El aumento mamario es una de las cirugías estéticas más realizadas a nivel mundial. La elección del plano adecuado para la colocación del implante es crucial para optimizar los resultados y reducir complicaciones. Se han descrito múltiples planos de colocación, cada uno con ventajas y desventajas. La técnica Tri-B-Plane se presenta como una evolución del plano dual (dual plane), integrando 3 conceptos clave: mejora de la cobertura del polo inferior, división estratégica del músculo pectoral y un tercer plano de cobertura; todos con la finalidad de optimizar la estabilidad, la cobertura y la proyección del implante.  Material y método. Descripción de la técnica quirúrgica y estudio retrospectivo, descriptivo y transversal en 67 pacientes sometidas a aumento mamario con la técnica Tri-B-Plane entre enero de 2020 y junio de 2024 en una clínica privada de Lima, Perú. Se excluyeron pacientes con recambio de implantes o mastopexia con aumento. Se documentaron características clínicas, técnica quirúrgics, tipo de implantes utilizados y complicaciones postoperatorias.  Resultados. Las pacientes tuvieron una edad promedio de 28 años. El 76.1% presentaba hipoplasia mamaria sin ptosis, y el 23.9% tenía mamas tuberosas. Se utilizaron implantes redondos, predominantemente lisos y de perfil alto, con volumen promedio de 295 cc. Se realizó acceso periareolar en todos los casos y el 77.6% de las cirugías se llevó a cabo con anestesia local y sedación. Las complicaciones fueron mínimas, con un 2.9% de asimetría del surco submamario y un 1.5% de dehiscencia parcial.  Conclusiones. La técnica Tri-B-Plane que describimos ofreció en nuestra serie de pacientes sometidas a mamoplastia de aumento con implantes, una cobertura óptima del implante mediante una estructura multiplanar, que contribuye a mejorar la estabilidad, la proyección mamaria y a reducir complicaciones como la mama dinámica. Su aplicación es versátil, especialmente en pacientes con mama tuberosa, deportistas o con necesidades específicas de cobertura y soporte. <abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract  Background and objective. Breast augmentation is one of the most performed aesthetic surgeries worldwide. Choosing the appropriate plane for implant placement is crucial to optimize results and reduce complications. Multiple placement planes have been described, each with advantages and disadvantages. The Tri-B-Plane technique is presented as an evolution of the dual plane, integrating 3 key concepts: improved coverage of the inferior pole, strategic division of the pectoralis muscle, and a third plane of coverage, all with the aim of optimizing implant stability, coverage, and projection.  Methods. Surgical technique is described and a retrospective, descriptive, cross-sectional study was performed on 67 patients who underwent breast implant augmentation with the Tri-B-Plane technique between January 2020 and June 2024 in a private clinic in Lima, Peru. Patients with implant replacement or mastopexy with augmentation were excluded. Clinical characteristics, surgical techniques, implant type, and postoperative complications were documented.  Results. The patients had a mean age of 28 years; 76.1% had breast hypoplasia without ptosis, while 23.9% had tuberous breasts. Round, predominantly smooth, high-profile implants were used, with an average volume of 295 cc. Periareolar access was performed in all cases, and 77.6% of surgeries were performed under local anesthesia and sedation. Complications were minimal, with 2.9% asymmetry of the submammary sulcus and 1.5% partial dehiscence.  Conclusions. The Tri-B-Plane technique offered in our group of patients who underwent breast augmentation with implants, optimal implant coverage by means of a multiplanar structure which contributes to improve stability, breast projection and reduce complications such as dynamic breast.
