Dado al envejecimiento general de la población mundial, cada vez será más frecuente la Cirugía Plástica en pacientes mayores de 60 años. Si bien se habla de mayor morbimortalidad en pacientes de la tercera edad, existen pocos estudios al respecto en esta especialidad. Su heterogeneidad no ha permitido predecir qué sujetos son más vulnerables al estrés del perioperatorio y presentarán mayor posibilidad de eventos adversos. Por ello, nuestro objetivo ha sido caracterizar a esta población y sus complicaciones postquirúrgicas en una muestra de población Chilena.  Material y método. Análisis retrospectivo de fichas clínicas entre 2015 - 2020 de pacientes mayores de 60 años de edad sometidos a Cirugía Plástica, en el Hospital Clínico de la Fuerza Aérea de Chile, en Santiago (Chile), determinando sus principales características, comorbilidades, y tipos de complicaciones.  Resultados. Analizamos 187 pacientes, de los que 86 correspondieron a Cirugía Estética y 101 a Reconstructiva. Los pacientes de Cirugía Estética fueron sometidos a cirugías cérvicofaciales y procedimientos menores (lipomas, quistes epidérmicos, lunares y lóbulos rasgados) en un 80%, mientras que, en Cirugía Reconstructiva, la intervención más frecuente fue la de colgajo con un 28%, seguida de la plastia de cicatriz con un 25%. Nuestra serie tuvo un 19.8% de complicaciones totales, correspondiendo un 2.3% a Cirugía Estética y un 34% a Reconstructiva. Mas de la mitad de las complicaciones se concentraron en pacientes mayores de 80 años. Todas las reoperaciones fueron en Cirugía Reconstructiva. No hubo casos de fallecimiento. El análisis multivariado mostró una asociación estadísticamente significativa entre complicaciones y riesgo anestésico elevado o pacientes sometidos a Cirugía Reconstructiva.  Conclusiones. La edad, por sí misma, no fue un factor determinante de complicación en nuestra serie. En base a los resultados obtenidos en nuestra población de estudio consideramos que es posible plantear la Cirugía Estética con cierta seguridad en el adulto mayor, considerando operar al paciente con sus patologías de base compensadas y con buena reserva fisiológica. Es fundamental, además, considerar la prehabilitación en cirugías reconstructivas según corresponda, para así disminuir la morbimortalidad en este grupo etáreo. <abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract  Background and objective. Given the general aging of the global population, Plastic Surgery will increasingly be performed on patients over 60 years of age. Although there is talk of higher morbidity and mortality in older patients, few studies exist in this specialty. Its heterogeneity has not allowed us to predict which subjects are more vulnerable to perioperative stress and more likely to experience adverse events. Therefore, our objective was to characterize this population and their postoperative complications in a sample of the Chilean population.  Methods. A retrospective analysis of clinical records of patients older than 60 years who underwent Plastic Surgery between 2015 and 2020, was carried out in Hospital Clínico de la Fuerza Aérea de Chile, Santiago (Chile), determining their main characteristics, comorbidities, and type of complications.  Results. One hundred and eighty-seven patients were analyzed; 86 corresponded to Aesthetic Surgery, and 101 to Reconstructive Surgery. Aesthetic Surgery patients underwent cervicofacial surgeries and minor procedures (lipomas, epidermal cysts, moles, torn earlobes) in 80%, while in Reconstructive Surgery, the most frequent surgery was flap surgery in 28%, followed by scar-plasty in 25%. We had a total of 19.8% complications, 2.3% of aesthetic surgeries and 34% of reconstructive surgeries. More than half of the complications were concentrated in patients over 80 years of age. All re-interventions were in reconstructive surgeries. There were no deaths. The multivariate analysis showed an independent statistically significant association between complications and patients with high anesthetic risk, or patients undergoing Reconstructive Surgery.  Conclusions. Age, by itself, was not a determining factor for complications. Based on the results obtained in our study group, we conclude it is possible to safely consider Aesthetic Surgery in the elderly patient with compensated underlying disease and with adequate physiological reserve.
