El aumento mamario con implantes es una intervención muy frecuente en Cirugía Plástica, y el surco submamario (SSM) es una estructura anatómica importante en esta práctica. Este surco no solo delimita la parte inferior de la mama y el inicio del tórax submamario, sino que también puede variar en rigidez, constricción y posición según las características individuales de cada paciente y su raza. Presentamos la experiencia del autor principal en el manejo del SSM y proponemos una nueva clasificación con implicaciones clínicas y terapéuticas.  Material y método. Analizamos 426 procedimientos de mamoplastia de aumento realizados entre 2010 y 2017, incluyendo cirugías primarias, reemplazos de implantes y correcciones de deformidades secundarias. Clasificamos el SSM según su origen, posición y definición, adaptando el manejo quirúrgico a los hallazgos preoperatorios  Resultados. En el 23% de los casos analizados se modificó la posición del SSM mediante procedimientos de descenso, elevación y refuerzo. El enfoque propuesto ayudó en la toma de decisiones quirúrgicas.  Conclusiones. Considerar la ubicación de la mama en el tórax y evaluar el SSM, son aspectos críticos en la planificación del aumento mamario. En algunos casos, es necesario modificar este surco para asegurar una colocación adecuada del implante en relación con el complejo areola-pezón. Nuestro estudio proporciona una clasificación detallada del SSM considerando su posición, firmeza y adherencia a otras estructuras torácicas, lo que facilita una planificación más precisa y ayuda a prevenir posibles complicaciones.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract  Background and objective. Breast augmentation with implants is a common practice in Plastic Surgery, and the unframmary fold (IMF) plays a crucial role in this procedure. This fold not only demarcates the lower part of the breast and the beginning of the submammary chest, but it can also vary in firmness, constriction, and position depending on the individual characteristics of each patient and their racial background. Our study presents a medical center experience in managing the IMF and proposes a new classification with clinical implications.  Methods. A total of 426 augmentation mammoplasty procedures performed between 2010 and 2017, including primary surgeries, implant replacements, and corrections of secondary deformities, were analyzed.The IMF was classified based on its origin, position, and definition, and surgical management was tailored to preoperative findings.  Results: In 23% of cases, it was necessary to modify the position of the IMF through procedures such as descent, elevation, and reinforcement for enhanced definition. Our proposal approach facilitated surgical decision-making.  Conclusions. Consideration of breast location on the chest and evaluation of the IMF are critical aspects in planning breast augmentation. In some cases, its necessary to modify this fold to ensure proper implant placement in relation to the areola-nipple complex. This study provides a detailed classification of the IMF, considering its position, firmness, and adherence to other thoracic structures, thereby facilitating more precise planning, and helping to prevent potential complications.]]></p></abstract>
