<body><![CDATA[<p><font face="Verdana" size="2"><b>Introducci&oacute;n</b></font></p>     <p><font face="Verdana" size="2">La fibromialgia es una enfermedad que cursa con dolor cr&oacute;nico generalizado al que se a&ntilde;aden otras condiciones cl&iacute;nicas como son la rigidez muscular, los trastornos del sue&ntilde;o, la fatiga cr&oacute;nica, la ansiedad, la depresi&oacute;n, o las deficiencias de tipo neurocognitivo y autoinmune.</font></p>     <p><font face="Verdana" size="2">Es una enfermedad que afecta a un var&oacute;n por cada diez mujeres, especialmente en la edad media de la vida (1). La prevalencia es elevada, llegando a cifrarse en torno al 2% de la poblaci&oacute;n general (2), y a pesar de ser actualmente un problema sociosanitario de envergadura (3), los tratamientos de efectividad contrastada son escasos.</font></p>     <p><font face="Verdana" size="2">En cuanto a la etiopatogenia, la fibromialgia es una enfermedad dif&iacute;cil de ubicar, en cuyo desarrollo se han involucrado aspectos neurol&oacute;gicos, psicol&oacute;gicos, endocrinol&oacute;gicos e inmunol&oacute;gicos.</font></p>     <p><font face="Verdana" size="2">Parece tratarse de la "enfermedad del agotamiento", en la que el individuo pierde su capacidad de adaptaci&oacute;n ante los diversos estresores, desarrollando una cl&iacute;nica en la que coexisten signos y s&iacute;ntomas de lo m&aacute;s diverso.</font></p>     <p><font face="Verdana" size="2">La similitud y diversidad sintom&aacute;tica con pacientes que han afrontado situaciones estresantes, como ocurre en los s&iacute;ndromes postconcusionales, avala la hip&oacute;tesis de la desadaptaci&oacute;n, bien primaria (que se desarrolla paulatinamente) o secundaria, es decir, con un evento estresante definido, de cualquier naturaleza, que el paciente relaciona con el comienzo del proceso morboso.</font></p>     <p><font face="Verdana" size="2">Se puede concluir que un trastorno inespec&iacute;fico de la desadaptaci&oacute;n desemboca en un crisol sintomatol&oacute;gico que se establece sobre una cl&iacute;nica b&aacute;sica com&uacute;n a todos los pacientes, por eso todos son similares, pero todos son distintos, ya que en cada paciente habr&aacute; una situaci&oacute;n concreta que limite en especial la calidad de vida.</font></p>     <p><font face="Verdana" size="2">En este trabajo se revisan las caracter&iacute;sticas principales de la enfermedad desde el punto de vista del estr&eacute;s oxidativo como base biol&oacute;gica del deterioro org&aacute;nico y de los principales s&iacute;ntomas que padece el paciente.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Estr&eacute;s emocional y estr&eacute;s oxidativo</b></font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">De entre las condiciones asociadas a la enfermedad se han descrito varias que se podr&iacute;an relacionar con situaciones estresantes; como tales tenemos la mayor tendencia a la somatizaci&oacute;n, los malos tratos recibidos en la infancia, el sufrimiento de traumatismos previos, el haber padecido determinados procesos infecciosos a lo largo de la vida, o incluso la edad media avanzada o la condici&oacute;n de divorciado. La diversidad de los factores desencadenantes hace pensar en procesos de desadaptaci&oacute;n m&aacute;s o menos generalizada, condicionados por la exposici&oacute;n a estresores de naturaleza diversa.</font></p>     <p><font face="Verdana" size="2">Para muchos autores, la enfermedad, a&uacute;n de etiopatogenia desconocida, es un s&iacute;ndrome som&aacute;tico funcional cuya cl&iacute;nica se solapa con otros similares, como pueden ser el s&iacute;ndrome de fatiga cr&oacute;nica o el trastorno de estr&eacute;s postraum&aacute;tico. Se supone que en el desarrollo de todos estos procesos jugar&iacute;a un papel clave el estr&eacute;s emocional ante experiencias vitales adversas (4). Van Houdenhove y colaboradores consideran a la fibromialgia como una enfermedad del estr&eacute;s, y llevaron a cabo una revisi&oacute;n sobre el papel etiopatog&eacute;nico y la relevancia cl&iacute;nica de los eventos estresantes en el desarrollo de s&iacute;ndromes que cursan con fatiga y dolor generalizado, proponiendo nuevos modelos de investigaci&oacute;n que conecten el estr&eacute;s psicol&oacute;gico con el neurobiol&oacute;gico (5). Y efectivamente, desde el punto de vista de la biolog&iacute;a celular, podemos observar que las caracter&iacute;sticas cl&iacute;nicas m&aacute;s relevantes de la enfermedad cursan con trastornos en los que se han descrito alteraciones en el balance de oxido-reducci&oacute;n celular.</font></p>     <p><font face="Verdana" size="2">Los problemas psiqui&aacute;tricos son comunes en fibromialgia, y se ha sugerido que una tercera parte de los pacientes padecen ansiedad o depresi&oacute;n; el trastorno de ansiedad se puede diagnosticar en el 27-60% de los pacientes, mientras que la depresi&oacute;n mayor oscila entre el 14 y el 23% (6). En comparaci&oacute;n con los pacientes con artritis reumatoide, Walker y su equipo demostraron tasas significativamente superiores de trastornos psiqui&aacute;tricos en los pacientes con fibromialgia (90% frente al 49%) (7). Desde el punto de vista biocelular, Sarandol y su equipo (8), al comparar 96 pacientes con depresi&oacute;n mayor con 54 sujetos sanos, pudieron demostrar marcadores de estr&eacute;s oxidativo estad&iacute;sticamente m&aacute;s elevados en los pacientes con depresi&oacute;n. As&iacute;, hallaron niveles plasm&aacute;ticos elevados de malondialdeh&iacute;do (marcador del da&ntilde;o lip&iacute;dico por estr&eacute;s oxidativo) y una mayor susceptibilidad de los eritrocitos a la oxidaci&oacute;n; ambos par&aacute;metros se correlacionaban con un aumento de la actividad de la super&oacute;xido dismutasa (SOD) eritrocitaria. Tambi&eacute;n se vio que estos marcadores no revert&iacute;an tras seis meses de tratamiento farmacol&oacute;gico.</font></p>     <p><font face="Verdana" size="2">Felicity y colaboradores (9) llevaron a cabo una revisi&oacute;n a cerca del papel del estr&eacute;s oxidativo en los trastornos psiqui&aacute;tricos, concluyendo que la implicaci&oacute;n del mismo es relevante, tanto en la depresi&oacute;n mayor como en los trastornos de ansiedad.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Fatiga cr&oacute;nica y estr&eacute;s oxidativo</b></font></p>     <p><font face="Verdana" size="2">Aparte de los trastornos an&iacute;micos, la fatigabilidad muscular en los pacientes con fibromialgia supone otra limitaci&oacute;n cl&iacute;nica importante, y la falta de lesiones musculares anatomopatol&oacute;gicamente concluyentes (10) podr&iacute;an apuntar hacia un trastorno de estr&eacute;s oxidativo a nivel fibrilar (11) con una mala gesti&oacute;n energ&eacute;tica por parte de la c&eacute;lula muscular. Es una realidad que el agotamiento del m&uacute;sculo esquel&eacute;tico se relaciona con una elevaci&oacute;n de especies reactivas de ox&iacute;geno m&aacute;s all&aacute; de las tasas normales, y se ha demostrado que el pretratamiento con suplementos antioxidantes es capaz de limitar la fatigabilidad muscular (12).</font></p>     <p><font face="Verdana" size="2">Como se ha comentado, la fibromialgia es una enfermedad estrechamente asociada al s&iacute;ndrome de fatiga cr&oacute;nica, y ambas condiciones a menudo se confunden. Las dos son m&aacute;s frecuentes en mujeres que en hombres y cursan con dolor y fatiga generalizados, alteraciones del sue&ntilde;o, trastornos afectivos, depresi&oacute;n inmunitaria, etc. Se ha sugerido que estas enfermedades y otras similares como el trastorno de estr&eacute;s postraum&aacute;tico y la hipersensibilidad a multiqu&iacute;micos cursan con niveles elevados de &oacute;xido n&iacute;trico, que en presencia de ani&oacute;n super&oacute;xido ser&iacute;a oxidado, formando peroxinitritos y agot&aacute;ndose, con el consiguiente d&eacute;ficit vascular (13). Kennedy y su grupo han demostrado que, tanto en ni&ntilde;os como en adultos con s&iacute;ndrome de fatiga cr&oacute;nica/encefalomielitis mi&aacute;lgica se registran niveles elevados de estr&eacute;s oxidativo en relaci&oacute;n con los controles (14).</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Trastornos del sue&ntilde;o y estr&eacute;s oxidativo</b></font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">El sue&ntilde;o no adecuado tambi&eacute;n es propio de la enfermedad, lo que se ha relacionado con d&eacute;ficit en la producci&oacute;n de hormona de crecimiento (directamente implicada con el deterioro muscular) (15) y con la disminuci&oacute;n de los niveles de tript&oacute;fano y serotonina, ambos precursores de la melatonina, de marcado car&aacute;cter antioxidante (16,17). En este contexto es interesante remarcar que en el modelo del s&iacute;ndrome de apnea obstructiva del sue&ntilde;o se han evidenciado niveles elevados de estr&eacute;s oxidativo, con aumento de la peroxidaci&oacute;n lip&iacute;dica a nivel plasm&aacute;tico, aumentos de adenosina y &aacute;cido &uacute;rico en orina y disminuci&oacute;n de &oacute;xido n&iacute;trico endotelial, circunstancias metab&oacute;licas similares a las que se han descrito en fibromialgia (18), y que supondr&iacute;an isquemia, acidosis y dolor.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Hiperexcitabilidad central y estr&eacute;s oxidativo</b></font></p>     <p><font face="Verdana" size="2">Hoy d&iacute;a se acepta la existencia de fen&oacute;menos de excitabilidad central; estos fen&oacute;menos se han relacionado con alteraciones de la neurotransmisi&oacute;n, anal&iacute;ticamente constatadas por alteraciones de las tasas de diferentes neurotransmisores en el plasma y el l&iacute;quido cefalorraqu&iacute;deo. Estudios precl&iacute;nicos recientes han demostrado el papel de las especies reactivas del ox&iacute;geno (Reactive Oxigen Species: ROS) en la hiperalgesia, v&iacute;a activaci&oacute;n de los receptores N-metil-D-aspartato (NMDA). Gao y su equipo (19), en un modelo precl&iacute;nico de dolor, tanto neurop&aacute;tico como inflamatorio, pudieron demostrar que las ROS a nivel del asta dorsal acompa&ntilde;aban al dolor, y que la administraci&oacute;n sist&eacute;mica de un agente neutralizador de las mismas reduc&iacute;a la hiperalgesia mediante el bloqueo de la fosforilaci&oacute;n a nivel de los NMDA. Posteriormente, el mismo grupo de investigaci&oacute;n (20) (induciendo hiperalgesia por capsaicina en ratas) pudo demostrar el papel del ani&oacute;n super&oacute;xido como responsable del procesamiento anormal de la se&ntilde;al dolorosa en el asta dorsal, sugiriendo el papel terap&eacute;utico de la super&oacute;xido dismutasa mitocondrial (SOD-2) en estos tipos de dolor.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>El estr&eacute;s oxidativo en fibromialgia</b></font></p>     <p><font face="Verdana" size="2">La etiopatogenia de la fibromialgia es tan oscura como diversa, y en ella se han implicado mecanismos de &iacute;ndole neurol&oacute;gica, neuroendocrina, traumatol&oacute;gica o inmunol&oacute;gica. Se ha sugerido que en la enfermedad subyace una alteraci&oacute;n del balance &oacute;xido-reductor, con acumulo de radicales libres y distorsi&oacute;n de las capacidades energ&eacute;ticas celulares. Eisinger y su grupo, con objeto de determinar el alcance del da&ntilde;o por estr&eacute;s oxidativo en la enfermedad registraron, a nivel plasm&aacute;tico y eritrocitario, el malondialdehido, los carbonilos proteicos y el &oacute;xido n&iacute;trico. Asimismo, para determinar la capacidad antioxidante tambi&eacute;n midieron las vitaminas A y E, los grupos tiol y los niveles de glutati&oacute;n y glutati&oacute;n peroxidasa. Estos autores pudieron observar que los carbonilos proteicos (productos de la peroxidaci&oacute;n proteica) estaban aumentados, y el &oacute;xido n&iacute;trico y los grupos tiol estaban disminuidos con respecto a los controles (21).</font></p>     <p><font face="Verdana" size="2">La pentosidina es un producto de la glicaci&oacute;n proteica que suele estar elevado en enfermedades propias del estr&eacute;s oxidativo tales como el Alzheimer, la diabetes o la arterioesclerosis, y Hein y Franke (22) encontraron niveles elevados de la misma en el plasma de pacientes con fibromialgia, sugiriendo procesos inflamatorios cr&oacute;nicos y disfunciones neuronales.</font></p>     <p><font face="Verdana" size="2">Posteriormente, Bagis y su grupo (23) en una muestra de 85 pacientes con fibromialgia primaria (comparados con 80 controles) hallaron niveles bajos de la enzima antioxidante super&oacute;xido dismutasa, a la vez que niveles elevados de malondialdehido.</font></p>     <p><font face="Verdana" size="2">En el a&ntilde;o 2006, Ozgocmen y su equipo tambi&eacute;n registraron, con respecto al grupo control, niveles elevados de malondialdehido y niveles reducidos de &oacute;xido n&iacute;trico. Al tratar a los pacientes con sertralina o amitriptilina, estos par&aacute;metros no mejoraron a pesar de hacerlo los registros del FIQ (dolor y ansiedad) y los de la escala de Hamilton, lo que sugiere que la mejor&iacute;a cl&iacute;nica no tiene porque correlacionarse con la mejor&iacute;a en el equilibrio de oxidorreducci&oacute;n de los pacientes (24).</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">Desde el punto de vista anatomopatol&oacute;gico, con microscop&iacute;a &oacute;ptica no se encuentran lesiones que no se hallen en otras miopat&iacute;as, pero con microscop&iacute;a electr&oacute;nica Lund (25) observ&oacute; im&aacute;genes sugerentes de mitocondriopat&iacute;a, alteraci&oacute;n del balance oxidorreductor a nivel miofibrilar y d&eacute;ficit de oxigenaci&oacute;n en torno a los puntos dolorosos. Otros autores tambi&eacute;n han referido que en los m&uacute;sculos de los pacientes con fibromialgia hay un d&eacute;ficit de fosfatos de alta energ&iacute;a, signo de procesos isqu&eacute;micos que generar&iacute;an el dolor y la rigidez en la enfermedad; un tono adren&eacute;rgico mantenido podr&iacute;a ser el origen de la situaci&oacute;n (26,27). Morf y cols. (28) evidenciaron alteraciones vasculares perif&eacute;ricas que mermar&iacute;an la oxigenaci&oacute;n tisular; con flujometr&iacute;a laser y microscop&iacute;a capilar observaron que los pacientes con fibromialgia ten&iacute;an menos capilaridad y m&aacute;s dilataciones e irregularidades capilares que los controles sanos. M&aacute;s recientemente, Mclver y cols. (29) demostraron un menor flujo capilar, tanto en reposo como en respuesta al ejercicio aer&oacute;bico, en los m&uacute;sculos de los pacientes con fibromialgia, hecho que se correlacion&oacute; con una mayor fatigabilidad en el grupo de pacientes que en los controles sanos.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Conclusi&oacute;n</b></font></p>     <p><font face="Verdana" size="2">A la vista de lo expuesto, se puede considerar que la fibromialgia es una &quot;enfermedad del estrés&quot; en la que subyace una alteración del balance de óxido-reducción celular, consecuencia de un aumento de la producción de radicales libres, una deficiencia de la capacidad antioxidante orgánica o de ambas circunstancias simultáneamente. Los hallazgos bioquímicos soportan esta realidad, y tratamientos tales como una dieta antioxidante, un plan de suplementos alimentarios o la ozonoterapia sist&eacute;mica, con capacidad para mejorar la capacidad antioxidante, podr&iacute;an ser opciones seguras y bien toleradas subsidiarias de ser ensayadas en el tratamiento de la fibromialgia.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Referencias</b></font></p>     <!-- ref --><p><font face="Verdana" size="2">1.
