Los injertos óseos han sido la técnica de elección para el tratamiento de estos casos, sin embargo, el índice de fracaso es alto debido al gran tamaño del injerto y la insuficiencia de tejido blando circundante a la fisura para poder cubrirlo satisfactoriamente. Se ha reportado la transportación ósea alveolar como alternativa para estos casos difíciles.  Presentación del caso:  Se presenta el caso de un paciente masculino de 18 años, con secuelas de labio paladar hendido bilateral, con fisuras alveolo palatinas amplias, tratado con trasportación alveolar mediante anclaje óseo y deslizamiento. Se evalúan los cambios tridimensionales en la disminución de la fisura y la zona distraída mediante los registros tomográficos pre y postratamiento.  Discusión:  La reconstrucción de áreas edéntulas con deficiencias óseas presenta un reto importante para cirujanos plásticos y ortodoncistas. La distracción osteogénica es una técnica segura para pacientes falta de tejido óseo en la región del vestíbulo alveolar. La transportación ósea alveolar como alternativa al tratamiento de las fisuras alveolo palatinas amplias ofrece ventajas sobre los injertos óseos.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[ABSTRACT  Introduction:  The management of alveolar palatine clefts in patients with cleft palate and lip sequalae, present challenge, for orthodontists and plastic surgeons. Bone grafts have been the elected procedure for the treatment of these cases, nevertheless presenting large failure rates, because of the lack of soft tissue for coverage and large graft intake. Alveolar transportation has been reported as an adequate alternative for these cases.  Case presentation:  We present an 18-year-old patient with cleft palate and lip sequalae, with large alveolar clefts, treated with alveolar transportation through bone anchorage and gliding. We evaluate the tridimensional changes with decrease in size of the cleft and distracted zone with tomographic evaluation pre and posterior to treatment  Discussion:  The reconstruction of edentulous areas with bone deficiency represents an important challenge for plastic surgeons and orthodontists. Osteogenic distraction is a safe technique for patients with lack of bone tissue in the alveolar area. Bone alveolar transportation is a better alternative than bone grafts, for large alveolar clefts.]]></p></abstract>
