Autologous fat infiltration has been established as an essential procedure in the armamentarium of the plastic surgeon, both in cosmetic procedures such as reconstructive. However, it was not always this way, to get up until today, it was necessary to go through many years of research and numerous and varied studies Despite all this progress, there are still many controversies surrounding the procedure. Research continues on the best body area for fat to be grafted, the way fat should be obtained, processing and handling before injecting it, the best technique for infiltration and layer of placement. However, to the delight of all surgeons that have advocated this procedure for decades, the utility of autologous fat infiltration is already a globally accepted procedure. The controversies are minimal in relation to the general acceptance of the procedure and only exist to obtain greater survival of infiltrated fat. It is accepted that the best donor areas are the abdomen and inner thighs, and must be obtained at a pressure less than one atmosphere. Centrifugation at 3,000 rpm and not more than 3 minutes and placed in multiple planes and small amounts in each pass of the cannula ensure better integration and fat survival. And now the main challenge is to maximize the innate advantages of pluripotent stem cells injected within the same process of autologous fat infiltration.
