Las quemaduras eléctricas siguen siendo una causa importante de morbimortalidad en Colombia, y hasta donde conocemos, no se tienen datos precisos sobre las quemaduras eléctricas que afectan a nivel plantar. La reconstrucción en esta zona es compleja dado el poco tejido donante en la zona, las características especiales y únicas de la piel plantar y la microanatomía. El objetivo del presente estudio es compartir nuestra experiencia en el uso de colgajos tipo keystone para reconstrucción de defectos por quemaduras eléctricas en la región plantar.  Material y método. Serie de 10 pacientes con defectos de cobertura plantar secundarios a quemaduras eléctricas, de 13.4 cm2 de diámetro promedio, con media de edad de 46 años, atendidos en la USS Simón Bolívar, en Bogotá, Colombia. A todos se le realizaron colgajos tipo keystone IIA y tipo IV.  Resultados. En todos los casos se logró la cobertura de los defectos. Solo hubo una complicación menor. Se consideró un resultado funcional y estético adecuado dado que se preservó el principio de reconstrucción &#8220;igual con igual&#8221; (like-to-like), es por ello que, en nuestro grupo de estudio, el colgajo keystone resultó ser una alternativa muy útil y ventajosa frente a otras opciones reconstructivas.  Conclusiones. Los colgajos para cobertura de defectos plantares pueden suponer una limitante para la reconstrucción del pie; con los colgajos keystone pudimos cubrir zonas de defecto a cualquier nivel de la planta del pie y de cualquier tamaño con una mínima morbilidad asociada. <abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract  Background and objective. Electrical burns are an important cause of morbidity and mortality in Colombia. To our knowledge, there is no accurate data on electrical burns at the sore foot. Reconstruction in this area represents a real challenge due to the lack of donor tissue, the special characteristics and unique plant skin microanatomy. The aim of this study is to share our experience in the use of keystone flaps for reconstruction of electrical burn defects in the plantar region.  Methods. We present a case series of 10 patients with coverage defects at the plantar level caused by electrical burns of an average diameter of 13.4cm2, with an average patients age of 46 years, treated at the USS Simón Bolívar in Bogotá, Colombia. All patients were treated with type IIA and type IV keystone flaps.  Results. Coverage of soft tissue defects at the plantar level was achieved with the design of keystone flaps. There was not any major complication, and it is considered a functional and aesthetic good result given that the &#8220;like-to-like&#8221; principle is preserved. In our series of patients, this flap was a very useful and advantageous alternative to other reconstructive options.  Conclusions. In our experience, flaps to cover plantar defects can be a limitation for foot reconstruction; keystone flaps can cover defective areas at any level and of any size with minimal associated morbidities.
