<body><![CDATA[<p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Introducci&oacute;n</b></font></p>     <p><font face="Verdana" size="2">La alveolitis seca es la complicaci&oacute;n postoperatoria m&aacute;s frecuente despu&eacute;s de una extracci&oacute;n dental<sup>1</sup>, con una frecuencia de aparici&oacute;n com&uacute;nmente entre el 1 al 5%<sup>2,3</sup>, aunque existen reportes de hasta un incidencia del 70%<sup>4</sup> de los pacientes en la pr&aacute;ctica dental. Se ha aceptado que la alveolitis tiene una etiolog&iacute;a multifactorial (sist&eacute;mica y local)<sup>5</sup>, lo que a su vez ha producido que se enumeren diferentes opciones de tratamiento<sup>6</sup>.</font></p>     <p><font face="Verdana" size="2">Actualmente el manejo de la alveolitis est&aacute; enfocado a aliviar la sintomatolog&iacute;a que refiere el paciente, mediante el uso de agentes paliativos, enjuagues<sup>7</sup> e incluso f&aacute;rmacos. Sin embargo, hasta el momento no existe un consenso establecido para su manejo, pues su exacta patog&eacute;nesis ha sido a&uacute;n poco estudiada<sup>8,9</sup>.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Definici&oacute;n y terminolog&iacute;a</b></font></p>     <p><font face="Verdana" size="2">Algunos autores la definen como un proceso inflamatorio<sup>4,10</sup> y otros como infeccioso<sup>11,12</sup>, pero todos concuerdan en que se presenta de forma dolorosa y localizada en la zona donde se realiz&oacute; la exodoncia dental<sup>1-13</sup>. Es reversible, superficial y de aparici&oacute;n tard&iacute;a, pues aparece entre 2 a 4 d&iacute;as despu&eacute;s de la extracci&oacute;n dentaria<sup>6</sup>.</font></p>     <p><font face="Verdana" size="2">Su m&aacute;s reciente concepci&oacute;n fue descrita por Blum<sup>14</sup> en 2002, quien la describe como un dolor postoperatorio dentro y alrededor del lugar de la extracci&oacute;n que aumenta en severidad entre el primer y tercer d&iacute;a postextracci&oacute;n, pudiendo estar acompa&ntilde;ado por la desintegraci&oacute;n total o parcial del co&aacute;gulo alveolar con o sin halitosis.</font></p>     <p><font face="Verdana" size="2">Se han establecido diferentes definiciones para la alveolitis desde que Crawford la describiera por primera vez en 1896 como alv&eacute;olo seco<sup>15</sup>. Otros t&eacute;rminos usados son: "oste&iacute;tis alveolar localizada", "alveolitis fibrionol&iacute;tica", alv&eacute;olo "necr&oacute;tico", "alveolagia"<sup>4,10,14</sup>, "alveolitis sicca dolorosa"<sup>16</sup>, alveolitis postoperatoria, alv&eacute;olo s&eacute;ptico y osteomielitis localizada<sup>14,17</sup>.</font></p>     <p>&nbsp;</p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2"><b>Clasificaci&oacute;n</b></font></p>     <p><font face="Verdana" size="2">Hupp<sup>10</sup> la divide en 2 entidades:</font></p>     <p><font face="Verdana" size="2">- Alveolitis seca primaria: corresponde a una periostitis del alv&eacute;olo desnudo. Los pacientes no sangran despu&eacute;s de la exodoncia; es inmediata.</font></p>     <p><font face="Verdana" size="2">- Alveolitis seca secundaria: es mediata, present&aacute;ndose entre el segundo y cuarto d&iacute;a posterior a la exodoncia. Hay p&eacute;rdida total del co&aacute;gulo. El paciente presenta mal sabor, dolor intenso e irradiado con linfadenopat&iacute;a y halitosis poco marcada.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Fisiopatolog&iacute;a de la alveolitis</b></font></p>     <p><font face="Verdana" size="2">Despu&eacute;s de la extracci&oacute;n dental el alv&eacute;olo debe llenarse completamente de sangre para que se forme un co&aacute;gulo sangu&iacute;neo que se organizar&aacute; a partir del tercer d&iacute;a<sup>18</sup>. Durante el proceso de la alveolitis existe una ausencia de vasos sangu&iacute;neos, que no permite la proliferaci&oacute;n de capilares, y por ende, no se organiza el co&aacute;gulo sangu&iacute;neo que f&aacute;cilmente se desintegra<sup>6</sup>. La p&eacute;rdida del co&aacute;gulo sangu&iacute;neo expone el hueso alveolar al aire, a los alimentos y los l&iacute;quidos, causando dolor<sup>18</sup>. Por su parte, el proceso infeccioso disminuye el tope de neoformaci&oacute;n &oacute;sea<sup>1</sup>, con lo cual la cicatrizaci&oacute;n del alv&eacute;olo se ve retrasada.</font></p>     <p><font face="Verdana" size="2">Gay Escoda<sup>19</sup> considera que, durante la alveolitis, se produce un estado necr&oacute;tico del proceso alveolar que ante la ausencia de vasos sangu&iacute;neos no permite la formaci&oacute;n de capilares ni de tejido de granulaci&oacute;n, claves para la organizaci&oacute;n del co&aacute;gulo sangu&iacute;neo.</font></p>     <p><font face="Verdana" size="2">El potencial de cicatrizaci&oacute;n del hu&eacute;sped determinar&aacute; la severidad y duraci&oacute;n de esta condici&oacute;n<sup>5</sup>. El traumatismo tisular produce una liberaci&oacute;n de quininas, que sumado a otros mediadores de dolor liberados por la exposici&oacute;n &oacute;sea y a un posible proceso infeccioso desencadena el malestar caracter&iacute;stico de este cuadro<sup>18</sup>.</font></p>     <p>&nbsp;</p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2"><b>Etiolog&iacute;a</b></font></p>     <p><font face="Verdana" size="2">Se han enumerado diversos factores de riesgo asociados a esta complicaci&oacute;n dental.</font></p>     <blockquote>     <p><font face="Verdana" size="2">- Edad: se cree que la edad avanzada podr&iacute;a predisponer al desarrollo de alveolitis, pues en los pacientes adultos el ligamento periodontal es m&aacute;s delgado y posee menor irrigaci&oacute;n<sup>19</sup>.</font></p>     <p><font face="Verdana" size="2">- Sexo y terapia hormonal: estudios revelan que la incidencia de alveolitis es mayor en el sexo femenino que en el masculino<sup>2,20</sup>. Un estudio realizado en 2013<sup>21</sup> evidenci&oacute; que las mujeres que se encontraban entre la segunda y tercera semana de su ciclo menstrual presentaban una mayor incidencia de alveolitis, independientemente de si usaban o no anticonceptivos orales. Se presume que durante la menstruaci&oacute;n los niveles de estr&oacute;geno aumentan, y con ello la actividad fibrinol&iacute;tica dentro del alv&eacute;olo<sup>22,23</sup>, lo cual ocurre tambi&eacute;n con el uso de terapia hormonal<sup>24</sup>.</font></p>     <p><font face="Verdana" size="2">- Zona operatoria: existe una mayor incidencia de alveolitis en piezas dentarias mandibulares, 2,5 veces mayor que en las piezas maxilares<sup>20</sup>. Esto podr&iacute;a atribuirse a que la maxila tiene una mejor irrigaci&oacute;n, menor densidad &oacute;sea y mayor capacidad de producci&oacute;n de tejido de granulaci&oacute;n<sup>19</sup>.</font></p>     <p><font face="Verdana" size="2">-El consumo de tabaco: se ha encontrado en la literatura una mayor incidencia de alveolitis en pacientes fumadores<sup>25-28</sup>, as&iacute; como de desarrollar una posible infecci&oacute;n<sup>25</sup>. El estudio de Bortoluzzi et al.<sup>27</sup> encontr&oacute; una OR de 12,3 en pacientes que fuman m&aacute;s de 20 cigarrillos al d&iacute;a. Se presume que el aumento de temperatura intraoral y el h&aacute;bito de succi&oacute;n durante el acto de fumar podr&iacute;a repercutir en el proceso normal de cicatrizaci&oacute;n. Por otro lado, la falta de cuidado postoperatorio en este tipo de pacientes tambi&eacute;n podr&iacute;a desempe&ntilde;ar un rol importante<sup>21</sup>.</font></p>     <p><font face="Verdana" size="2">- El tipo de cirug&iacute;a podr&iacute;a originar alveolitis: seg&uacute;n diversos autores existe una correlaci&oacute;n positiva entre la cantidad de traumatismo durante la cirug&iacute;a y la aparici&oacute;n de sintomatolog&iacute;a dolorosa<sup>20,28</sup>. Bortoluzzi et al. asociaron a la t&eacute;cnica de seccionar la pieza dentaria con la aparici&oacute;n de alveolitis seca<sup>27</sup>. En otro de sus estudios<sup>29</sup> se encontr&oacute; que periodos posoperatorios dolorosos mayores a 2 d&iacute;as estaban asociados a extracciones dentales mayores a 30 min. El da&ntilde;o quir&uacute;rgico conlleva la liberaci&oacute;n de diferentes agentes en los tejidos y la inoculaci&oacute;n de agentes bacterianos, factores predisponentes para el inicio de actividades fibrinol&iacute;ticas locales<sup>30</sup>. Por otro lado, la falta de experiencia del cirujano podr&iacute;a desempe&ntilde;ar un papel crucial para que se desarrolle la alveolitis<sup>2</sup>.</font></p>     <p><font face="Verdana" size="2">- Infecciones previas como la periodontitis<sup>31</sup> y la pericoronaritis podr&iacute;an predisponer al desarrollo de alveolitis<sup>32</sup>. Halabi et al.<sup>33</sup> encontraron en su estudio una OR de 3,3 al asociar la presencia de infecci&oacute;n previa en el sitio operatorio con la aparici&oacute;n de alveolitis. Los microorganismos dentro de los alv&eacute;olos infectados consisten fundamentalmente en bacilos fusiformes, espiroquetas y estreptococos; adem&aacute;s, los restos del ligamento periodontal desvitalizados, junto a la escasa irrigaci&oacute;n sangu&iacute;nea, constituyen las circunstancias favorecedoras del crecimiento bacteriano<sup>34</sup>.</font></p>     <p><font face="Verdana" size="2">- El uso de anest&eacute;sicos: aunque no hay evidencia de que el tipo de anest&eacute;sico pueda aumentar la incidencia de la alveolitis, existe la posibilidad que el uso excesivo de vasoconstrictores podr&iacute;a retardar la cicatrizaci&oacute;n al disminuir el sangrado y la tensi&oacute;n del ox&iacute;geno aumentando la fibrin&oacute;lisis. El uso y n&uacute;mero de cartuchos ejercer&iacute;a tambi&eacute;n un papel importante en el desarrollo de la alveolitis<sup>21</sup>. Adem&aacute;s, el condicionamiento anat&oacute;mico vascular mandibular predispone a un mayor riesgo de infecci&oacute;n, por lo tanto deben evitarse situaciones que favorezcan los estados isqu&eacute;micos, como el empleo de anest&eacute;sicos vasoconstrictores y la infiltraci&oacute;n de estos a temperaturas bajas<sup>34</sup>.</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">- Uso de irrigantes locales: tales como el suero fisiol&oacute;gico o la soluci&oacute;n salina podr&iacute;an limpiar la zona al remover detritus y restos bacterianos por arrastre. Pese a esta idea, el estudio de Tolstunov<sup>35</sup> demostr&oacute; que el sangrado postextracci&oacute;n es muy importante para la cicatrizaci&oacute;n &oacute;sea. Al no ser eliminado por una excesiva irrigaci&oacute;n, el co&aacute;gulo de sangre tiene una mayor probabilidad de formarse, y por lo tanto, se evitar&iacute;a el desarrollo de la oste&iacute;tis alveolar.</font></p> </blockquote>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Cuadro cl&iacute;nico y diagn&oacute;stico</b></font></p>     <p><font face="Verdana" size="2">El diagn&oacute;stico se realiza por las caracter&iacute;sticas cl&iacute;nicas y la sintomatolog&iacute;a del paciente, la cual es altamente dolorosa<sup>3-8</sup> y puede llegar a limitar las funciones en la cavidad bucal<sup>36</sup>. El dolor suele aumentar con la succi&oacute;n o la masticaci&oacute;n y persiste durante varios d&iacute;as<sup>4</sup>. Cl&iacute;nicamente se observa el alveolo con una p&eacute;rdida parcial o total del co&aacute;gulo sangu&iacute;neo<sup>10</sup>. En algunos casos se presenta un co&aacute;gulo gris&aacute;ceo que luego se desprende y desaparece completamente<sup>4</sup>.</font></p>     <p><font face="Verdana" size="2">Tambi&eacute;n se percibe mal olor bucal, fetidez a la exploraci&oacute;n y edema en la zona<sup>10</sup>. En algunos casos el hueso alveolar expuesto se observa de color blanquecino y la mucosa perialveolar tumefacta<sup>37</sup>.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Tratamiento</b></font></p>     <p><font face="Verdana" size="2">No existen a&uacute;n directrices para el tratamiento de la alveolitis. Algunos autores recomiendan desde el uso de medicamentos paliativos, como son los ap&oacute;sitos, y el uso de f&aacute;rmacos como los antibi&oacute;ticos, y en otros casos tratamientos m&aacute;s invasivos como el desbridamiento local.</font></p>     <p><font face="Verdana" size="2"><b><i>Pastas y ap&oacute;sitos</i></b></font></p>     <p><font face="Verdana" size="2">Los ap&oacute;sitos han sido el tratamiento m&aacute;s utilizado para la alveolitis. El ejemplo m&aacute;s com&uacute;nmente usado es la gasa iodoformada recubierta con &oacute;xido de cinc m&aacute;s eugenol, aunque en el mercado existen ya otros preparados a base de eugenol combinado con glicerina, lanolina, aceite de oliva, etc.<sup>13</sup>. Estos han sido criticados, pues se ha reportado que el uso de &oacute;xido de cinc m&aacute;s eugenol puede producir osteonecrosis y retrasar la cicatrizaci&oacute;n &oacute;sea<sup>8</sup>.</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">El Alveogyl (iodoformo m&aacute;s butylparaminobenzoato) ha reportado excelentes resultados para combatir los s&iacute;ntomas dolorosos<sup>9,18,38</sup>. Sus componentes deprimen los receptores sensoriales implicados en la percepci&oacute;n del dolor al inhibir la s&iacute;ntesis de prostaglandinas. Por otro lado, el iodoformo es considerado antis&eacute;ptico<sup>9,38</sup>.</font></p>     <p><font face="Verdana" size="2">Dentro de las nuevas opciones de tratamiento mediante el uso de ap&oacute;sitos en el &aacute;lveolo dental, la esponja de col&aacute;geno embebida con plasma rico en factores de crecimiento (PRFC), obtenido de sangre aut&oacute;loga, ha mostrado mejores resultados en el manejo de la alveolitis seca frente al uso de pasta de &oacute;xido de cinc eugenol<sup>38</sup>. Estudios previos sobre cicatrizaci&oacute;n &oacute;sea han demostrado su efectividad en el tratamiento de defectos &oacute;seos periodontales<sup>39</sup>. Se presume que los factores de crecimiento podr&iacute;an acelerar el proceso de cicatrizaci&oacute;n y la angiog&eacute;nesis, disminuyendo el dolor<sup>38,39</sup>, por lo que Haraji et al.<sup>40</sup> recomiendan su uso como profil&aacute;ctico en pacientes predisponentes a desarrollar alveolitis seca.</font></p>     <p><font face="Verdana" size="2">El uso de la pastilla GECBB a base de guaicol, eugenol, clorobutanol y el b&aacute;lsamo del Per&uacute; ha demostrado mejores resultados en el alivio del dolor frente a la terapia convencional a base de oxido de cinc m&aacute;s euguenol. Esta se trata de una pasta introducida en una pastilla de gelatina, glicerina y az&uacute;car colocada en el alv&eacute;olo a manera de ap&oacute;sito que adem&aacute;s no necesita ser preparada antes de su uso<sup>41</sup>.</font></p>     <p><font face="Verdana" size="2">Es importante recordar que todos los ap&oacute;sitos act&uacute;an como barrera f&iacute;sica para mantener el alv&eacute;olo obturado impidiendo la exposici&oacute;n de las terminaciones nerviosas &oacute;seas al ambiente<sup>4,8,9</sup>, aunque en unos casos pueden reaccionar como cuerpo extra&ntilde;o retardando el periodo de cicatrizaci&oacute;n<sup>13</sup>.</font></p>     <p><font face="Verdana" size="2"><b><i>Antis&eacute;pticos</i></b></font></p>     <p><font face="Verdana" size="2">La clorhexidina es un antis&eacute;ptico que puede ser usado como irrigante local y/o enjuague bucal al 0,12 y 2% con amplia evidencia de su efectividad<sup>42,43</sup>. Su uso como enjuague ha sido estandarizado cada 12 h y puede ser empleado incluso desde una semana antes de la cirug&iacute;a, aunque no se ha demostrado que esto potencie su efectividad<sup>44</sup>. Tambi&eacute;n suele usarse de forma t&oacute;pica como gel al 0,2% dentro del alv&eacute;olo cada 12 h por 7 d&iacute;as<sup>18,42,44</sup>. Esta forma de presentaci&oacute;n tiene como ventaja la posibilidad de uso dentro de las 24 h postextracci&oacute;n, pues se cree que la realizaci&oacute;n de enjuagues puede precipitar el desprendimiento del co&aacute;gulo<sup>35,44</sup>.</font></p>     <p><font face="Verdana" size="2">Lopes Cardoso et al.<sup>1</sup> recomiendan la combinaci&oacute;n de antis&eacute;pticos capaces de liberar grandes cantidades de ox&iacute;geno, ya que muestran efectividad frente a las bacterias anaer&oacute;bicas. Dentro de estas combinaciones est&aacute; el ioduro de sodio con per&oacute;xido de hidr&oacute;geno al 3%. Al entrar en contacto con los tejidos el ox&iacute;geno liberado act&uacute;a como un agente germicida. El mecanismo efervescente incentiva la limpieza de la herida y remoci&oacute;n del detritus.</font></p>     <p><font face="Verdana" size="2"><b><i>Analg&eacute;sicos y antiinflamatorios</i></b></font></p>     <p><font face="Verdana" size="2">Su prescripci&oacute;n es necesaria y apropiada, puesto que el principal objetivo de la terapia para la alveolitis es el manejo del dolor<sup>4,6,8,9</sup>. Diversos autores sugieren desde el uso de AINE hasta preparados de paracetamol con code&iacute;na<sup>8</sup>. Bello Santos<sup>13</sup> recomienda el uso de ketorolaco de 30 mg IM seguido de dosis oral de 10 mg cada 6 horas condicionado al dolor. Otros estudios recomiendan tambi&eacute;n el uso de celexocib oral de 200 mg<sup>45</sup>.</font></p>     <p><font face="Verdana" size="2"><b><i>Antibi&oacute;ticos</i></b></font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">Aunque se cree que su uso podr&iacute;a prevenir la aparici&oacute;n de infecciones y de alveolitis seca, diversos autores reportan que no es necesario el uso de antibi&oacute;ticos como profil&aacute;cticos<sup>2,8,46,47</sup>. Su uso no est&aacute; recomendado a menos que el paciente est&eacute; inmunocomprometido o si existiera el riesgo de desarrollar osteomielitis<sup>8,47</sup>.</font></p>     <p><font face="Verdana" size="2">En el caso de ser necesario su uso, la mayor&iacute;a de odont&oacute;logos evaluados en el estudio de Sener et al.<sup>48</sup> refirieron utilizar amoxicilina. Otros autores refieren que en el caso de presentar abscesos dentales pueden combinarlo con metronidazol<sup>49</sup>. La clindamicina fue recomendada como segunda opci&oacute;n por su baja especificidad contra pat&oacute;genos orales<sup>48</sup>. El aumento de pacientes con resistencia al tratamiento con antibi&oacute;ticos debe hacernos considerar si su uso como profil&aacute;ctico podr&iacute;a ser prejudicial o no<sup>50</sup>.</font></p>     <p><font face="Verdana" size="2"><b><i>Quir&uacute;rgicos</i></b></font></p>     <p><font face="Verdana" size="2">Dentro de estos protocolos se encuentran los curetajes e irrigaci&oacute;n con suero fisiol&oacute;gico hasta otros procedimientos m&aacute;s invasivos. El curetaje agresivo no es recomendado por algunos autores<sup>1,10,37</sup>, pues puede causar gran traumatismo alveolar e inducir a una posible bacteriemia. Otras maniobras incluyen la regularizaci&oacute;n previa de los bordes del alv&eacute;olo y el uso de colgajos para cubrir el alv&eacute;olo expuesto<sup>10</sup>.</font></p>     <p><font face="Verdana" size="2"><b><i>Laserterapia</i></b></font></p>     <p><font face="Verdana" size="2">La terapia con l&aacute;ser ha ganado reconocimiento al mostrar que estimula el metabolismo celular, la microcirculaci&oacute;n y producir un efecto analg&eacute;sico, anti edematoso y anti inflamatorio<sup>51,52</sup>. Esta ha mostrado resultados beneficiosos al acelerar el proceso de curaci&oacute;n de heridas en pacientes diab&eacute;ticos<sup>53</sup>.</font></p>     <p><font face="Verdana" size="2">Recientes estudios han confirmado que la terapia con l&aacute;ser de diodo de arseniuro de galio de baja potencia ha resultado beneficiosa en la aceleraci&oacute;n de la cicatrizaci&oacute;n &oacute;sea en ratas<sup>54,55</sup> y en la descontaminaci&oacute;n de implantes dentales<sup>56</sup>.</font></p>     <p><font face="Verdana" size="2">El estudio de Janovi&#x107; et al.<sup>51</sup> compar&oacute; el uso de aplicaciones diarias del l&aacute;ser de baja intensidad con la aplicaci&oacute;n cada 48 h de &oacute;xido de cinc con eugenol, encontrando que la reducci&oacute;n del dolor fue mayor en aquellos pacientes que usaron la terapia con l&aacute;ser.
