Presentamos un paciente de 70 años con carcinoma escamoso en región cigomática que, tras resección quirúrgica, presentó un defecto extenso de 14 x 8 cm. Se optó por reconstrucción con colgajo microquirúrgico basado en la perforante medial de la arteria circunfleja ilíaca superficial, una técnica novedosa en nuestro centro asistencial en Argentina. La planificación prequirúrgica se realizó mediante angiotomografía y ultrasonido doppler. La cirugía, realizada por un equipo que incluyó residentes en formación, se ejecutó sin complicaciones con una duración de 3 horas. El postoperatorio fue exitoso, tanto estética como funcionalmente. <abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract Facial reconstruction following tumor resection is a significant challenge due to the aesthetic and functional importance of the region. We present a 70-year-old patient with squamous cell carcinoma in the zygomatic region, who after surgical resection developed an extensive defect measuring 14 x 8 cm. Reconstruction was performed using a microvascular flap based on the medial perforator of the superficial circumflex iliac artery, a novel technique in our hospital in Argentina. Preoperative planning was carried out using angiotomography and doppler ultrasound. The surgery, performed by a team that included training residents, was completed without complications lasting 3 hours. The postoperative period showed successful results both aesthetically and functionally. Follow-up included the use of thermography to assess the vitality of the flap.
