<body><![CDATA[<p>&nbsp;</p>     <p><font face="Verdana" size="2">En los &uacute;ltimos a&ntilde;os ha habido un gran desarrollo de las t&eacute;cnicas intervencionistas en el tratamiento del dolor. &Eacute;stas han posibilitado la disminuci&oacute;n de las dosis de f&aacute;rmacos, menos efectos secundarios de los mismos y mayor capacidad de resoluci&oacute;n desde nuestras unidades. Uno de estos procedimientos, y que se ha mostrado m&aacute;s &uacute;til, es la radiofrecuencia en sus distintas modalidades, y est&aacute; siendo usada en multitud de patolog&iacute;as.</font></p>     <p><font face="Verdana" size="2">La radiofrecuencia convencional o t&eacute;rmica (RFC) fue usada para tratar el dolor por primera vez por Shealy en 1975, y consiste en la aplicaci&oacute;n de una onda de alta frecuencia (500 KHz) que logra un aumento de la temperatura local por vibraci&oacute;n de las part&iacute;culas circundantes a la punta activa, alcanzando as&iacute; temperaturas del rango 80-90<sup>o</sup>C. El modo de radiofrecuencia pulsada (RFP) se us&oacute; por primera vez por Sluijter en 1998 (1), y consiste en la aplicaci&oacute;n de dicha onda de radiofrecuencia en forma de pulsos, es decir, se intercalan periodos de tratamiento (periodo activo de 20 msg) con periodos silentes (480 msg) que permiten as&iacute; la disipaci&oacute;n del calor. De este modo, la RFP no produce lesi&oacute;n t&eacute;rmica destructiva (no supera los 42<sup>o</sup>C aprox.). As&iacute;, desde los primeros estudios (2) se se&ntilde;alaba que lo que originar&iacute;a el efecto analg&eacute;sico en la RFP ser&iacute;a el campo el&eacute;ctrico emitido en la punta de la aguja y que cumplir&iacute;a una funci&oacute;n neuromoduladora en la transmisi&oacute;n del est&iacute;mulo doloroso, catalogando a esta t&eacute;cnica como "non destructive". Todo esto ha facilitado que el uso de la RFP en nervios perif&eacute;ricos y ganglio dorsal de ra&iacute;ces raqu&iacute;deas se haya extendido con gran facilidad y se haya introducido en los protocolos de actuaci&oacute;n de nuestra especialidad (3-5).</font></p>     <p><font face="Verdana" size="2">Pero el mecanismo de acci&oacute;n de la RFP ha sido muy discutido durante muchos a&ntilde;os y ha dado lugar a multitud de estudios que han intentado aclarar este efecto analg&eacute;sico. Estudios en animales (6) han demostrado que la RFP ocasiona modificaciones a nivel microcelular en el ax&oacute;n, evidenci&aacute;ndose en microscop&iacute;a electr&oacute;nica distintos grados de da&ntilde;o o disrupci&oacute;n en membrana celular, mitocondria, microt&uacute;bulos y microfilamentos. Incluso se apunta hacia la posibilidad de que la RFP tenga un efecto selectivo sobre las fibras de menor calibre (fibras C o A-delta) y no sobre las m&aacute;s grandes (A-beta), justificando as&iacute; su efecto analg&eacute;sico sin da&ntilde;ar la funci&oacute;n motora de los nervios (6,7).</font></p>     <p><font face="Verdana" size="2">Un estudio se&ntilde;al&oacute; que la RFP del ganglio dorsal de la ra&iacute;z produc&iacute;a un aumento de la expresi&oacute;n de c-fos en el asta dorsal de la m&eacute;dula (incluso de forma contralateral) (8). Otros autores han indicado tambi&eacute;n el posible efecto de la RFP sobre la modulaci&oacute;n de la inflamaci&oacute;n a trav&eacute;s de la atenuaci&oacute;n de la expresi&oacute;n de citokinas y otros mediadores (9,10), as&iacute; como la posible acci&oacute;n sobre las v&iacute;as inhibitorias descendentes noradren&eacute;rgicas y serotonin&eacute;rgicas (11).</font></p>     <p><font face="Verdana" size="2">Incluso en los &uacute;ltimos a&ntilde;os, tambi&eacute;n se apunta hacia la posibilidad de que realmente la RFP no pueda ser catalogada como "non-thermal" o "non-destructive". De este modo, los picos de temperatura ("spikes") no detectados por la c&aacute;nula, que se han observado de forma te&oacute;rica con el uso de la RFP, y que rondan temperaturas de 50-60<sup>o</sup>C, podr&iacute;an ser las responsables de estas modificaciones ultraestructurales debido a mecanismos de microablaci&oacute;n (2,12,13).</font></p>     <p><font face="Verdana" size="2">As&iacute; pues, aunque el mecanismo de acci&oacute;n analg&eacute;sico de la RFC est&aacute; bastante claro, ya que se trata de una lesi&oacute;n puramente t&eacute;rmica que busca la destrucci&oacute;n de la v&iacute;a nociceptiva, no ocurre lo mismo con el mecanismo de la RFP que, a&uacute;n hoy en d&iacute;a, sigue estando en continuo debate (14,15). Por otra parte, tambi&eacute;n debemos evaluar los resultados de la radiofrecuencia sobre nuestra pr&aacute;ctica cl&iacute;nica habitual. En un estudio prospectivo en el que se analizaba el uso de la RFC de ramo medial y de la RFP de ganglio dorsal para tratar a los pacientes que estaban en lista de espera para cirug&iacute;a de columna por distintas patolog&iacute;as discales y/o facetarias, se observ&oacute; una mejor&iacute;a significativa de la intensidad del dolor, funcionalidad y calidad de vida hasta un a&ntilde;o posterior al tratamiento, logrando que el 76 % de los pacientes tratados con radiofrecuencia rechazaran la cirug&iacute;a al a&ntilde;o (16).</font></p>     <p><font face="Verdana" size="2">Sin embargo, una revisi&oacute;n de la Cochrane del pasado a&ntilde;o respecto al tratamiento con radiofrecuencia en el dolor lumbar cr&oacute;nico se&ntilde;ala la falta de evidencia de la mayor&iacute;a de nuestros tratamientos (17). Indica que apenas existen unos pocos estudios bien dise&ntilde;ados que, no obstante, usan una muestra peque&ntilde;a y presentan resultados contradictorios. Tekin y cols. (18) compararon la RFC, RFP y placebo mostrando resultados a largo plazo a favor de la RFC, y Kroll y cols. (19) tambi&eacute;n compararon ambos modos de radiofrecuencia y, no obstante, concluyeron que la RFC logr&oacute; mejor resultado a corto plazo pero que no hab&iacute;a diferencias significativas a largo plazo. M&aacute;s recientemente, un estudio prospectivo no comparado con 300 pacientes con dolor facetario s&iacute; demostr&oacute; eficacia con la RFP a los 6 meses (20).</font></p>     <p><font face="Verdana" size="2">Ante este panorama, el estudio presentado por Nebreda y cols. en el n&uacute;mero actual de nuestra revista intenta aportar nuevos datos en un tema en el que los resultados a&uacute;n no est&aacute;n claros. La muestra es peque&ntilde;a, aunque similar a la de anteriores estudios, y los resultados muestran una clara diferencia a favor de la RFC en el tratamiento del s&iacute;ndrome facetario. Por otro lado, no reporta ning&uacute;n caso de complicaci&oacute;n, coincidiendo en ello con lo presentado por otros investigadores (21).</font></p>     <p><font face="Verdana" size="2">En otras patolog&iacute;as, como la neuralgia del trig&eacute;mino, las conclusiones son m&aacute;s evidentes, pues la RFP se ha mostrado claramente ineficaz comparada con la RFC (6), mientras que en el tratamiento del hombro doloroso, la RFP del nervio supraescapular s&iacute; se ha mostrado eficaz (22,23).</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">Esto nos hace plantearnos que quiz&aacute;s en aquellos "target" en los que buscamos la denervaci&oacute;n completa es m&aacute;s eficaz la RFC y, sin embargo, en aquellos dolores neurop&aacute;ticos en los que no nos podemos permitir la destrucci&oacute;n de dicho nervio por las potenciales complicaciones, la RFP puede resultar una opci&oacute;n segura y eficaz. ¿Esto se debe a que la RFP da lugar a una lesi&oacute;n tambi&eacute;n t&eacute;rmica pero m&aacute;s peque&ntilde;a que en la RFC? A&uacute;n no lo sabemos.</font></p>     <p><font face="Verdana" size="2">Por todo lo se&ntilde;alado anteriormente, a&uacute;n hoy nos seguimos haciendo preguntas respecto a la radiofrecuencia pulsada a las que no hemos sido capaces de dar respuesta a&uacute;n:</font></p>     <p><font face="Verdana" size="2">- ¿Cu&aacute;l debe ser la colocaci&oacute;n &oacute;ptima de la aguja respecto al "target"?</font></p>     <p><font face="Verdana" size="2">- ¿La funci&oacute;n neuromoduladora de la RFP es producida por el campo el&eacute;ctrico o se debe a peque&ntilde;as lesiones microablativas producidas por los "spikes" de temperatura originados en la punta de la aguja?</font></p>     <p><font face="Verdana" size="2">- ¿Cu&aacute;l es el tiempo &oacute;ptimo de tratamiento?</font></p>     <p><font face="Verdana" size="2">- ¿Qu&eacute; par&aacute;metros el&eacute;ctricos ser&iacute;an los id&oacute;neos (voltaje, frecuencia, duraci&oacute;n del pulso, etc.)?</font></p>     <p><font face="Verdana" size="2">- ¿Qu&eacute; implicaci&oacute;n tiene la plasticidad central en estos tratamientos?</font></p>     <p><font face="Verdana" size="2">- ¿Repetidos procedimientos de RFP van a lograr un efecto analg&eacute;sico aditivo? (24).</font></p>     <p><font face="Verdana" size="2">As&iacute; pues, creemos que este tipo de estudios prospectivos comparados, como el publicado por Nebreda y cols., debe ser la l&iacute;nea que debemos seguir para dotar de evidencia a nuestra pr&aacute;ctica cl&iacute;nica y emprender un ciclo de mejora continua en nuestros tratamientos con radiofrecuencia, intentando as&iacute; dar respuesta a las preguntas que, todav&iacute;a hoy, nos seguimos haciendo.</font></p>     <p>&nbsp;</p> <hr align="left" width="30%" size="1"> <hr align="left" width="30%" size="1">     ]]></body>
