El ejercicio terapéutico, es uno de los elementos claves en los programas diseñados para el manejo de esta condición. Se realizará una búsqueda de la evidencia a través de una revisión sistemática de ensayos clínicos aleatorizados que hayan determinado la efectividad del ejercicio terapéutico para el manejo de estos pacientes. El objetivo de esta Revisión es determinar si existe evidencia científica que avale la efectividad clínica del ejercicio terapéutico sólo o dentro de un programa de tratamiento en pacientes adultos con epicondilalgia lateral. La estrategia de bús-queda incluyó ensayos clínicos aleatorizados (ECA) y ensayos clínicos controlados (ECC); las bases de datos usadas fueron: Medline, Cinahl, PEDro, Central, Lilacs. Los resultados obtenidos fueron de treinta y seis artículos que cumplían con nuestros criterios de elegibilidad. Se concluye que existe evidencia moderada que el ejercicio terapéutico comparado con una intervención única, disminuye el dolor, mejora la funcionalidad y aumenta la fuerza libre de dolor en corto, medio y largo plazo. Existe evidencia de que el ejercicio terapéutico disminuye el dolor, y aumenta la fuerza sin dolor a corto plazo. Existe evidencia fuerte de que el ejercicio terapéutico disminuye el dolor, aumenta la fuerza y mejora la funcionalidad a corto, medio y largo plazo.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[The lateral epicondylalgia is one of the most common injuries of the arm; with the muscle extensor carpi radialis brevis (ERCB) affected the structure, repetitive motion, rapids and monotonous, mainly affects between 1-3% of the general population, being more common in women. Therapeutic exercise is one of the key elements in programs designed for the management of this condition. There will be a synthesis of evidence through a systematic review of randomized clinical trials that established the effectiveness of exercise therapy in the management of these patients. The aim of this review is to determine whether there is scientific evidence to support the clinical effectiveness of exercise therapy alone or in a treatment program for adult patients with LE. The search strategy included randomized clinical trials (RCTs) and controlled clinical trials (CCTs) databases were used: Medline, CINAHL, PEDro, Central, Lilacs. The results obtained were thirty-six articles that met our eligibility criteria. We conclude that there is moderate evidence shows that therapeutic exercise alone compared with a single intervention, decreases pain, improves function and increases strength pain free in short, medium and long term. There is strong evidence that exercise therapy reduces pain, increases strength and free from short-term pain. There is strong evidence that exercise therapy reduces pain, increases strength and improves functionality in the short, medium and long term.]]></p></abstract>

Randomized clinical trial.</font></p> <hr size="1">     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Introducción</b></font></p>     <p><font face="Verdana" size="2">El t&eacute;rmino "tendinopat&iacute;a" es usado preferentemente para describir varias patolog&iacute;as del tend&oacute;n, incluyendo paratendinitis, tendinitis y tendinosis (1). La tendinopat&iacute;a lateral del codo com&uacute;nmente referida como epicondilitis lateral (EL), es una de las lesiones m&aacute;s comunes del brazo, siendo el m&uacute;sculo extensor radial corto del carpo (ERCB) la estructura m&aacute;s afectada (2). Se define como un s&iacute;ndrome de dolor en el &aacute;rea del epic&oacute;ndilo lateral (3-5), causado por el exceso de movimientos repetitivos, r&aacute;pidos y mon&oacute;tonos (6-9), teniendo como principales s&iacute;ntomas el dolor, la disminuci&oacute;n de la funci&oacute;n de agarre y resistencia en la flexi&oacute;n dorsal de mu&ntilde;eca (10-12). Esta condici&oacute;n m&uacute;sculo esquel&eacute;tica afecta principalmente entre 1-3% de la poblaci&oacute;n general (13-23), increment&aacute;ndose a un 15 % en trabajadores de industrias que requieren tareas manuales repetitivas (24). Adem&aacute;s, aunque ocurre en todas las edades (25), afecta predominantemente entre los 30 y 60 a&ntilde;os (2,26,27). A pesar que la proporci&oacute;n de los pacientes afectados por esta patolog&iacute;a no est&aacute; influenciado por el sexo del paciente, parece ser que la mayor duraci&oacute;n y gravedad de la injuria se encuentra en las mujeres (4). Aun as&iacute; la duraci&oacute;n promedio de los s&iacute;ntomas es muy variable; se reportan rangos entre 6 meses a 2 a&ntilde;os en pacientes que no han recibido tratamiento (28). Gran parte de los mecanismos patog&eacute;nicos de las tendinopat&iacute;as no est&aacute;n claramente establecidos (29); a&uacute;n siguen siendo un tema de debate en la literatura; si estas corresponden a un proceso inflamatorio o degenerativo. A pesar de que datos histopatol&oacute;gicos son categ&oacute;ricos en concluir que la tendinopat&iacute;a no es una condici&oacute;n de tipo inflamatoria (30). Sino que m&aacute;s bien, sugieren que corresponde a la presencia de col&aacute;geno desorganizado m&aacute;s que a la presencia de c&eacute;lulas inflamatorias (31-33). Por otra parte resulta importante mencionar que cuando el tend&oacute;n est&aacute; sobrecargado o sometido a estos microtraumatismos repetitivos, se traducen en la desorganizaci&oacute;n de las fibras de col&aacute;geno, aumento de fibroblastos, presencia de proteoglicanos y glicosaminoglicanos (10,34). Por lo tanto, dichas injurias acumulativas no s&oacute;lo debilitar&iacute;a al col&aacute;geno sino que adem&aacute;s se asocian a cambios patol&oacute;gicos de la matriz extracelular (MEC) y a elementos vasculares del tend&oacute;n (35,36). Todo esto debiera ser considerado en un an&aacute;lisis para las distintas intervenciones recomendadas para el tratamiento de esta condici&oacute;n m&uacute;sculo esquel&eacute;tica. Numerosos tratamientos han sido descritos en la literatura para las tendinopat&iacute;as dependiendo de su fase de presentaci&oacute;n (37,38), siendo las inyecciones de corticoides y la fisioterapia los tratamientos m&aacute;s comunes (39,40). Es por esto, que como en una primera instancia se establece la reducci&oacute;n de la sintomatolog&iacute;a a trav&eacute;s de hielo, reposo relativo (41) y algunas modalidades fisioterap&eacute;uticas como ultrasonido y l&aacute;ser (42). Es necesario establecer que existe una evidencia insuficiente con respecto a su efectividad a largo plazo (43). Pero cabe destacar, que por m&aacute;s que existan tratamientos con diferentes mecanismos te&oacute;ricos de acci&oacute;n, todos tienen un mismo objetivo, en reducir el dolor y mejorar la funci&oacute;n (44). Sin embargo; estudios han se&ntilde;alado que a pesar de la resoluci&oacute;n de s&iacute;ntomas mediante tratamientos conservadores permanece el d&eacute;ficit sensoriomotor (45), cuyas alteraciones se encuentran presentes de forma bilateral en pacientes con EL (46,47). Cabe destacar que el sistema sensoriomotor se define como un componente del control motor, el cual se utiliza para describir su componente sensorial y la integraci&oacute;n del procesamiento central junto con los elementos implicados en el mantenimiento de la estabilidad durante el movimiento din&aacute;mico (48), que incluye la posici&oacute;n sensorial de la articulaci&oacute;n, sensaci&oacute;n de fuerza y control neuromuscular, implicando ambos mecanismos de <i>feedback y feedforward</i> (49). A su vez, se ha establecido que el tiempo de reacci&oacute;n y la velocidad del movimiento son medidas simples de la funci&oacute;n sensoriomotora, esto se ha demostrado que en pacientes con EL resulta ser significativamente m&aacute;s lento comparado con pacientes sanos (46,47). Este d&eacute;ficit ha sido identificado previamente en otras condiciones m&uacute;sculo esquel&eacute;ticas cr&oacute;nicas tales como el s&iacute;ndrome de dolor patelofemoral (50), s&iacute;ndrome de dolor lumbar (51,52) y trastornos de hombro (53,54). Por lo general, el tratamiento conservador est&aacute; dirigido en disminuir el dolor y mejorar la funcionalidad en pacientes con EL. Algunos autores (23,55,56) sugieren que la mayor&iacute;a de estos pacientes reportan una mejor&iacute;a en su condici&oacute;n a largo plazo, en particular, si adoptan un programa de terapia f&iacute;sica y ejercicio terap&eacute;utico. Siendo ser m&aacute;s eficaz que la inyecci&oacute;n de corticoides (46,47). Cabe destacar; que un buen dise&ntilde;o de ejercicio terap&eacute;utico individual es siempre un componente fundamental en la terapia f&iacute;sica, cuyo objetivo resulta ser el logro de un nivel &oacute;ptimo de movimiento b&aacute;sico hasta llegar a niveles de actividad f&iacute;sica complejas, a su vez, tener presente el impacto que tiene este sobre los efectos en los tejidos. El ejercicio terap&eacute;utico toma su lugar, como uno de los elementos claves en los programas dise&ntilde;ados para mejorar o restaurar la funci&oacute;n (54). Y resulta sistem&aacute;tico, por el desempe&ntilde;o de los movimientos corporales, posturas f&iacute;sicas, o actividades destinadas a proporcionar al paciente el medio para prevenir el deterioro como as&iacute; tambi&eacute;n; restaurar o mejorar la funci&oacute;n f&iacute;sica, optimizando as&iacute; el estado general de salud o sentido de bienestar. Las t&eacute;cnicas seleccionadas para un ejercicio terap&eacute;utico individualizado se basan en la determinaci&oacute;n de las causas subyacentes de las deficiencias o limitaciones funcionales de un paciente (57).</font></p>     <p><font face="Verdana" size="2">Es fundamental destacar el rol del ejercicio ya que sigue siendo unos de los tratamientos m&aacute;s comunes (58-60), como as&iacute; tambi&eacute;n, ha demostrado tener efectos positivos en esta patolog&iacute;a (61). De acuerdo a esto; la literatura sugiere que un programa de ejercicio tanto de fortalecimiento como de estiramiento son componentes principales para el manejo de esta condici&oacute;n, ya que el tend&oacute;n no s&oacute;lo debe ser fuerte, sino que tambi&eacute;n flexible, estimulando de cierta manera los mecanoreceptores de los tenocitos, generando la s&iacute;ntesis de col&aacute;geno (60-65). Adem&aacute;s de que el flujo sangu&iacute;neo se detiene en el &aacute;rea de da&ntilde;o, produci&eacute;ndose neovascularizaci&oacute;n, lo cual mejora el flujo y reparaci&oacute;n a largo plazo (66). Otro fundamento que se atribuye al ejercicio es la analgesia que provocar&iacute;a, con mecanismos profundos en los tejidos despu&eacute;s del ejercicio, involucrando los procesos moduladores perif&eacute;ricos y centrales del dolor (67-71). A su vez, se han recomendado programas de ejercicio exc&eacute;ntrico dentro de los cuales se incorpora la carga, la velocidad y la frecuencia de contracci&oacute;n como elementos relevantes al momento de realizar el ejercicio (72), como as&iacute; tambi&eacute;n se plantean protocolos exc&eacute;ntricos isocin&eacute;ticos desarrollados espec&iacute;ficamente para el trabajo de esta condici&oacute;n pero del cual a&uacute;n no ha sido rigurosamente investigado (73). No obstante; un m&eacute;todo de tratamiento que est&aacute; abarcando la atenci&oacute;n es el fortalecimiento exc&eacute;ntrico (74). Puesto que este programa se basa en fortalecer tanto m&uacute;sculos y tendones del antebrazo (75). Es por esto, que algunos terapeutas confieren que la carga en los ejercicios exc&eacute;ntricos debe ser incrementada en funci&oacute;n a los s&iacute;ntomas del paciente, pues de lo contrario existe una posibilidad de una reagudizaci&oacute;n (76,77). Otro principio b&aacute;sico del ejercicio exc&eacute;ntrico es la velocidad, si bien es cierto, ha sido controversial puesto que algunos afirman que la velocidad debe ser incrementada en cada sesi&oacute;n de tratamiento lo que aumenta la carga del tend&oacute;n (78-80), sin embargo tambi&eacute;n se ha se&ntilde;alado que la velocidad debe ser lenta para evitar una posible lesi&oacute;n (60). De lo mencionado anteriormente uno de los fundamentos para que el ejercicio sea a baja velocidad es probablemente el hecho que no exceda el l&iacute;mite el&aacute;stico del tend&oacute;n y genere as&iacute; menos calor perjudicial para el tejido (76). El tercer principio b&aacute;sico de los ejercicios exc&eacute;ntricos es la frecuencia de contracciones. Las series y repeticiones pueden variar en la literatura, pero algunos autores han planteado tres series de diez repeticiones con el codo en extensi&oacute;n completa y el antebrazo en pronaci&oacute;n puede ser beneficioso, ya que se plantea que se consigue el mejor fortalecimiento de los tendones, seg&uacute;n la tolerancia del paciente (64,76,81). Otro factor com&uacute;n en los programas de ejercicios exc&eacute;ntrico es que pueden ser desarrollados en el hogar sin necesidad de supervisi&oacute;n de visitas de manera regular (82-84).</font></p>     <p><font face="Verdana" size="2">Por otra parte los ejercicios de <i>stretching</i> son propuestos como eficaces para aumentar la flexibilidad en esta condici&oacute;n, entendiendo este concepto como el rango de movimiento posible por medio de una o m&aacute;s articulaciones (84,86). Si bien es cierto; existe una variedad de t&eacute;cnicas para la flexibilidad tales como; bal&iacute;stica, est&aacute;tica y facilitaci&oacute;n neuromuscular propioceptiva, sin embargo no es una preocupaci&oacute;n en cuanto a las t&eacute;cnicas o procedimientos que deben ser utilizados para los &oacute;ptimos beneficios, a&uacute;n as&iacute; es la t&eacute;cnica est&aacute;tica la que resulta m&aacute;s utilizada (78,79,87). El stretching est&aacute;tico est&aacute; definido como un estiramiento pasivo dado en la unidad m&uacute;sculo-tendinosa, que se realiza en estiramiento m&aacute;ximo y mantenido en un per&iacute;odo de tiempo (64,79). En el caso de la EL, esta t&eacute;cnica deber&iacute;a ser realizada en el m&uacute;sculo ECRB, cuya posici&oacute;n se logra en extensi&oacute;n de codo, antebrazo en pronaci&oacute;n, la mu&ntilde;eca en flexi&oacute;n y con desviaci&oacute;n ulnar (64), con una duraci&oacute;n de 3 a 60 segundos (80). Aunque algunos autores sugieren que desde 30 a 45 segundos es m&aacute;s efectivo el incremento de flexibilidad del tend&oacute;n (87-90). El rol que cumple el programa de ejercicios en casa, resulta tambi&eacute;n fundamental en donde la seguridad del paciente mejora y el riesgo de lesi&oacute;n o recidiva se reduce al m&iacute;nimo, para ello es importante la instrucci&oacute;n y educaci&oacute;n de los pacientes. Es por esto, que se debe incorporar principios adecuados de la mec&aacute;nica corporal y la protecci&oacute;n de las articulaciones de manera de minimizar el riesgo de lesi&oacute;n (57).</font></p>     <p><font face="Verdana" size="2">De lo anteriormente expuesto nace nuestra pregunta de investigaci&oacute;n: en la rehabilitaci&oacute;n de pacientes esquel&eacute;ticamente maduros con epicondilalgia lateral, ¿Existe evidencia que el ejercicio terap&eacute;utico s&oacute;lo o dentro de un programa de tratamiento en comparaci&oacute;n con otras intervenciones fisioterap&eacute;uticas es m&aacute;s efectivo en el manejo cl&iacute;nico de la epicondilalgia lateral?</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Objetivo</b></font></p>     <p><font face="Verdana" size="2">Determinar si existe evidencia cient&iacute;fica que avale la efectividad cl&iacute;nica del ejercicio terap&eacute;utico s&oacute;lo o dentro de un programa de tratamiento en pacientes adultos con epicondilalgia lateral.</font></p>     ]]></body>

El riesgo de sesgo se evaluar&aacute; utilizando la herramienta propuesta en el <i>"Cochrane Handbook"</i> para Ensayos Cl&iacute;nicos Aleatorizados (91). <body><![CDATA[<p><font face="Verdana" size="2"><b>S&iacute;ntesis y an&aacute;lisis de datos</b></font></p>     <p><font face="Verdana" size="2">En base al an&aacute;lisis de los datos extra&iacute;dos de los art&iacute;culos que cumpl&iacute;an los criterios de elegibilidad de nuestra revisi&oacute;n sistem&aacute;tica, al determinar la efectividad del ejercicio terap&eacute;utico en EL, debido a la heterogeneidad por la naturaleza de la intervenci&oacute;n, no se pudo realizar la agrupaci&oacute;n de datos en las diferentes medidas de resultados. Debido a la dispersi&oacute;n de tal intervenci&oacute;n en cuanto a tipo y dosis es demasiado variable en cada estudio, por lo tanto, los resultados de los art&iacute;culos no se pueden combinar para llegar a un estimador puntual, debido a la disparidad en la homogeneidad cl&iacute;nica, que se considera cuando los pacientes, intervenciones, medidas de resultados y seguimiento son similares. Sin embargo, para llevar a cabo un meta-an&aacute;lisis se debe evaluar adem&aacute;s la homogeneidad estad&iacute;stica, que es menos plausible de realizar debido que no se cumple el primer criterio.</font></p>     <p><font face="Verdana" size="2"><b>Criterios para valoraci&oacute;n de los resultados</b></font></p>     <p><font face="Verdana" size="2">De los 36 art&iacute;culos seleccionados por nuestra b&uacute;squeda, no estaban en condiciones de poder agruparse en una comparaci&oacute;n en base a un estimador puntual. Por este motivo, se utilizar&aacute; un m&eacute;todo cualitativo recomendado por el Grupo Cochrane de Espalda (91) con el uso de niveles de evidencia para la s&iacute;ntesis de los datos (<a href="#t2">Tabla II</a>).</font></p>     <p>&nbsp;</p>     <p align="center"><font face="Verdana" size="2"><a name="t2"><img src="/img/revistas/dolor/v22n6/07_revisionmbe_tabla2.jpg"></a></font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Resultados</b></font></p>     <p><font face="Verdana" size="2"><b>Selecci&oacute;n y caracter&iacute;sticas de los estudios</b></font></p>     <p><font face="Verdana" size="2">De acuerdo a los criterios de nuestra revisi&oacute;n, la b&uacute;squeda preliminar identific&oacute; 1.144 art&iacute;culos potencialmente elegibles, al aplicar los l&iacute;mites de b&uacute;squeda y los criterios de selecci&oacute;n; quedaron 36 estudios En los cuales eran 21 ECAs y 15 ECC seleccionados (7-9,11-23,25,27,29,31-34,37-40,43-45,61,73-75,82-84) (n = 2.318 pacientes). Ocho art&iacute;culos fueron excluidos por los motivos previamente descritos (<a href="#t3">Tabla III</a>). Todos se realizaron en pacientes con epicondilalgia lateral. Se agruparon en ejercicio terap&eacute;utico s&oacute;lo <i>versus</i> cualquier otra intervenci&oacute;n sola (n = 206), ejercicio terap&eacute;utico dentro de un programa de tratamiento <i>versus</i> cualquier otra intervenci&oacute;n sola (n = 1.417 pacientes) y ejercicio terap&eacute;utico dentro de un programa de tratamiento comparado con otro programa de tratamiento (n = 695), los tama&ntilde;os de la muestra variaron entre 5 y 198 pacientes con un promedio de 72 pacientes por estudio, el rango de edad de los pacientes estaba entre 18 y 74 a&ntilde;os con un promedio de 46 a&ntilde;os.</font></p>     ]]></body>
<body><![CDATA[<p>&nbsp;</p>     <p align="center"><font face="Verdana" size="2"><a name="t3"><img src="/img/revistas/dolor/v22n6/07_revisionmbe_tabla3.jpg"></a></font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><i>Intervenciones y comparaciones</i></font></p>     <p><font face="Verdana" size="2">Las intervenciones terap&eacute;uticas estudiadas fueron: el ejercicio terap&eacute;utico s&oacute;lo y el ejercicio terap&eacute;utico junto a otras intervenciones. En base a los resultados de nuestra b&uacute;squeda agrupamos los art&iacute;culos seleccionados en las siguientes comparaciones: ejercicio terap&eacute;utico s&oacute;lo <i>versus</i> otra intervenci&oacute;n sola (15,18,33,61), ejercicio terap&eacute;utico s&oacute;lo <i>versus</i> un programa de tratamiento (7,9,12,17,19,20,29,34,37,39,43,45,73-75) y programa de tratamiento que incluya ejercicio terap&eacute;utico <i>versus</i> otro programa de tratamiento (8,11,14,16,21-23,25,27,31,38,40,44,82-84).</font></p>     <p><font face="Verdana" size="2"><i>Mediciones de resultados</i></font></p>     <p><font face="Verdana" size="2">Las medidas de resultado m&aacute;s com&uacute;nmente utilizadas en los art&iacute;culos seleccionados fueron: Dolor mediante la escala de EVA, funcionalidad medida con la escala DASH y la fuerza de agarre medida con un dinam&oacute;metro. Todos los art&iacute;culos seleccionados por nuestra b&uacute;squeda (7-9,11,12,14-23,25,27,29,31-34,37-40,43-45,61,73-75,82-84) estudiaron los efectos con un seguimiento m&aacute;ximo de 3 a&ntilde;os.</font></p>     <p><font face="Verdana" size="2"><b>S&iacute;ntesis y agrupaci&oacute;n de art&iacute;culos incluidos</b></font></p>     <p><font face="Verdana" size="2">El resumen de todos los aspectos metodol&oacute;gicos de los art&iacute;culos seleccionados se detalla en la <a target="_blank" href="/img/revistas/dolor/v22n6/07_revisionmbe_tabla4.pdf">tabla IV</a>, a continuaci&oacute;n se presentar&aacute;n los resultados obtenidos.</font></p>     <p><font face="Verdana" size="2"><i>Ejercicio terap&eacute;utico solo versus cualquier otra intervenci&oacute;n sola</i></font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">Los 4 estudios n = 206 pacientes (18,15,33,61) investigaron la efectividad del ejercicio terap&eacute;utico s&oacute;lo comparado con una intervenci&oacute;n sola en pacientes con EL.</font></p>     <p><font face="Verdana" size="2"><i>Ejercicio terap&eacute;utico dentro de un programa de tratamiento versus cualquier otra intervenci&oacute;n sola</i></font></p>     <p><font face="Verdana" size="2">Los 17 estudios n = 1.417 pacientes (7,9,12,17,19,20,23, 26,29,32,34,37,43,45,73-75) investigaron la efectividad del ejercicio terap&eacute;utico dentro de un programa de tratamiento comparado con una intervenci&oacute;n sola en pacientes con EL.</font></p>     <p><font face="Verdana" size="2"><i>Ejercicio terap&eacute;utico dentro de un programa de tratamiento versus otro programa de tratamiento</i></font></p>     <p><font face="Verdana" size="2">Los 15 estudios n = 695 pacientes (8,11,14,16,21,22,25, 27,31,38,40,44,82-84) investigaron la efectividad del ejercicio terap&eacute;utico dentro de un programa de tratamiento comparado con una intervenci&oacute;n sola en pacientes con EL.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Discusi&oacute;n</b></font></p>     <p><font face="Verdana" size="2">Se intent&oacute; determinar si existe evidencia cient&iacute;fica que avale la efectividad cl&iacute;nica del ejercicio terap&eacute;utico s&oacute;lo o dentro de un programa de tratamiento en comparaci&oacute;n con otra intervenci&oacute;n fisioterap&eacute;utica en pacientes adultos con epicondilalgia lateral. Para lograrlo se realiz&oacute; una s&iacute;ntesis de la evidencia disponible para responder la interrogante planteada precedentemente. En un comienzo debido a la amplia variedad y compleja clasificaci&oacute;n de todos los ejercicios terap&eacute;uticos se establecieron criterios de inclusi&oacute;n y exclusi&oacute;n para acotar la variabilidad de ejercicios propuestos. De acuerdo a esto seg&uacute;n la clasificaci&oacute;n de Kisner (60) adicionado a los criterios ya establecidos se propusieron los ejercicios terap&eacute;uticos m&aacute;s comunes como: ejercicios conc&eacute;ntricos, ejercicios exc&eacute;ntricos, ejercicios de resistencia, ejercicios sensoriomotrices, ejercicios de estiramientos, ejercicio isocin&eacute;tico y programa de ejercicio en casa. Seg&uacute;n lo planteado, nuestra b&uacute;squeda arroj&oacute; 36 art&iacute;culos seleccionados que no estaban en condiciones de agruparse en base a un estimador puntual, debido a la heterogeneidad de la naturalidad de las intervenciones de esta revisi&oacute;n sistem&aacute;tica sobre ejercicio terap&eacute;utico, se hace dif&iacute;cil la extracci&oacute;n de datos, entendiendo as&iacute; que para llevar a cabo un meta-an&aacute;lisis se debe evaluar adem&aacute;s la homogeneidad estad&iacute;stica, que es menos plausible de realizar debido que no se cumple el primer criterio propuesto. Por este motivo se utiliz&oacute; un m&eacute;todo cualitativo con el uso de niveles de evidencia para la s&iacute;ntesis de los datos, situaci&oacute;n que es dif&iacute;cil para el an&aacute;lisis final ya que carece de un poder estad&iacute;stico que proporcione pruebas concluyentes para la extrapolaci&oacute;n cl&iacute;nica. En cuanto a la evaluaci&oacute;n del riesgo de sesgo de los art&iacute;culos incluidos (riesgo de sobreestimar o subestimar los resultados) realizado por los autores de la presente revisi&oacute;n se establecieron criterios para designar bajo riesgo sesgo, alto riesgo sesgo y riesgo de sesgo incierto. En el primer punto de la evaluaci&oacute;n que corresponde a la generaci&oacute;n de la secuencia aleatoria se consider&oacute; bajo riesgo de sesgo cuando: se utiliza un generador de secuencia aleatoria, sorteo, utilizaci&oacute;n de una moneda, lanzamiento de dados. Alto riesgo de sesgo cuando: Secuencia generada por fecha de nacimiento, seg&uacute;n d&iacute;as de admisi&oacute;n, secuencia generada por juicio cl&iacute;nico. Riesgo de sesgo incierto cuando: si el m&eacute;todo de secuencia no se describe o falta informaci&oacute;n sobre el proceso. En el punto de cegamiento de participantes, terapeutas y evaluador, se decidi&oacute; realizar un consenso y juicio sobre la evaluaci&oacute;n de este punto. Debido a la naturaleza de la intervenci&oacute;n en este caso el ejercicio, resulta dif&iacute;cil cegar al paciente o al terapeuta, ya que siempre se conoce la terapia a realizar. Por lo tanto se consider&oacute; Bajo riesgo de sesgo en este punto para todos los art&iacute;culos. Sin embargo para el cegamiento de los evaluadores se puede castigar categ&oacute;ricamente si no cumple este punto. Es considerado Bajo riesgo de sesgo cuando: Si se reporta en el estudio. Alto riesgo de sesgo: Si no lo reporta en el estudio. Riesgo de sesgo incierto: Cuando el ocultamiento no se describe o falta informaci&oacute;n sobre el proceso. De todos los estudios seleccionados ninguno tiene inscrito el protocolo de investigaci&oacute;n en alg&uacute;n sistema de registro existente, sin embargo, presentaban los datos reportados para cada estudio. De acuerdo a esto, tienen bajo riesgo de sesgo; Bisset (23), Bisset (45), Blanchette (17), Chung (22), Hernandez (21), Kochar (27), Nagrale (31), Peterson (15), Pienimaki (7), Radpasand (16), Smidt (43), Stasinopoulos (72), Stasinopoulos (83), Stergioulas (14), Struijs (37), Struijs (19), Tyler (82), Verhaar (39), Viswas (11), con moderado riesgo de sesgo; Ajimsha (18), Bhardwaj (32), Coff (9), Croisier (73), Dreschler (29), Manias (44), Martinez-Silvestrini (74), Ho (12), Oken (8), Park (84), Soderberg (20) , Stasinopoulos (38) y con alto riesgo de sesgo; Luginbuhl (75), Newcomer (40), Nilsson (25), Pienimaki (61), Svernlov (33). Existe una RS publicada por Raman (92) que indica que los ejercicios de resistencia pueden reducir el dolor, aumentar la fuerza, y la funci&oacute;n en EL, sin embargo si existe una intervenci&oacute;n variable es el ejercicio, por lo cual, resulta complejo establecer una dosis &oacute;ptima y espec&iacute;fica para tal condici&oacute;n, lo que hace m&aacute;s dif&iacute;cil la aplicabilidad de los resultados. En cuanto a la comparaci&oacute;n del ejercicio s&oacute;lo versus cualquier otra intervenci&oacute;n sola, dentro de los 4 art&iacute;culos (15,18,33,61) agrupados, en los estudios de Peterson (15) y Ajimsha (18) establecen diferencias significativas a favor del grupo de ejercicios en las medidas de resultados de dolor y funcionalidad a corto plazo. En el estudio de Pienimaki (61) muestra diferencias significativas en la medida de resultado de dolor en el grupo de ejercicio a largo plazo. En el estudio de Svernlov (33) mostr&oacute; diferencias significativas en la medida de resultado de dolor y grip libre de dolor para ambos grupos a mediano plazo. Cabe consignar que ambos grupos eran de distinto tipo de ejercicio. En cuanto a la comparaci&oacute;n de un programa de tratamiento con ejercicio terap&eacute;utico versus una intervenci&oacute;n sola, dentro de los 17 art&iacute;culos seleccionados. Los estudios de Ho (12), Pienimaki (7), Bisset (23), Soderberg (20), Smidt (46), Struijs (19), Verhaar (41), Bhardwaj (34) establecen diferencias significativas a favor del programa de tratamiento con ejercicio versus una intervenci&oacute;n sola en las medidas de resultados de dolor a corto plazo. Cabe consignar que en el estudio Bisset (23), existe una disminuci&oacute;n del dolor a corto plazo atribuida a la inyecci&oacute;n de corticoides, pero a largo plazo era favorable al grupo de ejercicios. En cuanto al grip libre de dolor existen diferencias significativas a corto plazo (7,12,19,23,41,46). En los estudios de Struijs (39), Croisier (76) establecen diferencias significativas a favor de la intervenciones que no pose&iacute;an ejercicios (39) y a la intervenci&oacute;n sola (76), en las medidas de resultados de dolor existe una disminuci&oacute;n a corto plazo. En cuanto al grip libre de dolor existe diferencias significativas a corto plazo (39).Los estudios restantes de la comparaci&oacute;n no establecen diferencias significativas en las medidas de resultados establecidas. En cuanto a la comparaci    &oacute;n de ejercicio terap&eacute;utico dentro de un programa de tratamiento versus otro programa de tratamiento, dentro de los 15 art&iacute;culos seleccionados, los estudios de Chung (22), Park (84), Viswas (11), establecieron diferencias significativas a favor del programa de tratamiento con ejercicio terap&eacute;utico en comparaci&oacute;n con otro programa de tratamiento en las medidas de resultados de dolor a corto plazo. Los estudios de Stergioulas (13), Kochar (28), Radpasand (16), Stasinopoulos (86) establecieron diferencias significativas a favor del programa de tratamiento con ejercicio terap&eacute;utico en comparaci&oacute;n con otro programa de tratamiento en las medidas de resultados de dolor a mediano plazo. Los estudios de Hern&aacute;ndez (21), Nilsson (25), Park (84), Radpasand (16), Stasinopoulos (86), Tyler (85), Viswas (11), Stasinopoulos (86) establecieron diferencias significativas a favor del programa de tratamiento con ejercicio terap&eacute;utico en comparaci&oacute;n con otro programa de tratamiento en las medidas de resultados de la funcionalidad a corto, mediano y largo plazo. En cuanto al grip libre de dolor, los estudios de Stasinopoulos (86), Stergioulas (13), Tyler (85), Viswas (11) establecieron diferencias significativas a favor del programa de tratamiento con ejercicio terap&eacute;utico en comparaci&oacute;n con otro programa de tratamiento en la medida de resultado grip libre de dolor a corto y mediano plazo.</font></p>     <p><font face="Verdana" size="2">Cabe se&ntilde;alar que las m&uacute;ltiples diferencias en los tipos y dosis de ejercicios deben ser consideraciones a evaluar cuando se interpreten los resultados de los diferentes estudios y se intenten establecer conclusiones acerca de la efectividad cl&iacute;nica de las intervenciones terap&eacute;uticas estudiadas.</font></p>     ]]></body>
<body><![CDATA[<p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Conclusión</b></font></p>     <p><font face="Verdana" size="2">A pesar de la aparente homogeneidad cl&iacute;nica que presentaban algunos de los art&iacute;culos seleccionados por nuestra RS, era sumamente variable la dosis y el tipo de ejercicio, lo cual hace dif&iacute;cil realizar las comparaciones propuestas anteriormente, estas consignaciones no permitieron el agrupamiento estad&iacute;stico de los datos en base a un estimador puntual, a&uacute;n as&iacute; igual se consider&oacute; pertinente realizar un agrupamiento cualitativo o descriptivo de los resultados en las intervenciones cl&iacute;nicamente similares. Para la agrupaci&oacute;n de ejercicio s&oacute;lo comparado con una intervenci&oacute;n sola existe moderada evidencia que el ejercicio terap&eacute;utico disminuye el dolor, mejora la funcionalidad y aumenta la fuerza libre de dolor en corto, mediano y largo plazo. Para la agrupaci&oacute;n de ejercicio dentro de un programa de tratamiento comparado con una intervenci&oacute;n sola existe s&oacute;lida evidencia que el ejercicio terap&eacute;utico disminuye el dolor, y aumenta la fuerza libre de dolor a corto plazo. Para la agrupaci&oacute;n de ejercicio terap&eacute;utico dentro de un programa de tratamiento comparado con otro programa de tratamiento (Laser, Tens, US) existe s&oacute;lida evidencia que el ejercicio terap&eacute;utico disminuye el dolor, aumenta la fuerza y mejora la funcionalidad a corto, mediano y largo plazo. Cabe consignar que las conclusiones de nuestro estudio se realizaron sobre la base de los art&iacute;culos localizados por nuestra estrategia de b&uacute;squeda y seleccionados por nuestros crite-rios de elegibilidad; a&uacute;n as&iacute;, siempre se debe tomar en cuenta la proba-bilidad de que haya estudios que por problemas de indexaci&oacute;n o l&iacute;mites de b&uacute;squeda hayan quedado fuera de esta revisi&oacute;n.
