<self-uri xlink:href="http://scielo.isciii.es/scielo.php?script=sci_arttext&amp;pid=S0376-78922018000200009&amp;lng=en&amp;nrm=iso"></self-uri><self-uri xlink:href="http://scielo.isciii.es/scielo.php?script=sci_abstract&amp;pid=S0376-78922018000200009&amp;lng=en&amp;nrm=iso"></self-uri><self-uri xlink:href="http://scielo.isciii.es/scielo.php?script=sci_pdf&amp;pid=S0376-78922018000200009&amp;lng=en&amp;nrm=iso"></self-uri><abstract abstract-type="short" xml:lang="es"><p><![CDATA[Resumen  Introducción y Objetivo Debido al alto costo y a la dificultad de adquisición de las matrices dérmicas acelulares u otros substitutos biológicos en nuestro medio para reforzar el polo inferior del músculo pectoral en reconstrucción mamaria inmediata, diferida o al sustituir el expansor por un implante definitivo, utilizamos matrices dérmicas autólogas (MDA) obtenidas de cicatrices abdominales, cesáreas previas, abdominoplastias o de la mama contralateral, a fin de evitar la extrusión protésica y preservar la marcación del surco submamario. Presentamos de esta manera una alternativa segura, real, sencilla y accesible que abarata costos, y nuestra experiencia en su uso en reconstrucción mamaria.  Material y método Empleamos MDA obtenida principalmente del área suprapúbica, de cicatrices anteriores de cesáreas o abdominoplastias, de la mama contralateral o eventualmente de otras regiones del cuerpo (torsoplastias), en todos los casos de reconstrucción mamaria inmediata o diferida, tanto en el primer como en el segundo tiempo operatorio Para su obtención, marcamos el área a resecar, decorticamos la piel, obtenemos el segmento preciso de dermis y la preservamos en solución fisiológica hasta el momento de su uso.  Resultados Registramos 33 pacientes: 7 con cirugías diferidas y 26 con reconstrucción inmediata. Solo en 3 obtuvimos la MDA de la mama contralateral, en 1 de una cicatriz dorsal de torsoplastia, y el resto de la región suprapúbica, de cicatrices de cesáreas, abdominoplastias o nuevas incisiones. Contabilizamos 1 caso de seroma inmediato en reconstrucción diferida que cedió al 7º día. Ningún caso de infección de la herida operatoria, dehiscencia, extrusión del implante o fracaso de la técnica en un seguimiento clínico de 1 año de postoperatorio.  Conclusiones En nuestra experiencia, el uso de MDA en reconstrucción mamaria representa una buena alternativa técnica, segura, accesible y de bajo coste, que no prolonga el tiempo operatorio y no supone ninguna complicación para la paciente en cuanto a su obtención y aplicación.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[ Background and Objective Due to the high cost and the difficulty in acquiring acellular dermal matrices or other biological substitutes in our medium, we use autologous dermis matrices (ADM) obtained from abdominal scars, previous caesarean incisions, abdominoplasties, contralateral breast, or other areas, during immediate o deferred breast reconstruction, in order to reinforce the lower pole of the pectoral muscle and to avoid prosthetic extrusion by ensuring the submammary fold. By using these autologous tissues we present a real, safe, simple and accessible alternative that lowers costs and present our experience on the use of ADM in breast reconstruction.  Methods We obtain ADM mainly from the suprapubic area, previous scars of caesarean incision, tummy tucks, contralateral breast and even from other regions of the body (torsoplasty), either in the first or in the second surgical stage, in immediately or deferred breast reconstruction. The acquisition of the dermal matrix is technically simple. We first mark the area to be resected, then we decorticate the skin to obtain the dermis segment and finally we preserve it in a physiological solution until time to be used.  Results Thirty three patients were registered: 7 with deferred surgeries and 26 with immediate reconstruction. We obtained the ADM from the contra lateral breast in 3 patients, from the thoracic region of a torso scar in 1 woman and from suprapubic region, caesarean incisions scars , tummy tucks or even new incisions in the rest of the patients. We only had 1 immediate seroma case that yielded to the 7th day in a patient from a deferred reconstruction None of the presented cases evolved with wound infection, dehiscence, implant extrusion or technique failure over a 1-year postoperative clinical follow-up.  Conclusions In our experience, the use of ADM in breast reconstruction represents a good alternative, safe, accessible and low economic technic, which does not prolong the surgical time and with no complications in obtaining and application.]]></p></abstract>
