La desaparición espontánea de hernias de disco intervertebral es un hecho conocido con numerosas referencias en la bibliografía. Los autores presentamos el caso clínico de una paciente con radiculopatía secundaria a una hernia de disco lumbar diagnosticada con resonancia magnética y que se resolvió espontáneamente.]]></p></abstract>

<body><![CDATA[<p><font face="Verdana" size="2">La resoluci&oacute;n espont&aacute;nea de una hernia de disco intervertebral se observa con cierta frecuencia en la pr&aacute;ctica neuroquir&uacute;rgica habitual, sin embargo, su demostraci&oacute;n radiogr&aacute;fica con tomograf&iacute;a computarizada (TC) o resonancia magn&eacute;tica (RM) no es tan frecuente<Sup>4,10,15</Sup>.</font></p>     <p><font face="Verdana" size="2">Presentamos el caso cl&iacute;nico de una paciente de 65 a&ntilde;os que consulta por ci&aacute;tica y en el estudio con resonancia magn&eacute;tica se observa una hernia discal extru&iacute;da L4-L5. La paciente que rechaz&oacute; la intervenci&oacute;n quir&uacute;rgica fue tratada de forma conservadora, presentando un alivio progresivo de la sintomatolog&iacute;a. La nueva RM realizada con posterioridad mostr&oacute; la desaparici&oacute;n del fragmento discal extruido.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana"><b>Caso cl&iacute;nico </b></font></p>     <p><font face="Verdana" size="2">Mujer de 65 a&ntilde;os que consulta por presentar desde hace unos 2 a&ntilde;os dolor lumbar irradiado por la cara postero-externa de la extremidad inferior izquierda llegando hasta el tobillo. No presentaba antecedentes cl&iacute;nicos de inter&eacute;s. En la exploraci&oacute;n destacaba un signo de Lasegue a 45&deg; izquierdo, ligera p&eacute;rdida de fuerza para la dorsiflexi&oacute;n del pie izquierdo, y reflejos osteotendinosos y sensibilidad conservados en ambos miembros inferiores.</font></p>     <p><font face="Verdana" size="2">Se realiz&oacute; una resonancia magn&eacute;tica lumbar que mostr&oacute; una hernia de disco extru&iacute;da en el espacio L4L5 con un fragmento discal alojado en el receso lateral izquierdo (<a href="#f1">Figura 1</a>), La paciente rechazo la intervenci&oacute;n quir&uacute;rgica recibiendo tratamiento sintom&aacute;tico (analgesia y fisioterapia). Se revis&oacute; ambulatoriamente 7 meses m&aacute;s tarde presentando un alivio importante en el dolor ci&aacute;tico  y aportando una nueva RM que mostraba &uacute;nicamente cambios degenerativos en el disco L4-L5 sin evidencia de hernia y/o fragmento extrudido (<a href="#f2">Figura 2</a>).  </font> </p>     <p align="center"><font face="Verdana" size="2"><a name="f1"><img src="/img/revistas/neuro/v18n2/9_1.jpg" width="314" height="240"></a>    <br> Figura 1.<i> Resonancia magn&eacute;tica. Resonancia magn&eacute;tica que muestra     <br> la desaparici&oacute;n completa de la hernia de disco en L4-L5.</i></font></p>     <p>&nbsp;</p>     <p><font face="Verdana"><b>Discusi&oacute;n</b></font></p>     <p><font face="Verdana" size="2">En la literatura m&eacute;dica existen numerosas referencias sobre la desaparici&oacute;n de hernias de disco intervertebral sin tratamiento quir&uacute;rgico<Sup>1,3,7,9,12</Sup>. Seguramente el primer caso fue publicado por Key en 1945 comprobando la regresi&oacute;n del disco mediante mielografia<Sup>5</Sup>; a&ntilde;os m&aacute;s tarde Teplick y Haskin publicaron el primer caso demostrado con tomograf&iacute;a computarizada<Sup>13</Sup>. La introducci&oacute;n de la resonancia magn&eacute;tica en el diagnostico de la patolog&iacute;a discal nos ha permitido conocer detalladamente la historia natural de las hernias de disco. La regresi&oacute;n de discos herniados se ha publicado en todos los segmentos vertebrales y con diferentes formas de presentaci&oacute;n cl&iacute;nica, incluyendo radiculopat&iacute;a<Sup>15 </Sup>o mielopat&iacute;a cervical<Sup>11</Sup>, mielopat&iacute;a dorsal<Sup>9 </Sup>y radiculopat&iacute;a lumbo-sacra<Sup>12</Sup>. El mecanismo real por el que se resuelve la hernia del disco no est&aacute; perfectamente aclarado, postul&aacute;ndose al menos tres hip&oacute;tesis: la primera entiende que el fragmento herniado regresar&iacute;a espont&aacute;neamente al interior del anillo fibroso, esto ser&iacute;a posible en los casos en que la hernia protruye por el anillo pero manteniendo una continuidad con &eacute;ste, es decir no se producir&iacute;a si existe un fragmento muy extru&iacute;do o migrado<Sup>3</Sup>. La segunda hip&oacute;tesis sostiene que el fragmento herniado sufre un proceso de deshidrataci&oacute;n con retracci&oacute;n progresiva y la tercera sostiene que el fragmento herniado en el espacio epidural ocasiona un proceso inflamatorio que provocar&iacute;a una desaparici&oacute;n progresiva a manos de macr&oacute;fagos o una reabsorci&oacute;n enzim&aacute;tica<Sup>14</Sup>. El n&uacute;cleo pulposo normal es avascular y no contiene c&eacute;lulas inflamatorias. Lindblom y Hultqvist en 1950<Sup>7 </Sup>demostraron la proliferaci&oacute;n de fibroblastos en discos herniados. En animales de experimentaci&oacute;n, Minamide et al<Sup>8 </Sup>comprobaron que los factores de crecimiento fibrobl&aacute;stico juegan un papel importante en la formaci&oacute;n de neocapilares, proliferaci&oacute;n de c&eacute;lulas endoteliales y en la quimiotaxis de las c&eacute;lulas inflamatorias hacia el disco herniado, en un proceso similar al de la cicatrizaci&oacute;n de las heridas. Por lo tanto el fragmento herniado es considerado como un cuerpo extra&ntilde;o dando lugar a una reacci&oacute;n inflamatoria local, a la neovascularizaci&oacute;n y a la fagocitosis provocando una reducci&oacute;n progresiva del fragmento. Con t&eacute;cnicas de inmunohistoquimia se ha demostrado la presencia de positividad para CD68 en el disco, confirmando que hay macr&oacute;fagos. Posiblemente las hernias que no regresan espont&aacute;neamente podr&iacute;an estar formadas fundamentalmente por anillo fibroso y poco contenido de n&uacute;cleo pulposo<Sup>16</Sup>. Gallucci et al<Sup>2 </Sup>pudieron demostrar mediante RM con contraste la hipervascularizaci&oacute;n que acompa&ntilde;a al fragmento herniado y observar c&oacute;mo se produce una reducci&oacute;n progresiva en la vascularizaci&oacute;n a medida que decrece el fragmento herniado. El mismo grupo, con posterioridad y sobre una serie de 64 hernias resueltas espont&aacute;neamente observaron que el 83.3% de hernias que se realzan con contraste tienen una resoluci&oacute;n espont&aacute;nea<Sup>12</Sup>. Aunque posiblemente la tercera teor&iacute;a es la m&aacute;s aceptada no puede excluirse que los otros mecanismos tengan alguna participaci&oacute;n.</font></p>     <p><font face="Verdana" size="2">Sin embargo no siempre existe una correlaci&oacute;n entre la mejor&iacute;a cl&iacute;nica y la regresi&oacute;n de la hernia en las pruebas de imagen; Komori et al<Sup>6 </Sup>en una serie de 64 pacientes observaron que los 13 que mejoraron cl&iacute;nicamente no presentaban cambios significativos en la resonancia. Estos autores clasifican las hernias de disco en tres grupos, y entre estos grupos los que tienen discos extru&iacute;dos tienen una mayor incidencia de resoluci&oacute;n espont&aacute;nea.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana"><b>Bibliograf&iacute;a</b></font></p>     <!-- ref --><p><font face="Verdana" size="2">1.
