Estudios recientes han propuesto varias hipótesis biológicas entre las cuales, la inflamación, la disfunción mitocondrial y el estrés oxidativo han mostrado una gran implicación y plausibilidad biológica. Se postula que la disfunción mitocondrial origina estrés oxidativo, con elevación de radicales libres de oxígeno (ROS) y nitrógeno (NOS), la cual produce inflamación sistémica, que activaría los astrocitos y la microglía a nivel del sistema nervioso, lo que condicionaría hipersensibilidad y consecuentemente sensibilización periférica y central. El ozono es una terapia con propiedades multidiana que ha demostrado ser capaz de disminuir el estrés oxidativo y la inflamación y mejorar la respiración mitocondrial y la producción de ATP, por lo que postulamos su utilidad en pacientes con FM. Presentamos el caso clínico de una mujer de 44 años tratada con 8 sesiones de ozono rectal que mejoró el dolor y normalizó la habituación, medido por la respuesta simpática cutánea (RSC), prueba que postulamos como un biomarcador neurofisiológico útil tanto para diagnóstico como para pronóstico en pacientes con sensibilización central (SC).]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[ABSTRACT Fibromyalgia (FM) is a widespread chronic pain syndrome with a high worldwide prevalence that represents a true clinical challenge due to the variety of symptoms and its unknown etiology. Recent studies have proposed several biological hypotheses, among which inflammation, mitochondrial dysfunction and oxidative stress have shown great implication and biological plausibility. It is postulated that mitochondrial dysfunction causes oxidative stress, with an increase in free radicals of oxygen (ROS) and nitrogen (NOS), which produces systemic inflammation, activating astrocytes and microglia at the level of the nervous system; this cell activation would condition hypersensitivity and consequently peripheral and central sensitization. Ozone is a therapy with multitarget properties that has been shown to be capable of reducing oxidative stress and inflammation, improving mitochondrial respiration and ATP production, that is the reason why we postulate its usefulness in patients with FM. We present the clinical case of a 44-year-old woman treated with 8 sessions of rectal ozone who improved pain and normalized habituation, measured by the sympathetic skin response (SSR), a test that we postulate as a useful neurophysiological biomarker for both diagnosis and prognosis in patients with Central Sensitization (CS).]]></p></abstract>
