<body><![CDATA[<p>&nbsp;</p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2">La vitamina D<sub>3</sub> (colecalciferol) se forma a partir de su precursor 7-dehidrocolesterol en la piel por irradiaci&oacute;n ultravioleta. En el h&iacute;gado la vitamina D<sub>3</sub> se hidroxila para formar 25-hidroxivitamina D<sub>3</sub>, que se metaboliza a su metabolito activo 1,25-dihidroxivitamina D<sub>3</sub> preferentemente en el ri&ntilde;&oacute;n. La vitamina D<sub>3</sub> tambi&eacute;n puede ser aportada por la dieta, la cual supone una importante fuente de aporte s&oacute;lo en el caso de una insuficiente exposici&oacute;n al sol. El pescado azul contiene de forma natural grandes cantidades de vitamina D<sub>3</sub>, mientras que otros alimentos contienen cantidades importantes de vitamina D s&oacute;lo despu&eacute;s de la fortificaci&oacute;n. Para la fortificaci&oacute;n, en muchos pa&iacute;ses se emplea vitamina D<sub>2</sub> (ergocalciferol) obtenida de fuentes vegetales &#091;1&#093;.</font></p>     <p><font face="Verdana" size="2">La presencia de la enzima CYP27B1, que conduce la s&iacute;ntesis de 1,25-dihidroxivitamina D a partir de su principal sustrato la 25-hidroxivitamina D, y del receptor de la vitamina D (VDR), distribuidos de modo casi universal en las c&eacute;lulas y tejidos del organismo, le confiere a la vitamina D (aunque tal vez ser&iacute;a m&aacute;s correcto decir al sistema endocrino de la vitamina D) un amplio papel en la salud. No s&oacute;lo en la regulaci&oacute;n del calcio y el metabolismo &oacute;seo, sino tambi&eacute;n en lo referente al sistema cardiovascular, la inmunomodulaci&oacute;n innata o adquirida, la regulaci&oacute;n del crecimiento celular, etc., de tal manera que alrededor del 3% del genoma humano est&aacute; regulado por la hormona 1,25(OH)<sub>2</sub> vitamina D<sub>3</sub> &#091;1-3&#093;.</font></p>     <p><font face="Verdana" size="2">Por lo tanto, no es de extra&ntilde;ar que el inter&eacute;s por la vitamina D de cient&iacute;ficos b&aacute;sicos y cl&iacute;nicos &#091;4&#093;, as&iacute; como de la prensa no especializada y la poblaci&oacute;n en general &#091;5&#093;, se haya incrementado casi exponencialmente en la &uacute;ltima d&eacute;cada.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>La 25-hidroxivitamina D el marcador del estatus corporal de vitamina D</b></font></p>     <p><font face="Verdana" size="2">Desde hace a&ntilde;os existe un consenso universal en que la medici&oacute;n en sangre de los niveles del metabolito 25-hidroxivitamina D es el marcador del estatus corporal de vitamina D, incluyendo s&iacute;ntesis end&oacute;gena por exposici&oacute;n a la luz solar, ingesta diet&eacute;tica en comidas suplementadas o no y tratamientos farmacol&oacute;gicos &#091;1,2&#093;. Sin embargo, la medici&oacute;n de las concentraciones en sangre de 25-hidroxivitamina D ha sido, y contin&uacute;a siendo, muy problem&aacute;tica, pese a las mejoras actuales en cuanto a precisi&oacute;n y exactitud &#091;6&#093;.</font></p>     <p><font face="Verdana" size="2">Parad&oacute;jicamente, dos cuestiones esenciales, ¿cu&aacute;les son los niveles de vitamina D necesarios para la salud &oacute;ptima del hueso y del organismo en general?, y en consecuencia, ¿cu&aacute;l es la dosis a emplear para conseguir esos niveles?, permanecen a&uacute;n sin resolver a d&iacute;a de hoy. La diversidad de opiniones sobre este tema ha generado duras controversias entre investigadores &#091;7,8&#093; y sociedades cient&iacute;ficas. De hecho se han propuesto por las diferentes sociedades cient&iacute;ficas, como puntos de corte de normalidad para la vitamina D, dos niveles s&eacute;ricos de 25-hidroxivitamina D: por encima de 20 ng/ml para el Instituto de Medicina &#091;9&#093;, y por encima de 30 ng/ml para la Fundaci&oacute;n Internacional de Osteoporosis (IOF) &#091;10&#093;, apoyada &eacute;sta &uacute;ltima por la recomendaci&oacute;n de la Sociedad de Endocrinolog&iacute;a de EE.UU. &#091;11&#093; y, en Espa&ntilde;a, por la de la Sociedad Espa&ntilde;ola de Investigaci&oacute;n &Oacute;sea y del Metabolismo Mineral (SEIOMM) &#091;12&#093;.</font></p>     <p><font face="Verdana" size="2">Sin embargo, los estudios empleados para determinar esos puntos de corte med&iacute;an los niveles de 25-hidroxivitamina D por competici&oacute;n proteica (CBP) y/o radioinmunoensayo (RIA), y ten&iacute;an en aquella &eacute;poca una gran variabilidad de precisi&oacute;n y exactitud que llegaba hasta el 50%, lo que constituyen una limitaci&oacute;n importante, y nos sugiere que la prevalencia del d&eacute;ficit o la insuficiencia en vitamina D es ensayo-dependiente.</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">Los ensayos comerciales disponibles y muy simplificados a&uacute;n tienen problemas con exactitud cuando se les compara con el &quot;patr&oacute;n oro&quot;, la cromatograf&iacute;a l&iacute;quida, acoplada a espectrometr&iacute;a de masas en t&aacute;ndem &#091;6,13-15&#093;, hasta el punto que las plataformas para an&aacute;lisis de rutina de 25OHD pueden diferir hasta un 20% por encima o por debajo de los valores obtenidos con los ensayos patr&oacute;n-oro &#091;6&#093;. Sin embargo, aunque deben mejorar sustancialmente, los m&eacute;todos disponibles en nuestra pr&aacute;ctica habitual asistencial o de investigaci&oacute;n son suficientemente apropiados, y deber&iacute;an ser m&aacute;s empleados en nuestra pr&aacute;ctica cl&iacute;nica para el diagn&oacute;stico (y el seguimiento posterior de los tratamientos con vitamina D). No obstante, deber&iacute;amos ser cuidadosos con la interpretaci&oacute;n de nuestros propios resultados o de los estudios epidemiol&oacute;gicos o ensayos cl&iacute;nicos aleatorizados cuando en los ensayos utilizados para cuantificar niveles de 25OHD no se calibra conforme a est&aacute;ndares internacionales, tales como DEQAS (<a target="_blank" href="http://www.deqas.org">www.deqas.org</a>) o estandarizaci&oacute;n de NIST &#091;16&#093;.</font></p>     <p><font face="Verdana" size="2">Convendremos en que un objetivo m&iacute;nimo irrenunciable, debe ser conseguir niveles s&eacute;ricos de 25-hidroxivitamina D superiores a 20 ng/ml (para pasar a nmol/l multiplicar por 2,5) lo cual implica estar consiguiendo una media, en toda la poblaci&oacute;n cercana a los 30 ng/ml &#091;17&#093;, y preferiblemente mayores de 30 ng/ml, para asegurar un estatus &oacute;ptimo para la salud &oacute;sea &#091;18&#093; que probablemente deber&aacute; ser m&aacute;s alto si nos proponemos alcanzar otros objetivos de salud &#091;2,3,8,19&#093;. De tal modo, que si nuestros pacientes consiguen niveles de 25-hidroxivitamina D por encima de 30 ng/ml, estaremos armonizando las recomendaciones existentes, con las limitaciones metodol&oacute;gicas de los estudios que las generaron y matizaremos las limitaciones de nuestro propio m&eacute;todo de medida.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Estatus insuficiente de vitamina D en el mundo</b></font></p>     <p><font face="Verdana" size="2">En la actualidad, niveles insuficientes o incluso franca deficiencia de vitamina D determinada como 25-hidroxivitamina D constituye una "epidemia" en todo el mundo que afecta a m&aacute;s de la mitad de la poblaci&oacute;n &#091;3,20&#093;, descrita en ni&ntilde;os, j&oacute;venes, adultos, mujeres postmenop&aacute;usicas y ancianos; sobre todo si tienen fracturas osteopor&oacute;ticas, donde la prevalencia de niveles bajos de 25-hidroxivitamina D llega al 100% &#091;20&#093;.</font></p>     <p><font face="Verdana" size="2">En una excelente reciente revisi&oacute;n de los trabajos disponibles en todo el mundo se encontr&oacute; que el 88% de las muestras evaluadas ten&iacute;a niveles s&eacute;ricos de 25-hidroxivitamina D por debajo de 30 ng/ml; el 37% tienen valores medios por debajo de 20 ng/ml y hasta un 7% tienen valores medios inferiores a 10 ng/ml &#091;21&#093;.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Estatus insuficiente de vitamina D en Espa&ntilde;a</b></font></p>     <p><font face="Verdana" size="2">En Espa&ntilde;a se reproduce esta situaci&oacute;n de estatus inadecuado de calcifediol con resultados que exponemos en la <a href="#t1">tabla 1</a> &#091;22-38&#093;.</font></p>     <p>&nbsp;</p>     ]]></body>
<body><![CDATA[<p align="center"><font face="Verdana" size="2"><a name="t1"><img src="/img/revistas/romm/v6s1/original2_t1a.gif"></a></font></p>     <p align="center"><font face="Verdana" size="2"><img src="/img/revistas/romm/v6s1/original2_t1b.gif"></font></p>     <p align="center"><font face="Verdana" size="2"><img src="/img/revistas/romm/v6s1/original2_t1c.gif"></font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2">La variaci&oacute;n interlaboratorio de los diferentes m&eacute;todos empleados hace dif&iacute;cil una comparaci&oacute;n rigurosa, pero la tabla ilustra claramente que, pese a una te&oacute;rica facilidad climatol&oacute;gica para la s&iacute;ntesis de vitamina D en Espa&ntilde;a, los niveles son semejantes, o incluso inferiores, a los descritos para Europa central o Escandinavia, como ya se hab&iacute;a descrito en trabajos previos &#091;39&#093;. Esta aparente "paradoja" que Espa&ntilde;a comparte con otros pa&iacute;ses de la cuenca del Mediterr&aacute;neo &#091;39,40&#093; se ha tratado de explicar, de modo especulativo, por el escaso aporte diet&eacute;tico de vitamina D que no puede ser compensado por la s&iacute;ntesis cut&aacute;nea. La mayor parte de Espa&ntilde;a est&aacute; por encima del paralelo 35<sup>o</sup>N, donde la posibilidad de sintetizar vitamina D es escasa en invierno y primavera, y porque la mayor&iacute;a de los espa&ntilde;oles tienen una piel m&aacute;s oscura que dificulta la s&iacute;ntesis de vitamina D &#091;39&#093;.</font></p>     <p><font face="Verdana" size="2">Observamos que en Espa&ntilde;a como en el resto del mundo la insuficiencia de vitamina D o incluso franca deficiencia se encuentra ya en ni&ntilde;os o en j&oacute;venes, y persiste en adultos, en mujeres postmenop&aacute;usicas (osteopor&oacute;ticas o no) y en ancianos que viven en sus casas, y que en &eacute;stos es mayor si viven en residencias, con una variaci&oacute;n estacional que apenas llega a normalizarse despu&eacute;s del verano-oto&ntilde;o &#091;22-39&#093;.</font></p>     <p><font face="Verdana" size="2">Aunque esta elevada prevalencia de niveles bajos de vitamina D ocurre por una inadecuada exposici&oacute;n al sol, en ancianos espa&ntilde;oles se han descrito niveles m&aacute;s bajos en los meses de verano, debido a las elevadas temperaturas que ocurren en ciudades del sur de Espa&ntilde;a como Murcia o C&oacute;rdoba durante esta &eacute;poca, donde frecuentemente se sit&uacute;an entre los 30 y 40<sup>o</sup>C; las personas ancianas evitan estar al sol y prefieren estar en el interior de las casas donde la temperatura es m&aacute;s confortable. Adem&aacute;s, los ancianos est&aacute;n muy advertidos del riesgo de c&aacute;ncer de piel por la exposici&oacute;n directa al sol, pero en oto&ntilde;o o durante los meses de invierno se benefician de una temperatura m&aacute;s favorable (20-25<sup>o</sup>C) que les permite tomar el sol con ropas ligeras y, por tanto, sintetizar vitamina D &#091;22,23,32&#093;.</font></p>     <p><font face="Verdana" size="2">La elevada prevalencia de insuficiencia en vitamina D se da independientemente de la zona geogr&aacute;fica y del punto de corte establecido por los distintos autores, en mujeres postmenop&aacute;usicas espa&ntilde;olas y en ancianos espa&ntilde;oles (<a href="#t1">tabla 1</a>).</font></p>     <p><font face="Verdana" size="2">Estos resultados han sido confirmados en su conjunto por un estudio transversal llevado a cabo en unidades de estudio y tratamiento de osteoporosis de toda Espa&ntilde;a al final de la primavera. La 25-hidroxivitamina D se cuantific&oacute; tras separaci&oacute;n por HPLC &#091;38&#093;, y en el cual se evidencia que m&aacute;s de las tres cuartas partes (76%) de mujeres osteopor&oacute;ticas postmenop&aacute;usicas que a&uacute;n no han comenzado el tratamiento presentan niveles de 25-hidroxivitamina D por debajo de 30 ng/ml, y un 44% por debajo de 40 ng/ml.</font></p>     <p><font face="Verdana" size="2">Los datos disponibles confirman que existe una insuficiencia y a&uacute;n franca deficiencia en la poblaci&oacute;n espa&ntilde;ola en todas las edades estudiadas y en ambos sexos, similar a la existente en todo el mundo incluyendo regiones muy soleadas &#091;39,41&#093; y a la existente en otros pa&iacute;ses de la cuenca del Mediterr&aacute;neo &#091;40&#093; con similares posibilidades de exposici&oacute;n al sol. La prevalencia de deficiencia es a&uacute;n mayor en pacientes con factores de riesgo para presentar niveles s&eacute;ricos bajos de vitamina D, obesos y en &aacute;mbitos de pobreza &#091;42&#093;.</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana" size="2">Por tanto, la deficiencia de vitamina D en Espa&ntilde;a no es un mito (persona o cosa a las que se atribuyen cualidades o excelencias que no tienen, o bien una realidad de la que carecen), sino una realidad, de importante repercusi&oacute;n sobre la salud &oacute;sea y probablemente sobre la salud del organismo en su conjunto.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Impacto sobre el hueso</b></font></p>     <p><font face="Verdana" size="2">La deficiencia de vitamina D estimula la secreci&oacute;n de PTH, aumenta el remodelado &oacute;seo, produce p&eacute;rdida de la densidad mineral y la calidad &oacute;sea, aumenta el riesgo de ca&iacute;das factores que interactuando aumentan el riesgo de osteoporosis y fractura osteopor&oacute;tica &#091;43-45&#093;.</font></p>     <p><font face="Verdana" size="2">Sorprendentemente, el tratamiento convencional no normaliza de modo absoluto los niveles s&eacute;ricos de vitamina D, encontrando en mujeres postmenop&aacute;usicas espa&ntilde;olas en tratamiento por osteoporosis niveles de 25-hidroxivitamina D menores de 30 ng/ml y 20 ng/ml en el 63 y 30% &#091;38&#093;, datos consistentes con otros resultados previos descritos para Espa&ntilde;a &#091;46&#093;, otros pa&iacute;ses de Europa &#091;46&#093; o Estados Unidos de Norteam&eacute;rica &#091;46,47&#093;, lo que a primera vista puede resultar chocante, porque en todas las gu&iacute;as y recomendaciones terap&eacute;uticas de organizaciones profesionales para el tratamiento de la osteoporosis est&aacute; ampliamente reconocido y recomendado que el aporte adecuado de calcio y vitamina D constituye la base, y debe siempre asociarse al tratamiento con los f&aacute;rmacos osteoactivos para la osteoporosis &#091;48&#093;.</font></p>     <p><font face="Verdana" size="2">La elevada prevalencia de mujeres con niveles insuficientes de vitamina D pese a estar sometidas a tratamiento podr&iacute;a estar indicando una potencial ausencia de cumplimiento &#091;49&#093;. Otra posibilidad ser&iacute;a que la dotaci&oacute;n gen&eacute;tica de nuestros pacientes condicione niveles m&aacute;s bajos de vitamina D &#091;50&#093;. Ambas posibilidades deber&iacute;an ser valoradas al instaurar un tratamiento osteoactivo con terapias antirresortivas o anab&oacute;licas.</font></p>     <p><font face="Verdana" size="2">La evidencia disponible indica que, adem&aacute;s de un aporte insuficiente de calcio, niveles s&eacute;ricos inadecuados de vitamina D potencialmente disminuyen la respuesta a los tratamientos para la osteoporosis. De hecho, dos grupos espa&ntilde;oles han publicado que la insuficiencia o deficiencia de vitamina D (niveles s&eacute;ricos por debajo de 20 &oacute; 30 ng/ml) constituyen un importante factor contribuyente a la respuesta inadecuada al tratamiento antirresortivo &#091;51,52&#093;.</font></p>     <p><font face="Verdana" size="2">En conclusi&oacute;n, incluso en regiones soleadas como Espa&ntilde;a, es importante destacar la necesidad de mejorar el conocimiento m&eacute;dico y del paciente con respecto a la optimizaci&oacute;n del consumo de calcio y vitamina D en pacientes con osteoporosis. Esto aumentar&iacute;a la observancia del tratamiento y, por lo tanto, la optimizaci&oacute;n de la salud &oacute;sea por la mejora de la respuesta del hueso a medicamentos para la osteoporosis.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana" size="2"><b>Bibliograf&iacute;a</b></font></p>     ]]></body>
