La mastopexia es una de las cirugías más difíciles en el área de la estética mamaria ya que tiene que reposicionar la estructura mamaria, otorgar volumen y en ocasiones resecar tejidos. Suele tener un alto índice de complicaciones, pero la de mayor importancia es la necrosis del complejo areola-pezón (CAP) que se produce en el 2 al 5 % por fallo de la vascularización, no solo arterial sino también venosa. La elección de la técnica operatoria apropiada es fundamental, más aún en aquellas mamas que ya fueron operadas. La técnica de McKissock asegura en las gigantomastias una doble irrigación al CAP. El objetivo de nuestro trabajo es presentar nuestra experiencia con el uso de esta técnica de colgajo vertical bipediculado para prevenir la necrosis del CAP y la exposición del implante en caso de dehiscencia de la herida a nivel de la T invertida en la cirugía de pexia mamaria.  Material y método. Estudio descriptivo, retrospectivo y transversal de 71 pacientes sometidas a mastopexia primaria o secundaria con colgajo vertical bipediculado entre enero de 2019 y enero de 2023. Del grupo total se eligió al azar un grupo representativo al que se le realizó el cuestionario Breast Q adaptado y simplificado para evaluar en el postoperatorio la satisfacción personal, bienestar psicológico, social, sexual y físico.  Resultados. El grupo total se dividió en 4 grupos de estudio: Grupo 1: 5 mujeres con mastopexia con tejidos propios; Grupo 2: 31 con mastopexia con cambio de implantes; Grupo 3: 10 con mastopexia con retirada de implantes; y Grupo 4: 25 con mastopexia con implantes primarios. De las 71 pacientes, 46 tenían cirugía mamaria previa. La edad promedio fue de 37.94 años. Ninguna presentó necrosis o sufrimiento del CAP y 25 tuvieron dehiscencia pequeña de la T invertida, de las que 8 necesitaron sutura bajo anestesia local. Trece pacientes fueron sometidas al cuestionario Breast Q. Todas se sintieron muy cómodas y satisfechas en cuanto a los 5 módulos cuestionados.  Conclusiones. En nuestra experiencia, la técnica de reducción mamaria para gigantomastias de McKissock adaptada para mastopexias primarias y secundarias, resultó ser una opción terapéutica valida en mamas en las que no se conoce el patrón vascular o la técnica empleada con anterioridad. Reduce la probabilidad de necrosis del CAP y evita la exposición del implante en la T invertida en caso de dehiscencia. El grado de satisfacción personal también fue positivo en los 5 módulos cuestionados.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract  Background and objective. Mastopexy is one of the most difficult surgeries in breast aesthetic surgery since it has to reposition the breast structure, provide volume and sometimes resect tissues. It usually has a high rate of complications, but the most important is necrosis of the nipple-areola complex (NAC), which occurs in 2 to 5% due to failure of vascularization, not only arterial but also venous. Choosing the appropriate surgical technique is essential, even more in those breasts that have already been operated on. The McKissock technique ensures double irrigation to the NAC in gigantomastias. The objective of our study is to present our experience with the use of this bipedicled vertical flap technique to prevent necrosis of the NAC and the exposure of the implant in case of wound dehiscence at the level of the inverted T in mastopexy  Methods. Descriptive, retrospective, and cross-sectional study of 71 patients undergoing primary or secondary mastopexy with a bipedicled vertical flap between January 2019 and January 2023. From the total group, a representative group was randomly chosen and administered the adapted and simplified Breast Q questionnaire to evaluate personal satisfaction, psychological, social, sexual and physical well-being in the postoperative period.  Results. The total group was divided into 4 study groups: Group 1: 5 women with mastopexy with their own tissues; Group 2: 31 with mastopexy with implant change; Group 3: 10 with mastopexy with implant removal; and Group 4: 25 with mastopexy with primary implants. Of the 71 patients, 46 had previous breast surgery. The average age was 37.94 years. None presented necrosis or suffering of the NAC and 25 had small dehiscence of the inverted T, of which 8 required suturing under local anesthesia. Thirteen patients were submitted to the Breast Q questionnaire. All of them felt very comfortable and satisfied with the 5 modules questioned.  Conclusions. In our experience, the McKissock gigantomasty breast reduction technique adapted for primary and secondary mastopexies, turned out to be a therapeutic option valid in breasts in which the vascular pattern or the previously used technique is not known. Reduces the probability of necrosis of the NAC and avoids exposure of the implant in the inverted T in case of dehiscence. The degree of personal satisfaction was also positive in the 5 modules questioned.]]></p></abstract>
