En la cefalea en racimos se ha incorporado la administración de nuevos fármacos por vía intranasal. En el dolor masticatorio la aplicación tópica de capsaicina y el uso de férulas orales en combinación con ejercicios de movilización mandibular, son los tratamientos más utilizados. En el dolor neurógeno se han desarrollando nuevos fármacos anticonvulsi-vos con menos efectos secundarios que la carbamacepina. Los últimos avances farmacológicos se dirigen hacia el uso de AINES y opioides de forma tópica y hacia la combinación de distintos analgésicos. Comentamos en este trabajo de revisión los últimos avances en el tratamiento del dolor orofacial.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[New treatments for orofacial pain have been developed in recent years. In the case of cluster headache, new drugs are now administered via the intranasal route, while in patients with chewing pain the topical application of capsaicin and the use of oral splints in combination with jaw movement exercises are the most widely used management approaches. In the case of neurogenic pain new anticonvulsivants have been introduced, with fewer side effects than carbamazepine. The latest pharmacological advances involve the use of nonsteroidal antiinflammatory drugs and opioids via the topical route, and the combination of different analgesics. The present review discusses the latest advances in the treatment of orofacial pain.]]></p></abstract>

Comentamos en este trabajo de revisi&oacute;n los &uacute;ltimos avances en el tratamiento del dolor orofacial.</P>      <P><b>Palabras clave</b>: <i> Dolor orofacial, dolor vascular, dolor masticatorio, dolor neur&oacute;geno.</i></P>      <P><b>INTRODUCCION</b></P>      <P>Revisamos los art&iacute;culos m&aacute;s importantes publicados sobre los avances en el tratamiento del dolor orofacial desde 1998 hasta el 2002; se ha realizado una b&uacute;squeda bibliogr&aacute;fica en Medline y en las revistas espa&ntilde;olas de Odontolog&iacute;a.</P>      <P>1.- DOLOR LOCAL OROFACIAL: MUCOSO, DENTAL Y GLANDULAR</P>      <P>En caso de infecci&oacute;n de los tejidos orales, los antibi&oacute;ticos mejoran el dolor al disminuir la inflamaci&oacute;n (1); sin embargo, en los casos de pulpitis irreversible, la penicilina no disminuye el dolor, siendo el tratamiento de elecci&oacute;n la endodoncia (2).</P>      <P>El dolor en el tratamiento endod&oacute;ncico se puede producir tras la instrumentaci&oacute;n de conductos, como resultado de una inflamaci&oacute;n aguda de los tejidos periodontales. El acet&oacute;nido de triamcinolona es un potente corticosteroide que puede ser utilizado para eliminar o reducir la inflamaci&oacute;n severa secundaria al tratamiento endod&oacute;ncico. El uso de un gel de antibi&oacute;tico-corticosteroide en el interior del canal radicular, disminuye el dolor en la primera hora en la mayor&iacute;a de los pacientes, sin producir efectos adversos ni recurrencias (3).</P>      ]]></body>
<body><![CDATA[<P>En el dolor orofacial por obstrucci&oacute;n glandular la radiolog&iacute;a (4), la endoscopia (5,6), y los ultrasonidos (7), permiten localizar la obstrucci&oacute;n y evaluar las alteraciones glandulares; la retirada del c&aacute;lculo o la dilataci&oacute;n de la estenosis liberar&aacute; la presi&oacute;n de los conductos y eliminar&aacute; el dolor (8).</P>      <P>2.- DOLOR VASCULAR</P>      <P>Pe&ntilde;arrocha y Bag&aacute;n (9) revisaron los criterios diagn&oacute;sticos, el diagn&oacute;stico diferencial y el tratamiento de las cefaleas trigeminales auton&oacute;micas. En la cefalea en racimos se usa el sumatriptan por v&iacute;a subcut&aacute;nea en la fase aguda (10-13); recientemente se ha incorporado la administraci&oacute;n del f&aacute;rmaco por v&iacute;a intranasal (14). En el tratamiento profil&aacute;ctico, se ha usado la capsaicina y por sus efectos secundarios, se emplea un derivado suyo, la civamida, administrada una vez al d&iacute;a durante 7 d&iacute;as en la coana nasal homolateral al dolor, produciendo una reducci&oacute;n significativa del n&uacute;mero de ataques (15). Asimismo, se han descrito en estos a&ntilde;os otros f&aacute;rmacos como tratamiento de fondo, como: el verapamilo, los  corticoesteroides, el &aacute;cido valproico, los inhibidores de la serotonina (11,12), el topiramato (16) y el naratript&aacute;n (17). </P>      <P>La indometacina es el tratamiento de elecci&oacute;n en la hemicr&aacute;nea parox&iacute;stica, produce remisi&oacute;n absoluta del dolor (10). En la hemicr&aacute;nea continua tambi&eacute;n se obtienen buenos resultados  con la indometacina (9,18). Pareja y cols. (18) en un seguimiento durante 3.8 a&ntilde;os de media en 26 pacientes  administraron indometacina.  Un 42% de ellos experiment&oacute; una disminuci&oacute;n del dolor de m&aacute;s del 60%. Se han usado tambi&eacute;n f&aacute;rmacos como los inhibidores de la ciclooxigenasa. Peres y cols. (19)  trataron con celecoxib y rofecoxib a 14 pacientes con hemicr&aacute;nea continua, 5 y 9 respectivamente, observ&aacute;ndose una remisi&oacute;n de los s&iacute;ntomas en 3 pacientes de cada grupo.</P>      <P>El SUNCT ( Cefaleas de corta duraci&oacute;n con inyecci&oacute;n conjuntival y lagrimeo), es resistente a la indometacina, el sumatriptan y la amitriptilina (20), pero se han descrito remisiones con carbamacepina y con corticoides. En este tipo de cefalea tambi&eacute;n se ha referido el uso de nuevos antiepil&eacute;pticos como la lamotrigina  (20), la gabapentina, as&iacute; como el topiramato (21). D&#180;Andrea y cols. (20) administraron a 5 pacientes diagnosticados de SUNCT, rebeldes al tratamiento con carbamacepina, lamotrigina a dosis iniciales de 25 mg/d&iacute;a, increment&aacute;ndose  progresivamente hasta un m&aacute;ximo de 125-200 mg d&iacute;a, el dolor remiti&oacute; en 3 de ellos y en 2 casi completamente. </P>      <P>3.- DOLOR MASTICATORIO</P>      <P>El tratamiento t&oacute;pico del dolor localizado en el &aacute;rea de la uni&oacute;n temporomandibular con capsaicina demuestra ser igual de efectiva al uso de un placebo, produci&eacute;ndose en ambos una reducci&oacute;n del dolor del 50% (tras un periodo de 4 semanas) (22). </P>      <P>La administraci&oacute;n oral de glucosamina y condroitin sulfato es un m&eacute;todo efectivo y seguro para reducir los s&iacute;ntomas de la osteoartritis de la ATM. Una dosis diaria de 1500 mg de hidroclorato de glucosamina combinada con 1200 mg de condroitin sulfato, durante 12 semanas, disminuye la laxitud de las uniones temporomandibulares, los ruidos articulares y el n&uacute;mero de analg&eacute;sicos necesarios en pacientes con dolor de la ATM (23).</P>      <P>El uso de f&eacute;rulas orales acr&iacute;licas tiene un efecto moderado sobre el dolor en pacientes con dolor miofascial (24). Una combinaci&oacute;n de ejercicios de movilizaci&oacute;n mandibular, distracci&oacute;n manual de la uni&oacute;n, movilizaci&oacute;n del complejo c&oacute;ndilo/disco, correcci&oacute;n de la postura del cuerpo y las t&eacute;cnicas de relajaci&oacute;n alivian de forma significativa el dolor. Esta combinaci&oacute;n parece ser efectiva para el tratamiento de los s&iacute;ntomas de la osteoartritis de la ATM (25). Esta disminuci&oacute;n del dolor de ATM y de la disfunci&oacute;n se consigue a las varias semanas de la realizaci&oacute;n de ejercicios neuromusculares controlados en casa (26).</P>      <P>4.- DOLOR NEUR&Oacute;GENO</P>      ]]></body>
<body><![CDATA[<P>-<i>Tratamiento m&eacute;dico</i></P>      <P>La carbamacepina se sigue empleando en el tratamiento de la neuralgia trigeminal (27-29). Seg&uacute;n Reisner y cols (29), en una revisi&oacute;n sobre el uso de los anticonvulsivos en el tratamiento del dolor orofacial, refiere la lamotrigina, la gabapentina, la oxcarbacepina, la tiagabina y el topiramato tan eficaces como la carbamacepina, pero con la ventaja de producir menos efectos secundarios (30-32). En caso de no responder a la carbamacepina, la fenitoina podr&iacute;a ser el f&aacute;rmaco de segunda elecci&oacute;n. Si la presencia de efectos secundarios limita el uso de los anticonvulsivos, se podr&iacute;a usar el baclofeno (relajante muscular agonista del &aacute;cido gamma aminobut&iacute;rico). </P>      <P>Delvaux y cols. (30) administraron lamotrigina a 25 pacientes diagnosticados de neuralgia trigeminal, dolor facial at&iacute;pico, migra&ntilde;a, cefalea en racimos y cefalea cr&oacute;nica tensional.  Las dosis administradas fueron progresivas desde  25 mg d&iacute;a hasta obtener  dosis efectivas (250 mg/d&iacute;a de media). Tras  un seguimiento de 18 meses, la lamotrigina es m&aacute;s efectiva en el tratamiento de la neuralgia trigeminal que en el resto de los dolores faciales.</P>      <P>Krimsky Fischoff y cols (33) administraron gabapentina en un paciente con esclerosis m&uacute;ltiple asociada a neuralgia trigeminal. Precis&oacute; una dosis de 2700 mg/d&iacute;a, tolerando bien la medicaci&oacute;n y refiriendo ausencia de dolor en los dos a&ntilde;os de seguimiento. Rowbotham M y cols. (34) administraron gabapentina a lo largo de 8 semanas dosis crecientes hasta 3600mg/d&iacute;a en 109 pacientes con neuralg&iacute;a post-herp&eacute;tica, el dolor se redujo en un 33% de los pacientes frente al 7.7% del grupo control. </P>      <P>En el tratamiento del dolor neur&oacute;geno trigeminal se han usado f&aacute;rmacos de aplicaci&oacute;n t&oacute;pica (35,36). Epstein y cols. (35) realizaron un estudio en 7 pacientes aplicando clonidina (agonista adren&eacute;rgico) en crema a una concentraci&oacute;n de 0.2 mg/g, 4 veces/d&iacute;a. El 57% de ellos experimentaron una mejor&iacute;a (reducci&oacute;n del dolor a m&aacute;s de la mitad). Seg&uacute;n Padilla y cols. (36) la capsaicina (estimula la liberaci&oacute;n de la sustancia P y otros p&eacute;ptidos neurotransmisores) es eficaz, aplicada t&oacute;picamente, a dosis de 0.025% y 0.075%.</P>      <P>-<i>Tratamiento quir&uacute;rgico y radioter&aacute;pico</i></P>      <P><i>Neurocirug&iacute;a</i></P>      <P>Pollock y cols (37), sometieron a 68 pacientes con neuralgia trig&eacute;minal rebeldes al tratamiento farmacol&oacute;gico, a radiaciones de 70 o 90 Gy mediante el sistema Model B Leksell gamma knife. El dolor desapareci&oacute;  en el 41% de los casos con dosis bajas y en el 61% con dosis altas; recomiendan aplicar dosis menores de 90 Gy  para evitar la aparici&oacute;n de efectos secundarios como disfunciones permanentes  del nervio trig&eacute;mino y disestesias. </P>      <P>En el estudio de Tyler Kabara (38) se realiz&oacute; la descompresi&oacute;n microvascular intracraneal en un total de 2003 pacientes con neuralg&iacute;a trigeminal t&iacute;pica y en 673 con at&iacute;pica; en el 80% de sus pacientes, se redujo completamente el dolor en el postoperatorio inmediato y en un 73% se mantuvo la mejor&iacute;a a  largo plazo (m&aacute;s de 5 a&ntilde;os).</P>      <P>5.- DOLOR &Oacute;SEO</P>      ]]></body>
<body><![CDATA[<P>Bouquot y McMahon (39) ampl&iacute;an el t&eacute;rmino de osteopat&iacute;a alveolar defini&eacute;ndolo como un edema de la m&eacute;dula &oacute;sea, producido por alteraciones vasculares que conllevan a una trombosis y microinfartos &oacute;seos. El tratamiento consiste en la resecci&oacute;n completa del tejido &oacute;seo correspondiente a la zona del dolor, identificado tras el bloqueo anest&eacute;sico (39-41). Una vez extra&iacute;do el fragmento &oacute;seo se curetea la m&eacute;dula. Tras realizar cultivo y antibiograma, aplican un antibi&oacute;tico en la zona, para aumentar la concentraci&oacute;n del f&aacute;rmaco en esa &aacute;rea y disminuir los efectos secundarios. Este procedimiento tambi&eacute;n se ha realizado con la ayuda de ox&iacute;geno hiperb&aacute;rico (41).</P>      <P>Bouquot y McMahon (39) consideran la osteopat&iacute;a alveolar cavitaria como cualquier otra enfermedad oral, y en un futuro, deberemos prestar atenci&oacute;n a los peligrosos potenciales efectos protromb&oacute;ticos en nuestros pacientes, aprender m&aacute;s sobre t&eacute;cnicas de imagen e interesarse por los efectos sist&eacute;micos y locales de las infecciones alveolares locales y los procesos necr&oacute;ticos. Hoy en d&iacute;a, todav&iacute;a no existen criterios uniformes de diagn&oacute;stico y tratamiento (40).</P>      <P>6.- DOLOR POSTQUIR&Uacute;RGICO</P>      <P>Una higiene oral pobre est&aacute; asociada a un incremento de dolor tras cirug&iacute;a de cordales inferiores, y como consecuencia un mayor consumo de analg&eacute;sicos. El enjuague preoperatorio con clorhexidina reduce la incidencia de alveolitis seca en las extracciones simples y en las cirug&iacute;as de terceros molares inferiores (42). </P>      <P>Los &uacute;ltimos estudios se dirigen hacia el uso de AINEs y opioides de forma t&oacute;pica, para evitar los efectos secundarios producidos por la administraci&oacute;n sist&eacute;mica, (43) y hacia la combinaci&oacute;n de analg&eacute;sicos, para disminuir sus dosis y reducir as&iacute; los efectos adversos (44).</P>      <P>-<i>Analg&eacute;sicos-antiinflamatorios no esteroideos</i></P>      <P>El paracetamol a dosis de 1gr como terapia &uacute;nica o en combinaci&oacute;n con 60 mg de code&iacute;na se muestra eficaz para el tratamiento del dolor tras cirug&iacute;a oral (44). La code&iacute;na a dosis mayores de 60 mg produce un aumento significativo de la incidencia de efectos adversos, sin mejorar el nivel de analgesia (45). Para dolores m&aacute;s intensos se recomienda la combinaci&oacute;n de 1 gr de paracetamol con 10 mg de oxicodona (44). El uso de AINEs y antiinflamatorios esteroideos (AIEs) en las fases preoperatoria e intraoperatoria, es beneficioso para la reducci&oacute;n del dolor e hinchaz&oacute;n postquir&uacute;rgico (46).</P>      <P>Se han identificado ciertas citocinas relacionadas con el trauma tisular, entre ellas la IL-1b, cuya reducci&oacute;n farmacol&oacute;gica por medio del ibuprofeno produce una reducci&oacute;n del dolor (46). Una dosis de 400 mg de este f&aacute;rmaco cada 8 horas produce una reducci&oacute;n del dolor postquir&uacute;rgico en el 65% de los pacientes que se someten a cirug&iacute;a oral dento-alveolar (47). Los AINEs requieren una dosis mayor para alcanzar el efecto antiinflamatorio que la necesaria para conseguir el efecto analg&eacute;sico. En el caso del ibuprofeno se necesitan de 2400 a 3200 mg de ibuprofeno por d&iacute;a para obtener el efecto antiinflamatorio; mientras que el efecto analg&eacute;sico se consigue a dosis de 200 a 600 mg 4 veces al d&iacute;a u 800 mg 3 veces al d&iacute;a. (44).</P>      <P>El ketorolaco unido a la code&iacute;na tambi&eacute;n produce un control adecuado del dolor tras la extracci&oacute;n quir&uacute;rgica de cordales mandibulares incluidos. Los mejores resultados se consiguen con una combinaci&oacute;n de 10 mg de ketorolaco y 15 mg de fosfato de code&iacute;na. As&iacute; se reduce la cantidad de code&iacute;na (y sus efectos adversos) necesaria para la reducci&oacute;n del dolor (48).</P>      <P>El ketorolaco y el tramadol producen una analgesia posquir&uacute;rgica de eficacia similar, s&oacute;lo existen diferencias significativas cuando se consideran los efectos secundarios (49). No existe diferencia entre la dosis de 10 y 20 mg de ketorolaco en el tratamiento del dolor posquir&uacute;rgico, siendo mayor su eficacia al ketoprofeno 50 mg (50).</P>      ]]></body>
<body><![CDATA[<P>Se han comercializado dos inhibidores espec&iacute;ficos de la COX-2, el celecoxib y el rofecoxib, cuya eficacia es similar a naproxeno, ibuprofeno, diclofenaco y &aacute;cido acetil salic&iacute;lico en el tratamiento del dolor dental. El rofecoxib produce una analgesia comparable a la producida por otros AINEs (como el ibuprofeno 400 mg) (51), pero tiene duraci&oacute;n mayor de 24 horas tras una dosis simple y no se asocia con efectos adversos gastrointestinales. El celecoxib tiene una duraci&oacute;n similar al rofecoxib, pero su efecto analg&eacute;sico es ligeramente menor (52). Ambos tienen una f&aacute;cil posolog&iacute;a, la dosis prescrita se administra 1 o 2 veces al d&iacute;a (51). Una dosis simple de 50 mg de rofecoxib proporciona una mayor analgesia, mayor rapidez en el establecimiento del efecto analg&eacute;sico, mayor efecto analg&eacute;sico y una mayor duraci&oacute;n que una dosis simple de 50 mg de diclofenaco en pacientes con dolor asociado a cirug&iacute;a de cordales mandibulares (53). Pero debido al desconocimiento de sus efectos a largo plazo se recomienda su uso restringido a personas con riesgo de desarrollar complicaciones gastrointestinales graves por AINEs y cuando se trate de dolores cr&oacute;nicos (54,55). Aunque el precio de rofecoxib es superior al de los AINEs, es menor al de la suma de estos &uacute;ltimos m&aacute;s un gastroprotector como omeprazol (54).</P>      <P>- <i> Otros f&aacute;rmacos</i></P>      <P>La administraci&oacute;n de un opioide o un anest&eacute;sico local previo a la intervenci&oacute;n quir&uacute;rgica da como resultado un retraso en el establecimiento del dolor. La eficacia del anest&eacute;sico local cuando se utiliza antes o despu&eacute;s del tratamiento quir&uacute;rgico es la misma, siempre y cuando se administre antes de que aparezca el dolor (56).</P>      <P>El fentanilo (opioide) inyectado en combinaci&oacute;n con anest&eacute;sicos, como la mepivacaina, aumenta y prolonga el efecto anest&eacute;sico en &aacute;reas con inflamaci&oacute;n. Esto conlleva una reducci&oacute;n de los requerimientos analg&eacute;sicos posquir&uacute;rgicos (57).</P>      <P>El tartrato de butorphanol intranasal (Stadol NS&reg;), combinaci&oacute;n sint&eacute;tica agonista-antagonista analg&eacute;sico-opi&oacute;ide, presenta una gran eficacia y seguridad en el control del dolor postquir&uacute;rgico tras la extracci&oacute;n de terceros molares. Adem&aacute;s posee una f&aacute;cil administraci&oacute;n, un establecimiento r&aacute;pido del efecto analg&eacute;sico y puede ser administrado cuando el paciente tiene dificultad para tragar. Este preparado produce una reducci&oacute;n del 50% del dolor al cabo de 15 minutos de la administraci&oacute;n de la primera dosis, y su efecto dura aproximadamente 4 horas. Ladov y cols. (58) sugieren el uso de Stadol NS&reg; intranasal como f&aacute;rmaco de primera elecci&oacute;n para el manejo del dolor tras la extracci&oacute;n de terceros molares y cirug&iacute;a oral en general. Desjardins y cols.
