DIAGNÓSTICO DIFERENCIAL DE LOS DESÓRDENES TEMPOROMANDIBULARES (DTM)
CONCEPTO
Los DTM comprenden las miopatías y artropatías del sistema de relación cráneo mandibular. Originan el mayor número de dolores orofaciales después del dolor dental y periodontal.
Las miopatías más frecuentes son el dolor miofascial (dolor somático profundo con puntos de gatillo y áreas de referencia),la mialgia local (sin áreas de referencia) y el mioespasmo. Las artropatías más frecuentes son la luxación discocondilar y su consecuencia la artrosis. Miopatías y artropatías pueden ir asociadas. Estas patologías son más frecuentes en la mujer (5/1-9/1) y en su patogénesis se encuentran implicados el terreno (genético o adquirido), la parafunción (estrés con disparo del sistema dopaminérgico)y tal vez alteraciones oclusales.
