La búsqueda bibliográfica fue realizada en bases de datos electrónicas abarcando el período comprendido entre el 1 de julio de 2006 y el 30 de abril de 2012. Del total de 1.696 estudios encontrados inicialmente, se incluyeron 28 artículos que describieron 23 programas de AF y orientación alimentaria destinados a niños y adolescentes con sobrepeso/obesidad. Diecisiete programas fueron dirigidos a niños y adolescentes y 6 fueron diseñados específicamente para adolescentes. En 10 de los estudios, la condición física (CF) fue adicionalmente valorada. Los resultados parecen señalar que la AF asociada con orientación nutricional produce efectos positivos en la reducción del peso e índice de masa corporal (IMC). Resaltamos la necesidad de diseñar programas específicos para adolescentes, así como valorar objetivamente la eficacia de dichos programas a largo plazo en poblaciones de niños y adolescentes con sobrepeso/ obesidad.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[The aim of this study was to realize one systemic review the effectiveness of intervention programs which were based on physical activity (PA) and dietary control of overweight and/or obese children and adolescents. The literature search was conducted in electronic databases covering the time period between July 1th, 2006 to April 30th, 2012. From 1,696 studies initially reviewed, 28 articles describing 23 programs were included. We have included studies with exercise programs and nutritional counseling for overweight and/or obese children and adolescents. Interventions which were excluded were those which involved the use of drugs, studies conducted in adolescents with hormonal disorders and eating disorders, book chapters, conference proceedings and dissertations. Seventeen programs were targeted towards children and adolescents and six were designed specifically for adolescents. In ten studies, the physical fitness (PF) was further assessed. Due the methodological heterogeneity found, these results are insufficient to determine the effectiveness of intervention programs regarding PA and diet. The results seem to indicate that AF associated with nutrition counseling has positive effects on reducing weight and body mass index (BMI). We emphasize the need to design specific programs for adolescents, as well as the as well assess the effectiveness of long term programs in populations of children and adolescents with overweight and obesity.]]></p></abstract>

<body><![CDATA[<p><font face="Verdana" size="2"><b>Introducci&oacute;n</b></font></p>     <p><font face="Verdana" size="2">La obesidad es una patolog&iacute;a caracterizada por el aumento de la masa grasa corporal<sup>1</sup>, como consecuencia del desequilibrio entre la ingesta de alimentos y el gasto energ&eacute;tico<sup>2</sup>, constituyendo en la actualidad un problema de salud p&uacute;blica de alcance mundial, con prevalencia creciente en la poblaci&oacute;n infantil y adolescente en las &uacute;ltimas d&eacute;cadas<sup>3-4</sup>. Las estimaciones indican que el exceso de grasa es el trastorno m&aacute;s frecuente en Europa entre los ni&ntilde;os de 7-11 a&ntilde;os (10 - 30 %) y adolescentes de 14 - 17 a&ntilde;os (8 - 25 %)<sup>1</sup>. As&iacute; mismo, hay evidencias cient&iacute;ficas del negativo impacto f&iacute;sico y psicosocial causado por la presencia de sobrepeso y/u obesidad durante la infancia<sup>5-6</sup>.</font></p>     <p><font face="Verdana" size="2">Por este motivo, en los &uacute;ltimos a&ntilde;os, los esfuerzos se centran en aumentar la pr&aacute;ctica de actividad f&iacute;sica (AF) por parte de dichos grupos poblacionales como estrategia para reducir el riesgo de desarrollar la enfermedad<sup>7</sup>. En el trabajo de revisi&oacute;n de van Sluijs et al<sup>8</sup>, analizaron la efectividad de intervenciones para la promoci&oacute;n de AF en ni&ntilde;os y adolescentes, destacando la falta de calidad en las conclusiones de los estudios en lo que se refiere a la efectividad. Mac Goven et al<sup>9</sup>, analizaron los efectos de intervenciones en ensayos cl&iacute;nicos aleatorios incluyendo en su tratamiento estrategias de afrontamiento en ni&ntilde;os y adolescentes con sobrepeso/obesidad, el uso de f&aacute;rmacos, la AF y/o dieta, resaltando la falta de evidencia en la eficacia de tales tratamientos a largo plazo.</font></p>     <p><font face="Verdana" size="2">Considerando estos hallazgos, el objetivo del presente art&iacute;culo ha sido realizar una revisi&oacute;n sistem&aacute;tica de la efectividad de los programas de intervenci&oacute;n que han utilizado la AF y la alimentaci&oacute;n como estrategias para el afrontamiento del sobrepeso/obesidad en ni&ntilde;os y adolescentes.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>M&eacute;todo</b></font></p>     <p><font face="Verdana" size="2">Para la revisi&oacute;n se utilizaron las siguientes bases de datos: SCOPUS, Medline plataforma Web of Knowledge, PsychInfo, CINAHL plataforma OVID and ISI Web of Knowledge. Las palabras clave utilizadas en la b&uacute;squeda fueron: "<i>intervention</i>", "<i>overweight</i>, "<i>obesity</i>", "<i>adolescents</i>", "<i>physical activity</i>", "<i>physical fitness</i>" as&iacute; como la combinaci&oacute;n de &eacute;stas.</font></p>     <p><font face="Verdana" size="2"><b>Criterios de inclusi&oacute;n y exclusi&oacute;n</b></font></p>     <p><font face="Verdana" size="2">Los criterios de inclusi&oacute;n fueron los siguientes: a) estudios entre el 1 de julio de 2006 y el 30 de abril de 2012, publicados en revistas cient&iacute;ficas de texto completo con acceso electr&oacute;nico; b) adolescentes (12 - 19 a&ntilde;os) que presenten percentiles de &iacute;ndice de masa corporal (IMC) con valores considerados par&aacute;metros de sobrepeso y obesidad, seg&uacute;n los rangos de referencias establecidos por la Organizaci&oacute;n Mundial de la Salud (OMS), y c) intervenciones que incluyan AF, alimentaci&oacute;n o la combinaci&oacute;n de ambos para el tratamiento del sobrepeso/obesidad, con una duraci&oacute;n m&iacute;nima de cuatro semanas.</font></p>     <p><font face="Verdana" size="2">Se excluyeron aquellas intervenciones que emplearon f&aacute;rmacos, las centradas en desarrollar programas en adolescentes con alteraciones hormonales y des&oacute;rdenes alimentarios, as&iacute; como cap&iacute;tulos de libros, res&uacute;menes de congresos y tesis doctorales.</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">Inicialmente, un revisor seleccion&oacute; las referencias de acuerdo al t&iacute;tulo y resumen. Las identificadas como relevantes y que tras la lectura del texto completo generaban duda en el proceso de selecci&oacute;n, fueron analizadas de forma independiente por dos revisores. Las diferencias de acuerdo entre los revisores fueron resueltas mediante discusi&oacute;n, siguiendo los criterios de inclusi&oacute;n predeterminados.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Resultados de la b&uacute;squeda</b></font></p>     <p><font face="Verdana" size="2">El procedimiento de b&uacute;squeda ha permitido identificar 1.696 art&iacute;culos potencialmente seleccionables (SCOPUS, N = 344; Medline plataforma Web of Knowledge, N = 33; PsychInfo N = 925; CINAHL plataforma OVID N = 182; ISI Web of Knowledge N = 212). En el an&aacute;lisis inicial 913 referencias fueron excluidas tras comprobar los t&iacute;tulos de los art&iacute;culos. Un total de 722 art&iacute;culos fueron excluidos despu&eacute;s de la lectura del resumen. Entre los 191 restantes, tras la lectura del texto completo, se seleccionaron 44 art&iacute;culos como relevantes. Tras un an&aacute;lisis en profundidad, 28 art&iacute;culos que describen 23 programas de intervenci&oacute;n se ajustaron a los criterios de inclusi&oacute;n establecidos para esta revisi&oacute;n. (<a href="#f1">fig. 1</a>).</font></p>     <p>&nbsp;</p>     <p align="center"><font face="Verdana" size="2"><a name="f1"><img src="/img/revistas/ramd/v7n1/revision2_f1.jpg"></a>    <br><b>Fig. 1 </b>Diagrama de la selecci&oacute;n de referencias para la revisi&oacute;n.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Caracter&iacute;sticas de los programas de intervenci&oacute;n</b></font></p>     <p><font face="Verdana" size="2">Los 23 programas de intervenci&oacute;n tienen las siguientes caracter&iacute;sticas generales: todos son programas de intervenci&oacute;n que combinan AF y dieta, en 10 de ellos, adem&aacute;s se eval&uacute;an los cambios en la condici&oacute;n f&iacute;sica (CF)<sup>10-19</sup>.Diecisiete intervenciones fueron dirigidas para ni&ntilde;os y adolescentes<sup>10-12,14,16-28</sup> y 6 fueron desarrollados tan solo en adolescentes<sup>13,15,29-32</sup> (<a target="_blank" href="/img/revistas/ramd/v7n1/revision2_t1.html">tabla 1</a>). Seis programas presentaron un periodo de seguimiento<sup>12,20,22,24,33-36</sup>.</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">La duraci&oacute;n de la pr&aacute;ctica de AF vari&oacute; entre 45 y 90 minutos por sesi&oacute;n, con frecuencia desde un d&iacute;a a la semana hasta la pr&aacute;ctica diaria, incluyendo los fines de semana y vacaciones, desarrolladas con distinto tipo de pr&aacute;ctica de AF. Variadas estrategias se llevaron a cabo en la educaci&oacute;n alimentaria, tales como orientaci&oacute;n alimentaria (dieta y nutrici&oacute;n) o preparaci&oacute;n de alimentos. En la <a target="_blank" href="/img/revistas/ramd/v7n1/revision2_t1.html">tabla 1</a> se describen las caracter&iacute;sticas de los programas.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Caracter&iacute;sticas de las evaluaciones</b></font></p>     <p><font face="Verdana" size="2"><b>Composici&oacute;n corporal</b></font></p>     <p><font face="Verdana" size="2">La composici&oacute;n corporal se valor&oacute; mediante diferentes t&eacute;cnicas y par&aacute;metros: IMC, z-score de IMC, por el m&eacute;todo de suavizado "<i>Latent Moderated Structural</i>" (LMS); percentil de IMC a trav&eacute;s de <i>software</i>; an&aacute;lisis de per&iacute;metro y circunferencias corporales entre otras medidas antropom&eacute;tricas; estimaci&oacute;n de masa grasa/masa libre de grasa por an&aacute;lisis de bioimpedancia el&eacute;ctrica y porcentaje graso mediante absorciometr&iacute;a dual de rayos X (DEXA).</font></p>     <p><font face="Verdana" size="2"><b>Actividad f&iacute;sica</b></font></p>     <p><font face="Verdana" size="2">Para la valoraci&oacute;n de los niveles de AF fueron aplicados: cuestionarios autoadministrados de h&aacute;bitos de AF, conductas sedentarias, cuestionarios de h&aacute;bitos de vida "<i>The Family Habit Inventory</i>" y evaluaci&oacute;n del comportamiento "<i>Child Behaviour</i> <i>Checklis</i>t" (CBCL). Se emplearon monitores de frecuencia cardiaca, aceler&oacute;metros, diarios de AF y pod&oacute;metros.</font></p>     <p><font face="Verdana" size="2"><b>Condici&oacute;n f&iacute;sica</b></font></p>     <p><font face="Verdana" size="2">Para la valoraci&oacute;n de la CF se utilizaron los siguientes tests: a) test en cicloerg&oacute;metro con an&aacute;lisis de gases; b) test en tapiz rodante con an&aacute;lisis de gases y determinaci&oacute;n del VO<sub>2m&aacute;x</sub>. con carga de trabajo incremental (velocidad e inclinaci&oacute;n) hasta el agotamiento; c) aplicaci&oacute;n del test de "<i>Course Navette</i>" con modificaciones; d) an&aacute;lisis de la eficiencia del sistema cardiovascular (VO<sub>2</sub>/FC); e) valoraci&oacute;n de la tasa metab&oacute;lica basal; f) prueba subm&aacute;xima de campo; g) bater&iacute;a de test EUROFIT; h) prueba de subir y bajar escal&oacute;n (3 minutos); i) test de habilidades motoras; j) test de Balke, y k) caminar durante 6 minutos.</font></p>     <p><font face="Verdana" size="2"><b>Alimentaci&oacute;n y dieta</b></font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">Para el registro del consumo alimentario y de h&aacute;bitos diet&eacute;ticos se utilizaron los siguientes cuestionarios: a) h&aacute;bitos alimentarios "<i>Anti Cancer Council of Victorian Food Frequency Questionnaire</i>" (ACCVFFQ); b) "<i>Willet Youth Food Questionary</i>" y el cuestionario PACE + evaluaci&oacute;n de grasa en la dieta (<i>patient-centred assessment and counseling for exercise plus nutrition</i>); c) "<i>Short Fat Questionnaire</i>", al que se a&ntilde;adieron dos preguntas para evaluar el consumo de refrescos; d) "<i>Eating Disorder Examination</i>" (EDE-Q) cuestionario para la evaluaci&oacute;n psicopatol&oacute;gica de des&oacute;rdenes alimentarios; e) entrevista diagn&oacute;stico "<i>Eating Disorders</i>" (IDed-IV); f) diario de consumo de alimentos; g) valoraci&oacute;n de la necesidad cal&oacute;rica; h) c&aacute;lculo del gasto energ&eacute;tico; i) calorimetr&iacute;a, y j) recordatorio diet&eacute;tico de 24 horas.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Resultados alcanzados en las intervenciones</b></font></p>     <p><font face="Verdana" size="2">En la <a target="_blank" href="/img/revistas/ramd/v7n1/revision2_t2.jpg">tabla 2</a> se muestra el resumen de los resultados descritos en las intervenciones correspondientes a la composici&oacute;n corporal, AF, alimentaci&oacute;n/dieta y CF.</font></p>     <p><font face="Verdana" size="2"><b>Composici&oacute;n corporal</b></font></p> </p>     <p><font face="Verdana" size="2">Doce estudios de ensayos cl&iacute;nicos aleatorios observaron modificaciones significativas en el grupo de intervenci&oacute;n respecto al grupo control. El estudio "Hipteens", consigui&oacute; reducir el peso corporal (P &lt; 0,05) y porcentaje graso (P &lt; 0,05)<sup>20</sup>. El estudio piloto realizado por Williams et al<sup>23</sup>, logr&oacute; disminuir el peso (P &lt; 0,01), IMC (P &lt; 0,01) y las medidas antropom&eacute;tricas (circunferencia de la cadera y cintura P &lt; 0,05, pliegues cut&aacute;neos subcapulares (P &lt; 0,05, y supra&iacute;lico P &lt; 0,001). El estudio desarrollado por Johnston et al<sup>25</sup>, observ&oacute; mejoras en z-score de IMC (P &lt; 0,001). El programa "<i>Student Bodies 2</i>" evidenci&oacute; una reducci&oacute;n del IMC mediante &iacute;ndice z-score (P &lt; 0,05)<sup>30</sup>. El estudio "<i>Student Bodies 2-BED</i>" obtuvo cambios en z-score del IMC (P &lt; 0,01) y en el IMC (P &lt; 0,01)<sup>31</sup>. El estudio "<i>Challenge</i>" logr&oacute; reducir el porcentaje graso (P &lt; 0,01) y de masa grasa (P &lt; 0,05) y el incremento de masa libre de grasa (P &lt; 0,05)<sup>28</sup>. El estudio "<i>Obeldicks</i>" observ&oacute; una disminuci&oacute;n significativa del IMC (P &lt; 0,001)<sup>21</sup>. El estudio "<i>Bright Bodies</i>" mostr&oacute; cambios significativos en el IMC (- 3,3 Kg/m<sup>2</sup>), peso corporal (- 7,4 Kg) y porcentaje graso (-6,0 %)<sup>24</sup>. El programa "<i>Choose Health</i>", obtuvo reducciones significativas en el IMC (P &lt; 0,01), peso total (P &lt; 0,01), grasa corporal (P &lt; 0,01) y grasa abdominal (P &lt; 0,01)<sup>29</sup>. El programa "<i>Mend Exercise Nutrition</i>" (MEND) registr&oacute; una reducci&oacute;n significativa de la circunferencia de la cintura y de la z-score IMC (P &lt; 0,001)<sup>16</sup>. Finalmente, el programa "<i>Robbi Club</i>" redujo la puntuaci&oacute;n del &iacute;ndice en z-score del IMC (P &lt; 0,05), siendo tales efectos beneficiosos en el IMC y en masa grasa<sup>27</sup>. El estudio "CEMHaVi" redujo el IMC (P &gt; 0,05) y veinte sujetos pasaron del estado de obesidad al de sobrepeso<sup>19</sup>; en los participantes del estudio piloto fue posible observar una mejora del IMC (P &lt; 0,001)<sup>36</sup>.</font></p>     <p><font face="Verdana" size="2">Doce estudios de ensayo cl&iacute;nico sin grupo control presentaron sus resultados comparando los datos obtenidos entre pretest y postest. El estudio "<i>The Kids N Fitness</i>" obtuvo p&eacute;rdida del peso (P &lt; 0,001), IMC (P &lt; 0,001) y mejora en el z-score del IMC (P &lt; 0.001), siendo estas mejoras m&aacute;s acusadas en ni&ntilde;os mayores de 12 a&ntilde;os<sup>22</sup>. El programa "<i>Kids Fit/Fit Families</i>" mejor&oacute; el IMC del 81 % de los participantes y el 74 % presentaron un descenso en los per&iacute;metros corporales (P &lt; 0,01)<sup>26</sup>. El estudio "<i>Teaching, Encouragement, Exercise, Nutrition, and Support</i>" (TEENS) obtuvo mejoras en el &iacute;ndice de masa magra (P &lt; 0,01), masa grasa (P &lt; 0.001) y en z-score del IMC (P &lt; 0,05)<sup>14</sup>. El estudio "<i>Loozit</i>" mostr&oacute; una reducci&oacute;n del per&iacute;metro de la cintura (P &lt; 0,001) en los participantes<sup>32</sup>. Doyle et al<sup>30</sup>, observaron reducciones del IMC (P &lt; 0,001) y un incremento del porcentaje de masa magra (P &lt; 0,05), aunque estos cambios no llegaron a ser significativos para el porcentaje de IMC en los preadolescentes. El estudio desarrollado por Bougle et al<sup>10</sup>, observ&oacute; p&eacute;rdida significativa de masa grasa (P &lt; 0,001) y aumento de masa libre de grasa (P &lt; 0,05) tras la intervenci&oacute;n. El estudio piloto realizado por Klijn el al<sup>11</sup>, mostr&oacute; mejoras significativas en todos los componentes antropom&eacute;tricos (P &lt; 0,001). El estudio de Kn&ouml;pfli et al<sup>13</sup>, mostr&oacute; resultados significativos en la p&eacute;rdida de peso (P &lt; 0,001), grasa corporal absoluta (P &lt; 0,001), masa libre de grasa (P &lt; 0,001) y porcentaje graso (P &lt; 0,001). El programa "<i>Mi Piace Piacermi</i>" obtuvo mejoras de la circunferencia de la cintura, y los sujetos (N = 20) que completaron todo el programa (5 a&ntilde;os) consiguieron una reducci&oacute;n significativa del IMC y del IMC estandarizado (IMC-SDC)<sup>12</sup>. "<i>The Oslo Adiposity Intervention Study</i>" redujo el z-score del IMC (P &lt; 0,001) y un estudio desarrollado por Sola et al, tambi&eacute;n observ&oacute; una reducci&oacute;n del IMC (P = 0,01)<sup>18</sup>.</font></p>     <p><font face="Verdana" size="2"><b>Actividad f&iacute;sica</b></font></p>     <p><font face="Verdana" size="2">Doce estudios describieron los resultados relacionados con la AF. Los programas se mostraron eficaces en el aumento de la pr&aacute;ctica de AF en tiempo libre<sup>14,22,28,30</sup> y los fines de semana<sup>10</sup>, mejora de los h&aacute;bitos saludables relacionados con la pr&aacute;ctica de AF<sup>30,35</sup>, reducci&oacute;n de las actividades sedentarias por parte de los ni&ntilde;os<sup>26</sup> y adolescentes<sup>16,35</sup> , aumento de la pr&aacute;ctica de AF moderada e intensa<sup>15</sup>, mejora de la pr&aacute;ctica de AF habitual<sup>37</sup> e incremento del conocimientos respecto a la pr&aacute;ctica de AF y elaboraci&oacute;n de programas de ejercicio<sup>19</sup>. En los estudios "Choose Health"<sup>29</sup> y "Loozit"<sup>32</sup> no hubo diferencias significativas en el tiempo dedicado a la pr&aacute;ctica de AF, actividades sedentarias y tiempo dedicado a televisi&oacute;n.</font></p>     <p><font face="Verdana" size="2"><b>Alimentaci&oacute;n/dieta</b></font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">Ocho de los estudios describieron los resultados obtenidos sobre la intervenci&oacute;n alimentaria, que lograron: mejorar los comportamientos relacionados con una alimentaci&oacute;n saludable<sup>22,35</sup>, reducir el total de energ&iacute;a ingerida (P &lt; 0,01)<sup>12,29</sup>, hidratos de carbono (P &lt; 0,01), grasas (P &lt; 0,01) e ingesta total de az&uacute;car (P &lt; 0,01)<sup>29</sup>, cambiar positivamente los aspectos emocionales y sociales relacionados con el comportamiento en obesos<sup>12</sup>, mejorar los h&aacute;bitos relacionados con la restricci&oacute;n diet&eacute;tica de forma significativa (P &lt; 0,05)<sup>30</sup>, reducir los episodios de ingesta compulsiva de alimentos<sup>31</sup>, optimizar los conocimientos relacionados con las actitudes y h&aacute;bitos saludables de vida de ni&ntilde;os y adolescentes<sup>26</sup>, del consumo de postres y aperitivos (P = 0,001), grasa e ingesta total de calor&iacute;as (P &lt; 0,05)<sup>28</sup>.</font></p>     <p><font face="Verdana" size="2"><b>Condici&oacute;n f&iacute;sica</b></font></p>     <p><font face="Verdana" size="2">Los cambios en la CF fueron analizados en 10 de los estudios incluidos en la revisi&oacute;n. Las intervenciones mejoraron: la potencia aer&oacute;bica m&aacute;xima y frecuencia card&iacute;aca m&aacute;xima en los adolescentes, pero no en los ni&ntilde;os<sup>10</sup>; el consumo m&aacute;ximo de ox&iacute;geno, eficiencia cardiovascular y casi todos los componentes de la CF<sup>11,17-18</sup>; las capacidades motoras global y f&iacute;sica<sup>12</sup>; condici&oacute;n aer&oacute;bica<sup>13,17</sup>; condici&oacute;n cardiorrespiratoria<sup>14,16,37</sup>; en la estimativa de VO<sub>2m&aacute;x</sub> con respecto al peso corporal y masa libre de grasa<sup>15</sup>.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Resultados del per&iacute;odo de seguimiento</b></font></p>     <p><font face="Verdana" size="2">Para verificar la permanencia de los efectos alcanzados en las intervenciones a lo largo de tiempo, algunos estudios realizaron un per&iacute;odo de seguimiento.</font></p>     <p><font face="Verdana" size="2">El estudio "<i>HIPTEENS</i>" con educaci&oacute;n alimentaria, ejercicio y estilo de vida saludable, no produjo diferencias significativas en la grasa corporal entre el grupo de intervenci&oacute;n y grupo control despu&eacute;s de 2 a&ntilde;os<sup>20</sup>.</font></p>     <p><font face="Verdana" size="2">El estudio "<i>The Kids N Fitness</i>", present&oacute; un incremento no significativo del peso y aumento del IMC en el periodo de seguimiento<sup>22</sup>.</font></p>     <p><font face="Verdana" size="2">El estudio "<i>Obeldicks</i>", mostr&oacute; que la participaci&oacute;n en el programa se asoci&oacute; positivamente con la p&eacute;rdida del exceso de peso, que se mantuvo 3 a&ntilde;os m&aacute;s tarde de la finalizaci&oacute;n del programa<sup>33</sup>.</font></p>     <p><font face="Verdana" size="2">El estudio "<i>Bright Bodies</i>" se mostr&oacute; eficaz manteniendo despu&eacute;s de 12 meses los cambios alcanzados al final de la intervenci&oacute;n<sup>24</sup>.</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">El programa desarrollado por Johnston et al,<sup>34</sup> consigui&oacute; mantener los efectos positivos obtenidos en el z-score del IMC, la disminuci&oacute;n del pliegue del tr&iacute;ceps, IMC, peso y porcentaje de sobrepeso (todos P &lt; 0,001) despu&eacute;s de 2 a&ntilde;os.</font></p>     <p><font face="Verdana" size="2">El estudio "<i>Mi Piace Piacermi</i>" observ&oacute; la familia m&aacute;s activa, mejoras en la parte alimentaria con mayor consumo de frutas y vegetales, reducci&oacute;n de la mejoras alcanzadas en IMC y desviaciones est&aacute;ndar de IMC despu&eacute;s de 5 a&ntilde;os<sup>12</sup>.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Discusi&oacute;n</b></font></p>     <p><font face="Verdana" size="2">El objetivo de este trabajo ha sido realizar una revisi&oacute;n sistem&aacute;tica de la efectividad de intervenciones publicadas entre e1 julio de 2006 y el 30 de abril 2012, centradas en programas de AF y nutricionales para el tratamiento del sobrepeso/obesidad de ni&ntilde;os y adolescentes.</font></p>     <p><font face="Verdana" size="2">La mayor parte de los estudios seleccionados se dirigieron hacia ni&ntilde;os y adolescentes de forma conjunta (17 de 23) y en dos de los estudios los adolescentes obtuvieron mejores resultados que los ni&ntilde;os<sup>10,22</sup>. Seis de los estudios seleccionados para esta revisi&oacute;n fueron dise&ntilde;ados espec&iacute;ficamente para adolescentes<sup>13,15,29-32</sup>. Considerando el aumento progresivo en las &uacute;ltimas d&eacute;cadas del sobrepeso/obesidad en la adolescencia<sup>38-39</sup>, destacamos la necesidad de estudios futuros que analicen estrategias espec&iacute;ficas para el tratamiento en esta fase de la vida, m&aacute;s a&uacute;n si cabe teniendo en cuenta la mayor probabilidad de que estos adolescentes sean obesos en la edad adulta<sup>8,40</sup>.</font></p>     <p><font face="Verdana" size="2">Gran parte de los estudios seleccionados fueron desarrollados con la implicaci&oacute;n de la familia (22 de 23). El estudio descrito por Aguer et al<sup>15</sup>, fue el &uacute;nico estudio donde la familia no particip&oacute; directamente en el proceso de intervenci&oacute;n. Es cierto que la participaci&oacute;n de la familia es un componente beneficioso en el proceso de la p&eacute;rdida de peso<sup>8,40</sup>, pero no hemos observado evidencia de la efectividad de la participaci&oacute;n familiar en los estudios analizados. Eisenmann et al<sup>41</sup>, sugieren que intervenciones con la participaci&oacute;n de la familia, escuela y comunidad pueden ayudar a promover cambios hacia estilos de vida saludable, por lo que podemos destacar la necesidad de valorar la participaci&oacute;n de la familia en futuros programas de intervenci&oacute;n orientados al tratamiento del sobrepeso/obesidad de ni&ntilde;os y adolescentes.</font></p>     <p><font face="Verdana" size="2">De acuerdo con los estudios incluidos en esta revisi&oacute;n, podemos resaltar que las intervenciones con AF y orientaci&oacute;n alimentaria son eficaces para la p&eacute;rdida de peso, masa grasa y cambios favorables del IMC. Lo que no se puede afirmar es que tales modificaciones o el mantenimiento de los beneficios a lo largo del tiempo fueron obtenidos s&oacute;lo gracias a los cambios alimentarios o al incremento de la pr&aacute;ctica de AF<sup>5</sup>.</font></p>     <p><font face="Verdana" size="2">Aunque en todos los estudios el componente diet&eacute;tico estuviera presente, pocos estudios (8 de 23) consiguieron modificaciones positivas en los h&aacute;bitos alimentarios y diet&eacute;ticos de los participantes. La evidencia cient&iacute;fica ha demostrado que los cambios en los h&aacute;bitos diet&eacute;ticos son alcanzados mediante una reducci&oacute;n de la grasa y el aumento de la ingesta de fibra, frutas y verduras, es decir, mediante una alimentaci&oacute;n equilibrada<sup>42</sup>. Los resultados referentes a la dieta fueron descritos de forma global, con lo cual, no se puede concluir que las estrategias utilizadas fueran eficaces para promover cambios alimentarios y diet&eacute;ticos positivos.</font></p>     <p><font face="Verdana" size="2">El componente de la AF estuvo presente en todos los estudios. Ocho de estos estudios alcanzaron resultados positivos en lo que se refiere a la pr&aacute;ctica de AF. La diversidad encontrada en la orientaci&oacute;n de las actividades ofrecidas y en los m&eacute;todos de valoraci&oacute;n, dificulta los an&aacute;lisis para identificar qu&eacute; m&eacute;todo fue m&aacute;s eficaz para la obtenci&oacute;n de cambios positivos, hecho que coincide con hallazgos anteriores<sup>40,43</sup>. Es cierto que el n&uacute;mero de investigaciones en este campo van creciendo cada a&ntilde;o en poblaciones sanas<sup>44</sup>, pero existe la necesidad de futuras investigaciones en la promoci&oacute;n de AF en poblaci&oacute;n de ni&ntilde;os y adolescentes con sobrepeso/obesidad en lo que concierne a la metodolog&iacute;a y mantenimiento de los cambios alcanzados.</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">Atlantis et al<sup>43</sup>, destacan la prescripci&oacute;n de ejercicio f&iacute;sico como un complemento de la restricci&oacute;n diet&eacute;tica, por su gran potencial para la reducci&oacute;n del sobrepeso, as&iacute; como el peligro de la restricci&oacute;n en la dieta sin AF. Por lo tanto, los programas que combinan orientaci&oacute;n alimentaria y AF se presentan como los m&aacute;s eficaces y recomendables en el tratamiento del sobrepeso y la obesidad.</font></p>     <p><font face="Verdana" size="2">Algunos estudios valoraron la CF (10 de 23). Destacamos la necesidad de m&aacute;s estudios que incluyan la valoraci&oacute;n de la CF de ni&ntilde;os y adolescentes con sobrepeso/obesidad debido a que el nivel CF se correlaciona directamente con factores de riesgo cardiovascular y el estado de salud de dicha poblaci&oacute;n<sup>45-46</sup>. Ortega et al<sup>47</sup>, destacan la importancia de monitorizar y conocer este marcador de salud, resaltando la necesidad de su inclusi&oacute;n en los estudios cl&iacute;nicos para una mejor comprensi&oacute;n de los factores determinantes de la CF durante el crecimiento y maduraci&oacute;n.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Recomendaciones finales</b></font></p>     <p><font face="Verdana" size="2">En conclusi&oacute;n, los programas de intervenci&oacute;n incluidos en esta revisi&oacute;n se&ntilde;alan la necesidad de estudios espec&iacute;ficos para adolescentes con sobrepeso y/u obesidad. Los resultados apuntan a que programas con AF y orientaci&oacute;n alimentaria son eficaces en la p&eacute;rdida de peso y en la reducci&oacute;n del IMC de ni&ntilde;os y adolescentes con sobrepeso/obesidad. Sin embargo, es necesario investigar objetivamente el mantenimiento a largo plazo de tales cambios, as&iacute; como identificar elementos metodol&oacute;gicos en lo que se refiere a la AF y orientaci&oacute;n alimentaria que puedan valorar la eficacia de los programas de intervenci&oacute;n.
