La ritidoplastia en plano profundo extendida realizada a través de los espacios-cavidades propios de la cara, encuentra un plano de disección más seguro y de mayor aceptación para el promedio de los cirujanos, y más cuando se utiliza la visualización endoscópica a través de ellos gracias a la alta definición anatómica que se logra. Nos propusimos llevar las disecciones hasta el surco nasolabial, labiomentoniano y la línea media cervical, liberando todo el sistema fascioligamentario con ayuda del endoscopio, además de disminuir la extensión de las disecciones cutáneas.  Material y método. Estudio retrospectivo sobre pacientes operados entre 2022 y 2024, sometidos a ritidoplastia en plano profundo a través de los espacios-cavidades faciales y cervicales utilizando visualización endoscópica. Abordamos los espacios precigomático, premaséterico medio e inferior y cervical. La mayoría de los pacientes recibió anestesia general e infiltración vasoconstrictora.  Resultados. Recogemos una casuística de 70 pacientes sometidos a estiramiento facial con técnica de plano profundo endoscópico, con o sin extensión al cuello. En la mayoría de los casos primarios el exceso de piel cubría el pabellón auricular externo. El seguimiento promedio fue de 13 meses, con alta satisfacción en mejilla y cuello. No recogimos lesiones motoras ni sensitivas. Un caso presentó hematoma derecho en paciente hipertenso, que requirió drenaje inmediato.  Conclusiones. En nuestra experiencia, la ritidoplastia de plano profundo con o sin extensión al cuello proporciona resultados naturales, duraderos y con mayor seguridad vascular. Los avances en anatomía facial y la visualización endoscópica permiten disecciones precisas, minimizando riesgos y mejorando la identificación anatómica. Esta técnica llega a áreas profundas de la mejilla y cuello, permitiendo intervenciones más seguras y efectivas, adaptándose a la variabilidad anatómica de cada persona y cada proceso de envejecimiento. La visualización endoscópica facilita estrategias de acceso mínimas y aminora las cicatrices visibles.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract  Background and objective. Extended deep plane facelift performed through the facial spaces and cavities, finds a safer and more widely accepted dissection plane for the average surgeon, particularly when endoscopic visualization is used, thanks to the high definition anatomical views it ensures. We aimed to extend the dissections to the nasolabial groove, labiomental fold, and cervical midline, releasing the entire fascio-ligamentous system with the aid of the endoscope, in addition to reducing the extent of skin dissections.  Methods. Retrospective study on patients operated between 2022 and 2024 who underwent deep plane rhytidectomy through facial and cervical spaces-cavities using endoscopic visualization. The procedures involved approaches in the precigomatic, middle and lower premaseteric spaces, and the cervical area. Most patients received general anesthesia and vasoconstrictor infiltration. The technique allowed for excellent anatomical visualization, facilitating precise dissection, advancement, and release of anatomical structures.  Results. We compiled a case series of 70 patients who underwent facelift using the endoscopic deep plane technique, with or without neck extension. In most primary cases excess skin covered the external auricular pavilion. The average follow-up period was 13 months, with high satisfaction in the cheek and neck areas. No motor or sensory injuries were reported. One case involved a right-side hematoma in a hypertensive patient, which required immediate drainage.  Conclusions. In our experience, deep plane facelift, with or without neck extension, provides natural, long-lasting results and with greater vascular safety. Advances in facial anatomy and endoscopic visualization enable precise dissections, minimizing risks and improving anatomical identification. This endoscopic technique reaches deep areas of the cheek and neck, allowing for safer and more effective interventions, adapting to the anatomical variability of each person and aging processes. Endoscopic visualization facilitates minimal access strategies and avoids visible scars.]]></p></abstract>
