos que esta bacteria es la responsable en muchas ocasiones de una gastritis crónica y que incluso se le relaciona con la enfermedad periodontal (38). Un estudio retrospectivo indica una halitosis del 57%, pero es un estudio retrospectivo en cincuenta pacientes que evalúa muy poco el estado previo de la boca (35). Otra patología que podemos comentar es la hepatopatía, en el caso concreto de la cirrosis hepática se detecta sulfuro de hidrógeno, olor a limoneno, a ácidos alifáticos, a metilmercaptano, dimetil sulfuro y etanotiol (39). En la Insuficiencia renal crónica olor a urea) (40), en los trastornos intestinales (37,41), si bien no para todos los autores coincide esta relación (42) y de manera especial en la diabetes descompensada (con cetoacidosis) podemos encontrar un olor característico a cuerpos cetónicos (25,26, 43-45). Incluso se ha descrito una rara enfermedad metabólica, la hipermetiltionemia, que presenta un típico olor a pescado (46,47).</font></p>     <p><font face="Verdana" size="2">Un tema clásicamente relacionado con este cuadro, es el olor característico que tienen las mujeres durante la menstruación. Sólo hemos encontrado en MedLine-PubMed tres artículos que relacionen directamente estos dos cuadros (48-50). La relación no está clara, en un trabajo del 2010 (48), realizado en 10 mujeres sanas y 12 con periodontitis, los compuesto sulfurosos y el nivel de <i>Prebotella intermedia</i>, aumentó significativamente en los ciclos, pero sobre todo en las pacientes con enfermedad periodontal. Otro de los trabajos, realizado en el 2008 (49), que si que le otorga un valor importante a la relación, presenta una muestra muy pequeña. Finalmente, el tercer trabajo (50), del año 1978, al que no hemos tenido acceso, es frecuentemente referenciado en la literatura al relacionar la enfermedad periodontal y la halitosis (51,52).</font></p>     <p><font face="Verdana" size="2">Por otro lado diferentes alimentos y de manera muy especial el tabaco (53,54) y el alcohol también pueden ser los responsables (55,56). En este mismo grupo se incluyen algunos medicamentos que en general suelen contener azufre (57-61).</font></p>     <p><font face="Verdana" size="2">Dedicaremos unas palabras a hablar de la halitosis psicológica. La podemos clasificar en tres niveles:</font></p>     <blockquote>     <p><font face="Verdana" size="2">- La <i>pseudohalitosis</i>, causa de halitosis inexistente que suele solucionarse hablando con el paciente.</font></p>     <p><font face="Verdana" size="2">- La <i>halitofobia</i>, obsesión por el problema que constituye un trastorno siquiátrico, y</font></p>     <p><font face="Verdana" size="2">- El <i>síndrome de referencia olfativa</i>, que conforma alucinaciones olfativas asociadas a delirios de referencia (62).</font></p> </blockquote>     <p><font face="Verdana" size="2">Además, algunos pacientes confunden la disgeusia con el mal olor. Estos aspectos han sido ampliamente estudiados por diferente autores, destacando los trabajos de Yaegaki &amp; Coil (63,64) que hacen una magnifica revisión del problema. O los de Bosy (65) que ya en 1999 le otorga un valor del 4% y el de Filippi &amp; M&uuml;ller (66) que un trabajo de 144 pacientes encuentra un 84% de causa oral, un 3,5% de causa extra oral y un 12,5% de causa sicológica. Nosotros e un trabajo reciente, aceptado para publicar, hemos encontrado que, de 17 pacientes que creían tener mal olor (n= 98), 8 no lo tenían. En ninguno de ellos el problema pasaba más allá de la propia sensación del paciente y todos respondieron satisfactoriamente a las explicaciones del profesional (67).</font></p>     <p><font face="Verdana" size="2">Finalmente, a modo de conclusión, podemos decir que si bien hay múltiples causas de mal aliento, algunas de relevancia médica, la mayor parte de las veces su causa es oral, es por tanto el dentista el que debe estar muy implicado en el diagnóstico y tratamiento de este cuadro.</font></p>     ]]></body>
