La reconstrucción mamaria es indispensable para lograr un tratamiento integral en el cáncer de mama. El cuestionario BREAST-Q, validado por la Food and Drugs Administration (FDA) de los EE.UU., se utiliza ampliamente para evaluar la calidad de vida y la satisfacción con dicha reconstrucción. La literatura internacional sugiere que la reconstrucción autóloga tiende a tener resultados superiores en términos de calidad de vida en comparación con la aloplástica. Sin embargo, existe poca evidencia acerca de este tema en la población colombiana. El objetivo de este estudio es evaluar el impacto de dos tipos de reconstrucción mamaria: alopástica y con colgajo DIEP (colgajo de arteria perforante epigástrica inferior profunda - deep inferior epigastric perforator artery) en la calidad de vida de las pacientes colombianas, con el fin de generar conocimiento que contribuya a la toma de decisiones clínicas en Colombia y América Latina.  Material y método. Estudio transversal descriptivo, en el que se administró el cuestionario BREAST-Q de forma telefónica mediante el software UContact 5 a pacientes con antecedentes de cáncer de mama en la Clínica de los Nogales en Bogotá, Colombia, que finalizaron reconstrucción con colgajo DIEP o expansor/implante mamario entre 2017 y 2021. Los datos se analizaron utilizando el software IBM-SPSS versión 30 bajo la licencia de la Universidad El Bosque.  Resultados. Se realizaron 30 entrevistas, 15 por cada tipo de reconstrucción. El tiempo medio transcurrido desde la reconstrucción fue de 4.56 años (DE ± 1.45). La reconstrucción aloplástica tuvo un mejor puntaje en bienestar psicosocial (DM 13.47) y levemente una mejor puntuación en bienestar sexual (DM 1.12) y bienestar físico del tórax (DM 3.63), mientras que la reconstrucción con colgajo DIEP mostró ligeramente un mejor puntaje en cuanto a la satisfacción con el resultado (DM 2.63). En ambos tipos de reconstrucción se evidenció que la radioterapia disminuía el puntaje en la mayoría de los aspectos evaluados.  Conclusiones. Este estudio sugiere que, basados en la encuesta BREAST-Q, las mujeres colombianas de nuestro grupo de estudio con reconstrucción aloplástica después de 3 a 6 años de postoperatorio, tienen mejores puntajes en el bienestar psicosocial en comparación con las reconstruidas con colgajo DIEP. <abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract  Background and objective. Breast reconstruction is essential for achieving comprehensive breast cancer treatment. The BREAST-Q questionnaire, validated by the Food and Drugs Administration (FDA) in USA, is widely used to assess quality of life and satisfaction with reconstruction. International literature suggests that autologous reconstruction tends to yield superior outcomes in terms of quality of life compared to alloplastic reconstruction. However, there is limited evidence available in the Colombian population. The objective of this study is to assess the impact of two types of breast reconstruction, alloplastic and with DIEP (deep inferior epigastric perforator artery) flap on the quality of life of Colombian patients, aiming to generate knowledge that contributes to clinical decision-making in Colombia and Latin America.  Methods. A descriptive cross-sectional study was conducted, where the BREAST-Q questionnaire was administered via telephone using the UContact 5 software to patients with a history of breast cancer at the Clínica de Los Nogales in Bogotá, Colombia, who completed reconstruction with DIEP flap or expander/breast implants between 2017 and 2021. Data were analyzed using IBM-SPSS software version 30 under the license of Universidad El Bosque.  Results. Thirty interviews were conducted, 15 for each type of reconstruction. The mean time that had elapsed since reconstruction was 4.56 years (SD ± 1.45). Alloplastic reconstruction scored better in psychosocial well-being (MD 13.47), and slightly better in sexual well-being (MD 1.12) and physical chest well-being (MD 3.63), meanwhile, DIEP flap reconstruction showed barely better scores in satisfaction with the outcome of the breasts (MD 2.63). In both types of reconstruction, radiotherapy was found to decrease scores in most aspects evaluated.  Conclusions. This study suggests that based on the BREAST-Q survey, Colombian women of our study group with alloplastic reconstruction 3 to 6 years postoperatively, have better psychosocial well-being scores compared to those reconstructed with DIEP flap.
