La reconstrucción mamaria autóloga inmediata con colgajos libres, especialmente el colgajo DIEP (perforante epigástrica profunda inferior - deep inferior epigastric perforator), ha demostrado ser una técnica fiable y duradera. Sin embargo, la literatura que combina reconstrucción inmediata y técnicas autólogas microquirúrgicas es limitada, especialmente en medios de habla hispana. Presentamos la experiencia acumulada durante 10 años en un hospital de referencia del Sistema Público de Salud Español evaluando la evolución de la técnica, los resultados obtenidos y los factores que han influido en la optimización de los mismos con el fin de contribuir a la evidencia existente sobre reconstrucción mamaria inmediata autóloga.  Material y método. Estudio observacional retrospectivo en mujeres intervenidas entre 2013 y 2023 en el Hospital Universitario Virgen Macarena de Sevilla, España. Analizamos datos demográficos, quirúrgicos y resultados de 140 pacientes sometidas a mastectomía con reconstrucción inmediata mediante colgajos libres, principalmente DIEP. Aplicamos pruebas estadísticas para evaluar factores de riesgo y complicaciones.  Resultados. Se realizaron 168 colgajos libres: 160 DIEP, 6 PAP (perforante femoral profunda-profunda artery perforator) y 2 SIEA (epigástrica inferior superficial-superficial inferior epigastric). La supervivencia de los colgajos fue del 92.9% (95% en el caso de los DIEP). La introducción del verde de indocianina redujo significativamente la necrosis grasa (p=0.02). El tiempo quirúrgico disminuyó con la experiencia del equipo (p&lt;0.001). Se logró un 80% de resultados al menos aceptables y un 86% de satisfacción entre las pacientes de alta.  Conclusiones. En nuestra experiencia de 10 años, la reconstrucción mamaria autóloga con colgajos libres, con el DIEP a la cabeza, si bien requirió una curva de aprendizaje del equipo quirúrgico ofreció, resultados fiables y duraderos cuando se realizó de forma inmediata, especialmente en casos que necesiten radioterapia. <abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract  Background and objective. Immediate autologous breast reconstruction with free flaps, particularly the DIEP flap (deep inferior epigastric perforator), has proven to be a reliable and long-lasting technique. However, literature combining immediate reconstruction and microsurgical autologous techniques is limited, especially in Spanish-speaking settings. We present our 10-year experience in a referral care hospital within the Spanish Public Health System, evaluating the evolution of the technique, the outcomes achieved, and the factors that have contributed to their optimization with the aim of providing additional evidence on immediate autologous breast reconstruction.  Methods. A retrospective observational study was conducted on women who underwent surgery between 2013 and 2023 at Hospital Universitario Virgen Macarena, Sevilla, Spain. Demographic, surgical, and complication data were analyzed from 140 patients who underwent mastectomy with immediate free flap reconstruction, primarily DIEP. Statistical tests were applied to assess risk factors and complications.  Results.
