En México, las complicaciones de la diabetes son altamente prevalentes, como en Yucatán, donde se ha reportado que aproximadamente 38,4 % de los pacientes con diabetes también padecen enfermedad renal; y, la prescripción de múltiples medicamentos para su tratamiento representa un riesgo de interacciones farmacológicas que agraven la salud renal. El objetivo de este estudio fue identificar las interacciones farmacológicas potenciales y su nivel de relevancia clínica en la farmacoterapia de pacientes con diabetes tipo 2 y enfermedad renal. Método: Se incluyeron 34 pacientes registrados en el Centro de Atención a la Diabetes del Instituto Mexicano del Seguro Social. Se recolectó información de la historia clínica y farmacoterapia mediante expedientes clínicos electrónicos. Las interacciones farmacológicas potenciales se evaluaron utilizando Micromedex®, considerando la relevancia clínica. Resultados: La edad promedio de los pacientes fue 59 ± 9,6 años; 76,5 % fueron mujeres y 94,1 % estuvieron polimedicados. Se identificaron 117 interacciones farmacológicas potenciales; el 82,5 % de los pacientes presentó al menos una, siendo la más frecuente metformina-hidroclorotiazida de relevancia clínica media. El rango de prescripción de medicamentos fue de 1 a 10, con un promedio de 6,09 ± 2,3. Metformina, alopurinol y clortalidona requirieron ajuste de dosis según el aclaramiento de creatinina. Conclusiones: En este estudio se detectaron interacciones farmacológicas potenciales de relevancia clínica muy alta, alta y media en los pacientes con diabetes tipo 2 y enfermedad renal bajo tratamiento, lo cual podría agravar su salud y función renal.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract Introduction: Kidney disease is the most common complication of type 2 diabetes. In Mexico, diabetes-related complications are highly prevalent; for example, in Yucatán, approximately 38.4 % of patients with diabetes also suffer from chronic kidney disease; and the prescription of multiple drugs for its treatment increases the risk of drug-drug interactions that may further deteriorate renal function. The aim of this study was to identify potential drug-drug interactions and their level of clinical relevancy in the pharmacotherapy in patients with type 2 diabetes and kidney disease. Method: This study included 34 patients registered in the Diabetic Care Center program of the Mexican Social Security Institute. Information on clinical history and pharmacotherapy were collected through electronic clinical records. Potential drug interactions were evaluated using Micromedex® considering their clinical relevance. Results: The mean age for the patients was 59 ± 9.6 years old; 76.5 % were women and 94.1 % were polymedicated. A total of 117 potential drug interactions were identified; 82.35 % presented at least one, the most frequent pDDI was metformin-hydrochlorothiazide of medium clinical relevance. The range of medication prescription was between 1 and 10 drugs, with an average of 6.09 ± 2.3. Metformin, allopurinol and chlorthalidone require dose adjustment based on creatinine clearance. Conclusions: In this study, potential drug interactions of very high, high and medium clinical relevance were detected in patients with type 2 diabetes and kidney disease under treatment, which could worsen their health and kidney function.]]></p></abstract>
