There is controversy about when is the right time to start, what kind of food is the best and what is the way to introduce these foods to infants to improve eating behaviour. There are not specific guidelines for risk populations such as infants at high-risk of allergy or premature. The baby-led weaning has been proposed as a relaxing way to incorporate the infant to family feeding patterns. Current evidence supports the answer to all these questions lies in the degree of maturity and specific needs of each infant, however, the controversy is served.]]></p></abstract>

<body><![CDATA[<p><font face="Verdana" size="2">La diversificaci&oacute;n alimentaria es la introducci&oacute;n de alimentos distintos de la leche a partir del segundo semestre. M&aacute;s all&aacute; de las necesidades nutricionales propias de este periodo, los nuevos alimentos suponen un medio de incorporaci&oacute;n del lactante al contexto sociofamiliar y una ventana de oportunidad para la prevenci&oacute;n de enfermedades cr&oacute;nicas.</font></p>     <p><font face="Verdana" size="2">El objetivo fundamental es que este periodo sea f&aacute;cil, acompasado y saludable para evitar aversiones y preferencias alimentarias, prevenir la obesidad y disminuir la incidencia de alergias e intolerancias alimentarias.</font></p>     <p><font face="Verdana" size="2">Pese a la existencia de varias gu&iacute;as cl&iacute;nicas y consensos de expertos<sup>1</sup>, existen controversias acerca del cu&aacute;ndo, el c&oacute;mo, el orden de introducci&oacute;n de los alimentos distintos de la leche y sobre si influye la dieta de la madre durante el embarazo y la lactancia en su buena o mala evoluci&oacute;n.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Una oportunidad: la dieta de la gestante y de la madre que da de lactar</b></font></p>     <p><font face="Verdana" size="2">La diversificaci&oacute;n alimentaria forma parte del circulo intergeneracional de la alimentaci&oacute;n saludable y la prevenci&oacute;n de la enfermedad: los lactantes adecuadamente diversificados tienen unos h&aacute;bitos nutricionales m&aacute;s saludables en su infancia, y por extensi&oacute;n en la edad adulta, lo cual impacta en la aparici&oacute;n de enfermedades cr&oacute;nicas no transmisibles en las distintas generaciones<sup>2</sup>. Adem&aacute;s, una alimentaci&oacute;n saludable y variada durante el embarazo y la lactancia conlleva una mejor aceptaci&oacute;n de los alimentos distintos de la leche materna por exposici&oacute;n precoz a sabores y olores durante el periodo fetal<sup>3,4</sup>. A igualdad de elementos (sabor, contenido energ&eacute;tico, olor...), el lactante preferir&aacute; aquellos que le resulten m&aacute;s familiares o el sabor dulce de manera innata<sup>3,5</sup>.</font></p>     <p><font face="Verdana" size="2">La lactancia materna es hoy en d&iacute;a la mejor opci&oacute;n de alimentaci&oacute;n para el primer semestre por sus beneficios en la salud a corto y largo plazo<sup>6,7</sup>. Durante la misma no existen evidencia para recomendar que las mujeres deban modificar su dieta o tomar alg&uacute;n suplemento espec&iacute;fico durante el embarazo y la lactancia para prevenir el desarrollo de alergias alimentarias y atopia<sup>8</sup>. El modelo de dieta mediterr&aacute;nea, rico en frutas, verduras, pescado y fuentes de vitamina D, parece ser un modelo saludable a seguir<sup>9</sup>. Por consenso, se recomienda que la embarazada consuma un m&iacute;nimo de 200-300 mg de &aacute;cido docosahexaenoico (DHA) al d&iacute;a, procedente de fuentes como el pescado, los frutos secos y otros, por sus implicaciones en el crecimiento y desarrollo cerebral y visual<sup>10</sup>. Se recomienda que las mujeres embarazadas o que puedan estarlo, durante el periodo de lactancia y los ni&ntilde;os entre 12 y 30 meses consuman una amplia variedad de pescado, evitando las especies m&aacute;s contaminadas en metilmercurio, en especial el pez espada, el tibur&oacute;n, el at&uacute;n rojo y el lucio<sup>11</sup>. La Academy of Nutrition and Dietetics ha emitido recientemente un documento de posici&oacute;n sobre la nutrici&oacute;n y estilo de vida durante el embarazo<sup>12</sup>.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>El Cuándo</b></font></p>     <p><font face="Verdana" size="2">Las recomendaciones de cu&aacute;ndo introducir los alimentos han variado en relaci&oacute;n al aumento de incidencia de la enfermedad cel&iacute;aca, la aparici&oacute;n de alergias alimentarias y el incremento de la obesidad infantil<sup>13</sup>. En base a esto, la European Society for Paediatric Gastroenterology Hepatology and Nutrition (ESPGHAN) recomend&oacute; en 2008 no iniciar la introducci&oacute;n de alimentos antes de la 17.<sup>a</sup> ni despu&eacute;s de la 26.<sup>a</sup> semana de vida<sup>1</sup>, bas&aacute;ndose en el efecto que podr&iacute;a tener sobre el desarrollo de alergias alimentarias o la incidencia de la enfermedad cel&iacute;aca<sup>14</sup>. Sin embargo, una revisi&oacute;n Cochrane publicada en 2014 no encuentra beneficios ni desventajas en la introducci&oacute;n de alimentos distintos de la leche materna en este periodo, aunque tampoco encuentra inconveniente en mantener la lactancia materna exclusiva al menos los seis primeros meses, de acuerdo con las recomendaciones de la Organizaci&oacute;n Mundial de la Salud (OMS) para los lactantes sanos. Queda por esclarecer las consecuencias de introducir entre los cuatro y los seis meses de vida los alimentos complementarios en este grupo de pacientes<sup>15</sup>. Existe evidencia que asocia la introducci&oacute;n antes de los cuatro meses con mayor riesgo de obesidad infantil<sup>16</sup>.</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">M&aacute;s all&aacute; del incremento de las necesidades nutricionales, los signos que indican que el lactante puede empezar a recibir alimentos s&oacute;lidos son el tono axial y el sost&eacute;n cef&aacute;lico, la coordinaci&oacute;n motora entre los ojos, las manos y la boca y la desaparici&oacute;n del reflejo de extrusi&oacute;n, que permite introducir alimentos s&oacute;lidos en la boca<sup>14</sup>. La introducci&oacute;n del lactante en las rutinas de alimentaci&oacute;n de la familia tambi&eacute;n influye sobre cu&aacute;ndo iniciar la diversificaci&oacute;n. Es importante evitar transmitir h&aacute;bitos poco saludables durante este periodo<sup>17-19</sup>.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>El Orden de Introducción</b></font></p>     <p><font face="Verdana" size="2">Distintos comit&eacute;s de nutrici&oacute;n han emitido recomendaciones (calendarios) de introducci&oacute;n de alimentos, con mayor o menor coincidencia. El aumento de requerimientos espec&iacute;ficos (hierro, prote&iacute;nas, etc.), la maduraci&oacute;n intestinal, el desarrollo neurol&oacute;gico e inmunol&oacute;gico del lactante guiar&aacute;n el inicio individualizado durante este periodo en lo que se refiere al momento de inicio y el alimento de elecci&oacute;n. La European Food Safety Authority (EFSA) revis&oacute; en 2013 los requerimientos de macro y micronutrientes de los menores de tres a&ntilde;os<sup>20</sup>, incluyendo recomendaciones sobre el perfil de &aacute;cidos grasos de la dieta, indispensables para el desarrollo cerebral y visual. El estudio ALSALMA ha evidenciado desequilibrios en la ingesta global actual de los ni&ntilde;os menores de tres a&ntilde;os, ya presentes en el segundo semestre de la vida<sup>21</sup> asociadas al desarrollo precoz de obesidad<sup>22</sup>.</font></p>     <p><font face="Verdana" size="2">No existe evidencia en la actualidad que justifique el retraso en la introducci&oacute;n de alimentos potencialmente al&eacute;rgicos durante la diversificaci&oacute;n alimentaria en ni&ntilde;os de riesgo con antecedentes de asma, atopia, alergia alimentaria, aumento inexplicado de IgE en sangre o con familiares de primer grado con alg&uacute;n diagn&oacute;stico similar<sup>8</sup>.</font></p>     <p><font face="Verdana" size="2">La introducci&oacute;n del gluten debe ser gradual y evitar introducir en altas cantidades de forma muy precoz (antes de los cuatro meses) o tard&iacute;a (m&aacute;s de siete meses) para prevenir la aparici&oacute;n de enfermedad cel&iacute;aca, sin que haya, hasta el momento actual, bases para hacer una recomendaci&oacute;n m&aacute;s concreta. En grupos de riesgo la recomendaci&oacute;n debe ser similar, independientemente del riesgo individual y preferentemente en consonancia con la leche materna<sup>23</sup>.</font></p>     <p><font face="Verdana" size="2">Se concluye que no existe un orden &uacute;nico y la elecci&oacute;n es individualizada seg&uacute;n la ingesta global del lactante, la consonancia o no con la lactancia materna, los requerimientos espec&iacute;ficos y enfermedades asociadas. La introducci&oacute;n gradual, por grupos de alimentos, ayudar&aacute; a detectar las reacciones adversas<sup>24</sup>.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>El Cómo</b></font></p>     <p><font face="Verdana" size="2">Existen muchos factores que pueden condicionar la aparici&oacute;n de un trastorno del comportamiento alimentario en la primera infancia, lo que incluye factores gen&eacute;ticos, trastornos digestivos, caracter&iacute;sticas espec&iacute;ficas del cuidador, enfermedades intercurrentes y retraso o errores en la introducci&oacute;n de la alimentaci&oacute;n s&oacute;lida, entre otros<sup>18</sup>.</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">La preferencia innata por el sabor dulce, su efecto analg&eacute;sico y su uso como premio pueden ser la base de alimentaciones selectivas y desequilibradas<sup>3,5</sup>.</font></p>     <p><font face="Verdana" size="2">Es posible que se establezcan circuitos cerebrales de memoria ante la repetici&oacute;n de est&iacute;mulos no placenteros, alteraciones sensoriales (olor, gusto...) en relaci&oacute;n con la introducci&oacute;n de nuevos alimentos que perpet&uacute;en trastornos precoces del comportamiento alimentario y sean la base de la neofobia<sup>25</sup>.</font></p>     <p><font face="Verdana" size="2">La existencia de un periodo cr&iacute;tico de introducci&oacute;n de alimentos y de variaci&oacute;n de texturas apoya la hip&oacute;tesis de que el lactante acepta mejor el cambio a distintas texturas entre los 6-8 meses, y fuera de este periodo la aceptaci&oacute;n ser&iacute;a m&aacute;s tard&iacute;a y complicada; sin embargo, esta teor&iacute;a no ha llegado a confirmarse<sup>4</sup>. El <i>baby-led weaning</i> es un estilo de diversificaci&oacute;n guiado por el lactante cuyo objetivo es hacer del proceso algo placentero e incorporar precozmente al lactante a la din&aacute;mica familiar de comidas. Las madres que optan por este m&eacute;todo suelen ser poco controladoras y de car&aacute;cter relajado, puesto que en el caso contrario el proceso suele ser m&aacute;s complicado<sup>26, 27</sup>. Este estilo de alimentaci&oacute;n puede contribuir a un mejor desarrollo de la saciedad<sup>28</sup>, disminuir la aparici&oacute;n de obesidad y cambiar las preferencias de alimentaci&oacute;n hacia pautas m&aacute;s saludables<sup>29</sup>. Sin embargo, la evidencia procede de estudios observacionales y no existen estudios de mejor calidad metodol&oacute;gica que garanticen que el lactante a trav&eacute;s de este m&eacute;todo cubra todos sus requerimientos, sobre todo en caso de enfermedad intercurrente, ni su utilidad para la prevenci&oacute;n de alergias alimentarias. Su elecci&oacute;n atender&aacute; a las habilidades motoras del lactante y disposici&oacute;n de los padres, y debe ser monitorizada<sup>30</sup>. La propuesta a realizar podr&iacute;a ser una alimentaci&oacute;n m&aacute;s perceptiva que responda a las se&ntilde;ales de hambre y saciedad del ni&ntilde;o, a un ritmo adecuado, variada, comunicativa y sin distracciones y en un ambiente relajado que evite el establecimiento de aversiones.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Diversificación en el Prematuro</b></font></p>     <p><font face="Verdana" size="2">Existen gu&iacute;as que indican que el pret&eacute;rmino est&aacute; preparado para iniciar la diversificaci&oacute;n cuando pesa al menos 5 kg, ha perdido el reflejo de extrusi&oacute;n, acepta la cuchara y su dieta puede cubrir los requerimientos espec&iacute;ficos. Sin embargo, no todos los prematuros parten de una misma situaci&oacute;n cl&iacute;nica ni tienen el mismo grado de madurez (estimada por la edad gestacional corregida), por lo que es necesario el desarrollo de nuevas gu&iacute;as que valoren estos aspectos<sup>31</sup>. En general, la evidencia procede de estudios observacionales en los que se advierte una introducci&oacute;n precoz y desequilibrada de los alimentos distintos de la leche<sup>32</sup>. No hay bases para hacer recomendaciones para la introducci&oacute;n de alimentos potencialmente alerg&eacute;nicos o el gluten, por lo que las recomendaciones ser&aacute;n similares a las de los reci&eacute;n nacidos a t&eacute;rmino respetando la edad gestacional corregida, el grado de desarrollo neurol&oacute;gico y los requerimientos espec&iacute;ficos<sup>33</sup>.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Conflicto de Intereses</b></font></p>     <p><font face="Verdana" size="2">Los autores declaran no presentar conflictos de intereses en relaci&oacute;n con la preparaci&oacute;n y publicaci&oacute;n de este art&iacute;culo.</font></p>     <p>&nbsp;</p>     ]]></body>
