Objetivo: El propósito de este artículo es presentar una revisión crítica de las indicaciones actuales, resultados y complicaciones de la simpatectomía torácica endoscópica. Material y métodos: Se llevó a cabo una búsqueda y revisión exhaustiva de trabajos publicados sobre la simpatectomía toracoscópica. Resultados: La simpatectomía toracoscópica se ha desarrollado como una opción terapéutica en pacientes con hiperhidrosis focal, síndromes dolorosos y trastornos vasculares periféricos, fundamentalmente. Los resultados, recurrencias y complicaciones son equivalentes a los procedimientos abiertos previamente establecidos; sin embargo, la morbilidad, la estancia hospitalaria y el tiempo en volver a la vida diaria se reducen sustancialmente. Las mayores tasas de éxito y de satisfacción (más del 95%) se obtienen en sujetos tratados por hiperhidrosis focal. Conclusiones: Las tasas de éxito y de complicación de la simpatectomía endoscópica son similares a las de las técnicas abiertas, siendo mejor tolerado el postoperatorio y más precoz el retorno a la vida cotidiana y laboral.]]></p></abstract>

Raynaud disease.</font></p> <hr size="1">     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Introducci&oacute;n</b></font></p>     <p><font face="Verdana" size="2">Aunque la cirug&iacute;a del sistema nervioso simp&aacute;tico comenz&oacute; hacia 1880 para el tratamiento de espasticidad, epilepsia, angina de pecho, bocio e hipertensi&oacute;n, no fue hasta la d&eacute;cada de los a&ntilde;os 1920 cuando se realizaron numerosas simpatectom&iacute;as qu&iacute;micas con fenol o alcohol, para posteriormente disminuir mucho su uso tras publicar Leriche<sup>44</sup> en 1932 una complicaci&oacute;n seria por la entrada de alcohol en el espacio subaracnoideo tor&aacute;cico a trav&eacute;s de la vaina de un nervio. A partir de los a&ntilde;os 1930 se llevaron a cabo nuevas t&eacute;cnicas de simpatectom&iacute;a abierta a nivel espl&aacute;cnico, lumbar y tor&aacute;cico en el tratamiento de dolor, hiperhidrosis, vasculopat&iacute;as y cardiopat&iacute;as. En los a&ntilde;os 1980 se introdujo la simpatectom&iacute;a percut&aacute;nea por radiofrecuencia a nivel tor&aacute;cico y lumbar<sup>82</sup>.</font></p>     <p><font face="Verdana" size="2">Aunque la simpatectom&iacute;a endosc&oacute;pica tor&aacute;cica se inici&oacute; en los a&ntilde;os 1950, gracias a Kux<sup>39</sup>, debido a dificultades t&eacute;cnicas y alta tasa de recurrencias, no se populariz&oacute; hasta la adopci&oacute;n de la tecnolog&iacute;a de v&iacute;deo a comienzos de los 1990, recibiendo as&iacute; la denominaci&oacute;n de simpatectom&iacute;a toracosc&oacute;pica video-asistida<sup>3,6,8,13,17,20,21,48,69</sup>. Posteriormente estas t&eacute;cnicas endosc&oacute;picas se han ido refinando, reduci&eacute;ndose la invasividad con incisiones m&aacute;s cortas y menos puertos de entrada<sup>19,38,42,79,80,81</sup>.</font></p>     <p><font face="Verdana" size="2">La selecci&oacute;n de la literatura m&eacute;dica fue realizada a partir de b&uacute;squedas en las bases de datos de MEDLINE desde 1974 a diciembre de 2006, utilizando los siguientes t&eacute;rminos del Medical Subject Headings (MeSH) del Index Medicus: sympathectomy, thoracoscopy, hyperhidrosis, complex regional pain syndromes, Raynaud disease, long QT syndrome.</font></p>     ]]></body>
<body><![CDATA[<p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Indicaciones</b></font></p>     <p><font face="Verdana" size="2"><i><b>Hiperhidrosis focal patol&oacute;gica</b></i></font></p>     <p><font face="Verdana" size="2">Aunque es la indicaci&oacute;n m&aacute;s com&uacute;n en la actualidad, muchos pacientes siguen sin buscar ayuda m&eacute;dica al ignorar que tienen una enfermedad tratable con &eacute;xito<sup>6,8,17,30</sup>. Consiste en un incremento de la sudoraci&oacute;n ecrina por encima de lo requerido para el control de la termorregulaci&oacute;n en una o m&aacute;s &aacute;reas corporales, sobre todo a nivel palmar, siendo tambi&eacute;n frecuente a nivel axilar, plantar y facial. Habitualmente su causa es desconocida (idiop&aacute;tica) y se estima una incidencia inferior al 1%, siendo mayor en Estados Unidos (2.8%) y poblaciones asi&aacute;ticas (4-5%)<sup>2,11,55,59,82</sup>. En un 70% o m&aacute;s de los casos se inicia en la ni&ntilde;ez y en un 16-20% en la adolescencia, siendo poco frecuente su comienzo en adultos<sup>28,46,62,72</sup>. Tiene un discreto predominio femenino<sup>46</sup> y en un 25-65% de los casos existen otros familiares afectados (variante familiar), de forma que se ha propuesto una herencia con penetrancia variable<sup>68</sup>.</font></p>     <p><font face="Verdana" size="2">La fisiopatolog&iacute;a de la hiperhidrosis focal es poco conocida. Existen unos 4 millones (oscila entre 2 y 5 millones) de gl&aacute;ndulas sudor&iacute;paras en el cuerpo, de las cuales unos 3 millones son ecrinas y el resto apocrinas y apoecrinas<sup>70</sup>. Las gl&aacute;ndulas ecrinas est&aacute;n inervadas por fibras postganglionares colin&eacute;rgicas del sistema nervioso simp&aacute;tico. Su funci&oacute;n primaria es la secreci&oacute;n de sudor (l&iacute;quido incoloro e inodoro que interviene en la regulaci&oacute;n de la temperatura corporal) a una tasa inferior a 1 ml/m<sup>2</sup>/min en reposo y a temperatura ambiente, que se incrementa con est&iacute;mulos emocionales y gustativos. Estas gl&aacute;ndulas ecrinas est&aacute;n distribuidas por casi toda la superficie del cuerpo, aunque su densidad es mayor en plantas de pies y frente, seguidas de palmas de manos y mejillas. Por el contrario, las gl&aacute;ndulas apocrinas se localizan preferentemente en regiones axilares, &aacute;rea urogenital y ar&eacute;ola mamaria, su funci&oacute;n est&aacute; regulada por procesos hormonales, no son activas hasta la pubertad y no est&aacute;n implicadas en la hiperhidrosis focal. Adem&aacute;s, existen gl&aacute;ndulas mixtas apoecrinas, funcional y farmacol&oacute;gicamente diferentes, localizadas sobre todo en regiones axilares y perianales, cuyo papel en la hiperhidrosis focal es desconocido<sup>70</sup>.</font></p>     <p><font face="Verdana" size="2">En los estudios realizados en pacientes no se han encontrado cambios histopatol&oacute;gicos en las gl&aacute;ndulas sudor&iacute;paras ni incrementos del tama&ntilde;o o n&uacute;mero de las mismas. M&aacute;s bien, la hiperhidrosis focal representar&iacute;a una disfunci&oacute;n compleja del sistema nervioso aut&oacute;nomo<sup>70</sup>. En un estudio de Shih<sup>73</sup>, los pacientes con hiperhidrosis palmoplantar mostraban una menor bradicardia refleja en respuesta a maniobras de Valsalva y un mayor grado de vasoconstricci&oacute;n en respuesta a inmersi&oacute;n de los dedos en agua fr&iacute;a. Tal actividad simp&aacute;tica incrementada a trav&eacute;s de los ganglios T2-T3 podr&iacute;a causar sudoraci&oacute;n excesiva palmar, originando as&iacute; un c&iacute;rculo vicioso, ya que el enfriamiento de la piel por evaporaci&oacute;n desencadena una hiperactividad simp&aacute;tica, que a su vez aumenta la sudoraci&oacute;n<sup>73</sup>.</font></p>     <p><font face="Verdana" size="2">Para determinar la extensi&oacute;n de la hiperhidrosis se efect&uacute;a la prueba de Minor o test del yodo-almid&oacute;n (la aplicaci&oacute;n de yodo al 1-5% y almid&oacute;n en una zona cut&aacute;nea con excesiva sudoraci&oacute;n ti&ntilde;e dicha zona de un color purp&uacute;reo negruzco)<sup>15,23</sup>. Respecto a la severidad, se pueden distinguir los siguientes grados: leve (palmas frecuentemente h&uacute;medas), moderada (sudoraci&oacute;n palmar suficientemente profusa para empapar un pa&ntilde;uelo), grave (goteo de sudor espont&aacute;neo al cerrar el pu&ntilde;o similar al goteo de un grifo)<sup>41</sup>.</font></p>     <p><font face="Verdana" size="2">En los casos leves o moderados pueden ser &uacute;tiles terapias no quir&uacute;rgicas, como agentes t&oacute;picos (clorhidrato de aluminio hexahidratado al 20-25% en soluci&oacute;n de alcohol, soluciones de glutaraldehido al 2-10%), iontoforesis (3-6 sesiones por semana), f&aacute;rmacos orales (oxibutinina, glicopirrolato) y toxina botul&iacute;nica tipo A<sup>15.</sup> La simpatectom&iacute;a tor&aacute;cica endosc&oacute;pica estar&iacute;a indicada en la hiperhidrosis grave, la cual suele afectar a las esferas psicol&oacute;gica, profesional y social<sup>24,25</sup>. Otras indicaciones ser&iacute;an la hiperhidrosis asociada a cambios vasculares perif&eacute;ricos y la bromidrosis axilar (trastorno angustioso caracterizado por la combinaci&oacute;n de osmidrosis u olor desagradable que se desprende de las gl&aacute;ndulas apocrinas axilares e hiperhidrosis axilar provocada por las gl&aacute;ndulas ecrinas)<sup>29</sup>.</font></p>     <p><font face="Verdana" size="2"><i><b>Dolor mediado simp&aacute;ticamente (disautonom&iacute;a dolorosa)</b></i></font></p>     <p><font face="Verdana" size="2">El dolor neurop&aacute;tico se define como el dolor iniciado o causado por una lesi&oacute;n o disfunci&oacute;n primaria del sistema nervioso y suele ser de tipo quemante difuso, con alodinia o dolor inducido por simple contacto con el &aacute;rea<sup>53</sup>. Incluye numerosos cuadros cl&iacute;nicos mal definidos etiopatog&eacute;nicamente: s&iacute;ndrome de dolor regional complejo tipo I (distrofia simp&aacute;tica refleja) y tipo II (causalgia), s&iacute;ndrome mano-hombro, atrofia de Sudeck, dolor del miembro fantasma, s&iacute;ndrome del pie diab&eacute;tico quemante, dolor visceral mantenido simp&aacute;ticamente, dolor posterior a accidente cerebro-vascular<sup>53</sup>. La idea de que muchos s&iacute;ndromes de dolor neurop&aacute;tico, particularmente los tipos I y II, son mediados simp&aacute;ticamente ha llevado a tratarlos mediante interrupci&oacute;n temporal o permanente del sistema nervioso simp&aacute;tico<sup>53</sup>.</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">Se ha observado, tras lesiones de nervios perif&eacute;ricos o de ra&iacute;ces nerviosas, la formaci&oacute;n de ramificaciones desde las fibras simp&aacute;ticas a los ganglios de las ra&iacute;ces dorsales, donde forman cestillas alrededor de las neuronas sensitivas y disminuyen el umbral de disparo<sup>71,75</sup>.  En el mantenimiento del dolor por fibras simpáticas interviene la regulación ascendente de la actividad </font><font face="Symbol" size="2">a<sub>2</sub></font> <font face="Verdana" size="2">adren&eacute;rgica en la periferia, que disminuye el umbral de disparo de las fibras perif&eacute;ricas nociceptivas<sup>71,75</sup>. El hecho de que el bloqueo de los ganglios simp&aacute;ticos o la simpatectom&iacute;a realizados precozmente mejoran el dolor m&aacute;s que si se realizan tard&iacute;amente, sugiere la producci&oacute;n de alteraciones anatomofisiol&oacute;gicas duraderas<sup>71,75</sup>.</font></p>     <p><font face="Verdana" size="2">Para el diagn&oacute;stico del dolor inducido o mantenido simp&aacute;ticamente se deber&iacute;an llevar a cabo bloqueos anest&eacute;sicos de las v&iacute;as simp&aacute;ticas, simples o repetidos, los cuales a veces producen un alivio duradero del dolor<sup>58</sup>. Un bloqueo anest&eacute;sico que interrumpe eficazmente la transmisi&oacute;n simp&aacute;tica y origina un adecuado alivio del dolor y de la alodinia, aunque sea transitorio, confirma el diagn&oacute;stico y apoya la indicaci&oacute;n de la simpatectom&iacute;a<sup>58</sup>. As&iacute;, en aquellos pacientes con dolor refractario a otras terapias (f&aacute;rmacos, tratamientos f&iacute;sicos, intervenciones psicol&oacute;gicas), la simpatectom&iacute;a puede llegar a ser bastante gratificante en el manejo del dolor<sup>58</sup>.</font></p>     <p><font face="Verdana" size="2"><i><b>Vasculopat&iacute;as</b></i></font></p>     <p><font face="Verdana" size="2">Las enfermedades vasculares isqu&eacute;micas para las que puede ser &uacute;til la simpatectom&iacute;a son de tipo vasoesp&aacute;stico y vasoobstructivo<sup>22,37</sup>. Entre los procesos vasoesp&aacute;sticos destaca la enfermedad de Raynaud, en la cual se consigue en muchos casos una mejor&iacute;a duradera con simpatectom&iacute;a, aunque no son infrecuentes las recurrencias<sup>22,37</sup>. Los trastornos vasoobliterativos perif&eacute;ricos, como la enfermedad de Buerger o la macroglobulinemia, causan isquemia y necrosis tisular que se traducen en dolor y alteraci&oacute;n funcional<sup>22,37,76</sup>. El incremento del flujo vascular colateral tras la realizaci&oacute;n de simpatectom&iacute;a (especialmente de T2) lleva a un incremento de la perfusi&oacute;n cut&aacute;nea, una disminuci&oacute;n del dolor isqu&eacute;mico y a una mejor&iacute;a de las &uacute;lceras y tejidos necr&oacute;ticos, al menos temporalmente, ya que el proceso lesional suele continuar<sup>22,37,76,84</sup>.</font></p>     <p><font face="Verdana" size="2"><i><b>Cardiopat&iacute;as</b></i></font></p>     <p><font face="Verdana" size="2">Hoy d&iacute;a la indicaci&oacute;n de la simpatectom&iacute;a en cardiopat&iacute;as es excepcional, gracias al gran desarrollo de medicaciones efectivas y bypass de arterias coronarias<sup>36,74</sup>. La angina de Prinzmetal o s&iacute;ndrome X responde escasamente al bypass coronario, pero puede mejorar notablemente con la interrupci&oacute;n simp&aacute;tica<sup>36,74</sup>. Tambi&eacute;n el s&iacute;ndrome de intervalo QT largo cong&eacute;nito, consistente en episodios de s&iacute;ncope y taquicardia ventricular, responde a la simpatectom&iacute;a<sup>35,45,46,56,57,65</sup>.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>T&eacute;cnica de simpatectom&iacute;a tor&aacute;cica endosc&oacute;pica</b></font></p>     <p><font face="Verdana" size="2">El procedimiento de elecci&oacute;n es la endoscopia video-asistida, variando entre los diferentes cirujanos el n&uacute;mero de puertos de uno a tres. En el caso de la t&eacute;cnica uniportal se realiza a trav&eacute;s de una peque&ntilde;a incisi&oacute;n axilar por la cual se lleva a cabo la visualizaci&oacute;n, la manipulaci&oacute;n y la insuflaci&oacute;n de CO<sub>2</sub> (<a href="#f1">Figura 1</a>)<sup>42,79,81</sup>. Las ventajas de esta t&eacute;cnica uniportal son, entre otras, una duraci&oacute;n m&aacute;s corta (inferior a una hora), un menor dolor postoperatorio y unos mejores resultados est&eacute;ticos (tan s&oacute;lo una peque&ntilde;a cicatriz oculta en la regi&oacute;n axilar)<sup>79</sup>. En las t&eacute;cnicas biportales y triportales se suele utilizar un puerto para visualizar y el resto para inserci&oacute;n de instrumentos y manipulaci&oacute;n<sup>27</sup>. Los accesos biportales y triportales son extremadamente &uacute;tiles en el caso de adherencias y bridas intrator&aacute;cicas que impiden el acceso a la cadena simp&aacute;tica tor&aacute;cica, aunque afortunadamente estos casos son excepcionales<sup>5</sup>. Algunos autores practican la cirug&iacute;a de manera ambulatoria usando instrumentaci&oacute;n miniendosc&oacute;pica (2 mm)<sup>64</sup>.</font></p>     <p align="center"><font face="Verdana" size="2"><a name="f1"><img src="/img/revistas/neuro/v21n1/1_1.jpg" width="314" height="352"></a>    ]]></body>

En pacientes con hiperhidrosis palmar, que es la indicaci&oacute;n m&aacute;s frecuente, suele llevarse a cabo una lesi&oacute;n del ganglio simp&aacute;tico T2, que puede extenderse hasta los ganglios T3 y T4, aunque la mayor&iacute;a de los autores coinciden en que no es necesario salvo hiperhidrosis axilar asociada, ya que podr&iacute;a incrementar la tasa y el grado de hiperhidrosis compensatoria y la sequedad de las manos<sup>18,49,51</sup>.</font></p>     ]]></body>

La cadena simp&aacute;tica    <br>se dispone cruzando perpendicularmente    <br>sobre los cuellos costales.</i></font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2">La lesi&oacute;n del ganglio y de la cadena simp&aacute;tica es muy variada en la literatura: coagulaci&oacute;n del ganglio, resecci&oacute;n del ganglio, corte de la cadena simp&aacute;tica sin resecci&oacute;n del ganglio, coagulaci&oacute;n de la cadena simp&aacute;tica rostral y distal al ganglio, aplicaci&oacute;n de clips a la cadena simp&aacute;tica<sup>37,74</sup>. En cualquier caso, se recomienda seccionar la cadena simp&aacute;tica rostral y caudalmente al ganglio, adem&aacute;s de los ramos comunicantes y el nervio de Kuntz, que conecta los ganglios simp&aacute;ticos a los nervios intercostales y al plexo braquial<sup>38,63</sup>. Una vez lesionado el sistema simp&aacute;tico tor&aacute;cico alto se observa una elevaci&oacute;n de la temperatura cut&aacute;nea palmar, que var&iacute;a seg&uacute;n la temperatura basal en ese momento, siendo habitualmente de 1-3<sup>o</sup>C, especialmente tras simpatectom&iacute;a de T2<sup>9,31,52,73,84</sup>.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Resultados</b></font></p>     <p><font face="Verdana" size="2">El sistema nervioso simp&aacute;tico tiene una gran capacidad de regeneraci&oacute;n y de crecimiento, por lo que es posible la aparici&oacute;n de recidivas incluso despu&eacute;s de un a&ntilde;o<sup>28</sup>.</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2"><i><b>Hiperhidrosis focal patol&oacute;gica</b></i></font></p>     <p><font face="Verdana" size="2">Los resultados son generalmente muy buenos, con tasas de &eacute;xito superiores al 95% durante el primer a&ntilde;o, independientemente de si los ganglios lesionados son T2, T2 y T3 &oacute; T2, T3 y T4<sup>18,27,30,32,40,48,54</sup>. Dichos resultados se consiguen tanto si se extirpan los ganglios simp&aacute;ticos como si se destruyen mediante coagulaci&oacute;n monopolar o l&aacute;ser<sup>30,31,43,61,79</sup>. Hay que decir que esas altas tasas de desaparici&oacute;n de la hiperhidrosis palmar tambi&eacute;n se lograban en series antiguas con procedimientos tor&aacute;cicos abiertos<sup>5,14,77</sup>.</font></p>     <p><font face="Verdana" size="2">Wilkinson<sup>83</sup> obtuvo en m&aacute;s de 100 pacientes una tasa inicial de &eacute;xito del 98%, que baj&oacute; al 91% a los 3 a&ntilde;os. Chuang y Liu<sup>12</sup> consiguieron un control inicial de la hiperhidrosis en el 99% de sus 1742 casos, que baj&oacute; al 92% tras m&aacute;s de un a&ntilde;o de seguimiento. En aquellos casos que recidivaron llev&oacute; a cabo entre una y tres reoperaciones, obteniendo as&iacute; una tasa de control total del 99.9%. En la serie de Lin<sup>51</sup> con 2000 pacientes se consiguieron unas tasas de &eacute;xito inicial del 100% en las palmares y del 95.9% en las axilares, que bajaron progresivamente hasta 98.7 y 83.3% a los 5 a&ntilde;os, respectivamente. En el caso de la bromidrosis axilar, la simpatectom&iacute;a T3-T4 es un m&eacute;todo efectivo (resultados excelentes o buenos) en el 70% de los casos<sup>29</sup>.</font></p>     <p><font face="Verdana" size="2">Estudios preliminares conducidos por Pohjavaara<sup>62</sup> muestran que algunas fobias sociales pueden beneficiarse del bloqueo simp&aacute;tico endosc&oacute;pico, al reducir la sudoraci&oacute;n palmar y facial, el enrojecimiento facial y el temblor, por lo que podr&iacute;a ser un tratamiento de elecci&oacute;n en fobias sociales generalizadas que no se han beneficiado de un tratamiento adecuado con medicaci&oacute;n o psicoterapia.</font></p>     <p><font face="Verdana" size="2"><i><b>Dolor mediado simp&aacute;ticamente (disautonom&iacute;a dolorosa)</b></i></font></p> <font face="Verdana" size="2">Aunque se resuelva incluso completamente el componente de dolor mantenido simp&aacute;ticamente, el frecuente dolor som&aacute;tico concomitante, que puede representar s&oacute;lo una peque&ntilde;a parte en el conjunto global del dolor, puede llevar al paciente a referir que nota una mejor&iacute;a insatisfactoria de su dolor global<sup>34,53</sup>. Las reca&iacute;das del dolor son relativamente frecuentes, siendo dif&iacute;cil saber las razones o causas que las originan. Wilkinson<sup>83</sup>, sobre 28 pacientes, obtuvo una mejor&iacute;a del dolor mediado simp&aacute;ticamente a nivel de extremidades superiores del 69% durante m&aacute;s de un a&ntilde;o, aunque 24% desarrollaron recurrencia del dolor en un a&ntilde;o. Algunos pacientes en los que el dolor neurop&aacute;tico persiste tras la simpatectom&iacute;a pueden tener dolor incluso m&aacute;s intenso que antes de la misma<sup>34</sup>.</font></p>     <p><font face="Verdana" size="2">En una revisi&oacute;n sistem&aacute;tica de la literatura en 2003 sobre la utilidad de la simpatectom&iacute;a en el dolor neurop&aacute;tico, los autores encontraron s&oacute;lo 4 estudios con cierta calidad metodol&oacute;gica<sup>53</sup>. La informaci&oacute;n obtenida indicaba que la simpatectom&iacute;a no es, en general, un procedimiento eficaz para el alivio del dolor neurop&aacute;tico, por lo que esta t&eacute;cnica debe ser considerada muy cuidadosamente en cuanto a su utilidad, efectividad y riesgo potencial de efectos adversos<sup>53</sup>.</font></p>     <p><font face="Verdana" size="2"><i><b>Vasculopat&iacute;as</b></i></font></p>     <p><font face="Verdana" size="2">Los resultados son tambi&eacute;n mucho menos satisfactorios que en el caso de la hiperhidrosis. En pocas ocasiones se llega a realizar simpatectom&iacute;a para el vasoespasmo asociado a oclusi&oacute;n arterial aguda, aunque su tasa de &eacute;xito en estos casos es muy alta<sup>88</sup>. Por lo que respecta a la enfermedad de Raynaud, frecuentemente progresa a situaciones en las que el dolor isqu&eacute;mico y la necrosis tisular no pueden ser revertidas con la simpatectom&iacute;a<sup>88</sup>. Tambi&eacute;n en la enfermedad de Buerger y en otras vasculopat&iacute;as perif&eacute;ricas obliterativas la progresi&oacute;n es lo habitual, por lo que el efecto positivo de la interrupci&oacute;n simp&aacute;tica es de corta duraci&oacute;n<sup>87,88</sup>.</font></p>     <p><font face="Verdana" size="2">Por consiguiente, la mayor&iacute;a de los autores reservan la simpatectom&iacute;a para aquellos pacientes con vasculopat&iacute;as avanzadas y dolor isqu&eacute;mico intratable o p&eacute;rdida tisular, sobre todo si ha fallado la respuesta a la neuroestimulaci&oacute;n de los cordones medulares posteriores<sup>22,88</sup>.</font></p>     <p><font face="Verdana" size="2"><i><b>Cardiopat&iacute;as</b></i></font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">En el caso de las cardiopat&iacute;as vasoesp&aacute;sticas dolorosas (anginas intratables) o los s&iacute;ncopes o taquiarritmias epis&oacute;dicas por s&iacute;ndrome de intervalo Q-T largo cong&eacute;nito, la simpatectom&iacute;a puede ser una t&eacute;cnica atractiva, segura y efectiva que consigue un control del dolor precordial en muchos pacientes. El problema a la hora de evaluar los resultados es que las diferentes series presentan un n&uacute;mero bajo de casos<sup>36,45,46,65,78</sup>.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Complicaciones</b></font></p>     <p><font face="Verdana" size="2"><i><b>Hiperhidrosis compensatoria</b></i></font></p>     <p><font face="Verdana" size="2">Es la complicaci&oacute;n m&aacute;s com&uacute;n, de forma que la mayor parte de los pacientes (45-98.6%) sometidos a simpatectom&iacute;a por hiperhidrosis presentan alg&uacute;n grado de incremento de la sudoraci&oacute;n en las siguientes semanas o meses. Habitualmente aparece con el calor o el ejercicio sobre t&oacute;rax, abdomen y muslos. En general, es bien tolerada y disminuye en los siguientes meses<sup>2,18,32,40,48,49,50,51,66</sup>.</font></p>     <p><font face="Verdana" size="2">Se pueden distinguir tres grados seg&uacute;n su gravedad: leve (ropa interior seca a pesar de la sudoraci&oacute;n), moderada (ropa interior a veces empapada pero con sudoraci&oacute;n tolerable) y grave (ropa interior empapada con sudoraci&oacute;n excesiva e intolerable, requiriendo varios cambios de ropa al d&iacute;a)<sup>41</sup>. Cuando el incremento de la sudoraci&oacute;n es de gran intensidad, resultando molesta al sujeto, se denomina hiperhidrosis compensatoria patol&oacute;gica, que es m&aacute;s frecuente en sujetos con la variante familiar, en los cuales adem&aacute;s la hipersudoraci&oacute;n preoperatoria suele ser m&aacute;s extensa. En la serie de Gossot<sup>26</sup> result&oacute; ser grave en el 7.5% de las hiperhidrosis compensatorias, habiendo adem&aacute;s un 7.2% de pacientes con sudoraci&oacute;n facial gustativa patol&oacute;gica. Se ha sugerido en varios estudios que ocurre m&aacute;s frecuentemente si en la simpatectom&iacute;a se incluyen adem&aacute;s los ganglios simp&aacute;ticos tor&aacute;cicos tercero y cuarto<sup>1,33,61,67</sup>; sin embargo, otros autores han encontrado que la hiperhidrosis compensatoria es menos frecuente y menos grave tras simpaticotom&iacute;a toracosc&oacute;pica de T3, de forma que logra una mayor tasa de satisfacci&oacute;n a largo plazo<sup>85,86</sup>. En cualquier caso, no existe ninguna prueba prequir&uacute;rgica que permita predecir qu&eacute; pacientes podr&iacute;an desarrollar esta complicaci&oacute;n.</font></p>     <p><font face="Verdana" size="2">Cuando resulta problem&aacute;tica para el individuo puede tratarse con compuestos de hidr&oacute;xido de aluminio, oxibutinina o glicopirrolato e incluso con toxina botul&iacute;nica<sup>23</sup>. Respecto a la sudoraci&oacute;n facial gustativa, se han usado de forma eficaz parches de clonidina y toxina botul&iacute;nica<sup>23</sup>.</font></p>     <p><font face="Verdana" size="2"><i><b>S&iacute;ndrome de Horner</b></i></font></p>     <p><font face="Verdana" size="2">La tasa de aparici&oacute;n de s&iacute;ndrome de Horner var&iacute;a mucho entre las diferentes series (0-6.9%)<sup>16,32,51</sup>, aunque las &uacute;ltimas publicaciones se&ntilde;alan tasas muy bajas, como la de Chuang<sup>12</sup> y Lin<sup>51</sup>, que s&oacute;lo refieren tenerlo en el 0-0.15% de los casos. Muchos de estos casos son s&oacute;lo temporales, desapareciendo en semanas o meses<sup>12,16,32,51</sup>.</font></p>     <p><font face="Verdana" size="2"><i><b>Neumot&oacute;rax</b></i></font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">Aunque puede aparecer en cualquier simpatectom&iacute;a endosc&oacute;pica tor&aacute;cica, es m&aacute;s frecuente en aquellos casos que han requerido una extensa lisis de adhesiones pleurales<sup>12,51</sup>. La cl&iacute;nica sugestiva de neumot&oacute;rax consiste en dolor tor&aacute;cico progresivo, dolor con la respiraci&oacute;n profunda o acortamiento progresivo de los movimientos respiratorios, que se desarrollan en las primeras horas o en los primeros dos d&iacute;as. Habitualmente es de peque&ntilde;o tama&ntilde;o y sin repercusi&oacute;n ventilatoria, trat&aacute;ndose conservadoramente; en caso contrario, puede requerir la inserci&oacute;n de un tubo de drenaje tor&aacute;cico. En series amplias aparece tan s&oacute;lo en 0.2-0.5% de los pacientes<sup>12,51</sup>.</font></p>     <p><font face="Verdana" size="2"><i><b>Hemot&oacute;rax</b></i></font></p>     <p><font face="Verdana" size="2">Suelen ser peque&ntilde;os y poco frecuentes (0.1-0.3%) en la mayor&iacute;a de las series<sup>12,32,51</sup>. Peque&ntilde;as hemorragias intrator&aacute;cicas que no se aclaran r&aacute;pidamente pueden necesitar la colocaci&oacute;n de un tubo de drenaje tor&aacute;cico. Sin embargo, hemorragias persistentes pueden precisar la realizaci&oacute;n de video-toracoscopia, o si el sangrado es extenso y muy vivo toracotom&iacute;a abierta. Hay que tener en cuenta que esta complicaci&oacute;n puede llegar a ser fatal<sup>32,51</sup>.</font></p>     <p><font face="Verdana" size="2"><i><b>Infecci&oacute;n</b></i></font></p>     <p><font face="Verdana" size="2">Es otra complicaci&oacute;n muy poco frecuente (0.1%) en las diferentes series<sup>32,51</sup>. Si el paciente desarrolla una infecci&oacute;n en la cavidad tor&aacute;cica (empiema) suele ser necesario la inserci&oacute;n de un tubo de drenaje tor&aacute;cico y la instauraci&oacute;n de tratamiento antibi&oacute;tico. La formaci&oacute;n de empiema o absceso a otros niveles suele requerir drenaje quir&uacute;rgico y terapia antibi&oacute;tica<sup>51</sup>.</font></p>     <p><font face="Verdana" size="2"><i><b>Dolor postoperatorio</b></i></font></p> <font face="Verdana" size="2">Dolor pleur&iacute;tico persistente e intenso suele seguir a extensa lisis de adhesiones pleurales o a aparici&oacute;n de neumot&oacute;rax persistente, efusi&oacute;n pleural o empiema<sup>4,7,47</sup>. Para su tratamiento pueden usarse analg&eacute;sicos sist&eacute;micos (morfina, paracetamol, antiinflamatorios no esteroideos, etc.) y anest&eacute;sicos locales introducidos en cavidad tor&aacute;cica a trav&eacute;s de un tubo tor&aacute;cico. As&iacute;, el uso de anestesia intrapleural bilateral mediante la colocaci&oacute;n en la cavidad tor&aacute;cica de 10-20 ml de bupica&iacute;na al 0.5% y 5 mg/ml de adrenalina al final de la simpatectom&iacute;a, consigue una reducci&oacute;n significativa del dolor y del uso de analg&eacute;sicos<sup>4</sup>. Otra medida que puede reducir significativamente el dolor postoperatorio y el requerimiento de analg&eacute;sicos es el bloqueo de los nervios intercostales internos con 3 ml de bupica&iacute;na al 0.5% en cada uno de los niveles intercostales T2, T3 y T4<sup>7,47</sup>.</font></p>     <p><font face="Verdana" size="2">Tras la cirug&iacute;a algunos pacientes desarrollan cl&iacute;nica de neuralgia intercostal, consistente en dolor quemante e hipersensibilidad cut&aacute;nea a nivel de axila, aspecto cubital de miembro superior, pared tor&aacute;cica superior y mama<sup>27</sup>. Aunque suele ser autolimitada, puede persistir varios meses, siendo &uacute;til habitualmente medicaciones antineur&iacute;ticas (carbamazepina, baclof&eacute;n, gabapentina, amitriptilina). La incidencia de esta complicaci&oacute;n disminuye usando endoscopios flexibles o endoscopios r&iacute;gidos de peque&ntilde;o calibre y reduciendo el n&uacute;mero de puertos de entrada<sup>27,30</sup>.</font></p>     <p><font face="Verdana" size="2"><i><b>Mortalidad</b></i></font></p>     <p><font face="Verdana" size="2">Aunque es un procedimiento menos invasivo que la simpatectom&iacute;a abierta, se han comunicado varias muertes. Las causas de las mismas han sido infecci&oacute;n, empiema, hemoneumot&oacute;rax y complicaciones anest&eacute;sicas<sup>60,61</sup>. Una amenaza seria para la vida ocurre cuando se produce hemorragia grave por lesi&oacute;n de las venas cava o &aacute;zigos<sup>60</sup>.</font></p>     <p>&nbsp;</p>     ]]></body>
